    • The nucleosynthesis of elements beyond iron is dominated by neutron captures in the s and r processes. However, 32 stable, proton-rich isotopes cannot be formed during those processes, because they are shielded from the s-process flow and r-process beta-decay chains. These nuclei are attributed to the p and rp process. For all those processes, current research in nuclear astrophysics addresses the need for more precise reaction data involving radioactive isotopes. Depending on the particular reaction, direct or inverse kinematics, forward or time-reversed direction are investigated to determine or at least to constrain the desired reaction cross sections. The Facility for Antiproton and Ion Research (FAIR) will offer unique, unprecedented opportunities to investigate many of the important reactions. The high yield of radioactive isotopes, even far away from the valley of stability, allows the investigation of isotopes involved in processes as exotic as the r or rp processes.

  • Superdeformed and Triaxial States in Ca-42

    • Shape parameters of a weakly deformed ground-state band and highly deformed slightly triaxial sideband in Ca-42 were determined from E2 matrix elements measured in the first low-energy Coulomb excitation experiment performed with AGATA. The picture of two coexisting structures is well reproduced by new state-of-the-art large-scale shell model and beyond-mean-field calculations. Experimental evidence for superdeformation of the band built on 0(2)(+) has been obtained and the role of triaxiality in the A similar to 40 mass region is discussed. Furthermore, the potential of Coulomb excitation as a tool to study superdeformation has been demonstrated for the first time.

  • Shape evolution in the neutron-rich osmium isotopes : Prompt gamma-ray spectroscopy of Os-196

    • The shape transition in the neutron-rich Os isotopes is studied by investigating the neutron-rich Os-196 nucleus through in-beam gamma-ray spectroscopy using a two-proton transfer reaction from a Pt-198 target to a Se-82 beam. The beam-like recoils were detected and identified with the large-acceptance magnetic spectrometer PRISMA, and the coincident gamma rays were measured with the advanced gamma tracking array (AGATA) demonstrator. The de-excitation of the low-lying levels of the yrast-band of Os-196 were identified for the first time. The results are compared with state-of-the-art beyond-mean-field calculations, performed for the even-even Os188-198 isotopes. The new results suggest a smooth transition in the Os isotopes from a more axial rotational behavior towards predominately vibrational nuclei through triaxial configurations. An almost perfect gamma-unstable/triaxial rotor yrast band is predicted for Os-196 which is in agreement with the experimentally measured excited states.

  • Shell evolution beyond N=40 : Cu-69,Cu-71,Cu-73

    • The level structure of the neutron-rich Cu-69, Cu-71, and Cu-73 isotopes has been investigated by means of multinucleon transfer reactions. The experiment was performed at Laboratori Nazionali di Legnaro using the AGATA Demonstrator array coupled to the PRISMA magnetic spectrometer. Lifetimes of excited states in Cu nuclei were measured with the recoil-distance Doppler-shift method. The resulting electromagnetic matrix elements for transitions from excited states in Cu-69,Cu-71,Cu-73 nuclei are used to assess the collective or single-particle character of these states. The results are compared with predictions of large-scale shell-model calculations, giving further insight into the evolution of the proton pf shell as neutrons fill the 1g(9/2) orbital.

  • Spectroscopy of the neutron-rich actinide nucleus U-240 following multinucleon-transfer reactions

    • Background: Nuclear structure information for the neutron-rich actinide nuclei is important since it is the benchmark for theoretical models that provide predictions for the heaviest nuclei. Purpose: gamma-ray spectroscopy of neutron-rich heavy nuclei in the actinide region. Method: Multinucleon-transfer reactions in Zn-70 + U-238 and Xe-136 + U-238 have been measured in two experiments performed at the INFN Legnaro, Italy. In the Zn-70 experiment the high-resolution HPGe Clover Array (CLARA) coupled to the magnetic spectrometer PRISMA was employed. In the Xe-136 experiment the high-resolution Advanced Gamma Tracking Array (AGATA) was used in combination with PRISMA and the Detector Array for Multinucleon Transfer Ejectiles (DANTE). Results: The ground-state band (g.s. band) of U-240 was measured up to the 20(+) level and a tentative assignment was made up to the (24(+)) level. Results from gamma gamma coincidence and from particle coincidence analyses are shown. Moments of inertia (MoI) show a clear upbend. Evidence for an extended first negative-parity band of U-240 is found. Conclusions: A detailed comparison with latest calculations shows best agreement with cranked relativistic Hartree-Bogoliubov (CRHB) calculations for the g.s. band properties. The negative-parity band shows the characteristics of a K-pi = 0 band based on an octupole vibration.

