  • Overweight and obesity impair left ventricular systolic function as measured by left ventricular ejection fraction and global longitudinal strain

    • AimsObesity is associated with type 2 diabetes mellitus, left ventricular diastolic dysfunction and heart failure but it is unclear to which extent it is related to left ventricular systolic dysfunction. The aim of the study was to explore the effects of overweight and obesity on left ventricular systolic function in patients with type 2 diabetes mellitus and a control group of non-diabetic persons.MethodsWe prospectively investigated 384 patients with type 2 diabetes mellitus, and 184 controls who participated in the CARDIPP and CAREFUL studies. The participants were grouped according to body mass index (normal weight < 25 kg/m2, overweight 25–29 kg/m2, and obesity ≥ 30 kg/m2). Echocardiography was performed at the beginning of the study and after 4-years in the patient group.ResultsUnivariable and multivariable regression analysis revealed that variations in left ventricular ejection fraction, global longitudinal strain, left ventricular mass and diastolic function expressed as E/é (the ratio between early diastolic mitral flow and annular motion velocities) all are related to body mass index. The mean and standard deviation of left ventricular ejection fraction and global longitudinal strain values were 57% (8%) vs. − 18.6% (2.3%) for normal weight patients, 53% (8%) vs. − 17.5% (2.3%) for overweight, and 49% (9%) vs. − 16.2% (3.0%) for obese (p < 0.05 vs. p < 0.05). Corresponding results in the control group were 58% (6%) vs. − 22.3% (3.0%), 55% (7%) vs. − 20.8% (3.1%) and 54% (8%) − 19.6% (4.0%) (p < 0.05 vs. p < 0.05). Patients who gained weight from baseline to follow-up changed left ventricular ejection fraction (median and interquartile range) by − 1.0 (9.0) % (n = 187) and patients who lost weight changed left ventricular ejection fraction by 1.0 (10.0) % (n = 179) (p < 0.05).ConclusionOverweight and obesity impair left ventricular ejection fraction and global longitudinal strain in both patients with type 2 diabetes mellitus and non-diabetic persons.

    • Background: Progress in cancer biomarker discovery is dependent on access to high-quality biological materials and high-resolution clinical data from the same cases. To overcome current limitations, a systematic prospective longitudinal sampling of multidisciplinary clinical data, blood and tissue from cancer patients was therefore initiated in 2010 by Uppsala and Umeå Universities and involving their corresponding University Hospitals, which are referral centers for one third of the Swedish population.Material and Methods: Patients with cancer of selected types who are treated at one of the participating hospitals are eligible for inclusion. The healthcare-integrated sampling scheme encompasses clinical data, questionnaires, blood, fresh frozen and formalin-fixed paraffin-embedded tissue specimens, diagnostic slides and radiology bioimaging data.Results: In this ongoing effort, 12,265 patients with brain tumors, breast cancers, colorectal cancers, gynecological cancers, hematological malignancies, lung cancers, neuroendocrine tumors or prostate cancers have been included until the end of 2016. From the 6914 patients included during the first five years, 98% were sampled for blood at diagnosis, 83% had paraffin-embedded and 58% had fresh frozen tissues collected. For Uppsala County, 55% of all cancer patients were included in the cohort.Conclusions: Close collaboration between participating hospitals and universities enabled prospective, longitudinal biobanking of blood and tissues and collection of multidisciplinary clinical data from cancer patients in the U-CAN cohort. Here, we summarize the first five years of operations, present U-CAN as a highly valuable cohort that will contribute to enhanced cancer research and describe the procedures to access samples and data.

  • A five-axis parallel kinematic mirror unit for soft X-ray beamlines at MAX IV

    • With the introduction of the multi-bend achromats in the new fourth-generation storage rings the emittance has decreased by an order of magnitude resulting in increased brightness. However, the higher brightness comes with smaller beam sizes and narrower radiation cones. As a consequence, the requirements on mechanical stability regarding the beamline components increases. Here an innovative five-axis parallel kinematic mirror unit for use with soft X-ray beamlines using off-axis grazing-incidence optics is presented. Using simulations and measurements from the HIPPIE beamline at the MAX IV Laboratory it is shown that it has no Eigen frequencies below 90 Hz. Its positioning accuracy is better than 25 nm linearly and 17-35 mu rad angularly depending on the mirror chamber dimensions.

    • A report on a four-axis ultra-high-stability manipulator developed for use at the Veritas and Species RIXS beamlines at MAX IV Laboratory, Lund, Sweden, is presented. The manipulator consists of a compact, light-weight X-Y table with a stiffened Z tower carrying a platform with a rotary seal to which a manipulator rod holding the sample can be attached. Its design parameters have been optimized to achieve high eigen-frequencies via a light-weight yet stiff construction, to absorb forces without deformations, provide a low center of gravity, and have a compact footprint without compromising access to the manipulator rod. The manipulator system can house a multitude of different, easily exchangeable, manipulator rods that can be tailor-made for specific experimental requirements without having to rebuild the entire sample positioning system. It is shown that the manipulator has its lowest eigen-frequency at 48.5 Hz and that long-term stability is in the few tens of nanometres. Position accuracy is shown to be better than 100 nm. Angular accuracy is in the 500 nrad range with a long-term stability of a few hundred nanoradians.

