    • Soot, or black carbon (BC), aerosol is a major climate forcer with severe health effects. The impacts depend strongly on particle number concentration, size and mixing state. This work reports on two field campaigns at nearby urban and rural sites, 65gkm apart, in southern Sweden during late summer 2018. BC was measured using a single-particle soot photometer (SP2) and Aethalometers (AE33). Differences in BC concentrations between the sites are driven primarily by local traffic emissions. Equivalent and refractory BC mass concentrations at the urban site were on average a factor 2.2 and 2.5, with peaks during rush hour up to a factor g1/44, higher than the rural background levels. The number fraction of particles containing a soot core was significantly higher in the city. BC particles at the urban site were on average smaller by mass and had less coating owing to fresh traffic emissions. The organic components of the fresh traffic plumes were similar in mass spectral signature to hydrocarbon-like organic aerosol (HOA), commonly associated with traffic. Despite the intense local traffic (g1/4g30g000 vehicles passing per day), PM1, including organic aerosol, was dominated by aged continental air masses even at the curbside site. The fraction of thickly coated particles at the urban site was highly correlated with the mass concentrations of all measured chemical species of PM1, consistent with aged, internally mixed aerosol. Trajectory analysis for the whole year showed that air masses arriving at the rural site from eastern Europe contained approximately double the amount of BC compared to air masses from western Europe. Furthermore, the largest regional emissions of BC transported to the rural site, from the Malmö-Copenhagen urban area, are discernible above background levels only when precipitation events are excluded. We show that continental Europe and not the Malmö-Copenhagen region is the major contributor to the background BC mass concentrations in southern Sweden.

    • In coastal areas, there is increased concern aboutemissions from shipping activities and the associated impacton air quality. We have assessed the ship aerosol propertiesand the contribution to coastal particulate matter (PM) andnitrogen dioxide (NO2) levels by measuring ship plumes inambient conditions at a site in southern Sweden, within a SulfurEmission Control Area. Measurements took place duringa summer and a winter campaign, 10 km downwind ofa major shipping lane. Individual ships showed large variabilityin contribution to total particle mass, organics, sulfate,and NO2. The average emission contribution of theshipping lane was 2913 and 3720 ngm?3 to PM0:5,188 and 3419 ngm?3 to PM0:15, and 1:210:57 and1:110:61 μgm?3 to NO2, during winter and summer, respectively.Sulfate and organics dominated the particle massand most plumes contained undetectable amounts of equivalentblack carbon (eBC). The average eBC contribution was3:51:7 ngm?3 and the absorption Ångström exponent wasclose to 1. Simulated ageing of the ship aerosols using anoxidation flow reactor showed that on a few occasions, therewas an increase in sulfate and organic mass after photochemicalprocessing of the plumes. However, most plumes did notproduce measurable amounts of secondary PM upon simulatedageing.

    • Air pollution is a major contributor to the global burden of disease and has been linked to several diseases and conditions, including cardiovascular disease. The biological mechanisms are related to inflammation and increased coagulability, factors that play an important role in the pathogenesis of venous thromboembolism (VTE, i.e., deep vein thrombosis or pulmonary embolism). This study investigates if long-term exposure to air pollution is associated with increased VTE incidence. The study followed 29408 participants from the Malmö Diet and Cancer (MDC) cohort, which consists of adults aged 44-74 recruited in Malmö, Sweden between 1991 and 1996. For each participant, annual mean residential exposures to particulate matter <2.5μg (PM2.5) and <10μg (PM10), nitrogen oxides (NOx) and black carbon (BC) from 1990 up to 2016 were calculated. Associations with VTE were analysed using Cox proportional hazard models for air pollution in the year of the VTE event (lag0) and the mean of the prior 1-10 years (lag1-10). Annual air pollution exposures for the full follow-up period had the following means: 10.8μg/m3 for PM2.5, 15.8μg/m3 for PM10, 27.7μg/m3 for NOx, and 0.96μg/m3 for BC. The mean follow-up period was 19.5 years, with 1418 incident VTE events recorded during this period. Exposure to lag1-10 PM2.5 was associated with an increased risk of VTE (HR 1.17 (95%CI 1.01-1.37)) per interquartile range (IQR) of 1.2μg/m3 increase in PM2.5 exposure. No significant associations were found between other pollutants or lag0 PM2.5 and incident VTE. When VTE was divided into specific diagnoses, associations with lag1-10 PM2.5 exposure were similarly positive for deep vein thrombosis but not for pulmonary embolism. Results persisted in sensitivity analyses and in multi-pollutant models. Long-term exposure to moderate concentrations of ambient PM2.5 was associated with increased risks of VTE in the general population in Sweden.