    • The gamma decay from the high-lying states of Sn-124 was measured using the inelastic scattering of O-17 at 340 MeV. The emitted gamma rays were detected with high resolution with the AGATA demonstrator array and the scattered ions were detected in two segmented Delta E-E silicon telescopes. The angular distribution was measured both for the gamma rays and the scattered O-17 ions. An accumulation of E1 strength below the particle threshold was found and compared with previous data obtained with (gamma,gamma') and (alpha,alpha'gamma) reactions. The present results of elastic scattering, and excitation of E2 and E1 states were analysed using the DWBA approach. From this comprehensive description the isoscalar component of the 1-excited states was extracted. The obtained values are based on the comparison of the data with DWBA calculations including a form factor deduced using a microscopic transition density.

    • The NEutron Detector Array, NEDA, will form the next generation neutron detection system that has been designed to be operated in conjunction with gamma-ray arrays, such as the tracking-array AGATA, to aid nuclear spectroscopy studies. NEDA has been designed to be a versatile device, with high-detection efficiency, excellent neutron-gamma discrimination, and high rate capabilities. It will be employed in physics campaigns in order to maximise the scientific output, making use of the different stable and radioactive ion beams available in Europe. The first implementation of the neutron detector array NEDA with AGATA 1 pi was realised at GANIL. This manuscript reviews the various aspects of NEDA.

  • Spin-gap Isomer in 96Cd

    • Evidence has been obtained for the existence of the long predicted 16+ spin-gap isomer in 96Cd. The decay of the isomer was identified and studied following the use of an 850 MeV/u beam of 124Xe impinging on a Be target and the fragment recoil separator at the GSI Laboratory. Gamma decays from the fragments were detected using the RISING gamma ray array, in its stopped beam configuration, plus a silicon active stopper. The data obtained have been compared with shell model predictions, which indicate that the isoscalar neutron-proton interaction plays a key role in the formation of the isomer.

  • Centrality dependence of charged-neutral particle fluctuations in 158A (GeVPb)-Pb-208+Pb-208 collisions

    • Results on the study of localized fluctuations in the multiplicity of charged particles and photons produced in 158A GeV/c Pb+Pb collisions are presented for varying centralities. The charged versus neutral particle multiplicity correlations in common phase space regions of varying azimuthal sizes are analyzed by two different methods. Various types of mixed events are constructed to probe fluctuations arising from different sources. The measured results are compared to those from simulations and from mixed events. The comparison indicates the presence of nonstatistical fluctuations in both the charged particle and photon multiplicities in limited azimuthal regions. However, no correlated charged-neutral fluctuations, a possible signature of formation of disoriented chiral condensates, are observed. An upper limit on the production of disoriented chiral condensates is set.

    • The neutron-rich nucleus Pt-200 is investigated via in-beam gamma-ray spectroscopy to study the shape evolution in the neutron-rich platinum isotopes towards the N = 126 shell closure. The two-neutron transfer reaction Pt-198(Se-82, Se-80)Pt-200 is used to populate excited states of Pt-200. The Advanced Gamma Ray Tracking Array (AGATA) demonstrator coupled with the PRISMA spectrometer detects gamma rays coincident with the Se-80 recoils, the binary partner of Pt-200. The binary partner method is applied to extract the gamma-ray transitions and build the level scheme of Pt-200. The level at 1884 keV reported by Yates et al. [S. W. Yates, E. M. Baum, E. A. Henry, L. G. Mann, N. Roy, A. Aprahamian, R. A. Meyer, and R. Estep, Phys. Rev. C 37, 1889 (1988)] was confirmed to be at 1882.1 keV and assigned as the (6(1)(+)) state. An additional gamma ray was found and it presumably deexcites the (8(1)(+)) state. The results are compared with state-of-the-art beyond mean-field calculations, performed for the even-even Pt190-204 isotopes, revealing that Pt-200 marks the transition from the gamma-unstable behavior of lighter Pt nuclei towards a more spherical one when approaching the N = 126 shell closure.