    • The focus in this thesis is young peoples’ (aged 17–20) experiences of leaving out-of-home care (OHC) and making the transition into adulthood. Prior research shows care leavers as a vulnerable group making the transition to adulthood at younger age, in shorter duration and with less support than their peers. The overarching aim is to gain knowledge of how young care leavers experience and handle their transition from OHC to adulthood, and how their experiences and strategies change over time. The study design is longitudinal with three waves of interviews: when still in care (T1), 6–11 months later when most of them had left care (T2) and another 10–19 months later (T3). Article I focuses the informants’ (n=15) expectations for their future and how these are changing over time (T1–T2). The participants’ short term expectations are characterized by worries (T1) and ambivalence (T2) – their long-term expectations are more optimistic (both T1 and T2) and tend to be guided by normative developmental patterns. The aim of article II is to study care leavers’ (n=20) strategies for handling adversities during their OHC transition. The results show that the participants over time (T1–T2) develop externally oriented strategies by navigating towards available resources, and internally oriented reflexive strategies for re-negotiating the meaning of their earlier experiences. Departing from an agency perspective article III focuses care leavers’ (n=14) transitional patterns of leaving care (T1–T3). Three patterns are identified: one stable long-term future oriented, one unstable short-term future oriented and one ambivalent pattern shuttling between long- and short-term future orientations. The aim in article IV is to study occupational trajectories, i.e. care leavers’ (n=14) paths into education and employment from a theoretical framework of agency vs. structure. The results show three ideal types of trajectories where agency is: (1) facilitated by structure, (2) perceived as free from structural constraints, and (3) hindered by structural constraints. The longitudinal design provides an original contribution the field of study by uncovering how care leavers’ expectations for their future is changing during the process of transition, how increasingly successful strategies are developed over time, and how transitional OHC patterns are influenced by the agents’ time horizons as well as by structural forces. A conclusion from the study is that societal support targeting young care leavers is deficient and needs to be developed and strengthened. Furthermore, the transition could be facilitated by extending the duration of the transition process, by including care leavers as active participating agents in the planning process of their passage out from OHC, and by strengthening the maintenance of care leavers’ relationships to supportive members in their formal and informal network. ”Independence” as the ultimate goal for young people leaving OHC is criticized based on the results showing that interdependent relationships to significant others is an integrated part of care leavers’ perception of adult life.

  • Teaming up or struggling alone from care to occupation – A study of young Swedish care leavers’ occupational trajectories

    • Employment is crucial for independence and integration into society, but few studies have focused on care leavers’ own experiences of the transition to working life. This study presents longitudinal data on 14 young Swedish care leavers’ occupational trajectories from out-of-home care (OHC) towards working life. The first interview (T1) was conducted when the respondents were still in OHC but on the verge of leaving care, the second (T2) when the majority had left care, and the third (T3) when they had been out of care for some time. The aim was to study occupational trajectories from out-of-home care among young Swedish care leavers. What are the main features of their trajectories? How do they handle this transition? The thematic analysis generated three different occupational trajectories, recognized as “teamwork,” “lone wolf,” and “tug-of-war.” The theoretical concepts of structure and agency were used to understand these trajectories over time. “Agency enabled by structure” outlined the teamwork trajectory and seemed to imply a successful pathway towards working life. The lone wolf trajectory represented “agency perceived as free from structural constraint” and meant a successful “self-made” trajectory, however, at the expense of vulnerability, and the tug-of-war trajectory represented “agency hindered by structural constraint” as well as individual adversities.

  • Transitional patterns when leaving care – Care leavers’ agency in a longitudinal perspective

    • Based on three waves of semi-structured interviews, this longitudinal qualitative study aims to understand the transition from out-of-home care (OHC; foster care, residential care) to independent adulthood, for a group of Swedish care leavers aged 16–20 years (n = 14). What are the main patterns in their transition process? How is agency reflected in these processes? The first interview (T1) was conducted when they were all still in OHC but on the verge of leaving, the second (T2) when the majority had left care and the third (T3) when they had been out for some time. Average time from T1 to T3 was 21 months, ranging from 17 to 28 months. By directed content analysis and an abductive approach, three transitional patterns were identified: From care to societal insiders, From care to societal outsiders and From care to societal in-betweeners. These patterns were connected to different dimensions of agency. Agency with long-term goals was shown to be connected to a stable transition from care, facilitating the emerging identification as independent “insiders” of society. Agency oriented towards the short term instead seemed to be connected to unstable transitions with a growing perception of being left as “outsiders” of society. Shuttling between those agentic positions, trying to apply long-term agency but experiencing the need for instant and pragmatic decisions, implied a halted transition “in-between” OHC and independent adulthood.

    • AbstractThis qualitative longitudinal study of 20 young Swedish care leavers investigates their subjective experience of and strategies for handling adversities when being in the process of leaving out-of-home care. The empirical data is based on two sets of interviews, the first conducted at time 1 (T1) when they were still in care but the moving out process had begun, the second (T2) 6-10 months later when the vast majority had left care. The thematic analysis based on resilience theory showed that the majority of the informants over time developed process-oriented strategies, which in our categorization emanated either from the inner world of the informants (e.g. through re-framing of experiences and an emerging self-reliance) or from their outer contextual world (e.g. through a restructuring of the social network). The results are discussed from a resilience theoretical perspective in which the informants' strategies are illustrated by the conceptual pair of 'navigation' and 'negotiation', used to make sense of their inner and outer world-oriented strategies.