The aim of this study was to investigate if the uptake of N-methyl-2-pyrrolidone (NMP), a widely used industrial chemical, increases after exposure to NMP in humid air compared to dry air. NMP has been described to be an airway irritant and a developmentally toxic compound. Six male volunteers were exposed to NMP, three at the time, for 8h in an exposure chamber. They were each exposed on four different occasions to air levels of 0 and 20mg NMP/m(3) in dry and humid air. Blood and urine were sampled before, during and up to 5 days after the end of the 8-h exposure. Plasma and urine were analysed for NMP and its metabolites, using liquid chromatography-tandem mass spectrometry. There was no statistically significant increase in the total cumulated excretion of NMP and its metabolites in urine after exposure in humid air as compared to dry air. Furthermore, there were no differences in the levels of peak concentrations in either plasma or urine. Also, no differences were found in AUC between the exposures. However, there were large individual differences, especially for the exposure in humid air. A not previously identified metabolite in human, 2-pyrrolidone (2-P), was identified. The results do not support a significantly higher absorption of NMP at exposure in humid air as compared to dry air. However, the large individual differences support the use of biological monitoring for assessment of NMP exposure. In addition, 2-P was confirmed to be an NMP metabolite in humans. This may be of importance for the developmental toxicity of NMP since 2-P have been described to be a reproductively toxic substance.

  • Adsorption Efficiency of Respirator Filter Cartridges for Isocyanates

    • In some industries, the temperature and the humidity will vary greatly between different work places, such as outdoor work in arctic or tropical climates. There is therefore a need to test respirator filters at conditions that simulate conditions that are relevant for the industries that they are used in. Filter cartridges were exposed to controlled atmospheres of varying isocyanate concentration, air humidity, and temperature in an exposure chamber. For isocyanic acid (ICA) and methyl isocyanate (MIC), the exposure concentrations were between 100 and 200 p.p.b., monitored using a proton transfer reaction mass spectrometer. ICA and MIC were generated by continuous thermal degradation of urea and dimethylurea. The breakthrough was studied by collecting air samples at the outlet of the filter cartridges using impinger flasks or dry samplers with di-n-butylamine as derivatization reagent for isocyanates followed by liquid chromatography tandem mass spectrometry (LC-MS/MS) analysis. For hexamethylene diisocyanate (HDI) and isophorone diisocyanate (IPDI), the exposure concentrations were between 4 and 20 p.p.b. and were generated by wet membrane permeation. To reveal the profile of adsorption in different layers of the respirator filters, representative samples from each of the layers were hydrolyzed. The hydrolysis products hexamethylene diamine and isophorone diamine were determined after derivatization with pentafluoropropionic anhydride (PFPA) followed by LC-MS/MS analysis. The two filter types studied efficiently absorbed both ICA and MIC. There was no trend of impaired performance throughout 48-h exposure tests. Even when the filters were exposed to high concentrations (similar to 200 p.p.b.) of ICA and MIC for 96 h, the isocyanates were efficiently absorbed with only a limited breakthrough. The majority of the HDI and IPDI (> 90%) were absorbed in the top layers of the absorbant, but HDI and IPDI penetrated farther down into the respirator filters during 120 h of exposure as compared to 16 h exposure.

