    • The spectrum of disorders of the brain is large, covering hundreds of disorders that are listed in either the mental or neurological disorder chapters of the established international diagnostic classification systems. These disorders have a high prevalence as well as short- and long-term impairments and disabilities. Therefore they are an emotional, financial and social burden to the patients, their families and their social network. In a 2005 landmark study, we estimated for the first time the annual cost of 12 major groups of disorders of the brain in Europe and gave a conservative estimate of €386 billion for the year 2004. This estimate was limited in scope and conservative due to the lack of sufficiently comprehensive epidemiological and/or economic data on several important diagnostic groups. We are now in a position to substantially improve and revise the 2004 estimates. In the present report we cover 19 major groups of disorders, 7 more than previously, of an increased range of age groups and more cost items. We therefore present much improved cost estimates. Our revised estimates also now include the new EU member states, and hence a population of 514 million people.

Our revised estimates also now include the new EU member states, and hence a population of 514 million people.AIMS: To estimate the number of persons with defined disorders of the brain in Europe in 2010, the total cost per person related to each disease in terms of direct and indirect costs, and an estimate of the total cost per disorder and country.METHODS: The best available estimates of the prevalence and cost per person for 19 groups of disorders of the brain (covering well over 100 specific disorders) were identified via a systematic review of the published literature. Together with the twelve disorders included in 2004, the following range of mental and neurologic groups of disorders is covered: addictive disorders, affective disorders, anxiety disorders, brain tumor, childhood and adolescent disorders (developmental disorders), dementia, eating disorders, epilepsy, mental retardation, migraine, multiple sclerosis, neuromuscular disorders, Parkinson's disease, personality disorders, psychotic disorders, sleep disorders, somatoform disorders, stroke, and traumatic brain injury. Epidemiologic panels were charged to complete the literature review for each disorder in order to estimate the 12-month prevalence, and health economic panels were charged to estimate best cost-estimates. A cost model was developed to combine the epidemiologic and economic data and estimate the total cost of each disorder in each of 30 European countries (EU27+Iceland, Norway and Switzerland). The cost model was populated with national statistics from Eurostat to adjust all costs to 2010 values, converting all local currencies to Euro, imputing costs for countries where no data were available, and aggregating country estimates to purchasing power parity adjusted estimates for the total cost of disorders of the brain in Europe 2010.RESULTS: The total cost of disorders of the brain was estimated at €798 billion in 2010. Direct costs constitute the majority of costs (37% direct healthcare costs and 23% direct non-medical costs) whereas the remaining 40% were indirect costs associated with patients' production losses. On average, the estimated cost per person with a disorder of the brain in Europe ranged between €285 for headache and €30,000 for neuromuscular disorders. The European per capita cost of disorders of the brain was €1550 on average but varied by country. The cost (in billion €PPP 2010) of the disorders of the brain included in this study was as follows: addiction: €65.7; anxiety disorders: €74.4; brain tumor: €5.2; child/adolescent disorders: €21.3; dementia: €105.2; eating disorders: €0.8; epilepsy: €13.8; headache: €43.5; mental retardation: €43.3; mood disorders: €113.4; multiple sclerosis: €14.6; neuromuscular disorders: €7.7; Parkinson's disease: €13.9; personality disorders: €27.3; psychotic disorders: €93.9; sleep disorders: €35.4; somatoform disorder: €21.2; stroke: €64.1; traumatic brain injury: €33.0. It should be noted that the revised estimate of those disorders included in the previous 2004 report constituted €477 billion, by and large confirming our previous study results after considering the inflation and population increase since 2004. Further, our results were consistent with administrative data on the health care expenditure in Europe, and comparable to previous studies on the cost of specific disorders in Europe. Our estimates were lower than comparable estimates from the US.DISCUSSION: This study was based on the best currently available data in Europe and our model enabled extrapolation to countries where no data could be found. Still, the scarcity of data is an important source of uncertainty in our estimates and may imply over- or underestimations in some disorders and countries. Even though this review included many disorders, diagnoses, age groups and cost items that were omitted in 2004, there are still remaining disorders that could not be included due to limitations in the available data. We therefore consider our estimate of the total cost of the disorders of the brain in Europe to be conservative. In terms of the health economic burden outlined in this report, disorders of the brain likely constitute the number one economic challenge for European health care, now and in the future. Data presented in this report should be considered by all stakeholder groups, including policy makers, industry and patient advocacy groups, to reconsider the current science, research and public health agenda and define a coordinated plan of action of various levels to address the associated challenges.RECOMMENDATIONS: Political action is required in light of the present high cost of disorders of the brain. Funding of brain research must be increased; care for patients with brain disorders as well as teaching at medical schools and other health related educations must be quantitatively and qualitatively improved, including psychological treatments. The current move of the pharmaceutical industry away from brain related indications must be halted and reversed. Continued research into the cost of the many disorders not included in the present study is warranted. It is essential that not only the EU but also the national governments forcefully support these initiatives.

  • Industrial cleaning with Qlean Water : a case study of printed circuit boards

    • Many manufacturing companies are looking for ways to substitute environmentally problematic cleaning methods for surface treatments with more environmentally friendly ones. In this paper, one potential solution is described. The Qlean method, based on cleaning with highly pure water (in this paper defined as Qlean Water), is a novel cleaning method. This method, now utilized at one plant at a leading major international electronic company, has substituted previous chemical-based methods for cleaning printed circuit boards prior to lacquering. This paper presents, based on that company's primary data, a comparative study using environmental analysis and economic life cycle cost review between cleaning with Qlean Water and conventional cleaning. The focus is on the environmental and economic performance of the two alternatives. The conclusion is that Qlean Water offers both a significant economic and environmental cost reduction and a better product. This is the case even though all identified economic benefits derived from using Qlean Water, e.g. that the quality and technical lifetime have been extended for the printed circuit boards with the Qlean Water cleaning method, are not considered in the economic analysis.

