  • Healthcare Staff's Racialised talk : Examining Accounts of Racialisation in Healthcare

    • This thesis contributes to the literature on racism in healthcare and the scholarship on racism and racialisation by moving the current focus of healthcare literature from demonstrating the existence of racism to examining accounts of racialisation through analysing healthcare staff’s racialised talk. Drawing from critical ‘race’ and postcolonial theories, the thesis departs from the premise that racism is a structural phenomenon embedded in nation states and institutions, including healthcare across the globe. Through a scoping review ofstudies on racism in healthcare, this thesis maintains that the current literature does not conceptualise racism as structural, and does not attempt to uncover accounts of racialisation. The review argues that the trends uncovered are part of why racism continues to reproduce itself in healthcare, despite equality regulations and policy makers’ efforts to eradicate racism. The thesis posits racialisationas a process situated within the sociohistorical playing out of colonial domination, where in groups of people are stratified somatically and culturally within groups of subordination and supraordination. Societies, institutions, and interactions are viewed as racialised such that an analysis ofracialised talk captures the seemingly subtle racialisation intrinsic tohealthcare. Analytically, the excavation of racialised talk regards talk as reflective and constitutive of the dominant structures within which talk is situated. Drawing on qualitative interviews with 58 healthcare staff in Sweden, the thesis examines how healthcare staff’s racialised talk is used to devalue minority healthcare users and obfuscate racism. The findings of this thesis contradict previous characterisations of racism in today’s society as covert. Racialised talk against minority healthcare users is found to be overt and used to categorise minority users as ‘bad’ users and their health complaints as ‘unworthy’ by labeling symptoms as ‘ethnic’, ‘cultural’ or ‘functional’. The devaluingof minority healthcare users through talk further justifies differential and suboptimal care. Besides demonstrating that racialised talk in healthcare is overt, this thesis proposes that by emphasising healthcare neutrality and equality regulations, blaming minorities for racism, viewing racism as an individual aberration, locating racism outside both national and institutional contexts, healthcare staff manage (albeit inadvertently) to obfuscate racism. It is suggested that obfuscation of racism may serve to allow racism to be perpetuated, resulting in a culture of resignation, where resistance to racism isnegligible.

  • ”Gå eller rulla – Alla vill knulla!” : Funktionsrättsaktivism i nyliberala landskap

    • This study follows groups in Sweden that are involved in a political struggle to change norms regarding sexuality and disability. The groups are mostly connected to the disability movement or the movement of sexual politics. The following questions have guided the research process: How do the groups describe their activities? What strategies do they use? What relations can be identified between them? What obstacles and possibilities do they meet? What space of action is created and maintained for the struggle, taking into account the political, cultural and economic context that they act within?In total, 43 interviews were conducted, predominantly with persons active in the struggle. Furthermore, the analysis is based on participation in the groups’ activities and archival studies of their publications, applications and reports to the funding agencies.The study concludes that an emphasis on knowledge and education always has been and continues to be the main focus for the groups’ political struggle. During the decade of 2010, this focus on education was related to norm criticism (normkritik), which enabled the activists to turn the focus from the deviant to the prescriptive normal. During the years of austerity within welfare services for disabled persons, the political struggle has shifted from a struggle mostly run for and by disability organisations to a situation where more able-bodied allies participate in the struggle, a development that is analysed with help of Crip Theory and the concept of “discursive climate of tolerance”. The analysis also shows how the fund-giving agencies and the projectification of civil society depoliticise, professionalise and dematerialise the political struggle.This dissertation’s main contribution is thus a case study of neoliberal co-optation, a process in which neoliberal hegemony incorporates and appropriates other discourses and practices. The dissertation argues for the need to crip neoliberal co-optation, i.e. to ask questions about what subjects that are interpellated and what space of action is thereby created.

  • Intersectional Directions in Working Life Research : a Proposal

    • A central challenge to gender studies during the last 15 years has been the expanding field of intersectionality. The use of intersectional perspectives within working life research has explored how class, sexuality, and race difference affected women’s position in the labor market. The aim of this article is to argue for the need of including an intersectional perspective in the field of working life research. By taking our point of departure in the work of feminist scholars Joan Acker, Miriam Glucksmann, and Chandra Talpade Mohanty, we argue that an intersectional perspective can expand as well as challenge working life research. But we also argue that working life research in many ways can contribute to the field of intersectional studies, especially by placing issues of exploitation, distribution, and production at the core of intersectional analyses.