    • Results on transverse mass spectra of neutral pious measured at central rapidity are presented for impact parameter selected 158-A GeV Pb + Pb-1 and Pb + Nb collisions. The distributions cover the range 0.5 GeV/c(2) less than or equal to MT - Mo less than or equal to 4 GeV/c(2). The change of the spectral shape and the multiplicity with centrality is studied in detail. In going from p+p to semi-peripheral Pb+Pb collisions there is a nuclear enhancement increasing with transverse mass similar to the well known Cronin effect, while for very central collisions this enhancement appears to be weaker than expected.

  • Examination of how properties of a fissioning system impact isomeric yield ratios of the fragments

    • The population of isomeric states in the prompt decay of fission fragments-so-called isomeric yield ratios (IYRs)-is known to be sensitive to the angular momentum J that the fragment emerged with, and may therefore contain valuable information on the mechanism behind the fission process. In this work, we investigate how changes in the fissioning system impact the measured IYRs of fission fragments to learn more about what parameters affect angular momentum generation. To enable this, a new technique for measuring IYRs is first demonstrated. It is based on the time of arrival of discrete gamma rays, and has the advantage that it enables the study of the IYR as a function of properties of the partner nucleus. This technique is used to extract the IYR of 134Te, strongly populated in actinide fission, from the three different fissioning systems: 232Th(n, f), 238U(n, f), at two different neutron energies, as well as 252Cf(sf). The impacts of changing the fissioning system, the compound nuclear excitation energy, the minimum J of the binary partner, and the number of neutrons emitted on the IYR of 134Te are determined. The decay code TALYS is used in combination with the fission simulation code FREYA to calculate the primary fragment angular momentum from the IYR. We find that the IYR of 134Te has a slope of 0.004 +/- 0.002 with increase in compound nucleus (CN) mass. When investigating the impact on the IYR of increased CN excitation energy, we find no change with an energy increase similar to the difference between thermal and fast fission. By varying the mass of the partner fragment emerging with 134Te, it is revealed that the IYR of 134Te is independent of the total amount of prompt neutrons emitted from the fragment pair. This indicates that neutrons carry minimal angular momentum away from the fission fragments. Comparisons with the FREYA+TALYS simulations reveal that the average angular momentum in 134Te following 238U(n, f) is 6.0 h over bar . This is not consistent with the value deduced from recent CGMF calculations. Finally, the IYR sensitivity to the angular momentum of the primary fragment is discussed. These results are not only important to help understanding the underlying mechanism in nuclear fission, but can also be used to constrain and benchmark fission models, and are relevant to the gamma -ray heating problem of reactors.

    • The spectroscopy and structure of excited states of N similar to Z nuclei in the mass 70-100 region has been investigated using two techniques. In the A similar to 70-80 region fusion evaporation reactions coupled with the recoil-beta-tagging method have been employed at Jyvaskyla to study low-lying states in odd-odd N = Z nuclei. Results from these and other data for known odd-odd nuclei above mass 60 will be discussed. In the heavier mass 90 region a fragmentation experiment has been performed using the RIS-ING/FRS setup at GSI. This experiment was primarily aimed at searching for spin gap isomers in nuclei around A similar to 96. The objectives of the latter experiment will be discussed.

  • Pairing Forces Govern Population of Doubly Magic Ca-54 from Direct Reactions

    • Direct proton-knockout reactions of Sc-55 at similar to 220 MeV/nucleon were studied at the RIKEN Radioactive Isotope Beam Factory. Populated states of Ca-54 were investigated through -ray and invariant-mass spectroscopy. Level energies were calculated from the nuclear shell model employing a phenomenological intemucleon interaction. Theoretical cross sections to states were calculated from distorted-wave impulse approximation estimates multiplied by the shell model spectroscopic factors, which describe the wave function overlap of the Sc-55 ground state with states in Ca-54. Despite the calculations showing a significant amplitude of excited neutron configurations in the ground-state of Sc-55, valence proton removals populated predominantly the ground state of Ca-54. This counterintuitive result is attributed to pairing effects leading to a dominance of the ground-state spectroscopic factor. Owing to the ubiquity of the pairing interaction, this argument should be generally applicable to direct knockout reactions from odd-even to even-even nuclei.