  • Utvärdering av Kunskapsprogrammet Hållbar Sanering

    • Kunskapsprogrammet Hållbar Sanering är en del av Naturvårdsverkets arbete med att nå miljömålet Giftfri miljö. Genom Hållbar Sanering har myndigheter, forskare och företag fått bidrag för att bygga upp och sprida kunskap om efterbehandling av förorenade mark- och vattenområden. Via Naturvårdsverkets hemsida kan all tillgänglig information inhämtas om kunskapsprogrammet Hållbar Sanering, inklusive samtliga projektrapporter och resultatet av syntesarbetet. Totalt har i kunskapsprogrammet genom tre utlysningar 2004 - 2006 finansierat 54 projekt (56 rapporter) inom områdena Undersökningsmetoder, Riskbedömning, Riskvärdering, Riskkommunikation och Åtgärdslösningar. Närmare hälften av projekten avser riskbedömning. En utvärdering har på Naturvårdsverkets uppdrag genomförts av Professional Management AB under perioden januari – april 2009. Utvärderingen visar att Hållbar Sanering har gjort en betydande insats för kunskapsförsörjningen, särskilt när det gäller riskbedömning och riskvärdering. Den ena utvärderingsfrågan handlar om i vilken utsträckning kunskapsprogrammets syfte har uppnåtts. Enligt 70 % av respondenterna i de primära målgrupperna (Naturvårdsverket, SGI, SGU, länsstyrelser, konsulter, forskare och forskningsfinansiärer) har syftet med Hållbar Sanering uppnåtts helt, nästan helt eller i stor utsträckning. Genom kunskapsprogrammet Hållbar Sanering har branschen fått ny kunskap och resultat kan användas i arbetet med att efterbehandla förorenad mark. Det finns en bred samstämmighet bland respondenterna om att Hållbar Sanering har gett användbara resultat för målgrupperna. Nio av tio instämmer helt eller delvis i att Hållbar Sanering erbjuder kunskap och resultat till nytta för myndigheter, forskare och företag. Närmare tre fjärdedelar av respondenterna har redan i viss eller betydande utsträckning själva använt resultatet från Hållbar Sanering i sitt arbete. Tre fjärdedelar anger även att de har haft stor nytta eller viss nytta av dessa resultat. Det bör noteras att denna utvärdering har genomförts innan kunskapsprogrammet är helt avslutat. Det är därför för tidigt att förvänta sig full effekt av programmet. Trots detta kan således påvisas att programmet redan har skapat verifierbar nytta hos målgrupperna. Den andra utvärderingsfrågan gäller arbetssättet. Två av tre respondenter i de primära målgrupperna anser att arbetssättet i stor eller mycket stor utsträckning har varit ändamålsenligt för att öka kunskapen. Färre bedömer dock att arbetssättet hittills har varit ändamålsenligt för att ge underlag för att prioritera insatser. I detta avseende är det bara 28 % som anser att arbetssättet i stor eller mycket stor utsträckning har varit ändamålsenligt. Detta kan möjligen sammanhänga med att tyngdpunkten i kunskapsprogrammet har legat på projekt som gäller riskbedömning och riskvärdering medan få projekt har gällt åtgärdslösningar. För att kunna göra prioriteringar krävs att det går att ställa kostnadseffektiviteten i olika åtgärdslösningar mot varandra. Här uppfattas Hållbar Sanering inte ha bidragit med det underlag som är nödvändigt för beslutsfattarna. Syntesarbetet har dock lett fram till att Naturvårdsverket vet inom vilka områden (inom ramen för programmets kunskapsområden) det finns ytterligare kunskapsbehov. En eventuell fortsatt satsning bör inriktas mot åtgärdslösningar. Samtliga intressenter som har medverkat i arbetet gör bedömningen att kunskapsprogrammet Hållbar Sanering har fokuserats på rätt områden och att områden med viktiga kunskapsluckor har prioriterats i utlysningarna. Det finns en bra balans mellan tekniska, ekonomiska och samhälleliga aspekter. De projekt som har fått stöd synes ha täckt prioriterade kunskapsluckor inom större delen av dessa områden. Dock skulle det ha funnits behov av mer insatser vad gäller åtgärdstekniker, varför Naturvårdsverket bör överväga att använda en del av anslaget för efterbehandlingsarbetet för en ny projektomgång som tydligt är inriktad mot åtgärdsteknik och annan empiri som inte har täckts av Hållbar Sanering. Medan Hållbar Sanering har haft en bred inriktning bör således en ny utlysning vara betydligt smalare och fokusera på ett fåtal frågeställningar av tillämpningskaraktär. Både programkommittén, projektledaren och beredningsgruppen i övrigt har enligt samstämmiga uppgifter genomfört sitt uppdrag på ett mycket bra sätt. Programkommittén har haft en bred sammansättning, vilket har skapat goda förutsättningar för en gemensam kunskapsutveckling. Dock har representant för konsulterna saknats i programkommittén. Generellt sett har arbetssättet inom kunskapsprogrammet varit ändamålsenligt. Programmet har varit välorganiserat och arbetet har lagts upp på ett målmedvetet sätt. Resultatet är av stort värde för intressenterna. Publiceringen av rapporterna på Naturvårdsverkets hemsida anses ha fungerat bra. Den bristande tidshållningen i vissa av projekten och i syntesarbetet borde dock ha kunnat motverkas genom kraftfullare styrning och tydligare incitament. Vidare kunde programmet ha marknadsförts mera offensivt både i utlysningsfasen och i spridningen av resultaten. Materialet från Hållbar Sanering kommer inte att uppdateras eller på annat sätt ändras efter det att programmet är avslutat, men materialet kommer att finnas tillgängligt för alla intressenter för sökning i Naturvårdsverkets webbibliotek. Därutöver kan det finnas skäl att kontinuerligt sprida riktad information om att materialet som helhet finns och om enskilda rapporter till specifika målgrupper. Vissa rapporter kommer att vara aktuella under längre tid, medan det på andra områden relativt snabbt kan komma fram ny kunskap. Naturvårdsverket bör därför systematiskt analysera vad som kan användas som underlag för verkets framtida vägledningsarbete. Beslutsfattare och utförare har behov av tydliga rekommendationer utifrån dagens tillgängliga kunskap. En del av de kunskapsluckor som återstår kan täckas genom internationellt samarbete. Detta gäller inte minst grundläggande studier av föroreningars öde i mark och vatten och olika toxikologiska effekter. Den kunskap som tas fram internationellt kan anpassas till svenska förhållanden och föras vidare av t.ex. forskare (universitet eller forskningsinstitut), konsulter, entreprenörer eller handläggare på en myndighet eller annan organisation. I utvärderingen skisseras också avslutningsvis tre alternativ när det gäller en senare effektstudie.