    • Long-term exposure to air pollution increases the risk of cardiovascular morbidity and mortality, but the mechanisms are not fully known. Current evidence suggests that air pollution exposure contributes to the development of atherosclerosis. There are few studies investigating associations between air pollution and carotid plaques, a well-known precursor of cardiovascular disease.A Swedish population-based cohort (aged 45-64yearsat recruitment) was randomly selected from the Malmö Diet and Cancer study between 1991 and 1994, of which 6103 participants underwent ultrasound examination of the right carotid artery to determine carotid plaque presence and carotid intima media thickness (CIMT). Participants were assigned individual residential air pollution exposure (source-specific PM2.5, PM10, NOx, BC) at recruitment from Gaussian dispersion models. Logistic and linear regression models, adjusted for potential confounders and cardiovascular risk factors, were used to investigate associations between air pollutants and prevalence of carotid plaques, and CIMT, respectively.The prevalence of carotid plaques was 35%. The mean levels of PM2.5 and PM10 at recruitment were 11 and 14μg/m3, most of which was due to long range transport. The exposure contrast within the cohort was relatively low. PM2.5 exposure was associated with carotid plaques in a model including age and sex only (OR 1.10 (95% CI 1.01-1.20) per 1μg/m3), but after adjustment for cardiovascular risk factors and socioeconomic status (SES) the association was weak and not significant (OR 1.05 (95% CI 0.96-1.16) per 1μg/m3). The pattern was similar for PM10 and NOx exposure. Associations between air pollutants and plaques were slightly stronger for long-term residents and in younger participants with hypertension. There was no clear linear trend between air pollution exposure and plaque prevalence. Non-significant slightly positive associations were seen between air pollution exposures and CIMT.In this large, well-controlled cross-sectional study at low exposure levels we found no significant associations between air pollution exposures and subclinical atherosclerosis in the carotid arteries, after adjusting for cardiovascular risk factors and SES. Further epidemiological studies of air pollution and intermediate outcomes are needed to explain the link between air pollution and cardiovascular events.

    • Ambient air pollution remains the major environmental cause of disease. Accurate assessment of population exposure and small-scale spatial exposure variations over long time periods is essential for epidemiological studies. We estimated annual exposure to fine and coarse particulate matter (PM2.5, PM10), and nitrogen oxides (NOx, NO2) with high spatial resolution to examine time trends 2000‒2018, compliance with the WHO Air Quality Guidelines, and assess the health impact. The modelling area covered six metropolitan areas in Sweden with a combined population of 5.5 million. Long-range transported air pollutants were modelled using a chemical transport model with bias correction, and locally emitted air pollutants using source-specific Gaussian-type dispersion models at resolutions up to 50 × 50m. The modelled concentrations were validated using quality-controlled monitoring data. Lastly, we estimated the reduction in mortality associated with the decrease in population exposure. The validity of modelled air pollutant concentrations was good (R2 for PM2.5 0.84, PM10 0.61, and NOx 0.87). Air pollution exposure decreased substantially, from a population weighted mean exposure to PM2.5 of 12.2µgm−3 in 2000 to 5.4µgm−3 in 2018. We estimated that the decreased exposure was associated with a reduction of 2719 (95% CI 2046–3055) premature deaths annually. However, in 2018, 65%, 8%, and 42% of residents in the modelled areas were still exposed to PM2.5, PM10, or NO2 levels, respectively, that exceeded the current WHO Air Quality Guidelines for annual average exposure. This emphasises the potential public health benefits of reductions in air pollution emissions.

Concentrations of total and locally emitted particulate matter <2.5 μm (PM2.5), <10 μm (PM10), and nitrogen oxides (NOx) at the residential address were modelled using high-resolution dispersion models. We estimated associations between air pollution exposures and segment involvement score (SIS), coronary artery calcification score (CACS), number of non-calcified plaques (NCP), and number of significant stenoses, using ordinal regression models extensively adjusted for potential confounders.Results: Median 10-year average PM2.5 exposure was 6.2 μg/m3 (range 3.5–13.4 μg/m3). 51 % of participants were women and 51 % were never-smokers. None of the assessed pollutants were associated with a higher SIS or CACS. Exposure to PM2.5 was associated with NCP (adjusted OR 1.34, 95 % CI 1.13, 1.58, per 2.05 μg/m3). Associations with significant stenoses were inconsistent.Conclusions: In this large, middle-aged general population sample with low exposure levels, air pollution was not associated with measures of total burden of coronary atherosclerosis. However, PM2.5 appeared to be associated with a higher prevalence of non-calcified plaques. The results suggest that increased risk of early-stage atherosclerosis or rupture, but not increased total atherosclerotic burden, may be a pathway for long-term air pollution effects on cardiovascular disease.

