    • Landstinget har i samarbete med Karlstads universitet gjort flera befolkningsundersökningar i Värmland och publicerat rapporter om både den vuxna befolkningens och barns hälsa, senast med rapporten Värmlänningarnas Liv och Hälsa 2004. Någon befolkningsstudie av de äldres hälsa och livsvillkor har dock inte gjorts tidigare.Efter tre års förarbeten och samråd mellan landstinget, Värmlands kommuner och flera institutioner vid Karlstads universitet genomfördes den föreliggande studien av de äldres hälsa under senhösten 2006, i form av en omfattande postenkät. Urvalet utgjordes av 2500 slumpmässigt uttagna kvinnor och män som var 80 år eller äldre. Svarsfrekvensen var 60 procent, vilket får betraktas som mycket bra med tanke på att cirka 15 procent i denna åldersgrupp lever i särskilda boenden.Studien avspeglar således hälsan och livsvillkoren för de äldre. Överlag visar det sig att de äldre mår bra och har stor kapacitet att fungera som aktiva samhällsmedborgare. Samtidigt är det många som har kroniska besvär, med värk, syn- och hörselnedsättning, inkontinensbesvär etc. Det föreligger också en klar social gradient, där särskilt äldre kvinnor med arbetarbakgrund har både mer sjukdom och större ekonomiska problem än de övriga.

    • This study illuminates a deeper understanding of the processes of confidence building and emotion management among boundary spanners from organisations with a bureaucratic structure in a multinational civil-military operational context. Twenty-one informants (mainly military officers) were interviewed. They had a variety of experiences and occupational roles in civil-military collaboration contexts. A grounded theory analysis of interview data yielded a model according to which the most central aspects of civil-military collaboration are confidence building, maintaining existing confidence, avoiding distrust and rebuilding damaged confidence. The study shows that boundary spanners strategically utilise a variety of emotion management strategies in order to fulfil the demands laid upon them by their collaborating counterparts in the hostile environment and by their own organisation. Three interrelated dimensions of smoothness were identified: cultural, structural and smoothness in risky situations. By acting ‘smoothly’, an adaption to the dynamic environment can be achieved. The proposed model is compared to existing writings on emotional labour and self-presentation.

Pandemin blev en sjukvårdskatastrof men också en kris med sociala och emotionella konsekvenser för arbetstagare. Syftet med denna artikel, som har ett emotionssociologiskt perspektiv, är att sondera förekomsten av hot mot det sociala självet inom arbetslivet som tar sig uttryck i termer som signalerar skam eller skamrelaterade känslor och beteenden, som kan kopplas till pandemin. Hotet mot det sociala självet kan vara avsiktligt eller oavsiktligt och bestå av beteenden och gester som upplevs som nedsättande och i termer av mobbning, kränkningar, förödmjukelser, trakasserier eller organisatoriska ingrepp och förändringar.

  • Parental monitoring, peer activities and alcohol use : A study based on data on Swedish adolescents

    • Aim: This study investigates the association between two types of social relations during leisure time (to parents and peers) and the frequency of alcohol use among Swedish adolescents, taking possible interaction effects into account.Methods: The data were collected during the 1995–2005 time period by using a questionnaire handed out in the class room. The study includes about 10,000 Swedish adolescents aged 15–16 years.Results: The results show that there are strong associations between the social relations adolescents have during leisure time (both to parents and peers) and the frequency of alcohol use. High levels of peer activity were associated with higher frequencies of alcohol use. Although the effects of relations with parents were modified by peer activity frequencies, high levels of parental monitoring were significantly associated with lower frequencies of alcohol use, regardless of the peer activity frequencies.Conclusions: Parental monitoring is an efficient way to prevent or reduce adolescents’ alcohol use, although its importance may vary due to peer activity frequency.

    • Background: The purpose of this study was to analyse the connection between two types of social relations in school (to peers and to teachers) and psychosomatic health complaints among adolescents in school Year 9 in the Swedish compulsory school. In particular,              the focus is on the importance of students’ academic orientation as a possible modifier of the association between social relations and psychosomatic health complaints.Methods: The data were collected during the 1995–2005 time period from approximately 10 000 Swedish adolescents in the age of 15–16 years by using a questionnaire that was handed out in the class room.Results: There are strong associations between adolescents’ social relations in school (both to peers and to teachers) and psychosomatic health complaints. Worse relationships are connected to worse psychosomatic health. The health effects of teacher contacts                     were significantly modified by academic orientation; they were stronger for theoretically (i.e. those with better health) than for non-theoretically oriented students.Conclusion: Interpreted from a social class perspective, the results may reflect that the theoretically oriented students to a higher degree strive to conform to the culture present in school making this group of students more sensitive for teacher relations                     manifested as recognitions, rewards or penalties. In order to promote social equity in health, efforts to improve social relations in school should not solely focus on the teacher–student relationships but also on the relationships between peers.