    • The role of exosomes in cancer can be inferred from the observation that they transfer tumor cell derived genetic material and signalling proteins, resulting in e.g. increased tumor angiogenesis and metastasis. However, the membrane transport mechanisms and the signalling events involved in the uptake of these virus-like particles remain ill-defined. We now report that internalization of exosomes derived from glioblastoma (GBM) cells involves nonclassical, lipid raft-dependent endocytosis. Importantly, we show that the lipid raft associated protein caveolin-1 (CAV1), in analogy with its previously described role in virus uptake, negatively regulates the uptake of exosomes. We find that exosomes induce the phosphorylation of several downstream targets known to associate with lipid rafts as signalling and sorting platforms, such as extracellular signal-regulated kinase-1/2 (ERK1/2) and heat shock protein 27 (HSP27). Interestingly, exosome uptake appears dependent on unperturbed ERK1/2-HSP27 signalling, and ERK1/2 phosphorylation is under negative influence by CAV1 during internalization of exosomes. These findings significantly advance our general understanding of exosome-mediated uptake and offer potential strategies for how this pathway may be targeted through modulation of CAV1 expression and ERK1/2 signaling.

    • We present detailed continuous wave (cw) and transient photoinduced absorption (PA) measurements in thin films of a novel alternating polyfluorene copolymer, poly[2,7-(9,9-dioctyl-fluorene)-alt-5,5-(4',7'-di-2-thienyl-2',1',3-benzo-thiadiazole)] (DiO-PFDTBT), and its blends with the sol. fullerene deriv. [6,6]-phenyl-C61-butyric acid Me ester (PCBM) in wt. ratios of 1:0, 4:1 and 1:4. We measure the frequency, intensity and temp. dependence of the PA signal in the frequency domain, and compare with the results obtained from the transient PA decay measurements in the time domain. In all blends, the PA spectrum shows a broad high energy PA band ranging from .apprx.1 eV to 2 eV as well as a low energy band peaking at .apprx.0.35 eV. We attribute the low energy band to the P1 transition of polarons and part of the high energy band to the correlated P2 transition of polarons. Both frequency and time domain measurements show that the high energy band has two decay components, a faster component in the microsecond time regime and a slower component in the millisecond time regime. The slow component is strongly dispersive, whereas the fast component is practically non-dispersive. [on SciFinder (R)]

    • This text provides a brief overview of the principles of enzymatic lipid conversion and some recent advances in the enzymatic conversion of glycerophospholipids and galactolipids. Lipases and phospholipases are used to exchange fatty acids or the polar group in the lipids. The reactions can be carried out either as hydrolysis-esterification sequences or as one-step transferase reactions. The scope and limitations of the different methods are discussed.

    • Rheumatoid arthritis (RA) is an immune-mediated disease affecting diarthrodial joints that remains an unmet medical need despite improved therapy. This limitation likely reflects the diversity of pathogenic pathways in RA, with individual patients demonstrating variable responses to targeted therapies. Better understanding of RA pathogenesis would be aided by a more complete characterization of the disease. To tackle this challenge, we develop and apply a systems biology approach to identify important transcription factors (TFs) in individual RA fibroblast-like synoviocyte (FLS) cell lines by integrating transcriptomic and epigenomic information. Based on the relative importance of the identified TFs, we stratify the RA FLS cell lines into two subtypes with distinct phenotypes and predicted activepathways. We biologically validate these predictions for the top subtype-specific TF RARα and demonstrate differential regulation of TGFβ signaling in the two subtypes. This study characterizes clusters of RA cell lines with distinctive TF biology by integrating transcriptomic and epigenomic data, which could pave the way towards a greater understanding of disease heterogeneity.