  • Young care leavers’ expectations of their future : A question of time horizon

    • This paper investigates young care leavers’ expectations of their future after discharge from care. The results are based on qualitative longitudinal data where 16- to 21-year-old care leavers (n = 15) were interviewed twice, first when still in care but planning for their discharge (T1) and the second time 6–9 months later (T2). The analysis using a general inductive approach showed that their expectations were dependent on the time horizon and that there was an obvious difference between the young informants’ short- and long-term expectations. Their short-term expectations consisted of worries connected to their approaching discharge (at T1) and how to cope with challenges of everyday life after discharge from care (at T2). These results seem to echo negative outcomes shown in previous quantitative research. However, the informants’ long-term expectations provide a different picture, being mainly positive in both interviews (T1 and T2). The results are discussed from a life course perspective, where the informants’ visions of their future are framed and understood in terms of the different stages of their transition process.

  • Prevalence of Dental Caries in Past European Populations: A Systematic Review

    • Objective: Within the fields of anthropology and osteoarcheology, human teeth have long been studied to understand the diet, habits, and diseases of past civilizations. However, no complete review has been published to collect and analyze the extensive available data on caries prevalence in European man (Homo sapiens) over time. Method: In this current study, the two databases, Scopus and Art, Design, and Architecture Collection, were searched using predefined search terms. The literature was systematically reviewed and assessed by two of the authors. Results: The findings include a significant nonlinear correlation with increasing caries prevalence in European populations from 9000 BC to 1850 AD, for both the number of carious teeth and the number of affected individuals. Conclusion: Despite the well-established collective belief that caries rates fluctuate between different locations and time and the general view that caries rates have increased from prehistoric times and onwards, this is to our knowledge the first time this relationship has been proven based on published data.

    • The tumor stroma is vital to tumor development, progression, and metastasis. Cancer-associated fibroblasts (CAF) are among the abundant cell types in the tumor stroma, but the range of their contributions to cancer pathogenicity has yet to be fully understood. Here, we report a critical role for upregulation of the TGF beta/BMP family member GDF15 (MIC-1) in tumor stroma. GDF15 was found upregulated in situ and in primary cultures of CAF from prostate cancer. Ectopic expression of GDF15 in fibroblasts produced prominent paracrine effects on prostate cancer cell migration, invasion, and tumor growth. Notably, GDF15-expressing fibroblasts exerted systemic in vivo effects on the outgrowth of distant and otherwise indolent prostate cancer cells. Our findings identify tumor stromal cells as a novel source of GDF15 in human prostate cancer and illustrate a systemic mechanism of cancer progression driven by the tumor microenvironment. Further, they provide a functional basis to understand GDF15 as a biomarker of poor prognosis and a candidate therapeutic target in prostate cancer.

  • The importance of understanding annual and shorter term temperature patterns and variation in the surface levels of polar soils for terrestrial biota

    • Ground temperatures in the top few centimetres of the soil profile are key in many biological processes yet remain very poorly documented, especially in the polar regions or over longer timescales. They can vary greatly seasonally and at various spatial scales across the often highly complex and heterogeneous polar landscapes. It is challenging and often impossible to extrapolate soil profile temperatures from meteorological air temperature records. Furthermore, despite the justifiably considerable profile given to contemporary large-scale climate change trends, with the exception of some sites on Greenland, few biological microclimate datasets exist that are of sufficient duration to allow robust linkage and comparison with these large-scale trends. However, it is also clear that the responses of the soil-associated biota of the polar regions to projected climate change cannot be adequately understood without improved knowledge of how landscape heterogeneity affects ground and sub-surface biological microclimates, and of descriptions of these microclimates and their patterns and trends at biologically relevant physical and temporal scales. To stimulate research and discussion in this field, we provide an overview of multi-annual temperature records from 20 High Arctic (Svalbard) and maritime Antarctic (Antarctic Peninsula and Scotia Arc) sites. We highlight important features in the datasets that are likely to have influence on biology in polar terrestrial ecosystems, including (a) summer ground and sub-surface temperatures vary much more than air temperatures; (b) winter ground temperatures are generally uncoupled from air temperatures; (c) the ground thawing period may be considerably shorter than that of positive air temperatures; (d) ground and air freeze–thaw patterns differ seasonally between Arctic and Antarctic; (e) rates of ground temperature change are generally low; (f) accumulated thermal sum in the ground usually greatly exceeds air cumulative degree days. The primary purpose of this article is to highlight the utility and biological relevance of such data, and to this end the full datasets are provided here to enable further analyses by the research community, and incorporation in future wider comparative studies.

    • Chemotherapy-induced mucositis is characterized by diarrhoea and villous atrophy. However, it is not well-understood why diarrhoea arises, why it only occurs with some chemotherapeutics and how it is related to villus atrophy. The objectives in this study were to determine (i) the relationship between chemotherapy-induced diarrhoea and villus atrophy and to (ii) establish and validate a rat diarrhoea model with clinically relevant endpoints. Male Wistar Han IGS rats were treated with saline, doxorubicin, idarubicin, methotrexate, 5-fluorouracil, irinotecan or 5-fluorouracil+irinotecan. After 72 h, jejunal tissue was taken for morphological, apoptotic and proliferative analyses, and faecal water content and change in body weight were determined. All treatments except methotrexate caused a similar reduction (≈42%) in villus height, but none of them altered mucosal crypt cell proliferation or apoptosis. Doxorubicin, idarubicin, irinotecan and 5-fluorouracil+irinotecan caused body weight reduction, but only irinotecan and idarubicin caused diarrhoea. No direct correlation between diarrhoea and villus height or body weight loss was observed. Therefore, studies of the mechanisms for chemotherapy-induced diarrhoea should focus on functional factors. Finally, the irinotecan and idarubicin diarrhoea models established in this study will be useful in developing supportive treatments of this common and serious adverse effect in patients undergoing chemotherapy.