    • The aim of this study was to analyse the link between psychosocial factors in the neighbourhood and work environments, and psychosomatic health problems.Methods   The data were collected in the survey “Life and Health ”, which was conducted in 2000 in six Swedish county councils. A total of 71,580 questionnaires were distributed to randomly selected individuals aged 18 –79. A total of 46,636 respondents completed the questionnaire. This gives a response rate of around 65%. For the purpose of this study only gainfully employed individuals aged 18 – 64 are included, which gives a total of 22,164 individuals: 11,247 (50.7%) women and 10,917 (49.3%) men. Two scales were used to measure the psychosocial environments in the neighbourhood and at work. The link between these scales and psychosomatic health problems was analysed by using multinomial logistic regression.Results  The results show that both “Psychosocial Neighbourhood Environment” (PNE) and “Psychosocial Working Environment” (PWE), independently, are related to psychosomatic health problems. Hence, the health effects of social relations in the neighbourhood were not modified by the quality of social relations at work, or vice versa. The levels of psychosomatic health problems are highest for people experiencing a low degree of social solidarity in the neighbourhood and for those experiencing low degrees of supportive work relationships.

  • BMI and psychological distress in 68, 000 Swedish adults : A weak association when controlling for an age-gender combination

    • Background Study results concerning associations between body mass index (BMI) and psychological distress are conflicting. The purpose of this study was to describe the shape of the association between BMI and psychological distress in a large sample of Swedish adults. MethodsData was measured with the General Health Questionnaire-12 (GHQ-12), in 68,311 adults aged 18–74. Self-reported data was derived from a merger of the 2000, 2004 and 2008 Life and Health (Liv och Hälsa) questionnaires focusing general health and living conditions. Logistic regression analysis was used to describe the association between BMI and psychological distress when controlled for age and gender in combination. Results Women reported an overall higher psychological distress than men. A significant pattern of decreasing psychological distress with increasing age emerged among women in all BMI categories. Trends of this same pattern showed for men. Small or no differences were seen in psychological distress between those in normal weight, overweight, and obesity I categories (among women: 20.4 %, 18.4 %, 20.5 %; among men: 12.8 %, 11.2 %, 12.9 %). For both genders, any notable increase in psychological distress appeared first in the obesity II category (among women: 27.2 %. Among men: 17.8 %). Conclusions Our results raise questions concerning cultural norms regarding body norms. Does aging increase norm resistance while youth increases norm sensitivity, especially among women? The finding that psychological distress indifference between normal weight and overweight also included the obesity I category should be a point of departure in a search for important cut-off points in the BMI/ psychological distress association. Results Women reported an overall higher psychological distress than men.   • Waiting management at the emergency department - a grounded theory study

    • Background: An emergency department (ED) should offer timely care for acutely ill or injured persons that require the attention of specialized nurses and physicians. This study was aimed at exploring what is actually going on at an ED. Methods: Qualitative data was collected 2009 to 2011 at one Swedish ED (ED1) with 53.000 yearly visits serving a population of 251.000. Constant comparative analysis according to classic grounded theory was applied to both focus group interviews with ED1 staff, participant observation data, and literature data. Quantitative data from ED1 and two other Swedish EDs were later analyzed and compared with the qualitative data. Results: The main driver of the ED staff in this study was to reduce non-acceptable waiting. Signs of non-acceptable waiting are physical densification, contact seeking, and the emergence of critical situations. The staff reacts with frustration, shame, and eventually resignation when they cannot reduce non-acceptable waiting. Waiting management resolves the problems and is done either by reducing actual waiting time by increasing throughput of patient flow through structure pushing and shuffling around patients, or by changing the experience of waiting by calming patients and feinting maneuvers to cover up. Conclusion: To manage non-acceptable waiting is a driving force behind much of the staff behavior at an ED. Waiting management is done either by increasing throughput of patient flow or by changing the waiting experience.

  • Financial stress in late adulthood and diverse risks of incident cardiovascular disease and all-cause mortality in women and men

    • Background: Financial stress may have adverse health effects. The main aim of this study was to investigate whether having a cash margin and living alone or cohabiting is associated with incident cardiovascular disease (CVD) and all-cause mortality. Methods: Representative population-based prospective cohort study of 60-year-old women (n = 2065) and men (n = 1939) in Stockholm County, Sweden. National registers were used to identify cases of incident CVD (n = 375) and all-cause mortality (n = 385). The presence of a cash margin was determined in the questionnaire with the following question: Would you, if an unexpected situation occurred, be able to raise 10 000 SEK within a week? (This was equivalent to US$ 1250 in 1998). Results: Compared with cohabiting women with a cash margin, the risk of all-cause mortality was higher among cohabiting women without a cash margin, with hazard ratios (HRs) of 1.97 (95% confidence interval (CI) 1.06-3.66). Using cohabiting men with cash margin as referent, single men without a cash margin were at an increased risk of both incident CVD and all-cause mortality: HR 2.84 (95% CI 1.61-4.99) and 2.78 (95% CI 1.69-4.56), respectively. Single men with cash margins still had an increased risk of all-cause mortality when compared with cohabiting men with a cash margin: HR 1.67 (95% CI 1.22-2.28). Conclusions: Financial stress may increase the risks of incident CVD and all-cause mortality, especially among men. Furthermore these risks are likely to be greater in men living in single households and in women without cash margins. Living with a partner seems to protect men, but not women, from ill-health associated with financial stress due to the lack of a cash margin.