  • Illuminating Genetic Mysteries of the Dead Sea Scrolls

    • The discovery of the 2,000-year-old Dead Sea Scrolls had an incomparable impact on the historical understanding of Judaism and Christianity. "Piecing together'' scroll fragments is like solving jigsaw puzzles with an unknown number of missing parts. We used the fact that most scrolls are made from animal skins to "fingerprint'' pieces based on DNA sequences. Genetic sorting of the scrolls illuminates their textual relationship and historical significance. Disambiguating the contested relationship between Jeremiah fragments supplies evidence that some scrolls were brought to the Qumran caves from elsewhere; significantly, they demonstrate that divergent versions of Jeremiah circulated in parallel throughout Israel (ancient Judea). Similarly, patterns discovered in non-biblical scrolls, particularly the Songs of the Sabbath Sacrifice, suggest that the Qumran scrolls represent the broader cultural milieu of the period. Finally, genetic analysis divorces debated fragments from the Qumran scrolls. Our study demonstrates that interdisciplinary approaches enrich the scholar's toolkit.

    • Survivin is a proto-oncogene that regulates cell division and apoptosis. It is a molecular marker of cancer. Recently, survivin has emerged as a feature of RA, associated with severe joint damage and poor treatment response. The present study examined if inhibition of survivin affects experimental arthritis, which was induced in mBSA-immunized mice by an injection of mBSA in the knee joint or developed spontaneously in collagen type II-immunized mice. The inhibition of survivin transcription by a lentivirus shRNA construct alleviated joint inflammation and reduced bone damage. The inhibition of survivin reduced the levels of metalloproteinases, β-catenin, and vimentin, limiting the invasive capacity of synovia, while no inhibition of osteoclastogenesis could be found. The inhibition of survivin led to a p53-independent reduction of T cell proliferation and favored the transcription and activity of Blimp-1, which limited IL-2 production and facilitated formation of regulatory Foxp3(+)CD4(+) and effector CD8(+) T cells. The study shows that the inhibition of survivin is sufficient to reduce joint inflammation and bone damage in preclinical models of arthritis. Antiarthritic effects of survivin inhibition are related to p53-independent control of lymphocyte proliferation.

    • Follicular T helper (Tfh) cells are recognized by the expression of CXCR5 and the transcriptional regulator Bcl-6. Tfh cells control B cell maturation and antibody production, and if deregulated, may lead to autoimmunity. Here, we study the role of the proto-oncogene survivin in the formation of Tfh cells. We show that blood Tfh cells of patients with the autoimmune condition rheumatoid arthritis, have intracellular expression of survivin. Survivin was co-localized with Bcl-6 in the nuclei of CXCR5(+)CD4 lymphocytes and was immunoprecipitated with the Bcl-6 responsive element of the target genes. Inhibition of survivin in arthritic mice led to the reduction of CXCR5(+) Tfh cells and to low production of autoantibodies. Exposure to survivin activated STAT3 and induced enrichment of PD-1(+)Bcl-6(+) subset within Tfh cells. Collectively, our study demonstrates that survivin belongs to the Tfh cell phenotype and ensures their optimal function by regulating transcriptional activity of Bcl-6.

    • Seed-eating Apion weevils (Coleoptera: Apionidae) cause large economic losses in white and red clover seed production across Europe. Monitoring and control of clover weevils would be facilitated by semiochemical-based methods. Until now, however, nothing was known about physiological or behavioral responses to semiochemicals in this insect group. Here we analyzed the antenna of the white clover (Trifolium repens L.) specialist Apion fulvipes Geoffroy with scanning electron microscopy, and used single sensillum recordings with a set of 28 host compounds to characterize 18 classes of olfactory sensory neurons (OSNs). Nine of the OSN classes responded strongly to synthetic compounds with high abundance in clover leaves, flowers, or buds. Eight classes responded only weakly to the synthetic stimuli, whereas one collective class responded exclusively to volatiles released from a crushed clover leaf. The OSNs showed a remarkable degree of specificity, responding to only one or a few chemically related compounds. In addition, we recorded a marked difference in the temporal dynamics of responses between different neurons, compounds, and doses. The identified physiologically active compounds will be screened for behavioral activity, with the ultimate goal to develop an odor-based control strategy for this pest.

    • Survivin is known as an inhibitor of apoptosis and a positive regulator of cell division. We have recently identified survivin as a predictor of joint destruction in patients with rheumatoid arthritis (RA). Flt3 ligand (Flt3L) is expressed in the inflamed joints and has adjuvant properties in arthritis. Studies on 90 RA patients (median age 60.5 years [range, 24-87], disease duration 10.5 years [range, 0-35]) show a strong positive association between the levels of survivin and Flt3L in blood. Here, we present experimental evidence connecting survivin and Flt3L signaling. Treatment of BALB/c mice with Flt3L led to an increase of survivin in the bone marrow and in splenic dendritic cells. Flt3L changed the profile of survivin splice variants, increasing transcription of the short survivin40 in the bone marrow. Treatment with an Flt3 inhibitor reduced total survivin expression in bone marrow and in the dendritic cell population in spleen. Inhibition of survivin transcription in mice, by shRNA lentiviral constructs, reduced the gene expression of Flt3L. We conclude that expression of survivin is a downstream event of Flt3 signaling, which serves as an essential mechanism supporting survival of leukocytes during their differentiation, and maturation of dendritic cells, in RA.