  • Real Utopias at Work. Conflicts and dreams among nurses in the public sector.

    • This chapter illuminates what nurses experience as central conflicts shaping their everyday/everynight work, and what forms of injustice they identify as arising from those circumstances. Through the concept of real utopia, we explore what forms of radical visions of care nurses harbour and express. A central argument is that the interviewed nurses suggest a new system of value - in which care constitutes the moral centre of the healthcare organisation,and in which care is allowed to take time and caring time is valued as an asset, rather than viewed as an avoidable cost. We argue that to sustain social reproduction, a new notion of value must evolve in society, one that places primacy on care instead of on accumulation. In the nurses’ accounts, we see radical proposals of such a new system of value - one in which care is the prime resource and a prime profit. Such a vision moves beyond demands for new managerial models, and instead suggests a re-interpretation of how to measure output and what to conceptualise as productivity.

    • In Sweden, austerity politics and market reforms have resulted in a renegotiation of working conditions negatively impacting female-dominated public sector- and welfare state workers. A growing body of research locates the harshening conditions of nurses in a context of colliding logics between professions and professional authority, the specificities of interpersonal work, and public sector restructuring. Feminist scholars also tend to focus on the special character of care work in explaining nurses’ growing discontent under austerity. This article starts out from the position that the characteristics of care work are central in understanding care workers’ experiences, but the analysis also shows a more general conflict between labor and capital play out in care work. Based on a qualitative analysis interviews with 50 nurses, the article shows that this contradiction is expressed through conflicts over time, management and resources.

    • This article introduces the concept of the exit spiral to conceptualize processes in whichpoorly functioning hospital clinics or wards enter a phase of decline due to widespreaddissatisfaction among staff, prompting turnover contagion. In describing and analyzingcases amassed during observations and interviews, the article provides tools to identify,analyze and halt exit spirals. Once a hospital ward finds itself in an exit spiral, employeesexperience increased workloads, loss of control and discontent. Managers may react withincreased rigidity. Instability, discontinuity, and insufficient knowledge transfer areprocesses sparked by exit spirals. The article suggests that exit spirals can be halted ifmanagement recognizes the depth of organizational decline and directs resources toresolve it. This requires further analysis, stimulation of employee voice, innovation, andflexibility.

  • Femininity and feminist consciousness among female Swedish healthcare workers

    • Femininity and feminist consciousness among Swedish healthcare workers Focusing on the question of a “feminist consciousness” among female doctors, nurses, assistant nurses and physiotherapists, the aim of this paper is to explore patterns of class based practices and professional strategies within the context of care. The aim is to capture the complexity and tensions within discourses on gender equality in relation to different forms of (class based and racialised) femininity in the workplace, and among female professionals on different positions within healthcare. How do healthcare professionals on different positions within the hierarchy of the healthcare organization, who regard themselves as feminists, negotiate and sustain professional boundaries? Two examples can be deployed in order to highlight such tensions. First, in-depth interviews with female healthcare employees suggest that professionals higher up in the hierarchy distance themselves from what they see as traditional forms of femininity, represented by e.g. assistant nurses, especially women of colour. Another example is the Swedish nurses’ association (SSF, Svensk sjuksköterskeförening), who drew on a feminist discourse to protest against a government bill to allow assistant nurses to become nurses without having to go through full nurse training. According to the nurses’ association, the bill was “permeated with an old-fashioned view of women’s work and knowledge” because it did not value the theoretical aspects of the nursing profession.

  • Femininity at Work : Gender, Labour, and Changing Relations of Power in a Swedish Hospital

    • Gender scholarship has identified how paid care work reproduces male dominance and reinforces women’s subordination, but also how labour and workplaces provide a critical space for women through the development of new forms of identity and struggle. In this ethnographic study of Swedish nurses’ work, the concept of normative femininity is used in order to explore gender, labour, and changing relations of power in the context of the neoliberal transformation of the Swedish welfare state.The study shows how nursing has undergone dramatic changes in terms of work intensification and new forms of subordination and class boundaries. At the same time, the nursing profession has embraced nurses’ new role as adjunct managers in running clinics and taking on new responsibilities offered by New Public Management.Gendered subjectivities are at the level of the work place produced and reproduced through notions of femininities that shape and are shaped by the labour process. The study is located within the emerging field of ethnographies of neoliberalism and offers an empirical analysis of change and continuity in the relationship between femininity and care work among nurses employed in a Swedish hospital.