    • This paper presents the final design and results of the FADC Mezzanine for the EXOGAM (EXOtic GAMma array spectrometer) and NEDA (Neutron Detector Array) detectors. The measurements performed include those of studying the effective number of bits, the energy resolution using HP-Ge detectors, as well as timing histograms and discrimination performance. Finally, the conclusion shows how a common digitizing device has been integrated in the experimental environment of two very different detectors which combine both low-noise acquisition and fast sampling rates. Not only the integration fulfilled the expected specifications on both systems, but it also showed how a study of synergy between detectors could lead to the reduction of resources and time by applying a common strategy.

    • In 98Cd a new high-energy isomeric γ-ray transition was identified, which confirms previous spin-parity assignments and enables for the first time the measurement of the E2 and E4 strength for the two decay branches of the isomer. Preliminary results on the 98Cd high-excitation level scheme are presented. A comparison to shell-model calculations as well as implications for the nuclear structure around 100Sn are discussed.

  • Identification and rejection of scattered neutrons in AGATA

    • gamma Rays and neutrons, emitted following spontaneous fission of Cf-252, were measured in an AGATA experiment performed at INFN Laboratori Nazionali di Legnaro in Italy. The setup consisted of four AGATA triple cluster detectors (12 36 fold segmented high purity germanium crystals), placed at a distance of 50 cm from the source, and 16 HELENA BaF2 detectors. The aim of the experiment was to study the interaction of neutrons in the segmented high purity germanium detectors of AGATA and to investigate the possibility to discriminate neutrons and gamma rays with the gamma-ray tracking technique. The BaF2 detectors were used for a time measurement, which gave an independent discrimination of neutrons and gamma rays and which was used to optimise the gamma-ray tracking based neutron rejection methods. It was found that standard gamma-ray tracking, without any additional neutron rejection features, eliminates effectively most of the interaction points clue to recoiling Ge nuclei after elastic scattering of neutrons. Standard Cracking rejects also a significant amount of the events due to inelastic scattering of neutrons in the germanium crystals. Further enhancements of the neutron rejection was obtained by setting conditions on the following quantities, which were evaluated for each event by the Cracking algorithm: energy of the first and second interaction point, difference in the calculated incoming direction of the gamma ray, and figure-of-merit value. The experimental results of Cracking with neutron rejection agree rather well with GEANT4 simulations.

  • The role of core excitations in the structure and decay of the 16(+) spin-gap isomer in Cd-96

    • The first evidence for beta-delayed proton emission from the 16(+) spin gap isomer in Cd-96 is presented. The data were obtained from the Rare Isotope Beam Factory, at the RIKEN Nishina Center, using the BigRIPS spectrometer and the EURICA decay station. beta p branching ratios for the ground state and 16(+) isomer have been extracted along with more precise lifetimes for these states and the lifetime for the ground state decay of Cd-95. Large scale shell model (LSSM) calculations have been performed and WKB estimates made for l = 0, 2, 4 proton emission from three resonance-like states in Ag-96, that are populated by the beta decay of the isomer, and the results compared to the new data. The calculations suggest that l = 2 proton emission from the resonance states, which reside similar to 5 MeV above the proton separation energy, dominates the proton decay. The results highlight the importance of core-excited wavefunction components for the 16(+) state.

    • The study of nuclei in the region around the N = Z doubly-magic nucleus 100Sn has been of long standing interest for the nuclear structure and nuclear astrophysics. Recently, Park et al.  have reported on properties of γ-decaying isomers and isomeric ratios in the vicinity of 100Sn. That experiment was performed at the Radioactive Ion Beam Factory (RIBF) of the RIKEN Nishina Center in Japan as a part of the EURICA campaign. Neutron-deficient nuclei were produced in a fragmentation reaction of a 124Xe primary beam on a 9Be target at an energy of 345 MeV/A. Secondary ions were separated and identified in the BigRIPS fragment separator and implanted in the silicon detector array WAS3ABi. The data presented here were obtained in another experiment performed at the RIBF using the same reaction but slightly different separator settings. New results of ratios of isomeric population and half-lives of γ-decaying isomers populated in the experiment are presented.