This is done by either defocusing a single laser pulse or by using a split-pulse setup and irradiating the target with two identical laser pulses with variable spatial separation. The resulting proton beam profile and the energy spectrum are recorded as functions of the focal spot size of the single laser pulse and of the separation between the two pulses. A shaping of the resulting proton beam profile, related to both an increase in flux of low-energy protons in the target normal direction and a decrease in their divergence, in one or two dimensions, is observed. The results are explained by simple modelling of rear surface sheath field expansion, ionization, and projection of the resulting proton beam.

    • Background: In animal models, early initiation of therapeutic cooling, intra-arrest, or restored circulation has been shown to be neuroprotective shortly after cardiac arrest. We aimed to assess the feasibility and cooling efficacy of transnasal evaporative cooling, initiated as early as possible after hospital arrival in patients randomized to cooling in the TTM2 trial. Methods: This study took the form of a single-center (Södersjukhuset, Stockholm) substudy of the TTM2 trial (NCT02908308) comparing target temperature management (TTM) to 33 °C versus normothermia in OHCA. In patients randomized to TTM33 °C, transnasal evaporative cooling was applied as fast as possible. The primary objectives were the feasibility aspects of initiating cooling in different hospital locations (i.e., in the emergency department, coronary cathlab, intensive care unit (ICU), and during intrahospital transport) and its effectiveness (i.e., time to reach target temperature). Transnasal cooling was continued for two hours or until patients reached a core temperature of <34 °C. Cooling intervals were compared to participants at the same site who were randomized to hypothermia and treated at 33 °C but who for different reasons did not receive transnasal evaporative cooling. Results: From October 2018 to January 2020, 32 patients were recruited, of which 17 were randomized to the TTM33. Among them, 10 patients (8 men, median age 69 years) received transnasal evaporative cooling prior to surface systemic cooling in the ICU. In three patients, cooling was started in the emergency department; in two patients, it was started in the coronary cathlab, and in five patients, it was started in the ICU, of which three patients were subsequently transported to the coronary cathlab or to perform a CT scan. The median time to initiate transnasal cooling from randomization was 9 min (range: 5 to 39 min). The median time from randomization to a core body temperature of 34 °C was 120 min (range 60 to 334) compared to 178 min among those in the TTM33 group that did not receive TNEC and to 33 °C 230 min (range: 152 to 351) vs. 276 min (range: 150 to 546). No feasibility or technical issues were reported. No adverse events occurred besides minor nosebleeds. Conclusions: The early induction of transnasal cooling in out-of-hospital cardiac arrest patients was feasible to initiate in the emergency department, coronary cathlab, ICU, and during intrahospital transport. Time to target temperature was shortened compared to standard cooling.

Det krävs ofta långa produktionsserier för att det ska bli en rimlig investering att ställa om produktionen. Därför används människor för tillverkningsuppgifter som ofta ställs om. Denna avhandling fokuserar på konceptet Plug & Produce, som gör det enklare att flytta, lägga till och ta bort resurser från ett tillverkningssystem. Tanken är att resurser placeras i processmoduler som alla har samma fysiska gränssnitt för att kopplas in i tillverkningssystemet. Styrningen av tillverkningssystemet görs av ett multiagentsystem där varje detalj som ska produceras för produkter får en egen agent som representerar detaljen och agerar som styrningsmjukvara. Varje detaljs agent tar hand on sina egna tillverkningsmål genom att kommunicera med resursagenter i systemet som används för styrning av resurserna. I detta arbete, presenteras ett ramverk för Plug & Produce som består av ett konfigurerbart multiagentsystem, samt ett konfigurationsverktyg som kan användas för att definiera agenterna. Arbetet inkluderar metoder för att identifiera inkopplade resurser, kommunikation mellan agenter, schemaläggning som kan undvika konflikter mellan agenter, samt metoder för att automatiskt hitta vägar för transport genom tillverkningssystemet.

A configuration tool is also designed that works with the proposed method by connecting to an agent configuration database. The overall aim of the method is to simplify the steps taken for adapting a manufacturing system for new parts and resources.

    • This thesis presents methods for simplifying the use of multi-agent systems in Plug & Produce. The demand for customized products and low volume production is constantly increasing. The industry has for many years used dedicated manufacturing systems where it is difficult and expensive to adapt to new product designs. Instead, factories are forced to use human workers for certain tasks that demand high flexibility and rapid adaption for new product designs. Several solutions have been proposed over the years to create highly flexible automation systems that automatically handles rapid adaption for new products. A concept called Plug & Produce aims at creating a system where resources and parts can be added in minutes rather than days in dedicated systems. One promising solution for implementing Plug & Produce is the distributed approach called multi-agent systems, where each resource and part get its own controller that communicates with each other to reach manufacturing goals. The idea is that the system automatically handles the adaption for new products. However, still today the use of such systems is extremely limited in the industry. One reason is the lack of mature multi-agent systems that are easy to use and that hides the complexity of the underlying agent system from the users. This is a huge problem since these systems tend to be more complex than traditional approaches. Thus, this thesis focuses on simplifying the use of multi-agent systems by proposing various methods for bringing the multi-agent technology for Plug & Produce closer to the industry.

    • This article presents a framework for Plug & Produce that makes it possible to use configurations rather than programming to adapt a manufacturing system for new resources and parts. This is solved by defining skills on resources, and goals for parts. To reach these goals, process plans are defined with a sequence of skills to be utilized without specifying specific resources. This makes it possible to separate the physical world from the process plans. When a process plan requires a skill, e.g., grip with a gripper resource, then that skill may require further skills, e.g., move with a robot resource. This creates a tree of connected resources that are not defined in the process plan. Physical and logical compatibility between resources in this tree is checked by comparing several parameters defined on the resources and the part. This article presents an algorithm together with a multiagent system framework that handles the search and matching required for selecting the correct resources.