    • There is a renewed interest from the pharmaceutical field to develop oral formulations of compounds, such as peptides, oligonucleotides, and polar drugs. However, these often suffer from insufficient absorption across the intestinal mucosal barrier. One approach to circumvent this problem is the use of absorption modifying excipient(s) (AME). This study determined the absorption enhancing effect of four AMEs (sodium dodecyl sulfate, caprate, chitosan, N-acetylcysteine) on five model compounds in a rat jejunal perfusion model. The aim was to correlate the model compound absorption to the blood-to-lumen clearance of the mucosal marker for barrier integrity, 51Cr-EDTA. Sodium dodecyl sulfate and chitosan increased the absorption of the low permeation compounds but had no effect on the high permeation compound, ketoprofen. Caprate and N-acetylcysteine did not affect the absorption of any of the model compounds. The increase in absorption of the model compounds was highly correlated to an increased blood-to-lumen clearance of 51Cr-EDTA, independent of the AME. Thus, 51Cr-EDTA could be used as a general, sensitive, and validated marker molecule for absorption enhancement when developing novel formulations.

  • Signet Ring Cell Colorectal and Appendiceal Cancer : A Small Signet Ring Cell Component Is Also Associated with Poor Outcome

    • Background: Colorectal signet ring cell (SRC) carcinoma with ≥50% SRCs (SRC ≥ 50) has a poor prognosis, but the prognostic role of SRCs < 50% (SRC < 50) is unclear. The aim of this study was to provide a clinicopathological characterization of SRC colorectal and appendiceal tumours and analyse the importance of the SRC component size.Methods: All patients in the Swedish Colorectal Cancer Registry diagnosed with colorectal or appendiceal cancer in 2009–2020 at Uppsala University Hospital, Sweden, were included. The SRCs were verified, and the components estimated by a gastrointestinal pathologist.Results: Of the 2229 colorectal cancers, 51 (2.3%) had SRCs, with a median component size of 30% (interquartile range of 12.5–40) and 10 (0.45%) had SRC ≥ 50. The SRC tumours were primarily localized in the right colon (59%) and appendix (16%). No patients with SRCs had stage I disease, and 26 (51%) had stage IV, of whom, 18 (69%) had peritoneal metastases. The SRC tumours were often high grade with perineural and vascular invasion. The 5-year overall survival (OS) rate for patients with SRC ≥ 50 were 20% (95% confidence interval (CI) 6–70), for SRC < 50, 39% (95% CI 24–61); and for non-SRCs, 55% (95% CI 55–60). Among the patients with SRC < 50 and <50% extracellular mucin, the 5-year OS was 34% (95% CI 19–61), while those with ≥50% extracellular mucin had an OS of 50% (95% CI 25–99). The 5-year recurrence-free survival rates were 51% (95% CI 13–83) for patients with SRC tumours, as compared to 83% (95% CI 77–89) and 81% (95% CI 79–84) for mucinous and non-mucinous adenocarcinoma, respectively.Conclusions: The presence of SRCs was strongly associated with aggressive clinicopathological features, peritoneal metastases, and poor prognosis, also when they make up <50% of a tumour.Simple SummarySignet ring cell (SRC) carcinoma of colorectal and appendiceal cancer is rare but is recognized as the histopathological subtype with the poorest prognosis. However, the prognostic relevance of a SRC component <50% is unclear. The aim of this study was to provide a clinicopathological characterization of all SRC-containing colorectal and appendiceal cancers, including those with <50% SRCs. The results showed that SRCs, both ≥50% and <50%, were associated with aggressive histopathological features, advanced stages, and, particularly, peritoneal metastases. Information about the presence of SRCs in tumour tissue, not only in the case of ≥50% SRCs, should be routinely registered in pathology reports and clinical registers to enable larger studies that can aid our understanding of SRCs in colorectal and appendiceal cancers.

It is therefore important to investigate these properties during the drug discovery process. The liver is of particular interest in ADME-Tox studies, as it is highly metabolically active and oral administrated drugs needs to pass the liver before reaching the systemic circulation. However, a dose of a drug that is efficacious and safe for one individual may be inefficacious or toxic, because of inter-individual variability. Therefore, it is important to investigate the ADME-Tox properties in a sufficiently large population. Investigations on ADME-Tox is usually done in in vitro cell models.  Therefore, a variety of models to simulate liver functions have been developed and ranging from subcellular microsomes to complex 3D organoid cultures. This thesis investigates variability of ADME proteins in human liver tissue and in liver cell models.First, mass spectrometry based targeted proteomics was used to quantify ADME relevant proteins from 149 human liver samples. The observed inter-individual protein variability could not solely be explained by genotype. Therefore, a single transporter protein, the bile and drug transporting protein, NTCP, was investigate in detail.  Non-genetic factors, e.g. smoking and alcohol consumption, and epigenetic factors such as DNA methylation, were found to contribute to the observed inter-individual variability of NTCP. Next, hepatocytes (PHH) were isolated from 54 human livers tissues and after which the hepatocytes where cryopreserved. The variable attachment efficiency of cryopreserved hepatocytes where investigated and an apoptosis inhibition protocol for restoration of attachment properties was developed. This protocol was also successfully applied to 3D cultured PHH spheroids resulting in increased ability to form 3D spheroids. The effect of culture conditions on the quality of the 3D cultures was also investigated.  3D PHH spheroids were formed and maintained in different, commonly used culture media. The spheroids were characterized by a variety of functional assays including global proteomics. The proteome analysis showed that while no epithelial to mesenchymal transitions was observed, 3D cultures maintained in fasting glucose and insulin levels resembling the in vivo situation showed a more liver-like phenotype with a high expression of ADME proteins and functional cytochrome P450 metabolism. Transporter kinetics were also investigated in the 3D cultured PHH. Finally, we investigated if global proteomics data from 56 human liver tissues could be deconvoluted to give information about the liver composition. The cell type proportions generated by deconvolution where similar to literature values. Liver samples that displayed deviating cell composition were identified. The deviating liver compositions were in agreement with clinical markers of inflammation in the patient´s blood samples and with altered extracellular matrix protein composition, comparable to that found in liver steatosis.  In conclusion, this thesis have investigated variability in ADME proteins in human liver and in in vitro cultures of human hepatocytes, characterized cofounding factors for in vitro cultured hepatocytes and further extended drug disposition studies in 3D cultured hepatocytes.