Syftet med denna studie var att beräkna hälsoeffekter som kan tillskrivas hypotetisk nedgång i luftföroreningshalter i Malmö i södra Sverige om en policy med avgasfri innerstad skulle genomföras.

  • The Long-Term Mortality Effects Associated with Exposure to Particles and NOx in the Malmö Diet and Cancer Cohort

    • In this study, the long-term mortality effects associated with exposure to PM10 (particles with an aerodynamic diameter smaller than or equal to 10 µm), PM2.5 (particles with an aerodynamic diameter smaller than or equal to 2.5 µm), BC (black carbon), and NOx (nitrogen oxides) were analyzed in a cohort in southern Sweden during the period from 1991 to 2016. Participants (those residing in Malmö, Sweden, born between 1923 and 1950) were randomly recruited from 1991 to 1996. At enrollment, 30,438 participants underwent a health screening, which consisted of questionnaires about lifestyle and diet, a clinical examination, and blood sampling. Mortality data were retrieved from the Swedish National Cause of Death Register. The modeled concentrations of PM10, PM2.5, BC, and NOx at the cohort participants’ home addresses were used to assess air pollution exposure. Cox proportional hazard models were used to estimate the associations between long-term exposure to PM10, PM2.5, BC, and NOx and the time until death among the participants during the period from 1991 to 2016. The hazard ratios (HRs) associated with an interquartile range (IQR) increase in each air pollutant were calculated based on the exposure lag windows of the same year (lag0), 1–5 years (lag1–5), and 6–10 years (lag6–10). Three models were used with varying adjustments for possible confounders including both single-pollutant estimates and two-pollutant estimates. With adjustments for all covariates, the HRs for PM10, PM2.5, BC, and NOx in the single-pollutant models at lag1–5 were 1.06 (95% CI: 1.02–1.11), 1.01 (95% CI: 0.95–1.08), 1.07 (95% CI: 1.04–1.11), and 1.11 (95% CI: 1.07–1.16) per IQR increase, respectively. The HRs, in most cases, decreased with the inclusion of a larger number of covariates in the models. The most robust associations were shown for NOx, with statistically significant positive HRs in all the models. An overall conclusion is that road traffic-related pollutants had a significant association with mortality in the cohort.

    • Health impact assessments of differential air pollution rely on epidemiologically established relationships between concentration levels where people are exposed and adverse health outcomes. To assess air pollution concentrations, land use regression is commonly used. However, an alternative tool is dispersion modelling, where a detailed inventory of pollution sources together with meteorological data drives calculations of compound dispersion. With this, both spatial and temporal variation can be assessed. In this study, we evaluated results of a Gaussian dispersion model applied to an emissions inventory for Scania, the southernmost county in Sweden. The dispersion considered was particulate matter of aerodynamic diameter < 10 µm (PM10), particulate matter of aerodynamic diameter < 2.5 µm (PM2.5) and black carbon (BC) during an 11-year period (2000–2011). Mean concentrations and 95th percentiles expressed in µg/m3 ranged from 10.1 to 12.6 and 16.6 to 20.7 for PM2.5 and from 14.0 to 18.8 and 22.6 to 27.0 for PM10, respectively. Seven monitoring stations were used for evaluation. Correlations (R2) ranged from 0.44 to 0.86 for PM2.5 (mean bias from − 9.0 to 0.1 µg/m3) and from 0.46 to 0.83 for PM10 (mean bias − 6.1 to 3.5 µg/m3). An evaluated database of PM and BC concentrations for Scania is now available for future exposure assessment projects. Calculations were based on a well-known dispersion model with detailed emission data as input. The evaluation showed correlation coefficients for PM in line with previous literature. The data on PM10, PM2.5 and BC concentrations will, therefore, be used in subsequent studies, epidemiological as well as health impact assessments.