  • Improving the quality of healthcare through interdisciplinary education - a collaboration between Medicine and Gender Studies

    • While Gender studies has sometimes been placed in opposition to biomedicine and medical practices in public debate, there is increasing scholarly recognition of the relevance of social factors and relationships for biological and medical phenomena and for the quality and equity of healthcare practices (Chin et al., 2016; Meloni, Cromby, Fitzgerald, & Lloyd, 2018; SKL, 2014). In Sweden and internationally, medical schools have identified the need to integrate approaches from social science and humanities in their curricula (LagroJanssen, 2012; Miller et al., 2013; Polianski, 2012; Viberg, 2010). This emerging interdisciplinarity in higher education, wherein different scientific traditions are bridged, has inspired new forms of collaborations and educational efforts. In this presentation, researchers from the Medical Faculty and the Faculty of Social Sciences at Lund University will present experiences of developing and teaching a course in gender studies aimed at healthcare practitioners. The 7,5 credit course Det jämlika vårdmötet has attracted a large number of students working in different positions within the Swedish healthcare system. The course builds on gender research as well as research in public health, social epidemiology and organization studies, thus bridging theoretical traditions, methodological approaches and research themes for the purpose of encouraging students to reflect on their professional practices in new ways. The focus of this presentation is what the course can teach us about interdisciplinary collaborations and their potential for reaching new groups of students with new and challenging questions and approaches in the interest of quality assurance in public service.

  • Intensivvårdsavdelningarna i Lund och Malmö under Covid-19 : En analys av den organisatoriska responsen och personalens erfarenheter

    • Rapporten sammanfattar det forskningsprojekt, finansierat av Riksbankens Jubileumsfond, som Rebecca Selberg utfört inom verksamhetsområde intensiv- och perioperativ vård (IPV) vid Skånes universitetssjukhus januari 2021 – juni 2022. Rapporten bygger på djupintervjuer och observationer, samt på en strategisk genomgång av tidigare forskning kring krishantering och covidpandemins effekter på vårdpersonal.Rapporten visar att coronapandemin skapade en djupgående kris för IPV och dess medarbetare. Inledningsvis präglades krisen av splittring och brist på tillit, men relativt snart fann organisationen sin kraft i teamarbetet och lyckades hantera ett kraftigt utökat samhällsbärande uppdrag på ett effektivt sätt. Personalens initiativförmåga och tillit till varandra och de närmaste cheferna utgjorde en central resurs för organisationen. Samtidigt ställdes chefer och anställda inför svåra etiska dilemman och avvägningar, som organisationen inte fullt ut lyckades skapa kommunikation och reflektion kring. När krisen blev långdragen drabbades personalen av empatitrötthet och frustration, till rätt stora individuella kostnader. Trots allt klarade organisationen krisen väl, och inte minst berodde detta på chefernas vilja att lyssna på personalen och stötta dem med de resurser som fanns till hands. Organisationens kanske främsta styrka är den flexibla, engagerade och lojala personalen, och chefernas tilltro till sina medarbetare.Viss risk finns för att IPV inte tillräckligt väl lär sig av de erfarenheter och kunskaper som chefer och anställda samlade på sig under pandemin. Krisplaneringen behöver förbättras framöver, och särskilt bör IPV utveckla strategier för att mobilisera resiliens och det som kallas adaptiv kapacitet.

    • Genom att ställa hälso- och sjukvårdskuratorns uppdrag i relation till det som feministiska och marxistiska forskare kallat ”vårdens moraliska ekonomi” bidrar artikeln till att fördjupa förståelsen av kuratorns betydelse i samhällets reproduktiva sfär. Begreppet ”vårdens moraliska ekonomi” syftar på de ”bindningar och skyldigheter” som mer än själva anställningskontraktet formar de vårdande professionernas engagemang. Det är i den moraliska ekonomin som de grundläggande normerna ryms, om hur vi bör handla i en given kontext. Det är också här som legitimitet och lojalitet etableras och förhandlas. När dessa normer utmanas rubbas också förtroendet mellan, exempelvis, professionen och organisationen eller den enskilda anställda och ledningen. Artikeln kan bidra till analyser av såväl betydelsen av som svårigheterna med en stark kuratorsprofession inom sjukvården.

