  • Oral health and dental care of older persons-A systematic map of systematic reviews

    • Objectives: To examine the current knowledge on oral health status and dental care of older persons through a systematic mapping of systematic reviews of low or moderate risk of bias. Background: Geriatric dentistry covers all aspects of oral health and oral care of older persons. Oral health is part of general health and contributes to a person's physical, psychological and social wellbeing. Methods: A literature search was performed in three different databases (PubMed, The Cochrane Library and Cinahl) within 12 domains: Dental caries, periodontitis, Orofacial pain and temporomandibular joint (TMJ) pain, mucosal lesions, oral motor function, dry mouth, halitosis, interaction between oral status and other medical conditions, ability to interrelate and communicate, quality of life, ethics and organisation of dental care for older persons. Systematic reviews were identified and scrutinised, highlighting scientific knowledge and knowledge gaps. Results: We included 32 systematic reviews of which 14 were judged to be of low/moderate risk of bias. Most of the domains lack systematic reviews with low or moderate risk of bias. In two of the domains evidence was identified; in institutionalised people aged 65 or older, effective oral hygiene can prevent pneumonia. Furthermore, there is an evidence of a relationship between malnutrition (protein energy-related malnutrition, PEM) and poor appetite and edentulousness. Conclusions: There is an urgent need for further research and evidence-based knowledge within most domains in geriatric dentistry and in other fields related to oral health and dental care for older persons striving for multi-disciplinary research programmes.

  • Consequences of eliminating adenosine A(1) receptors in mice

    • The second coding exon of the adenosine A, receptor gene was eliminated by homologous recombination. The phenotype of mice (mixed C57B6/129OlaHsd background) was studied, using siblings from matings of heterozygous mice. Among the offspring the ratio between+/+, +/-and -/-animals was 1:2:1. Over the first half-year-at least-growth and viability were the same in all genotypes. Binding of A(1) ligands was eliminated in-/-mice and halved in+/-mice. Blood pressure was increased in-/-mice and this was paralleled by an increase in plasma renin. Heart rate was unaffected, as was contractility. Furthermore, the response of the perfused heart to ischemia was similar in+/+and -/-hearts. However, remote preconditioning was eliminated in-/-mouse hearts. Tubuloglomerular feedback in the kidney was also lost in-/-mice. The analgesic response to a non-selective adenosing receptor agonist was lost in-/-mice, which also showed hyperalgesia in the tail-flick test. There was a slight hypoactivity in-/-mice, but responses to caffeine were essentially normal. The inhibition of excitatory neurotransmission in hippocampus by adenosine was lost in-/-mice and reduced in+/-mice. Responses to ATP were affected similarly. Hypoxic depression of synaptic transmission was essentially eliminated in hippocampus and hypoxic decrease in spinal respiratory neuron firing was markedly reduced. These results show that adenosine A, receptors play a physiologically important role in the kidney, spinal cord, and hippocampus and that they are critically important in the adaptive responses to hypoxia.     • Research on teachers’ knowledge and beliefs has grown big in recent years. Thelarger parts of these fields are built on acquisitionist interpretations of humanfunctioning. We explore the potentials of a participationist framework forunderstanding the role of the teacher for emerging classroom practices. Theframework is built on social practice theory and symbolic interactionism and adoptsa processual approach to understanding the role of the teacher. We use theframework in a qualitative study of two teachers with different prior experiences. Thestudy suggests that the framework has some potential and sheds light on the dynamicrelationships between the teacher’s engagement in the practices of the mathematicsclassroom and other, personally significant, past and present ones.

  • Home Care Encounters in a Multicultural Context - A Diverse Space for Caring

    • The aim of this study was to explore care encounters in homes of patients living in multicultural area. The shadowing method enabled researchers to closely follow nurses in their every-day work. Complexities in the care environment, an ambivalent role played by the family, and varying meanings of care encounters were found. Despite difficulties in communication, nurses and patients together created mutual space for caring. Regular meetings were essential for creation of trust, support and consolation. Nurses’ sensitivity regarding patient’s needs and awareness of diversity of space for caring gave insights into the complexity of home care encounters in multicultural contexts.

    • The aim of this study was to explore the meaning of experience for home care nurses in a multicultural area of Sweden. Interviews and group discussions with a team of five home care nurses were interpreted according to a hermeneutical perspective. The meaning was expressed in connection with the complexities of place, and space for care. Contradictions developed from diversities of perspectives incorporated in this specific multicultural area. Nurses saw themselves as mediators and allowed complexity to be considered in order to manage care. They took on responsibility of creating a ‘space of care’ while the organizational structures of home care were perceived as something outside the care praxis. Making contradictions evident render an intermediary caring strategy feasible.

    • With the aim to expose how situations of valued caring were described by the participants, written episodes were analyzed and interpreted. The value of a caring act in the specific situation was narrated as an image of what actually happened. The scene in which a caring act took place shaped the meaning of the act. Caring was connected to a diversity of acts designed to achieve different purposes. Comparison between the described episodes revealed that recovery, alleviation, security and comfort were most often seen as purposes of the caring acts. The all-encompassing motive was fulfillment of the participants desired change.