  • Long-term exposure to traffic noise and risk of incident colon cancer : A pooled study of eleven Nordic cohorts

    • BackgroundColon cancer incidence is rising globally, and factors pertaining to urbanization have been proposed involved in this development. Traffic noise may increase colon cancer risk by causing sleep disturbance and stress, thereby inducing known colon cancer risk-factors, e.g. obesity, diabetes, physical inactivity, and alcohol consumption, but few studies have examined this.ObjectivesThe objective of this study was to investigate the association between traffic noise and colon cancer (all, proximal, distal) in a pooled population of 11 Nordic cohorts, totaling 155,203 persons.MethodsWe identified residential address history and estimated road, railway, and aircraft noise, as well as air pollution, for all addresses, using similar exposure models across cohorts. Colon cancer cases were identified through national registries. We analyzed data using Cox Proportional Hazards Models, adjusting main models for harmonized sociodemographic and lifestyle data.ResultsDuring follow-up (median 18.8 years), 2757 colon cancer cases developed. We found a hazard ratio (HR) of 1.05 (95% confidence interval (CI): 0.99–1.10) per 10-dB higher 5-year mean time-weighted road traffic noise. In sub-type analyses, the association seemed confined to distal colon cancer: HR 1.06 (95% CI: 0.98–1.14). Railway and aircraft noise was not associated with colon cancer, albeit there was some indication in sub-type analyses that railway noise may also be associated with distal colon cancer. In interaction-analyses, the association between road traffic noise and colon cancer was strongest among obese persons and those with high NO2-exposure.DiscussionA prominent study strength is the large population with harmonized data across eleven cohorts, and the complete address-history during follow-up. However, each cohort estimated noise independently, and only at the most exposed façade, which may introduce exposure misclassification. Despite this, the results of this pooled study suggest that traffic noise may be a risk factor for colon cancer, especially of distal origin.

  • Long-Term Exposure to Transportation Noise and Risk of Incident Stroke : A Pooled Study of Nine Scandinavian Cohorts

    • Background: Transportation noise is increasingly acknowledged as a cardiovascular risk factor, but the evidence base for an association with stroke is sparse.Objective: We aimed to investigate the association between transportation noise and stroke incidence in a large Scandinavian population.Methods: We harmonized and pooled data from nine Scandinavian cohorts (seven Swedish, two Danish), totaling 135,951 participants. We identified residential address history and estimated road, railway, and aircraft noise for all addresses. Information on stroke incidence was acquired through linkage to national patient and mortality registries. We analyzed data using Cox proportional hazards models, including socioeconomic and lifestyle confounders, and air pollution.Results: During follow-up (median=19.5y), 11,056 stroke cases were identified. Road traffic noise (Lden) was associated with risk of stroke, with a hazard ratio (HR) of 1.06 [95% confidence interval (CI): 1.03, 1.08] per 10-dB higher 5-y mean time-weighted exposure in analyses adjusted for individual- and area-level socioeconomic covariates. The association was approximately linear and persisted after adjustment for air pollution [particulate matter (PM) with an aerodynamic diameter of ≤2.5μm (PM2.5) and NO2]. Stroke was associated with moderate levels of 5-y aircraft noise exposure (40–50 vs. ≤40 dB) (HR=1.12; 95% CI: 0.99, 1.27), but not with higher exposure (≥50 dB, HR=0.94HR; 95% CI: 0.79, 1.11). Railway noise was not associated with stroke.Discussion: In this pooled study, road traffic noise was associated with a higher risk of stroke. This finding supports road traffic noise as an important cardiovascular risk factor that should be included when estimating the burden of disease due to traffic noise.