    • Background: The thiosemicarbazone CD 02750 (VLX50) was recently reported as a hit compound in a phenotype-based drug screen in primary cultures of patient tumor cells. We synthesized a copper complex of VLX50, denoted VLX60, and characterized its antitumor and mechanistic properties. Materials and Methods: The cytotoxic effects and mechanistic properties of VLX60 were investigated in monolayer cultures of multiple human cell lines, in tumor cells from patients, in a 3-D spheroid cell culture system and in vivo and were compared with those of VLX50. Results: VLX60 showed ?3-fold higher cytotoxic activity than VLX50 in 2-D cultures and, in contrast to VLX50, retained its activity in the presence of additional iron. VLX60 was effective against non-proliferative spheroids and against tumor xenografts in vivo in a murine model. In contrast to VLX50, gene expression analysis demonstrated that genes associated with oxidative stress were considerably enriched in cells exposed to VLX60 as was induction of reactive oxygen. VLX60 compromised the ubiquitin-proteasome system and was more active in BRAF mutated versus BRAF wild-type colon cancer cells. Conclusions: The cytotoxic effects of the copper thiosemicarbazone VLX60 differ from those of VLX50 and shows interesting features as a potential antitumor drug, notably against BRAF mutated colorectal cancer.

    • For decades, the standard procedure when screening for candidate anticancer drug combinations has been to search for synergy, defined as any positive deviation from trivial cases like when the drugs are regarded as diluted versions of each other (Loewe additivity), independent actions (Bliss independence), or no interaction terms in a response surface model (no interaction). Here, we show that this kind of conventional synergy analysis may be completely misleading when the goal is to detect if there is a promising in vitro therapeutic window. Motivated by this result, and the fact that a drug combination offering a promising therapeutic window seldom is interesting if one of its constituent drugs can provide the same window alone, the largely overlooked concept of therapeutic synergy (TS) is reintroduced. In vitro TS is said to occur when the largest therapeutic window obtained by the best drug combination cannot be achieved by any single drug within the concentration range studied. Using this definition of TS, we introduce a procedure that enables its use in modern massively parallel experiments supported by a statistical omnibus test for TS designed to avoid the multiple testing problem. Finally, we suggest how one may perform TS analysis, via computational predictions of the reference cell responses, when only the target cell responses are available. In conclusion, the conventional error-prone search for promising drug combinations may be improved by replacing conventional (toxicology-rooted) synergy analysis with an analysis focused on (clinically motivated) TS.

    • BACKGROUND: Identification of biomarkers associated with benefit of adjuvant chemotherapy in stage II/III colon cancer is an important task. METHODS: Vessel density (VD) and tumour stroma were analysed in a randomised-trial-derived discovery cohort (n = 312) and in a stage II/III group of a population-based validation cohort (n = 85). VD was scored separately in the tumour centre, invasive margin and peritumoral stroma compartments and quantitated as VD/total analysed tissue area or VD/stroma area. RESULTS: High stroma-normalised VD in the invasive margin was associated with significantly longer time to recurrence and overall survival (OS) (p = 0.002 and p = 0.006, respectively) in adjuvant-treated patients of the discovery cohort, but not in surgery-only patients. Stroma-normalised VD in the invasive margin and treatment effect were significantly associated according to a formal interaction test (p = 0.009). Similarly, in the validation cohort, high stroma-normalised VD was associated with OS in adjuvant-treated patients, although statistical significance was not reached (p = 0.051). CONCLUSION: Through the use of novel digitally scored vessel-density-related metrics, this exploratory study identifies stroma-normalised VD in the invasive margin as a candidate marker for benefit of adjuvant 5-FU-based chemotherapy in stage II/III colon cancer. The findings, indicating particular importance of vessels in the invasive margin, also suggest biological mechanisms for further exploration.

    • Intestinal mucosal barrier dysfunction caused by disease and/or chemotherapy lacks an effective treatment, which highlights a strong medical need. Our group has previously demonstrated the potential of melatonin and misoprostol to treat increases in intestinal mucosal permeability induced by 15-min luminal exposure to a surfactant, sodium dodecyl sulfate (SDS). However, it is not known which luminal melatonin and misoprostol concentrations are effective, and whether they are effective for a longer SDS exposure time. The objective of this single-pass intestinal perfusion study in rats was to investigate the concentration-dependent effect of melatonin and misoprostol on an increase in intestinal permeability induced by 60-min luminal SDS exposure. The cytoprotective effect was investigated by evaluating the intestinal clearance of Cr-51-labeled EDTA in response to luminal SDS as well as a histological evaluation of the exposed tissue. Melatonin at both 10 and 100 mu M reduced SDS-induced increase in permeability by 50%. Misoprostol at 1 and 10 mu M reduced the permeability by 50 and 75%, respectively. Combination of the two drugs at their respective highest concentrations had no additive protective effect. These in vivo results support further investigations of melatonin and misoprostol for oral treatments of a dysfunctional intestinal barrier.

