  • Outcomes after perioperative SARS-CoV-2 infection in patients with proximal femoral fractures: an international cohort study

    • Studies have demonstrated high rates of mortality in people with proximal femoral fracture and SARS-CoV-2, but there is limited published data on the factors that influence mortality for clinicians to make informed treatment decisions. This study aims to report the 30-day mortality associated with perioperative infection of patients undergoing surgery for proximal femoral fractures and to examine the factors that influence mortality in a multivariate analysis.SettingProspective, international, multicentre, observational cohort study.ParticipantsPatients undergoing any operation for a proximal femoral fracture from 1 February to 30 April 2020 and with perioperative SARS-CoV-2 infection (either 7 days prior or 30-day postoperative).Primary outcome30-day mortality. Multivariate modelling was performed to identify factors associated with 30-day mortality.ResultsThis study reports included 1063 patients from 174 hospitals in 19 countries. Overall 30-day mortality was 29.4% (313/1063). In an adjusted model, 30-day mortality was associated with male gender (OR 2.29, 95% CI 1.68 to 3.13, p<0.001), age >80 years (OR 1.60, 95% CI 1.1 to 2.31, p=0.013), preoperative diagnosis of dementia (OR 1.57, 95% CI 1.15 to 2.16, p=0.005), kidney disease (OR 1.73, 95% CI 1.18 to 2.55, p=0.005) and congestive heart failure (OR 1.62, 95% CI 1.06 to 2.48, p=0.025). Mortality at 30 days was lower in patients with a preoperative diagnosis of SARS-CoV-2 (OR 0.6, 95% CI 0.6 (0.42 to 0.85), p=0.004). There was no difference in mortality in patients with an increase to delay in surgery (p=0.220) or type of anaesthetic given (p=0.787).ConclusionsPatients undergoing surgery for a proximal femoral fracture with a perioperative infection of SARS-CoV-2 have a high rate of mortality. This study would support the need for providing these patients with individualised medical and anaesthetic care, including medical optimisation before theatre. Careful preoperative counselling is needed for those with a proximal femoral fracture and SARS-CoV-2, especially those in the highest risk groups.Trial registration numberNCT04323644

    • Nanoparticles (NPs) are new inspiring clinical targets that have emerged from persistent efforts with unique properties and diverse applications. However, the main methods currently utilized in their production are not environmentally friendly. With the aim of promoting a green approach for the synthesis of NPs, this review describes eco-friendly methods for the preparation of biogenic NPs and the known mechanisms for their biosynthesis. Natural plant extracts contain many different secondary metabolites and biomolecules, including flavonoids, alkaloids, terpenoids, phenolic compounds and enzymes. Secondary metabolites can enable the reduction of metal ions to NPs in eco-friendly one-step synthetic processes. Moreover, the green synthesis of NPs using plant extracts often obviates the need for stabilizing and capping agents and yields biologically active shape- and size-dependent products. Herein, we review the formation of metallic NPs induced by natural extracts and list the plant extracts used in the synthesis of NPs. In addition, the use of bacterial and fungal extracts in the synthesis of NPs is highlighted, and the parameters that influence the rate of particle production, size, and morphology are discussed. Finally, the importance and uniqueness of NP-based products are illustrated, and their commercial applications in various fields are briefly featured.

  • Synthesis of Heart/Dumbbell-Like CuO Functional Nanostructures for the Development of Uric Acid Biosensor

    • It is always demanded to prepare a nanostructured material with prominent functional properties for the development of a new generation of devices. This study is focused on the synthesis of heart/dumbbell-like CuO nanostructures using a low-temperature aqueous chemical growth method with vitamin B-12 as a soft template and growth directing agent. CuO nanostructures are characterized by scanning electron microscopy (SEM), X-ray diffraction (XRD), and X-ray photoelectron spectroscopy (XPS) techniques. CuO nanostructures are heart/dumbbell like in shape, exhibit high crystalline quality as demonstrated by XRD, and have no impurity as confirmed by XPS. Apparently, CuO material seems to be porous in structure, which can easily carry large amount of enzyme molecules, thus enhanced performance is shown for the determination of uric acid. The working linear range of the biosensor is 0.001 mM to 10 mM with a detection limit of 0.0005 mM and a sensitivity of 61.88 mV/decade. The presented uric acid biosensor is highly stable, repeatable, and reproducible. The analytical practicality of the proposed uric acid biosensor is also monitored. The fabrication methodology is inexpensive, simple, and scalable, which ensures the capitalization of the developed uric acid biosensor for commercialization. Also, CuO material can be used for various applications such as solar cells, lithium ion batteries, and supercapacitors.