    • The Finnish National Esophago-Gastric Cancer Cohort (FINEGO) was established to combine the available registry data with detailed patient information to form a comprehensive, retrospective, population-based research platform of surgically treated oesophageal and gastric cancer in Finland. This cohort profile describes the 2045 surgically treated patients with oesophageal cancer included in the FINEGO cohort.ParticipantsRegistry data were collected from the National Cancer, Patient, Education and Death Registries from 1 January 1987 to 31 December 2016. All patients over 18 years of age, who had either curative surgery, palliative surgery or salvage surgery for primary cancer in the oesophagus are included in this study.Findings to date2045 patients had surgery for oesophageal cancer in the selected time period. 67.2% were man, and the majority had only minor comorbidities. The proportions of adenocarcinomas and squamous cell carcinomas were 43.1% and 44.4%, respectively, and 12.5% had other or missing histology. Only about 23% of patients received neoadjuvant therapy. Oesophagectomy was the treatment of choice and most patients were treated at low-volume centres, but median annual hospital volume increased over time. Median overall survival was 23 months, 5-year survival for all patients in the cohort was 32.9% and cancer-specific survival was 36.5%.Future plansEven though Finland only has a population of 5.5 million, surgery for oesophageal carcinoma has not been centralised and therefore previously reported results have mostly been small, single-centre cohorts. Because of FINEGO, we now have a population-based, unselected cohort of surgically treated patients, enabling research on national trends over time regarding oesophageal cancer, including patient characteristics, tumour histology, stage and neoadjuvant treatment, surgical techniques, hospital volumes and patient mortality. Data collection is ongoing, and the cohort will be expanded to include more detailed data from patient records and national biobanks.

    • Isomeric states in Ti-59,Ti-61 have been populated in the projectile fragmentation of a 345 AMeV( 238)U beam at the Radioactive Isotope Beam Factory. The decay lifetimes and delayed gamma-ray transitions were measured with the EURICA array. Besides the known isomeric state in Ti-59, two isomeric states in Ti-61 are observed for the first time. Based on the measured lifetimes, transition multipolarities as well as tentative spins and parities are assigned. Large-scale shell model calculations based on the modified LNPS interaction show that both Ti-59 and Ti-61 belong to the Island of Inversion at N = 40 with ground state configurations dominated by particle-hole excitations to the g(9/2 )and d(5/2) orbits.

    • BACKGROUND: In the decade following the introduction of ICSI, a higher prevalence of de novo chromosome abnormalities, in particular sex chromosome and autosomal structural abnormalities, as well as inherited abnormalities was described in children conceived by ICSI compared to both naturally conceived (NC) children and children conceived by standard IVF. The explanation for the observed increase in prevalence is not clear and has been suggested to reflect parental factors (e.g. age or sperm quality) or to be a result of the ICSI procedure itself. Over the years, the procedure, as well as the patient group, and indications for ICSI treatment have changed. OBJECTIVE AND RATIONALE: The objective of this systematic review and meta-analysis was to assess the prevalence of chromosome abnormalities in ICSI pregnancies and children and to examine any potentially increased risk compared to standard IVF and NC. SEARCH METHODS: Pubmed, Embase, Cochrane Libraries and Web of Science up to October 2020 were searched. Primary outcome measures were overall chromosome abnormalities and de novo abnormalities (including sex chromosome abnormalities and autosomal abnormalities). The secondary outcome was inherited abnormalities. We followed the PRISMA guidelines and relevant meta-analyses were performed. OUTCOMES: The search included 4648 articles, out of which 27 met the inclusion criteria, and 19 were included in quantitative synthesis (meta-analyses). The prevalence of chromosome abnormalities varied considerably between studies, possibly explained by large differences in sample size and patient demographics. Only five studies were eligible for pooled analyses on adjusted data. All studies had a critical risk of bias. Results from pooled adjusted data showed no evidence of an increased risk of overall chromosome abnormalities when comparing ICSI to either standard IVF (aOR 0.75 (95% CI 0.41-1.38)) or NC (aOR 1.29 (95% CI 0.69-2.43)). In contrast, meta-analyses on unadjusted data showed an increased risk of overall chromosome abnormalities in ICSI compared to both standard IVF (OR 1.42 (95% CI 1.09-1.85)) and NC (OR 2.46 (95% CI 1.52-3.99)) and an increased risk of de novo abnormalities in ICSI compared to NC (OR 2.62 (95% CI 2.07-3.31)). Yet, based on a very low certainty of evidence, the conclusion remains, that no indication of an increased risk of chromosome abnormalities in ICSI offspring could be found. If an increased risk of chromosome abnormalities in selected ICSI offspring should exist, the absolute risk continues to be small. WIDER IMPLICATIONS: This review provides an extensive overview of the existing evidence on the relationship between ICSI and chromosome abnormalities in the offspring. We highlight the need for well-designed large, prospective, controlled studies with systematic cytogenetic testing. Existing data are limited and, in many cases, marred by critical levels of bias.