    • Through a historization of the Swedish public sector together with vignettes grounded in interviews, this paper questions managerialism in the public sector. The chapter argues for leaderless management as a shift of attention from managerialism toward worker control. The chapter explores what nurses identify as central areas of conflict shaping their work, and what forms of change they view as necessary to create a more sustainable work situation, for themselves and the patients. Workers—not managers—within the public sector are key knowledge bearers in confronting the crisis of care. Inspired by Nancy Fraser’s idea of a care crisis, as well as Erik Olin Wright’s discussion on real utopia, the chapter analyzes how public sector workers frame their work-life. We use this to frame a vision of leaderless management. Specifically, we draw out three arguments for leaderless management of (1) time, (2) caring, and (3) resources.

    • Ett av de senaste årens mest uppmärksammade fenomen inom högre utbildning handlar om olika former av gränshållande kring potentiellt upprörande undervisningsinnehåll. Dessa konflikter har lett till intensiv mediabevakning och politisk mobilisering. Men bortom de mest uppmärksammade konflikterna finns återkommande och vardagliga situationer där frågor om exempelvis bild- och språkbruk utmanar studenter och lärare när de möts i undervisningen. Essän argumenterar för att det behövs fler och bättre samtal om sådana konflikter och spänningar. Men debatten bör präglas av en analytisk hållning som sätter undervisningsuppdraget i centrum. Syftet med essän är att erbjuda lärare analytiska ingångar till fördjupad reflektion genom att först problematisera begreppet akademisk frihet; därefter diskutera realiseringen av frihet och ansvarstagande inom ramen för specifika styrmekanismer; och slutligen lyfta fram reflexivitet och etik som centrala i utvecklingen av universitetslärares professionalitet.

  • Nursing in Times of Neoliberal Change: An Ethnographic Study of Nurses' Experiences of Work Intensification

    • The aim of this article is to analyze how nurses respond to and act upon neoliberal transformations of work. The article identifies and explores those transformations considered by my informants, nurses working in public sector hospital wards, as central to changing conditions of work and experiences of work intensifications. It further analyzes nurses’ responses toward these transformations and locates these responses within a particular form of femininity evolving from rationalities of care, the working conditions within the organization, and experiences of care work, studied here through a class and gender informed analysis of care work and work intensifications. By using ethnographic data to explore issues of work intensification in the public sector, the article contributes to a deepened understanding of effects of welfare state transformation on care workers, as well as continuities and change in processes of exploitation and worker adaptation involved in the everyday production of welfare services. The article is organized as follows. First, I will describe changes in the Swedish healthcare sector, focusing on New Public Management (NPM). In the succeeding section I outline the analytical frame of the article, focusing on gendered aspects of the conditions of labor processes in public sector care work. I then go on to describe the methodology. The remaining part of the article is an analysis based on an empirical study of ward nurses and their experiences of changing working conditions in a Swedish hospital.

    • Svenska genusforskare som lägger tonvikt vid klass börjar ofta med det kritiska konstaterandet att klass och klassrelaterade frågor varit frånvarande från senaste årens genusvetenskapliga forskningsfront. Däremot kopplas sällan detta argument till det teoretiska skifte, från marxistiska till poststrukturalistiska utgångspunkter, som genusforskningen genomgått sedan 1980-talet, och som fått konsekvenser för hur och i vilka sammanhang klass analyseras. Syftet med denna artikel är att diskutera hur klass kan gestalta sig i nyare genusvetenskapliga produktioner som utforskar klass och identitet, och problematisera några av de former genom vilka klass teoretiserats inom ramen för den poststrukturalistiska traditionen. Utgångspunkten är att kritiskt granska det sätt på vilket denna specifika relation av ojämlikhet ”återinförts” i den svenska genusvetenskapliga och poststrukturalistiskt influerade forskningen.Problemställningen är aktuell inte minst i ljuset av diskussionen kring begreppet intersektionalitet, som kommit att engagera många forskare både i Sverige och internationellt. Det har beskrivits som ”det hittills viktigaste teoretiska bidraget från genusvetenskapen och de närliggande forskningsfälten” (McCall 2005:31). Frågorna man har ställt sig har handlat om hur man ska lokalisera skärningspunkten mellan olika typer av maktordningar, och hur dessa skärningspunkter ska konceptualiseras på individ- och på institutionell nivå. Artikeln ska ses som ett bidrag till den debatten, men argumentet som lyfts fram här handlar om vikten av att i diskussionen kring klass, som en av de relationer som med nödvändighet måste finnas med i ett analytiskt användbart intersektionalitetsbegrepp, relatera till frågor om arbetsprocessen som den ser ut inom det ekonomiska systemet.
