    • From 1985 to 2016, the prevalence of underweight decreased, and that of obesity and severe obesity increased, in most regions, with significant variation in the magnitude of these changes across regions. We investigated how much change in mean body mass index (BMI) explains changes in the prevalence of underweight, obesity, and severe obesity in different regions using data from 2896 population-based studies with 187 million participants. Changes in the prevalence of underweight and total obesity, and to a lesser extent severe obesity, are largely driven by shifts in the distribution of BMI, with smaller contributions from changes in the shape of the distribution. In East and Southeast Asia and sub-Saharan Africa, the underweight tail of the BMI distribution was left behind as the distribution shifted. There is a need for policies that address all forms of malnutrition by making healthy foods accessible and affordable, while restricting unhealthy foods through fiscal and regulatory restrictions.

  • Diminishing benefits of urban living for children and adolescents' growth and development

    • Optimal growth and development in childhood and adolescence is crucial for lifelong health and well-being1–6. Here we used data from 2,325 population-based studies, with measurements of height and weight from 71 million participants, to report the height and body-mass index (BMI) of children and adolescents aged 5–19 years on the basis of rural and urban place of residence in 200 countries and territories from 1990 to 2020. In 1990, children and adolescents residing in cities were taller than their rural counterparts in all but a few high-income countries. By 2020, the urban height advantage became smaller in most countries, and in many high-income western countries it reversed into a small urban-based disadvantage. The exception was for boys in most countries in sub-Saharan Africa and in some countries in Oceania, south Asia and the region of central Asia, Middle East and north Africa. In these countries, successive cohorts of boys from rural places either did not gain height or possibly became shorter, and hence fell further behind their urban peers. The difference between the age-standardized mean BMI of children in urban and rural areas was <1.1 kg m–2 in the vast majority of countries. Within this small range, BMI increased slightly more in cities than in rural areas, except in south Asia, sub-Saharan Africa and some countries in central and eastern Europe. Our results show that in much of the world, the growth and developmental advantages of living in cities have diminished in the twenty-first century, whereas in much of sub-Saharan Africa they have amplified.

  • Design and Baseline Characteristics of the Finerenone in Reducing Cardiovascular Mortality and Morbidity in Diabetic Kidney Disease Trial

    • <b><i>Background:</i></b> Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. <b><i>Patients and</i></b> <b><i>Methods:</i></b> The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate ≥25 mL/min/1.73 m<sup>2</sup> and albuminuria (urinary albumin-to-creatinine ratio ≥30 to ≤5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level α = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. <b><i>Conclusions:</i></b> FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.   • Nrf2-interacting nutrients and COVID-19: time for research to develop adaptation strategies

    • There are large between- and within-country variations in COVID-19 death rates. Some very low death rate settings such as Eastern Asia, Central Europe, the Balkans and Africa have a common feature of eating large quantities of fermented foods whose intake is associated with the activation of the Nrf2 (Nuclear factor (erythroid-derived 2)-like 2) anti-oxidant transcription factor. There are many Nrf2-interacting nutrients (berberine, curcumin, epigallocatechin gallate, genistein, quercetin, resveratrol, sulforaphane) that all act similarly to reduce insulin resistance, endothelial damage, lung injury and cytokine storm. They also act on the same mechanisms (mTOR: Mammalian target of rapamycin, PPARγ:Peroxisome proliferator-activated receptor, NFκB: Nuclear factor kappa B, ERK: Extracellular signal-regulated kinases and eIF2α:Elongation initiation factor 2α). They may as a result be important in mitigating the severity of COVID-19, acting through the endoplasmic reticulum stress or ACE-Angiotensin-II-AT1R axis (AT1R) pathway. Many Nrf2-interacting nutrients are also interacting with TRPA1 and/or TRPV1. Interestingly, geographical areas with very low COVID-19 mortality are those with the lowest prevalence of obesity (Sub-Saharan Africa and Asia). It is tempting to propose that Nrf2-interacting foods and nutrients can re-balance insulin resistance and have a significant effect on COVID-19 severity. It is therefore possible that the intake of these foods may restore an optimal natural balance for the Nrf2 pathway and may be of interest in the mitigation of COVID-19 severity.