    • This paper describes a method together with an implementation for automating the detection, identification and configuration of newly added resources and parts in a Plug and Produce system using OPC UA. In a Plug and Produce system, resources and parts are usually controlled by agents, forming a multi-agent system of collaborating resources. Hence, when a resource or part is connected to the system, a corresponding agent must be instantiated and associated with that specific device. In order to automate this, the system needs information about newly connected devices. This information could, for example, be positional data describing where the device is connected. Some devices like tools and parts to be processed have no own network connection, but still, they should get an agent with correct configuration instantiated. In this work, OPC UA is used for communication between devices and the corresponding agents. All agents and their communication are handled by an Agent Handling System, consisting of an OPC UA HUB together with functions for device detection and agent instantiation. The HUB is used for transferring data between devices and their agents in the network by OPC UA protocols. When a device is connected to the network, it is detected, and a connection is automatically created to the HUB that becomes configured for transmitting data between the device and its corresponding agent. © 2019 The Authors.     • Importance  Smartphone dispatch of volunteer responders to nearby out-of-hospital cardiac arrests (OHCAs) has emerged in several emergency medical services, but no randomized clinical trials have evaluated the effect on bystander use of automated external defibrillators (AEDs).Objective  To evaluate if bystander AED use could be increased by smartphone-aided dispatch of lay volunteer responders with instructions to collect nearby AEDs compared with instructions to go directly to patients with OHCAs to start cardiopulmonary resuscitation (CPR).Design, Setting, and Participants  This randomized clinical trial assessed a system for smartphone dispatch of volunteer responders to individuals experiencing OHCAs that was triggered at emergency dispatch centers in response to suspected OHCAs and randomized 1:1. The study was conducted in 2 main Swedish regions: Stockholm and Västra Götaland between December 2018 and January 2020. At study start, there were 3123 AEDs in Stockholm and 3195 in Västra Götaland and 24 493 volunteer responders in Stockholm and 19 117 in Västra Götaland. All OHCAs in which the volunteer responder system was activated by dispatchers were included. Excluded were patients with no OHCAs, those with OHCAs not treated by the emergency medical services, and those with OHCAs witnessed by the emergency medical services.Interventions  Volunteer responders were alerted through the volunteer responder system smartphone application and received map-aided instructions to retrieve nearest available public AEDs on their way to the OHCAs. The control arm included volunteer responders who were instructed to go directly to the OHCAs to perform CPR.Main Outcomes and Measures  Overall bystander AED attachment, including those attached by volunteer responders and lay volunteers who did not use the smartphone application.Results  Volunteer responders were activated for 947 patients with OHCAs. Of those, 461 were randomized to the intervention group (median [IQR] age of patients, 73 [61-81] years; 295 male patients [65.3%]) and 486 were randomized to the control group (median [IQR] age of patients, 73 [63-82] years; 312 male patients [65.3%]). Primary outcome of AED attachment occurred in 61 patients (13.2%) in the intervention arm vs 46 patients (9.5%) in the control arm (difference, 3.8% [95% CI, −0.3% to 7.9%]; P = .08). The majority of AEDs were attached by lay volunteers who were not using the smartphone application (37 in intervention arm, 28 in control). There were no significant differences in secondary outcomes. Among the volunteer responders using the application, crossover was 11% and compliance to instructions was 31%. Volunteer responders attached 38% (41 of 107) of all AEDs and provided 45% (16 of 36) of all defibrillations and 43% (293 of 666) of all CPR.Conclusions and Relevance  In this study, smartphone dispatch of volunteer responders to OHCAs to retrieve nearby AEDs vs instructions to directly perform CPR did not significantly increase volunteer AED use. High baseline AED attachement rate and crossover may explain why the difference was not significant.Trial Registration  ClinicalTrials.gov Identifier: NCT02992873

    • Background: Systems for smartphone dispatch of lay responders to perform cardio-pulmonary resuscitation (CPR) and bring automated external defibrillators to out-of-hospital cardiac arrests (OHCAs) are advocated by recent international guidelines and emerging worldwide. Objectives: This study aimed to investigate the emotional responses, posttraumatic stress reactions and levels of wellbeing among smartphone-alerted lay responders dispatched to suspected OHCAs. Methods: Lay responders were stratified by level of exposure: unexposed (Exp-0), tried to reach (Exp-1), and reached the suspected OHCA (Exp-2). Participants rated their emotional responses online, at 90 minutes and at 4–6 weeks after an incident. Level of emotional response was measured in two dimensions of core affect: “alertness” – from deactivation to activation, and “pleasantness” – from unpleasant to pleasant. At 4–6 weeks, WHO wellbeing index and level of posttraumatic stress (PTSD) were also rated. Results: Altogether, 915 (28%) unexposed and 1471 (64%) exposed responders completed the survey. Alertness was elevated in the exposed groups: Exp-0: 6.7 vs. Exp-1: 7.3 and Exp-2: 7.5, (p < 0.001) and pleasantness was highest in the unexposed group: 6.5, vs. Exp-1: 6.3, and Exp-2: 6.1, (p < 0.001). Mean scores for PTSD at follow-up was below clinical cut-off, Exp-0: 9.9, Exp-1: 8.9 and Exp-2: 8.8 (p = 0.065). Wellbeing index showed no differences, Exp-0: 78.0, Exp-1: 78.5 and Exp-2: 79.9 (p = 0.596). Conclusion: Smartphone dispatched lay responders rated the experience as high-energy and mainly positive. No harm to the lay responders was seen. The exposed groups had low posttraumatic stress scores and high-level general wellbeing at follow-up. © 2021