    • Breast cancer is one of the most common cancer types among women, and it is a deadly disease caused by the uncontrolled proliferation of cells. Pathologists face a challenging issue of mitotic cell identification and counting during manual detection and identification of cancer. Artificial intelligence can help the medical professional with early, quick, and accurate diagnosis of breast cancer. Consequently, the survival rate will be improved and mortality rate will be decreased. Different deep learning techniques are used in computational pathology for cancer diagnosis. In this study, the SMDetector is proposed which is a deep learning model for detecting small objects such as mitotic and non-mitotic nuclei. This model employs dilated layers in the backbone to prevent small objects from disappearing in the deep layers. The purpose of the dilated layers in this model is to reduce the size gap between the image and the objects it contains. Region proposal network is optimized to accurately identify small objects. The proposed model yielded overall average precision (AP) of 50.31% and average recall (AR) of 55.90% that outperforms the existing standard object detection models on ICPR 2014 (Mitos-Atypia-14) dataset. To best of our knowledge the proposed model is state-of-the-art model for precision and recall of mitotic object detection on ICPR 2014 (Mitos-Atypia-14) dataset. The proposed model has achieved average precision for mitotic nuclei 68.49%, average recall for mitotic nuclei 59.86% and f-measure 63.88%. © 2022 The Authors

    • In this paper, a compact, geometrically simple, dual band and high gain coplanar waveguide (CPW) fed antenna for WLAN and X-band is proposed. The antenna is designed on Rogers RT/Duroid 5880 substrate with overall size of 25 mm x 33 mm x 0.79 mm. The proposed antenna consists of circular and rectangular stubs with one rectangular slot. The stubs and slots are introduced in order to achieve the desired bandwidth and high gain. The proposed antenna is designed and analyzed using the electromagnetic (EM) tool HFSS. Moreover, the proposed antenna design is compared to previously published work, and the comparison shows that our proposed antenna is a good candidate for radar applications.

    • Although numerous works have been done, most of the studies in fault diagnosis are limited to single fault type at a time. Majority of the works reported in the literature do not extend the diagnosis of the root cause of the fault for simultaneous faults specifically in the distillation column. However, an industrial system is susceptible to more than one fault at a time, which may or may not be interrelated. These faults not only reduce the diagnosis performance but also increase the computational complexity of the diagnosis algorithm. In this work, therefore, a multiple kernel support vector machine (MK-SVM) algorithm is proposed to diagnose simultaneous faults in the distillation column. In the developed MK-SVM algorithm, multilabel approach based on various kernel functions has been utilized for the classification of simultaneous faults. Dynamic simulation of a pilot-scale distillation column using Aspen Plus(R) is used for generating data in normal and faulty operation. Eight different fault types are considered, including valve sticking at reflux and reboiler, tray upsets, loss of feed flow, feed composition, and feed temperature changes. In the classification of simultaneous faults, a combination of two, three, and four faults is introduced for the performance evaluation of the proposed MK-SVM algorithm. The result showed that the proposed MK-SVM has a high fault detection rate (FDR) of 99.51% and a very low misclassification rate (MR) of 0.49%. The MK-SVM-based classification is better with the F1 score of >97% for all combinations of faults. Moreover, it is observed that the proposed MK-SVM shows better fault diagnosis for single, multiple, and simultaneous faults as compared to other established machine-learning algorithms.

    • The development of a PEG-decorated lipid-polymer hybrid system camouflaged with natural and synthetic chemotherapeutic moieties is an influential approach melding the biomimetic properties of long-circulating vesicles to utilize different mechanisms to dwindle the tumor growth. Therefore, a safe and efficient lipid-coated nano-particulate system (LCNPs) was proposed to investigate the in-vitro, ex-vivo and in-vivo demeanors of such amalgamation.Docetaxel loaded PLGA nanoparticles (DTX-NPs) were prepared by solvent evaporation while curcumin liposomes were mapped out using the film hydration method. Physicochemical characterizations were executed in terms of size, surface morphology, differential scanning calorimetry (DSC) and fourier-transform infrared spectroscopy (FTIR). In-vitro cytotoxicity was effectuated using MCF-7 cell line. Hemolysis, erythrocyte aggregation and acute in-vivo toxicity were carried out to establish the biocompatibility. The hydrodynamic diameters of samples were in the nano-range and corresponded to the findings of scanning electron microscopy (SEM) and atomic force microscopy (AFM). The absence of distinctive peaks of DTX-NPs in FTIR and DSC analysis of LCNPs depicts the shielding of the lipid bilayer over the nanoparticle. Cytotoxicity induced by the LCNPs represented the efficient delivery of cargo to the tumor cells. LCNPs also exhibited the least toxicity under ex-vivo and in-vivo circumstances compared to free drugs. Additionally, histological studies showed no evidence of substantial necrosis. Additionally, histological studies showed no evidence of notable necrosis.

    • Backscatter communication has recently emerged as a promising technology to enable passive sensing-based Internet-of-things (IoT) applications. In a backscatter communication network, uplink transmissions of multiple nodes are usually multiplexed in time- or frequency-domain to avoid collisions, yet it is desirable to improve the uplink capacity further. In this paper, we study a wireless-powered backscatter communication system, where the sensors use a hybrid channel access scheme by combining time division multiplexing access(TDMA) with power-domain non-orthogonal multiple access(PD-NOMA) to enhance the system performance in terms of outage probability and throughput. Our analysis shows that the proposed PD-NOMA increases both the spectrum efficiency and the throughput of the system.