  • Characteristics of Particle Emissions at Paint Manufacturing

    • The handling of pigments and powders in paint manufacturing cause emissions of airborne particles. A great variety of substances occur. Previous studies (Bohgard et al., 1994) of simulations of pouring of pigments, which occur in small scale weighing procedures, showed typically bimodal size distribution including a sub-micron and a super-micron mode.The objective of the study was to get detailed knowledge on the relation between different source processes (depending on production technology, equipment and the manual handling routines) and aerosol characteristics.Super-micron particles were continuously measured with a time-of-flight instrument (Aerodynamic Particle Sizer, APS TSI Inc.) and sub-micron particles with an electric mobility spectrometer (Scanning Mobility Particle Sizer, SMPS TSI Inc.). In addition filter sampling techniques for standardised work-place sampling (so-called total dust filter cassettes, a cyclone sampler for respirable dust and IOM-samplers for inhalable dust) have been used for idealised exposure/area measurements using a rotating mannequin as a carrier of these samplers. The filters were analysed gravimetrically and with Particle Induced X-ray Emission Analysis. The aerosols were measured during manufacturing of batches of paint. Sources and source mechanisms, as pouring pigments from bags, disposal of packages, mixing of powders and pigments and suspected re-suspension sources, were documented simultaneously with the performance of the measurements.Figure 1 shows examples of size distributions obtained with the APS and SMPS. The size distributions have essentially bimodal shapes with median diameters of 0.05 -0.4 [mu ]m and of 2-5 [mu ]m respectively. Rapid variation in concentration occurs.Preliminary results show that several sources occur. Irregular activities and events as disposal of package material, cleaning, leakage from conveyor tubes and re-suspension due to wind velocities from open gates and moving vehicles significantly contribute to the particle concentrations.Filter sampling with the samplers mounted on a mannequin torso, rotating 360 degrees forth and back, is advantageous by giving idealised measures on exposure. Samplers mounted on equal horizontal level will have equal sampling location integrated over time. Furthermore, the sampling procedure emulate sampling in accordance with sampling conventions. Inlet characteristics of samplers mounted on a body differ from characteristics of free-hanging samplers (Vincent, 1989). Higher estimated mass concentrations were obtained with the IOM-samplers for inhalable dust than with the 37 mm open-face [ldquo ]total dust[rdquo ] cassettes.Figure omitted

  • On the Determination of Reactive Compounds in Aerosols

    • Many sources of airborne contaminants have always been present in various work environments. With the development of thermosetting plastic systems, the use of reactive chemical compounds was greatly increased and thus also the risk of exposure to reactive and potentially hazardous compounds. Exposure can occur in many situations, not only at the workplace where the raw materials are handled. The finished products may emit unreacted or excess compounds, like in furniture made of particleboard with phenol-formaldehyde resin that may emit formaldehyde for a long time. Another emission source can arise when some thermosetting plastics like polyurethane and phenol-formaldehyde-urea resins are heated, intentionally or by accident. Many different types of isocyanates in a complex mixture are released at such occasions. In an effort to characterise those emissions in more detail, a new analytical method for the derivatisation and analysis of isocyanates was developed. The new derivatisation reagent, di-n-butylamine, was found to be suitable for the determination of many types of isocyanates even when complicated mixtures of air pollutants were present in the work environment. However, it was found that the used standard sampler, the midget impinger, had almost negligible collection efficiency for particles with an aerodynamic diameter smaller than 1.5 µm. In the work operations where polyurethane products were heated and decomposed, leading to the emission of isocyanate-containing aerosols, the particles were found to be mainly in the sub-micron range and thus not collected by the impinger sampler. In car repair shops, very high concentrations of airborne isocyanates found to be emitted at common work operations as welding and cutting in polyurethane painted vehicles. As an initial solution, a glass-fibre filter was connected in series after the impinger to collect the particles that otherwise would have escaped sampling. The relatively volatile reagent dibutylamine proved to be well suited for this sampling technique as the reagent evaporates from the sampling liquid and impregnates particles collected on the filter. In air samples taken with this method in car repair shops, very high concentrations of isocyanates were found to be emitted during operations such as welding and cutting in vehicles. In order to avoid undesirable reactions that can take place between the studied compound and other compounds present in the work environment, it is desirable to supply the reagent at the earliest possible moment in the sampling process. The ideal might be to supply the reagent to the sample even before collection. Using the aerosol particles as condensation nuclei for a condensing reagent vapour, this can be done. A new method to supply a large excess of liquid reagent to particles in an aerosol before collection of the particles was presented. This new type of sampler may become useful in the investigation of the actual chemical composition of reactive air contaminants at workplaces.

Reagensångan blandas med provaerosolen till en resulterande aerosolblandning, vilken överförs till en kondensationsenhet där reagensångan kondenserar på partiklarna hos aerosolen varigenom en kemisk reaktion orsakas mellan reagensen och de reaktiva föreningarna hos partiklarna. Ett stabilt derivat erhålles, vilket uppsamlas med en partikeluppsamlingsanordning för efterföljande analys.