    • Influenza A virus (IAV) has its natural reservoir in wild waterfowl and emerging human IAVs often contain gene segments from avian viruses. The active drug metabolite of oseltamivir (oseltamivir carboxylate (OC)), stockpiled as Tamiflu® for influenza pandemic preparedness, is not removed by conventional sewage treatment and has been detected in river water. There, it may there exert evolutionary pressure on avian IAV in waterfowl, resulting in development of resistant viral variants. A resistant avian IAV can circulate among wild birds only if resistance does not restrict viral fitness and if the resistant virus can persist without continuous drug pressure. In this in vivo Mallard (Anas platyrhynchos) study we tested if an OC-resistant avian IAV strain (A(H1N1)/NA-H274Y) could retain resistance while drug pressure was gradually removed. Successively infected Mallards were exposed to decreasing levels of OC, and fecal samples were analyzed for neuraminidase sequence and phenotypic resistance. No reversion to wild-type virus was observed during the experiment, which included 17 days of viral transmission in 10 ducks exposed to OC concentrations below resistance induction levels. We conclude that resistance in avian IAV, induced by OC exposure of the natural host, can persist in absence of the drug. Thus, there is a risk that human pathogenic IAVs that evolve from IAVs circulating among wild birds may contain resistance mutations. An oseltamivir resistant pandemic IAV would be a substantial public health threat. Therefore, our observations underscore the need for prudent oseltamivir use, upgraded sewage treatment and resistance surveillance of IAV in wild birds.

  • The Association Between High Birth Weight and Long-Term Outcomes-Implications for Assisted Reproductive Technologies: A Systematic Review and Meta-Analysis

    • Background: Studies have shown that the prevalence of children born with high birth weight or large for gestational age (LGA) is increasing. This is true for spontaneous pregnancies; however, children born after frozen embryo transfer (FET) as part of assisted reproductive technology (ART) also have an elevated risk. In recent years, the practice of FET has increased rapidly and while the perinatal and obstetric risks are well-studied, less is known about the long-term health consequences. Objective: The aim of this systematic review was to describe the association between high birth weight and LGA on long-term child outcomes. Data Sources: PubMed, Scopus, and Web of Science were searched up to January 2021. Exposure included high birth weight and LGA. Long-term outcome variables included malignancies, psychiatric disorders, cardiovascular disease, and diabetes. Study Selection: Original studies published in English or Scandinavian languages were included. Studies with a control group were included while studies published as abstracts and case reports were excluded. Data Extraction: The methodological quality, in terms of risk of bias, was assessed by pairs of reviewers. Robins-I (www.methods.cochrane.org) was used for risk of bias assessment in original articles. For systematic reviews, AMSTAR (www.amstar.ca) was used. For certainty of evidence, we used the GRADE system. The systematic review followed PRISMA guidelines. When possible, meta-analyses were performed. Results: The search included 11,767 articles out of which 173 met the inclusion criteria and were included in the qualitative analysis, while 63 were included in quantitative synthesis (meta-analyses). High birth weight and/or LGA was associated with low to moderately elevated risks for certain malignancies in childhood, breast cancer, several psychiatric disorders, hypertension in childhood, and type 1 and 2 diabetes. Conclusions: Although the increased risks for adverse outcome in offspring associated with high birth weight and LGA represent serious health effects in childhood and in adulthood, the size of these effects seems moderate. The identified risk association should, however, be taken into account in decisions concerning fresh and frozen ART cycles and is of general importance in view of the increasing prevalence in high birthweight babies.