    • BACKGROUND: Coronary atherosclerosis detected by imaging is a marker of elevated cardiovascular risk. However, imaging involves large resources and exposure to radiation. The aim was, therefore, to test whether nonimaging data, specifically data that can be self-reported, could be used to identify individuals with moderate to severe coronary atherosclerosis.METHODS AND RESULTS: We used data from the population-based SCAPIS (Swedish CardioPulmonary BioImage Study) in individuals with coronary computed tomography angiography (n=25 182) and coronary artery calcification score (n=28 701), aged 50 to 64 years without previous ischemic heart disease. We developed a risk prediction tool using variables that could be assessed from home (self-report tool). For comparison, we also developed a tool using variables from laboratory tests, physical examinations, and self-report (clinical tool) and evaluated both models using receiver operating characteristic curve analysis, external validation, and benchmarked against factors in the pooled cohort equation. The self-report tool (n=14 variables) and the clinical tool (n=23 variables) showed high-to-excellent discriminative ability to identify a segment involvement score ≥4 (area under the curve 0.79 and 0.80, respectively) and significantly better than the pooled cohort equation (area under the curve 0.76, P<0.001). The tools showed a larger net benefit in clinical decision-making at relevant threshold probabilities. The self-report tool identified 65% of all individuals with a segment involvement score ≥4 in the top 30% of the highest-risk individuals. Tools developed for coronary artery calcification score ≥100 performed similarly.CONCLUSIONS: We have developed a self-report tool that effectively identifies individuals with moderate to severe coronary atherosclerosis. The self-report tool may serve as prescreening tool toward a cost-effective computed tomography-based screening program for high-risk individuals.

  • Self-Report Tool for Identification of Individuals With Coronary Atherosclerosis : The Swedish CardioPulmonary BioImage Study

    • BACKGROUND: Coronary atherosclerosis detected by imaging is a marker of elevated cardiovascular risk. However, imaging involves large resources and exposure to radiation. The aim was, therefore, to test whether nonimaging data, specifically data that can be self-reported, could be used to identify individuals with moderate to severe coronary atherosclerosis.METHODS AND RESULTS: We used data from the population-based SCAPIS (Swedish CardioPulmonary BioImage Study) in individuals with coronary computed tomography angiography (n=25 182) and coronary artery calcification score (n=28 701), aged 50 to 64 years without previous ischemic heart disease. We developed a risk prediction tool using variables that could be assessed from home (self-report tool). For comparison, we also developed a tool using variables from laboratory tests, physical examinations, and self-report (clinical tool) and evaluated both models using receiver operating characteristic curve analysis, external validation, and benchmarked against factors in the pooled cohort equation. The self-report tool (n=14 variables) and the clinical tool (n=23 variables) showed high-to-excellent discriminative ability to identify a segment involvement score ≥4 (area under the curve 0.79 and 0.80, respectively) and significantly better than the pooled cohort equation (area under the curve 0.76, P<0.001). The tools showed a larger net benefit in clinical decision-making at relevant threshold probabilities. The self-report tool identified 65% of all individuals with a segment involvement score ≥4 in the top 30% of the highest-risk individuals. Tools developed for coronary artery calcification score ≥100 performed similarly. CONCLUSIONS: We have developed a self-report tool that effectively identifies individuals with moderate to severe coronary atherosclerosis. The self-report tool may serve as prescreening tool toward a cost-effective computed tomography-based screening program for high-risk individuals.

    • Rheumatoid arthritis-associated joint pain is frequently observed independent of disease activity, suggesting unidentified pain mechanisms. We demonstrate that antibodies binding to cartilage, specific for collagen type II (CII) or cartilage oligomeric matrix protein (COMP), elicit mechanical hypersensitivity in mice, uncoupled from visual, histological and molecular indications of inflammation. Cartilage antibody-induced pain-like behavior does not depend on complement activation or joint inflammation, but instead on tissue antigen recognition and local immune complex (IC) formation. smFISH and IHC suggest that neuronal Fcgr1 and Fcgr2b mRNA are transported to peripheral ends of primary afferents. CII-ICs directly activate cultured WT but not FcRγ chain-deficient DRG neurons. In line with this observation, CII-IC does not induce mechanical hypersensitivity in FcRγ chain-deficient mice. Furthermore, injection of CII antibodies does not generate pain-like behavior in FcRγ chain-deficient mice or mice lacking activating FcγRs in neurons. In summary, this study defines functional coupling between autoantibodies and pain transmission that may facilitate the development of new disease-relevant pain therapeutics.

    • A growing concern over incidents of widespread human exposure to arsenic (As) from groundwater sources has been noticed during the past three decades. Vaasterbotten county in northern Sweden hosts a large number of sulphide ore deposits and a number of gold deposits are recently discovered. Both are accompanied by elevated arsenic contents. Proterozoic metasediments sandwiched in the bedrock and mixed into the till contains elevated amounts of arsenic as well. During the present study about 80 groundwater samples were collected from dug wells, bore-wells and springs in the Skellefte field in Vasterbotten County in northern Sweden. Data from community environmental offices were also collected and included in the study. Arsenic concentrations were elevated in borewells and wetland springs while none of the dug wells had arsenic contents above 10 mg/l. The highest content seen in borewells was 300 mg/l and in wetland springs 100 mg/l. The As(III)/As(tot) varied largely in borewells while it was mostly above 0.8 in wetland springs indicating more reducing contents in the latter. The use of a redox classification indicated that two nechanisms were involved in the mobilisation of he arsenic, oxidation of sulphides and reduction of ferric oxyhydroxides. In some cases the borewells showed a mixed pattern, indicating inflow from different environments.