    • The platelet-derived growth factors are implicated in development of fibrotic reactions and disease in several organs. We have overexpressed platelet-derived growth factor-C in the heart using the alpha-myosin heavy chain promoter and created a transgenic mouse that exhibits cardiac fibrosis followed by hypertrophy with sex-dependent phenotypes. The transgenic mice developed several pathological changes including cardiac fibroblast proliferation and deposition of collagen, hypertrophy, vascular defects, and the presence of Anitschkow cells in the adult myocardium. Male mice developed a hypertrophic phenotype, whereas female mice were more severely affected and developed dilated cardiomyopathy, leading to heart failure and sudden death. The vascular defects initially included dilation of microvessels and vascular leakage. Subsequently, a marked loss of microvessels, formation of large vascular sac-like structures, and an increased density of smooth muscle-coated vessels were observed in the myocardium. In part, the observed vascular changes may be because of an up-regulation of vascular endothelial growth factor in cardiac fibroblasts of the transgenic hearts. This unique animal model reveals that a potent mitogen for cardiac fibroblasts result in an expansion of the interstitium that induce a secondary sex-dependent hypertrophic response in the cardiomyocytes.

  • Exploiting loss of heterozygosity for allele-selective colorectal cancer chemotherapy

    • Allelic losses occurring in cancer cells have been suggested as potential targets for therapy. Here, the authors show how recurring loss of heterozygosity of a drug metabolic gene in colorectal cancers can be exploited using a low molecular weight compound. Cancer chemotherapy targeting frequent loss of heterozygosity events is an attractive concept, since tumor cells may lack enzymatic activities present in normal constitutional cells. To find exploitable targets, we map prevalent genetic polymorphisms to protein structures and identify 45 nsSNVs (non-synonymous small nucleotide variations) near the catalytic sites of 17 enzymes frequently lost in cancer. For proof of concept, we select the gastrointestinal drug metabolic enzyme NAT2 at 8p22, which is frequently lost in colorectal cancers and has a common variant with 10-fold reduced activity. Small molecule screening results in a cytotoxic kinase inhibitor that impairs growth of cells with slow NAT2 and decreases the growth of tumors with slow NAT2 by half as compared to those with wild-type NAT2. Most of the patient-derived CRC cells expressing slow NAT2 also show sensitivity to 6-(4-aminophenyl)-N-(3,4,5-trimethoxyphenyl)pyrazin-2-amine (APA) treatment. These findings indicate that the therapeutic index of anti-cancer drugs can be altered by bystander mutations affecting drug metabolic genes.

    • Earlier studies have reported on both proinflammatory and anti-inflammatory activities of cholera toxin (CT). As CT is a powerful adjuvant, we were interested in identifying genes with a possible involvement in these functions. A global gene expression analysis in mouse B cells showed that CT regulated <100 annotated genes, which encoded transcription factors, G proteins, cell-cycle regulators, and immunoregulating molecules. Interestingly, CT regulated the expression of the signal transducer and activator of transcription (STAT)3 gene and influenced the level and activation of both isoforms STAT3alpha and STAT3beta, in vitro in a B-cell line and in Peyer's patch (PP) B cells and in vivo in freshly isolated splenic B cells from CT-treated mice. This effect was cAMP dependent and was not seen with CTB. B cells pre-exposed to CT were significantly more susceptible to the activation of STAT3 by interleukin (IL)-6 and IL-10. This exerted a stronger inhibitory effect of IL-10 on lipopolysaccharide (LPS)-stimulated B-cell proliferation and cytokine production (IL-6). Moreover, IgG1 and IgA production induced by LPS and IL-10 were enhanced by the addition of CT to cultures of PP or splenic B cells. This is the first study to provide a molecular mechanism that can reconcile previous findings of proinflammatory and anti-inflammatory effects by CT adjuvant.Mucosal Immunology advance online publication 7 April 2010. doi:10.1038/mi.2010.16.

    • The difficulties encountered in matching an antenna to its optimal impedance are reduced with an adaptive 0.35-mu m CMOS circuit based on several switched shunt capacitors arranged in capacitor banks and on a few external series inductors. As high-quality inductors are difficult to obtain in CMOS, the inductors are placed either in an low-temperature cofired ceramic (LTCC) substrate or is a lumped component outside the core circuit. The circuits, presented here through a range of simulations, are optimized to function within the ISM 2.4-GHz band, but the general approach employed to improve matching can be used for other frequency bands as well. The circuits discussed provide a VSWR <= 2 match for every impedance with VSWR <= 5. There is a 1-dB power loss for a perfect 50 Omega -> 50 Omega transformation, a break-even point at VSWR = 1.5, and a 3-dB increase in delivered power for VSWR = 4.3.

    • Aim: To explore the feasibility of withdrawal of diabetes medication in elderly patients with HbA1c £ 6.0%.Methods: HbA1c was measured in 98 patients with known diabetes in 17 nursing homes in Sweden. 32 subjects with HbA1c £ 6.0% participated in the drug withdrawal study. After measuring plasma glucose on three consecutive days, diabetes drugs were reduced, i.e. complete withdrawal of oral anti-diabetic drugs (OADs), complete insulin withdrawal when doses were £ 20 units/day and reduced by half in patients on more than 20 units/day.Results: We identified 31 episodes of plasma glucose £ 4.4 mmol/l, most of them nocturnal (n=17). Mean HbA1c was 5.2 % ± 0.4 compared to 7.1 % ± 1.6 in the non-intervention group. Three months after the diabetes drug discontinuation, 24 patients (75%) remained in the intervention group and mean HbA1c was then 5.8 %. ± 0.9. Six months after baseline investigation mean HbA1c in the intervention group was 5.8 % ± 1.1 compared with 6.6 % ± 1.4 in the non-intervention group.Conclusions: Hypoglycaemic events are common among elderly patients with type 2 diabetes. The withdrawal of diabetes medication in elderly with tight glycaemic control is safe and may decrease the risk for hypoglycaemia.