    • The PREDICTS project-Projecting Responses of Ecological Diversity In Changing Terrestrial Systems (www.predicts.org.uk)-has collated from published studies a large, reasonably representative database of comparable samples of biodiversity from multiple sites that differ in the nature or intensity of human impacts relating to land use. We have used this evidence base to develop global and regional statistical models of how local biodiversity responds to these measures. We describe and make freely available this 2016 release of the database, containing more than 3.2 million records sampled at over 26,000 locations and representing over 47,000 species. We outline how the database can help in answering a range of questions in ecology and conservation biology. To our knowledge, this is the largest and most geographically and taxonomically representative database of spatial comparisons of biodiversity that has been collated to date; it will be useful to researchers and international efforts wishing to model and understand the global status of biodiversity.

    • The High Altitude Water Cherenkov (HAWC) observatory and the High Energy Stereoscopic System (H.E.S.S.) are two leading instruments in the ground-based very-high-energy gamma-ray domain. HAWC employs the water Cherenkov detection (WCD) technique, while H.E.S.S. is an array of Imaging Atmospheric Cherenkov Telescopes (IACTs). The two facilities therefore differ in multiple aspects, including their observation strategy, the size of their field of view, and their angular resolution, leading to different analysis approaches. Until now, it has been unclear if the results of observations by both types of instruments are consistent: several of the recently discovered HAWC sources have been followed up by IACTs, resulting in a confirmed detection only in a minority of cases. With this paper, we go further and try to resolve the tensions between previous results by performing a new analysis of the H.E.S.S. Galactic plane survey data, applying an analysis technique comparable between H.E.S.S. and HAWC. Events above 1 TeV are selected for both data sets, the point-spread function of H.E.S.S. is broadened to approach that of HAWC, and a similar background estimation method is used. This is the first detailed comparison of the Galactic plane observed by both instruments. H.E.S.S. can confirm the gamma-ray emission of four HAWC sources among seven previously undetected by IACTs, while the three others have measured fluxes below the sensitivity of the H.E.S.S. data set. Remaining differences in the overall gamma-ray flux can be explained by the systematic uncertainties. Therefore, we confirm a consistent view of the gamma-ray sky between WCD and IACT techniques.

  • Association between convalescent plasma treatment and mortality in COVID-19 : a collaborative systematic review and meta-analysis of randomized clinical trials

    • Background: Convalescent plasma has been widely used to treat COVID-19 and is under investigation in numerous randomized clinical trials, but results are publicly available only for a small number of trials. The objective of this study was to assess the benefits of convalescent plasma treatment compared to placebo or no treatment and all-cause mortality in patients with COVID-19, using data from all available randomized clinical trials, including unpublished and ongoing trials (Open Science Framework, ). Methods: In this collaborative systematic review and meta-analysis, clinical trial registries (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform), the Cochrane COVID-19 register, the LOVE database, and PubMed were searched until April 8, 2021. Investigators of trials registered by March 1, 2021, without published results were contacted via email. Eligible were ongoing, discontinued and completed randomized clinical trials that compared convalescent plasma with placebo or no treatment in COVID-19 patients, regardless of setting or treatment schedule. Aggregated mortality data were extracted from publications or provided by investigators of unpublished trials and combined using the Hartung-Knapp-Sidik-Jonkman random effects model. We investigated the contribution of unpublished trials to the overall evidence. Results: A total of 16,477 patients were included in 33 trials (20 unpublished with 3190 patients, 13 published with 13,287 patients). 32 trials enrolled only hospitalized patients (including 3 with only intensive care unit patients). Risk of bias was low for 29/33 trials. Of 8495 patients who received convalescent plasma, 1997 died (23%), and of 7982 control patients, 1952 died (24%). The combined risk ratio for all-cause mortality was 0.97 (95% confidence interval: 0.92; 1.02) with between-study heterogeneity not beyond chance (I-2 = 0%). The RECOVERY trial had 69.8% and the unpublished evidence 25.3% of the weight in the meta-analysis. Conclusions: Convalescent plasma treatment of patients with COVID-19 did not reduce all-cause mortality. These results provide strong evidence that convalescent plasma treatment for patients with COVID-19 should not be used outside of randomized trials. Evidence synthesis from collaborations among trial investigators can inform both evidence generation and evidence application in patient care.

    • We propose an operational degree of polarization in terms of the variance of the Stokes vector minimized over all the directions of the Poincare sphere. We examine the properties of this second-order definition and carry out its experimental determination. Quantum states with the same standard (first-order) degree of polarization are correctly discriminated by this new measure. We argue that a comprehensive quantum characterization of polarization properties requires a whole hierarchy of higher-order degrees.

