  • Impact of SARS-CoV-2 Pandemic on Patients with Primary Immunodeficiency

    • Although it is estimated that COVID-19 life-threatening conditions may be diagnosed in less than 1:1000 infected individuals below the age of 50, but the real impact of this pandemic on pediatric patients with different types of primary immunodeficiency (PID) is not elucidated. The current prospective study on a national registry of PID patients showed that with only 1.23 folds higher incidence of infections, these patients present a 10-folds higher mortality rate compared to population mainly in patients with combined immunodeficiency and immune dysregulation. Therefore, further management modalities against COVID-19 should be considered to improve the survival rate in these two PID entities using hematopoietic stem cell transplantation and immunomodulatory agents.

    • Genetic defects in the development, maturation, and/or function of the immune cells can lead to Inborn errors of immunity (IEI) which may predispose patients to malignancies. The overall risk for cancer in children with IEI ranges from 4 to 25% and the type of malignancy is highly dependent on the specific mutant gene underlying IEI. We investigated 3056 IEI patients registered in the Iranian national registry between the years 1999 and 2020 in this retrospective cohort study. The frequency of malignancy and its association with the type of IEI in these patients were evaluated. A total of 82 IEI patients with malignancy were enrolled in this study. Among them, predominantly lymphoma was the most common type of malignancy (67.1%), followed by leukemia (11%), and cancers of the head and neck (7.3%). Among identified lymphoma cancers, non-Hodgkin’s lymphomas were the most frequent type (43.9%) followed by different subtypes of Hodgkin’s lymphoma (23.2%). Solid tumors (18.3%) appeared to be very heterogeneous by type and localization. The correlation between the type of malignancy and survival status and the association between the type of malignancy and IEI entities were unremarkable. The awareness of the association between the presence of IEI and cancer highlights the importance of a synergistic effort by oncologists and immunologists in the early diagnosis of malignancy and personalized therapeutic strategies in IEI patients.

  • Immunization with HER2 extracellular subdomain proteins induces cellular response and tumor growth inhibition in mice

    • Aim: We investigated cellular and protective immune responses in mice vaccinated with recombinant HER2 extracellular subdomains. Materials & methods: Balb/C mice were immunized with recombinant full HER2 extracellular domain and subdomain proteins. Humoral and cellular immune response and antitumor effect was evaluated using a syngeneic mice tumor model. Results: All recombinant proteins induced secretion of IL-4 and particularly IFN-γ and IL-17 cytokines. Challenging of immunized mice with stable 4T1-HER2 transfected cells resulted in partial but significant tumor growth inhibition in all groups of mice particularly those immunized with fHER2-ECD together with CPG. Conclusion: Our results suggest that the recombinant HER2-ECD subdomains induce mainly Th1 and Th17 responses, which seem to contribute to tumor growth inhibition in syngeneic mice.

    • Background:HER2 proto-oncogene is critical in the biology of breast cancer and an important therapeutic target of monoclonal antibodies (mAbs). We have recently established a panel of anti-HER2 mAbs recognizing different epitopes within the extracellular domain of HER2. Materials & methods: In the present study the antiproliferative effect of these mAbs was investigated on HER2-overexpressing human breast cancer cell line BT474, using radioactive thymidine incorporation assay. Results: Our results demonstrated that while two of the mAbs (1T0 and 2A8) inhibited cell proliferation dose dependently, similar to trastuzumab, six mAbs (1F2, 1B5, 1H9, 4C7, 1H6 and 2A9) augmented cell proliferation. Treatment of BT474 cells with different combinations of two mAbs induced either synergistic inhibitory or stimulatory effects. Discussion: Our findings indicate that combination of some stimulatory mAbs could completely abolish the inhibitory effect of other mAbs against HER2. Employment of some combinations of mAbs with significant synergistic inhibitory function may improve the therapeutic efficacy of HER2-specific mAbs.

  • On the relay-fallback tradeoff in millimeter wave wireless system

    • Millimeter wave (mmWave) communications systems are promising candidate to support extremely high data rate services in future wireless networks. MmWave communications exhibit high penetration loss (blockage) and require directional transmissions to compensate for severe channel attenuations and for high noise powers. When blockage occurs, there are at least two simple prominent options: 1) switching to the conventional microwave frequencies (fallback option) and 2) using an alternative non-blocked path (relay option). However, currently it is not clear under which conditions and network parameters one option is better than the other. To investigate the performance of the two options, this paper proposes a novel blockage model that allows deriving maximum achievable throughput and delay performance of both options. A simple criterion to decide which option should be taken under which network condition is provided. By a comprehensive performance analysis, it is shown that the right option depends on the payload size, beam training overhead, and blockage probability. For a network with light traffic and low probability of blockage in the direct link, the fallback option is throughput- and delay-optimal. For a network with heavy traffic demands and semistatic topology (low beam-training overhead), the relay option is preferable.

    • Many use cases in the Internet of Things (IoT) will require or benefit from location information, making positioning a vital dimension of the IoT. The 3GPP has dedicated a significant effort during its Release 14 to enhance positioning support for its IoT technologies to further improve the 3GPPbased IoT eco-system. In this article, we identify the design challenges of positioning support in LTE-M and NB-IoT, and overview the 3GPP's work in enhancing the positioning support for LTE-M and NB-IoT. We focus on OTDOA, which is a downlink based positioning method. We provide an overview of the OTDOA architecture and protocols, summarize the designs of OTDOA positioning reference signals, and present simulation results to illustrate the positioning performance.