    • The use of reconfigurable matching has become more and more desirable as mismatches between such interfaces as antenna and receiver increase. The present paper investigates reconfigurable matching by measurements of a CMOS Impedance Tuning Unit (ITU) designed for Digital Video Broadcasting for Handheld devices (DVB-H). The ITU consists of switched capacitors arranged in three capacitor banks, with inductors between them, thus creating a reconfigurable ladder matching network. It is demonstrated that any mismatch up to VSWR = 8 can be transformed to a mismatch better than VSWR = 2, that IIP2 = 7 dBm and IIP3 = 0.5 dBm, and that loss is 2.1 dB.

    • An adaptive matching network provides a number of states between which it can be reconfigured. At a certain frequency, the matching network transforms a reference impedance to different impedances for the different states. The circuit will be able to match those impedances perfectly to the reference impedance, and impedances close to those will also be fairly well matched. The matching will, however, decline for impedances further away and eventually become too low. By defining the level of acceptable matching, a boundary can be formed that identifies an area of impedances that are matched well enough. In the same manner, a boundary is possible to form for constant mismatch loss. It is shown that the boundaries are circular if plotted in a Smith chart and that the sizes of the circles depend on the location of $z_{L}$. With these results, it is then investigated how the impedances should be distributed to minimize the number of states while still achieving the matching performance and coverage.

    • A dual band antenna for two ISM bands at 2.4 GHz and 5 GHz has been evaluated. The antenna structure derives its advantage by deploying several radiators as fingers spreading from a single feeding point. Each radiator functions as both a half-wave and a quarter-wave radiator. Arranging them in parallel matches the half-wave resonance to 50 W. By carefully selecting the angle to ground plane and using mutual coupling, the quarter-wave resonance is also matched to 50 W. On average, the efficiency on the 2.4 GHz band is 71%, while on the 5 GHz band the efficiency is 75%. When placed close to a large shielding plane, performance is somewhat lowered, but still continues at a high level.

    • When implementing low frequency antennas in small devices, e.g., mobile phones using DVB-H, a large mismatch is inevitable. If a impedance tuning unit is introduced reflection losses and noise are reduced while gain is improved. A ITU consisting of 6 switched capacitances and 2 fixed LTCC inductors is studied. Simulations show that since a VSWR = 10 mismatch is reduced to better than VSWR = 2, the ITU increases the power delivered by 2.9 dB to 4.2 dB for an antenna mismatch between VSWR = 4 and VSWR = 8. In the same time, the average noise figure for VSWR = 4 is reduced by 1.4 dB from 4.6 dB to 3.2 dB; and for VSWR = 8 it is reduced by 2.8 dB from 6.4 dB to 3.7 dB. The highest noise figure for VSWR = 8 is reduced by 4.2 dB from 9.1 dB to 4.9 dB.

Slutsatsen är att det idag är lättare mekaniskt, men betydligt svårare elektriskt, att konstruera en mobiltelefonantenn.

    • The possibility of building compact, low-ohmic antennas suitable for mobile handsets is investigated. An F-antenna is built with a self-impedance of 10 Ω instead of the commonly used 50 Ω. Low impedance is chosen to minimize voltages in circuits prior to the antenna, in this case an adaptive matching network designed in CMOS or a similar process. To optimize this adaptive matching network, the antenna structure is two-fold: one antenna for transmission and one for reception. A resulting benefit is increased isolation between transmitter and receiver, as shown for a typical case. The 10 Ω antenna has an average efficiency of over 70% and an omnidirectional pattern; isolation increases by distance and reaches about 10dB at 4 cm

    • Switched capacitors are here investigated for use in reconfigurable matching networks, particularly for DVB-H frequencies. A $0.13$ $mu$m CMOS circuit is evaluated through both simulations and measurements. Source grounded NMOS transistors are used to switch high quality metal capacitors located above metal layer 8. The quality factor and tuning range depend on frequency, switch voltage, capacitor size, and transistor width. There is a clear trade-off between quality factor and tuning range, and measurements show quality factors above 50, 100, and 150 at tuning ranges of 3.9, 2.4, and 1.6, respectively. A reconfigurable matching network with the switched capacitors has been realized using external inductors and the measured matching domain for the DVB-H frequency band is shown. The total loss of the network is 1.0 dB, a result of the high quality switched capacitors.

    • Switched capacitors are here investigated for use in reconfigurable matching networks, particularly for DVB-H frequencies. A 0.13 um CMOS circuit is evaluated through both simulations and measurements. Source grounded NMOS transistors are used to switch high quality metal capacitors located above metal layer 8. The quality factor and tuning range depend on frequency, switch voltage, capacitor size, and transistor width.     • An Impedance Tuning Unit (ITU) has the ability to tune the impedance of for instance the antenna to better match another impedance. To effectively characterize a circuit containing an ITU, several functions in a measurement system need to be remotely controlled. In this paper such a measurement system is described. A line tune screw creates impedances for the ITU to match and a digital acquisition hardware (DAQ) controls the ITU. A LabVIEW interface is used to controll the DAQ and a network analyzer for measurements, as well as to collect, calculate and store data.