    • We work out the phase-space structure for a system of n qubits. We replace the field of real numbers that label the axes of the continuous phase space by the finite field GF(2(n)) and investigate the geometrical structures compatible with the notion of unbiasedness. These consist of bundles of discrete curves intersecting only at the origin and satisfying certain additional properties. We provide a simple classification of such curves and study in detail the four- and eight-dimensional cases, analyzing also the effect of local transformations. In this way, we provide a comprehensive phase-space approach to the construction of mutually unbiased bases for n qubits.

    • For a system of N qubits, living in a Hilbert space of dimension d=2(N), it is known that there exists d+1 mutually unbiased bases. Different construction algorithms exist, and it is remarkable that different methods lead to sets of bases with different properties as far as separability is concerned. Here we derive four sets of nine bases for three qubits, and show how they are unitarily related. We also briefly discuss the four-qubit case, give the entanglement structure of 16 sets of bases, and show some of them and their interrelations, as examples. The extension of the method to the general case of N qubits is outlined.

This may be sufficient for most classical problems, but for quantum fields higher-order correlations are crucial. We claim that a complete polarization characterization must involve a whole hierarchy of polarization degrees constructed order by order, as it happens in coherence theory. We present here a second-order degree and determine it experimentally.

    • We present a moment expansion for the systematic characterization of the polarization properties of quantum states of light. Specifically, we link the method to the measurements of the Stokes operator in different directions on the Poincare sphere and provide a scheme for polarization tomography without resorting to full-state tomography. We apply these ideas to the experimental first-and second-order polarization characterization of some two-photon quantum states. In addition, we show that there are classes of states whose polarization characteristics are dominated not by their first-order moments (i.e., the Stokes vector) but by higher-order polarization moments.

    • We put forward an operational degree of polarization that can be extended in a natural way to fields whose wave fronts are not necessarily planar. This measure appears as a distance from a state to the set of all of its polarization-transformed counterparts. By using the Hilbert-Schmidt metric, the resulting degree is a sum of two terms: one is the purity of the state and the other can be interpreted as a classical distinguishability, which can be experimentally determined in an interferometric setup. For transverse fields, this reduces to the standard approach, whereas it allows one to get a straight expression for nonparaxial fields.

By controlling the exposure pattern with a simple bank of birefringent plates, any pixel pattern on a (N + 1) X (N + 1) grid, occupying a square with the side half a wavelength long, can be generated from a 2N-photon state.

    • The characterization of quantum polarization of light requires knowledge of all the moments of the Stokes variables, which are appropriately encoded in the multipole expansion of the density matrix. We look into the cumulative distribution of those multipoles and work out the corresponding extremal pure states. We find that SU(2) coherent states are maximal to any order whereas the converse case of minimal states (which can be seen as the most quantum ones) is investigated for a diverse range of the number of photons. Taking advantage of the Majorana representation, we recast the problem as that of distributing a number of points uniformly over the surface of the Poincare sphere.

    • Mutually unbiased bases and discrete Wigner functions are closely but not uniquely related. Such a connection becomes more interesting when the Hilbert space has a dimension that is a power of a prime N=d(n), which describes a composite system of n qudits. Hence, entanglement naturally enters the picture. Although our results are general, we concentrate on the simplest nontrivial example of dimension N=8=2(3). It is shown that the number of fundamentally different Wigner functions is severely limited if one simultaneously imposes translational covariance and that the generating operators consist of rotations around two orthogonal axes, acting on the individual qubits only.

    • We propose a single-particle experiment that is equivalent to the conventional two-particle experiment used to demonstrate a violation of Bell's inequalities. Hence, we argue that quantum mechanical nonlocality can be demonstrated by single-particle states. The validity of such a claim has been discussed in the literature, but without reaching a clear consensus. We show that the disagreement can be traced to what part of the total state of the experiment one assigns to the (macroscopic) measurement apparatus. However, with a conventional and legitimate interpretation of the measurement process one is led to the conclusion that even a single particle can show nonlocal properties.

    • We demonstrate a systematic approach to subwavelength resolution Lithographic image formation on films covering areas larger than a wavelength squared. For example, it is possible to make a lithographic pattern with a feature size resolution of lambda/[2(N+1)] by using a particular 2M-photon, multimode entangled state, where N less than or equal toM, and banks of birefringent plates. By preparing such a statistically mixed state, one can form any pixel pattern on a 2(M-N)(N+1) x 2(M-N)(N+1) pixel grid occupying a square with side L = 2(M-N-1)lambda. Hence, there is a trade off between the exposed area, the minimum lithographic feature size resolution, and the number of photons used for the exposure. We also show that the proposed method will work even under nonideal conditions, albeit with somewhat poorer performance.