  • The effect of paternal factors on perinatal and paediatric outcomes: a systematic review and meta-analysis

    • BACKGROUND: Maternal factors, including increasing childbearing age and various life-style factors, are associated with poorer short- and long-term outcomes for children, whereas knowledge of paternal parameters is limited. Recently, increasing paternal age has been associated with adverse obstetric outcomes, birth defects, autism spectrum disorders and schizophrenia in children. OBJECTIVE AND RATIONALE: The aim of this systematic review is to describe the influence of paternal factors on adverse short- and long-term child outcomes. SEARCH METHODS: PubMed, Embase and Cochrane databases up to January 2017 were searched. Paternal factors examined included paternal age and life-style factors such as body mass index (BMI), adiposity and cigarette smoking. The outcome variables assessed were short-term outcomes such as preterm birth, low birth weight, small for gestational age (SGA), stillbirth, birth defects and chromosomal anomalies. Long-term outcome variables included mortality, cancers, psychiatric diseases/disorders and metabolic diseases. The systematic review follows PRISMA guidelines. Relevant meta-analyses were performed. OUTCOMES: The search included 14 371 articles out of which 238 met the inclusion criteria, and 81 were included in quantitative synthesis (meta-analyses). Paternal age and paternal life-style factors have an association with adverse outcome in offspring. This is particularly evident for psychiatric disorders such as autism, autism spectrum disorders and schizophrenia, but an association is also found with stillbirth, any birth defects, orofacial clefts and trisomy 21. Paternal height, but not BMI, is associated with birth weight in offspring while paternal BMI is associated with BMI, weight and/or body fat in childhood. Paternal smoking is found to be associated with an increase in SGA, birth defects such as congenital heart defects, and orofacial clefts, cancers, brain tumours and acute lymphoblastic leukaemia. These associations are significant although moderate in size, with most pooled estimates between 1.05 and 1.5, and none exceeding 2.0. WIDER IMPLICATIONS: Although the increased risks of adverse outcome in offspring associated with paternal factors and identified in this report represent serious health effects, the magnitude of these effects seems modest.

    • Background Approximately 50 000 oocyte donation OD) treatment cycles are now performed annually in Europe and the US. Objectives To ascertain whether the risk of adverse obstetric and perinatal/neonatal outcomes is higher in pregnancies conceived by OD than in pregnancies conceived by conventional in-vitro fertilisation IVF)/intracytoplasmic sperm injection ICSI) or spontaneously. Search Strategy A systematic search was performed in the PubMed, Cochrane and Embase databases from 1982-2016. Primary outcomes were hypertensive disorders of pregnancy, preeclampsia PE), gestational diabetes mellitus, postpartum haemorrhage, caesarean section, preterm birth, low birthweight and small for gestational age. Selection criteria Inclusion criteria were original studies including at least five OD pregnancies with a control group of pregnancies conceived by conventional IVF/ICSI or spontaneous conception, and case series with > 500 cases reporting one or more of the selected complications. Studies not adjusting for plurality were excluded. Data collection and analysis Thirty-five studies met the inclusion criteria. A random-effects model was used for the meta-analyses. Main results For OD pregnancies versus conventional IVF/ ICSI pregnancies the risk of PE was adjusted odds ratio (AOR) 2.11 (95% CI, 1.42-3.15) in singleton and AOR 3.31 (95% CI, 1.616.80) in multiple pregnancies. The risks of preterm birth and low birthweight in singletons were AOR 1.75 (95% CI, 1.39-2.20) and 1.53 (95% CI, 1.16-2.01), respectively. Conclusions OD conceptions are associated with adverse obstetric and neonatal outcomes. To avoid the additional increase in risk from multiplicity, single-embryo transfer should be the choice of option in OD cycles.