  • Unresolved conflicts and shaming processes: risk factors for long-term sick leave for mental-health reasons

    • Mental illness is the most common diagnosis resulting in long-term sick leave in Sweden today, especially stress-related syndromes and mood disorders. The aim of this article is to analyse the relational and emotional processes in the workplace that may contribute to the understanding of long-term sick leave for mental-health reasons. We conducted interviews with twenty-six people who were on sick-leave because of diagnoses of mental ill-health. The empirical material was analysed using Classic Grounded Theory. We suggest that the risk of being afflicted with mental illness, and forced into long-term sick leave, increases when there are conflicts at work that remain unresolved and which lead to malignant shaming processes that jeopardize personal dignity. In their struggle to maintain self-esteem, the afflicted escalate their work efforts by increasing work intensity, putting in overtime, and working when ill. Eventually, this behaviour affects their health and results in sick-listing. The strengths and weaknesses of the study are discussed along with the need for further research.

  • Long-Term Sickness Absence Due to Burnout : Absentees' Experiences

    • In the late 1990s, there was a marked increase in Sweden in long-term sickness absence with mental diagnoses. In the extensive research on burnout, little attention has been paid to the link with sickness absence. Our aim in this study was to discover what caused burnout, or such severe stress that it led to long-term sickness absence (> 28 days). We interviewed 32 individuals on long-term sickness absence with a diagnosis of burnout and used grounded theory to analyze the data. We believe that events prior to the sickness absence can be understood as a process of emotional deprivation, and we described the process in terms of a flight of stairs with eight steps: “The Burnout Stairs.” We argue that the emotion conveying the process of emotional deprivation is unacknowledged shame, which has its basis in poor social relations at work. We discuss the study's limitations and the need for further research in this field.

    • Adolescent girls' subjective health, or well-being, is of international concern as the frequency of psychological and psychosomatic complaints is continuously increasing in several countries world-wide. The causes of this development are still obscure. The aim of this study was to explore well-being and strategies for increased well-being among adolescent girls. Grounded Theory method was used, in which in-depth interviews were held with 18 adolescent girls, 17-18 years of age. Results showed that striving for acceptance and avoiding rejection were central for their well-being. When rejection was experienced, emotions of stress-shame were recognized, a phenomena we call the stress-shame cycle. In the struggle to prevent rejection and to become accepted, the girls strived to boost their social attractiveness by impression management.

    • Aims: This study examined whether social capital and a sense of coherence are associated with hazardous alcohol use in a large population-based Swedish sample. In particular, the objectives were (a) to examine which of five subdimensions of social capital is associated with hazardous alcohol use, (b) to investigate the moderating role of sense of coherence and (c) to examine possible sex differences. Methods: A postal survey was distributed to a sample of respondents (aged 18-84 years) from five Swedish counties that was stratified by sex, age and city; 40,674 (59.2%) participants responded, of which 45.5% were men and 54.5% were women with a mean +/- SD age of 53.8 +/- 17.9 years. Results: Structural dimensions of social capital were associated with an increased probability of hazardous alcohol use among both men and women, whereas the increased probability associated with cognitive dimensions occurred mostly among women. Sense of coherence was robustly associated with a decreased probability of hazardous alcohol use among both men and women. There were few moderating effects of sense of coherence and sex differences emerged mainly for the cognitive dimension of social capital. Conclusions: Associations between social capital dimensions and hazardous alcohol use were partly sex-specific, whereas the benefits of a sense of coherence accrued to both sexes. Social capital dimensions and sense of coherence were generally unrelated to each other. Only associations between the cognitive dimensions of social capital and hazardous alcohol use differed by sex.

    • This study aimed to explore adolescent boys’ views of masculinity and emotion management and their potential effects on well-being. Interviews with 33 adolescent boys aged 16–17 years in Sweden were analysed using grounded theory. We found two main categories of masculine conceptions in adolescent boys: gender-normative masculinity with emphasis on group-based values, and non-gender-normative masculinity based on personal values. Gender-normative masculinity comprised two seemingly opposite emotional masculinity orientations, one towards toughness and the other towards sensitivity, both of which were highly influenced by contextual and situational group norms and demands, despite their expressions contrasting each other. Non-gender-normative masculinity included an orientation towards sincerity emphasising the personal values of the boys; emotions were expressed more independently of peer group norms. Our findings suggest that different masculinities and the expression of emotions are strongly intertwined and that managing emotions is vital for well-being.