    • AIM: This study explored the association between reduced estimated glomerular filtration rate (eGFR) and microalbuminuria vs. subclinical organ damage in patients with type 2 diabetes.METHODS: Data from middle-aged patients with type 2 diabetes (n=706) treated in primary care were analyzed for microalbuminura, defined as a urinary albumin/creatinine ratio (uACR)≥3.0mmol/mol, and reduced eGFR, defined as<60mL/min/1.73m(2), in relation to blood pressure, pulse wave velocity (PWV), left ventricular mass index (LVMI), and carotid intima-media thickness (IMT) and lumen diameter (LD).RESULTS: Patients with microalbuminuria had significantly higher 24-h ambulatory systolic blood pressure (ASBP) compared with subjects with uACR<3mg/mmol: 137 vs. 128mmHg (P<0.001). There were no differences in ASBP in patients with eGFR<60mL/min/1.73m(2). However, patients with vs. without microalbuminuria had increased PWV (11.4 vs. 10.1m/s; P<0.001), LVMI (134.4 vs. 118.6g/m(2); P<0.001), LD (7.01±0.93 vs. 6.46±0.74mm; P<0.001) and IMT (0.78 vs. 0.74mm; P=0.047), respectively. The associations between uACR vs. PWV and LVMI were more robust after adjusting for age, diabetes duration, ASBP, HbA1c, LDL-cholesterol, and antihypertensive and lipid-lowering therapy compared with uACR vs. IMT. There were no statistically significant differences in PWV, LVMI or IMT between patients with reduced (<60mL/min/1.73m(2)) vs. normal eGFR.CONCLUSION: Levels of urinary albumin excretion, but not reduced eGFR, were associated with increased arterial stiffness, left ventricular mass and atherosclerosis in patients with type 2 diabetes.

    • At the MAX IV Laboratory, five new soft x-ray beamlines are under development. The first is Species and it will be used to develop and set the standard of the control system, which will be common across the facility. All motion axes at MAX IV will be motorized using stepper motors steered by the IcePAP motion controller and a mixture of absolute and incremental encoders following a predefined coordinate system. The control system software is built in Tango and uses the Python-based Sardana framework. The user controls the entire beamline through a synoptic overview and Sardana is used to run the scans.

    • Mobile phones become smaller but include more advanced functions for every new generation. This paradox is possible since reduced transistor size and increased integration of digital building blocks free space for new functions. Unfortunately, not all components are easily integrated or scaled, such as the antenna, since its performance is dependent on the size. Shrinking mobile phone size results in increased impedance mismatch, as the available antenna volume and the length of the ground plane are reduced. As a consequence, the performance of the whole mobile phone transceiver is degraded, since the noise increases and the output power decreases with increased mismatch. In this work, study of reconfigurable matching networks, and their improvement of receiver and transmitter performance with mismatched antennas, has been performed. In the general introduction, the matching network is introduced as an Impedance Tuning Unit (ITU) and the behaviour from the fundamental building blocks to system aspects are studied. Also, interaction with other building blocks in the receiver chain, such as low noise amplifier, mixer, and filter, is investigated. It is shown that noise performance and power transfer are improved by introducing an ITU, which has the ability to change its matching configuration. It is also shown that it is possible to realize an ITU for receivers in CMOS. Paper I and II examine antenna design and aspects such as shielding and coupling. Paper III is a parameter study of a switched capacitor. In paper IV, V, and VI ITU topologies are simulated and measured for 0.35 um and 0.13 um and for 2.4 to 2.5 GHz and 470 to 702 MHz. The results are presented together with system aspects relevant for receivers.

    • MAX IV is a fourth-generation, or diffraction-limited, synchrotron light source with a number of state-of-the-art beamlines. The performance of a beamline is, to a high degree, set by the energy resolution it can achieve, which in turn is governed to a large extent by the monochromator. During the design phase of a monochromator, the mechanical requirements must be fully understood and met with margin. During commissioning, the performance must be verified and optimized. In this paper, six soft X-ray monochromators at MAX IV beamlines (Bloch, Veritas, HIPPIE, SPECIES, FinEstBeAMS and SoftiMAX) are examined with a focus on their resolving power, energy range and the time required to change measurement range, as those parameters are dependent on each other. The monochromators have a modern commercial design, planned and developed in close collaboration with the vendors. This paper aims to present the current status of the commissioning at MAX IV with emphasis on elucidating the mechanical limitations on the performance of the monochromators. It contains analysis of the outcome and our approach to achieve fast and high-resolution monochromators.

    • ISP provides long-term support for the development of active and sustainable research environments within these basic sciences in developing countries. The support is directed to research groups and regional networks. The work is carried out in close cooperation with research groups at more advanced host institutions. Although Uppsala University is the base of the operation, ISP functions as a national programme and host laboratories may be at other Swedish universities, in other Nordic and European countries, and in the regions. This is to meet the requests from developing countries on their own terms.ISP funding is used for equipment, spare parts, consumables, technical services, literature, computer hard- and software, and costs associated with arranging and participating in meetings, exchange visits, training, etc. Besides what has been achieved in terms of development and maintenance of resources, contacts, etc., the results of ISP support in 2009 can be summarized based on the expenditures. For each MEUR spent (assuming that one EUR equals 10 SEK) the outcome was:19 PhD graduations,59 MSc/MPhil graduations,45 international publications,58 regional or national publicationst,113 conference contributions (34% of which international), and20 conferences/workshops etc. arranged.