In particular, we consider two recently proposed schemes with 'bucket detection' of one of the photons.     • Quantum metrology allows for a tremendous boost in the accuracy of measurement of diverse physical parameters. The estimation of a rotation constitutes a remarkable example of this quantum-enhanced precision. The recently introduced Kings of Quantumness are especially germane for this task when the rotation axis is unknown, as they have a sensitivity independent of that axis and they achieve a Heisenberg-limit scaling. Here, we report the experimental realization of these states by generating up to 21-dimensional orbital angular momentum states of single photons, and confirm their high metrological abilities.

    • We discuss the polarization of paraxial and nonparaxial classical light fields by resorting to a multipole expansion of the corresponding polarization matrix. It turns out that only a dipolar term contributes when one considers SU(2) (paraxial) or SU(3) (nonparaxial) as fundamental symmetries. In this latter case, one can alternatively expand in SU(2) multipoles, and then both a dipolar and a quadrupolar component contribute, which explains the richer structure of this nonparaxial instance. These multipoles uniquely determine Wigner functions, in terms of which we examine some intriguing hallmarks arising in this classical scenario.

    • We capitalize on a multipolar expansion of the polarization density matrix, in which multipoles appear as successive moments of the Stokes variables. When all the multipoles up to a given order K vanish, we can properly say that the state is Kth-order unpolarized, as it lacks of polarization information to that order. First-order unpolarized states coincide with the corresponding classical ones, whereas unpolarized to any order tally with the quantum notion of fully invariant states. In between these two extreme cases, there is a rich variety of situations that are explored here. The existence of hidden polarization emerges in a natural way in this context.

  • TOI-1749: an M dwarf with a Trio of Planets including a Near-resonant Pair

    • We report the discovery of one super-Earth- (TOI-1749b) and two sub-Neptune-sized planets (TOI-1749c and TOI-1749d) transiting an early M dwarf at a distance of 100 pc, which were first identified as planetary candidates using data from the TESS photometric survey. We have followed up this system from the ground by means of multiband transit photometry, adaptive optics imaging, and low-resolution spectroscopy, from which we have validated the planetary nature of the candidates. We find that TOI-1749b, c, and d have orbital periods of 2.39, 4.49, and 9.05 days, and radii of 1.4, 2.1, and 2.5 R (circle plus), respectively. We also place 95% confidence upper limits on the masses of 57, 14, and 15 M (circle plus) for TOI-1749b, c, and d, respectively, from transit timing variations. The periods, sizes, and tentative masses of these planets are in line with a scenario in which all three planets initially had a hydrogen envelope on top of a rocky core, and only the envelope of the innermost planet has been stripped away by photoevaporation and/or core-powered mass-loss mechanisms. These planets are similar to other planetary trios found around M dwarfs, such as TOI-175b,c,d and TOI-270b,c,d, in the sense that the outer pair has a period ratio within 1% of 2. Such a characteristic orbital configuration, in which an additional planet is located interior to a near 2:1 period-ratio pair, is relatively rare around FGK dwarfs.

  • Measurable entanglement criterion for two qubits

    • We propose a directly measurable criterion for the entanglement of two qubits. We compare the criterion with other criteria, and we find that for pure states, and some mixed states, it coincides with the state's concurrence. The measure can be obtained with a Bell-state analyzer and the ability to make general local unitary transformations. However, the procedure fails to measure the entanglement of a general mixed two-qubit state.

    • It is well established that unpolarized light is invariant with respect to any SU(2) polarization transformation. This requirement fully characterizes the set of density matrices representing unpolarized states. We introduce the degree of polarization of a quantum state as its distance to the set of unpolarized states. We use two different candidates of distance, namely the Hilbert-Schmidt and the Bures metric, showing that they induce fundamentally different degrees of polarization. We apply these notions to relevant field states and we demonstrate that they avoid some of the problems arising with the classical definition.

    • We propose a unifying phase-space approach to the construction of mutually unbiased bases for a two-qubit system. It is based on an explicit classification of the geometrical structures compatible with the notion of unbiasedness. These consist of bundles of discrete curves intersecting only at the origin and satisfying certain additional properties. We also consider the feasible transformations between different kinds of curves and show that they correspond to local rotations around the Bloch-sphere principal axes. We suggest how to generalize the method to systems in dimensions that are powers of a prime.