  • Exposure to long-term source-specific transportation noise and incident breast cancer : A pooled study of eight Nordic cohorts

    • Background: Environmental noise is an important environmental exposure that can affect health. An association between transportation noise and breast cancer incidence has been suggested, although current evidence is limited. We investigated the pooled association between long-term exposure to transportation noise and breast cancer incidence.Methods: Pooled data from eight Nordic cohorts provided a study population of 111,492 women. Road, railway, and aircraft noise were modelled at residential addresses. Breast cancer incidence (all, estrogen receptor (ER) positive, and ER negative) was derived from cancer registries. Hazard ratios (HR) were estimated using Cox Proportional Hazards Models, adjusting main models for sociodemographic and lifestyle variables together with long-term exposure to air pollution.Results: A total of 93,859 women were included in the analyses, of whom 5,875 developed breast cancer. The median (5th–95th percentile) 5-year residential road traffic noise was 54.8 (40.0–67.8) dB Lden, and among those exposed, the median railway noise was 51.0 (41.2–65.8) dB Lden. We observed a pooled HR for breast cancer (95 % confidence interval (CI)) of 1.03 (0.99–1.06) per 10 dB increase in 5-year mean exposure to road traffic noise, and 1.03 (95 % CI: 0.96–1.11) for railway noise, after adjustment for lifestyle and sociodemographic covariates. HRs remained unchanged in analyses with further adjustment for PM2.5 and attenuated when adjusted for NO2 (HRs from 1.02 to 1.01), in analyses using the same sample. For aircraft noise, no association was observed. The associations did not vary by ER status for any noise source. In analyses using <60 dB as a cutoff, we found HRs of 1.08 (0.99–1.18) for road traffic and 1.19 (0.95–1.49) for railway noise.Conclusions: We found weak associations between road and railway noise and breast cancer risk. More high-quality prospective studies are needed, particularly among those exposed to railway and aircraft noise before conclusions regarding noise as a risk factor for breast cancer can be made.

  • Luftföroreningars effekt på akutbesök för andningsorganen och uttag av astmamedicin

    • Denna rapport presenterar den femte studien för Naturvårdsverkets hälsorelaterade miljöövervakning inom detta område, och syftar till att analysera luftföroreningars korttidseffekter på antalet akutbesök för astma och övriga sjukdomar i andningsorganen under perioden 2013-2018 i tre storstadsområden: Stockholm, Göteborg och Malmö. Som ett tillägg i denna studie har också samband mellan luftföroreningar och läkemedelsuttag för astmabesvär utvärderats som en alternativ indikator på akuta effekter av luftföroreningar på astma. Luftföroreningsdata kommer från respektive miljöförvaltning och avser bakgrundshalter (taknivå) vid huvudmätstationen för respektive studieområde (Stockholm: Södermalm, Malmö: Rådhuset, Göteborg: Femmanhuset). Även pollenhalter, väder, veckodag, tid på året mm har beaktats i analyserna. Uppgifter om akutbesök och läkemedelsuttag kommer från Socialstyrelsens register.Analysen med alla tre områden och hela befolkningen (alla åldrar) visar att akutbesök för astma påverkas av de senaste två dygnens halt av alla tre föroreningstyperna: partiklar som PM10, ozon och kvävedioxid, medan det för övriga akutbesök för andningsorganen enbart är PM10 som visar ett statistisk säkerställt samband totalt sett (alla orter och alla åldrar).I jämförelse med tidigare analysers resultat angående sambanden mellan PM10 och akutbesök för astma så ses ingen säkerställd trend sedan 2001, men den senaste perioden ger störst ökning med omkring 4% fler akutbesök per 10 μg/m3 för PM10 (alla åldrar).Medan det främst finns samband mellan partikelkoncentrationen som PM10 och akutbesök, ses för uttag av astmaläkemedel främst ett samband med halten av kvävedioxid, och särskilt för barn 0-17 år. För alla tre studieområdena och båda typerna av astmaläkemedel uppvisades liknande samband mellan NO2 och uttag av medicin, vilket kan motivera fördjupade analyser kring avgaskomponenter och mildare uttryck för astmabesvär än akutbesök.