    • Mutually unbiased bases determine an optimal set of measurements to extract complete information about the quantum state of a system. However, quite often a priori information about the state exists, making some of the measurement bases superfluous. This is, for example, the case when multiqubit states belong to the permutationally invariant subspace. In this paper we derive the minimal sets of mutually unbiased bases needed to tomographically reconstruct such states.

    • We consider various approaches to treat the phases of a qutrit. Although it is possible to represent qutrits in a convenient geometrical manner by resorting to a generalization of the Poincare sphere, we argue that the appropriate way of dealing with this problem is through phase operators associated with the algebra su(3). The rather unusual properties of these phases are caused by the small dimension of the system and are explored in detail. We also examine the positive operator-valued measures that can describe the qutrit phase properties.

  • Global Impact of the COVID-19 Pandemic on Stroke Volumes and Cerebrovascular Events: A 1-Year Follow-up.

    • Declines in stroke admission, IV thrombolysis (IVT), and mechanical thrombectomy volumes were reported during the first wave of the COVID-19 pandemic. There is a paucity of data on the longer-term effect of the pandemic on stroke volumes over the course of a year and through the second wave of the pandemic. We sought to measure the effect of the COVID-19 pandemic on the volumes of stroke admissions, intracranial hemorrhage (ICH), IVT, and mechanical thrombectomy over a 1-year period at the onset of the pandemic (March 1, 2020, to February 28, 2021) compared with the immediately preceding year (March 1, 2019, to February 29, 2020).We conducted a longitudinal retrospective study across 6 continents, 56 countries, and 275 stroke centers. We collected volume data for COVID-19 admissions and 4 stroke metrics: ischemic stroke admissions, ICH admissions, IVT treatments, and mechanical thrombectomy procedures. Diagnoses were identified by their ICD-10 codes or classifications in stroke databases.There were 148,895 stroke admissions in the 1 year immediately before compared with 138,453 admissions during the 1-year pandemic, representing a 7% decline (95% CI [95% CI 7.1-6.9]; p < 0.0001). ICH volumes declined from 29,585 to 28,156 (4.8% [5.1-4.6]; p < 0.0001) and IVT volume from 24,584 to 23,077 (6.1% [6.4-5.8]; p < 0.0001). Larger declines were observed at high-volume compared with low-volume centers (all p < 0.0001). There was no significant change in mechanical thrombectomy volumes (0.7% [0.6-0.9]; p = 0.49). Stroke was diagnosed in 1.3% [1.31-1.38] of 406,792 COVID-19 hospitalizations. SARS-CoV-2 infection was present in 2.9% ([2.82-2.97], 5,656/195,539) of all stroke hospitalizations.There was a global decline and shift to lower-volume centers of stroke admission volumes, ICH volumes, and IVT volumes during the 1st year of the COVID-19 pandemic compared with the prior year. Mechanical thrombectomy volumes were preserved. These results suggest preservation in the stroke care of higher severity of disease through the first pandemic year.This study is registered under NCT04934020.

    • We explore the role played by the phase in an accurate description of the entanglement of bipartite systems. We first present an appropriate polar decomposition that leads to a truly Hermitian operator for the phase of a single qubit. We also examine the positive operator-valued measures that can describe the qubit phase properties. When dealing with two qubits, the relative phase seems to be a natural variable to understand entanglement. In this spirit, we propose a measure of entanglement based on this variable.

    • The advantages of using nonclassical states of light for optical imaging are discussed, with special emphasis on the new field of quantum optical lithography. The classical resolution limit given by the Rayleigh criterion is approximately half of the optical wavelength. However, it has been argued that, by using special quantum states of light and a multiphoton-sensitive material or detector, this limit can be broken. Here, a brief and rigorous overview of this problem is provided, some particularly widespread misconceptions are addressed, and quantum lithography into a practical technology is discussed.

    • We show that any pure, two-mode, N-photon state with N odd or equal to two can be transformed into an orthogonal state using only linear optics. According to a recently suggested definition of polarization degree, this implies that all such states are fully polarized. This is also found to be true for any pure, two-mode, energy eigenstate belonging to a two-dimensional SU(2) orbit. Complete two- and three-photon bases whose basis states are related by only phase shifts or geometrical rotations are also derived.

    • We present a complete polarization characterization of any quantum state of two orthogonal polarization modes and give a systematic measurement procedure to collect the necessary data. Full characterization requires measurements of the photon number in both modes and linear optics. In the case where only the photon-number difference can be determined, a limited but useful characterization is obtained. The characteristic Stokes moment profiles are given for several common quantum states.