    • Matrix metalloproteinase (MMP)-12, S100A8/A9, and S100A12 are involved in innate immune responses. We addressed whether different aspects of oral health and non-disease-related covariates influence their levels in saliva. 436 participants were clinically examined, completed a health questionnaire, and provided stimulated saliva. Salivary levels of MMP-12, S100A8/A9, and S100A12 were determined by enzyme-linked immunosorbent assays. Lower MMP-12 levels were observed in individuals 40-64years old (yo) compared to < 40yo, and higher S100A8/A9 levels were found in individuals > 64yo compared to 40-64yo. Smokers exhibited lower MMP-12 and S100A12 levels compared to non-smokers. All three proteins were elevated in individuals with bleeding on probing (BOP)>20% compared to those with BOP/= 10% gingival pocket depths (PPD)>/=4mm compared to the ones with shallow pockets < 4mm. The extent of alveolar bone loss or presence of manifest caries did not alter any of the markers. MMP-12, S100A8/A9, and S100A12 levels were higher in participants with high periodontal inflammatory burden. All three proteins correlated positively to BOP, PPD, and to several inflammatory mediators. The explanatory variables for MMP-12 in saliva were age, smoking, presence of any tumor, and percentage of PPD>/=4mm. The determinant of salivary S100A8/A9 was percentage of BOP, while S100A12 levels were associated with percentage of BOP and presence of any tumor. Taken together, MMP-12 and the S100/calgranulin levels in saliva reflect different aspects of periodontal inflammation. Smoking and age should be taken into account in further investigation of these proteins as biomarker candidates of periodontal disease.

    • In this paper, we investigate population dynamics in the Scandinavian Neolithic and Early Bronze Age in southwestern Sweden. Human mobility patterns in Falbygden were studied by applying strontium isotope analysis combined with archaeological and bioarchaeological data, including mtDNA and sex assessment on a large dataset encompassing 141 individuals from 21 megalithic graves. In combination with other archaeological and anthropological records, we investigated the temporal and spatial scale of individual movement, mobility patterns of specific categories of people and possible social drivers behind them. Our results of strontium and biomolecular analyses suggest that mobility increased in the Late Neolithic and Early Bronze Age compared to the earlier parts of the Neolithic. The data indicate individuals moving both into and away from Falbygden. Mobility patterns and contact networks also shift over time.

  • Autonomy in PhD-education – Supervising for Independence

    • According to the Swedish higher ordinance of education the development of autonomy of PhD students is of high importance. Graduates should be able to formulate new ideas and to independently assess and evaluate scientific results. Therefore it is interesting to investigate how the relationship between student and supervisor impacts the development of autonomy. In this work, we have evaluated the impact of the supervision relationship by a supervisor/studentalignment test (N=25) where both students and their supervisors perform a self-evaluation of the student’s level of autonomy and the supervisor’s supervision style. The test results show that the autonomy is greater for students with longer experience and also more aligned with the level of autonomy as perceived by their supervisors. The misalignment between the assessment done by students and supervisors decreases with age and time spent as a PhD student. However, we find that the misalignment increases when supervisors have many concurrent PhD students. We observe no statistical differences with respect to gender or nationality.

    • Det finns stora effekter att vinna med digitalisering och BIM kopplat till projekterings- och byggprocessen. Att enkelt kunna identifiera och kontrollera att krav är uppfyllda är av stor vikt i multidisciplinära bygg- och anläggningsprojekt som ofta är komplexa och har många osäkerheter. Här anses BIM vara användbart för att bland annat förbättra kvaliteten genom att eliminera konflikter och minska omarbetningen. En styrd, kvalitetssäkrad och effektiv hantering av information om anläggningen kan rätt hanterat bland annat lämna ett ökat utrymme för innovation. Detta projekt presenterar en kartläggning över hur kvalitetssäkringsprocessen och kvalitetskontrollen genomförs idag och testar möjliga kvalitetskontroller. Genom att få en tydlig bild av hur kvalitetskontroller genomförs ges en inblick i de öar av kvalitetssäkringsprocessen som är generiska och vilka moment som kan automatiseras, vilket underlättar för alla aktörer och frigör tid för teknisk lösningsorienterade arbetsuppgifter och utveckling. Syftet med forskningsprojektet har varit att utveckla och testa kvalitetskontroller för kravuppfyllnad och kvalitetssäkring samt beslutsfattande kopplat till användande av digitala modeller. Projektet har genomförs baserat på ett Design Science angreppssätt vilket tar sin utgångspunkt i att både tekniska och mänskliga aspekter kopplas till det problem som ska lösas. Projektet uppdelades i 7 arbetspaket och är ett samarbete mellan Trafikverket, Chalmers, Trimble, Norconsult och Sweco. Del 1 i rapporten är en kartläggning av nuvarande process för leveranser, kvalitetssäkring och kravhantering inom projekteringsfasen, både hos Trafikverket och leverantör. Kartläggningen ger insyn i möjliga förbättringar och effektiviseringsvinster inom kvalitetssäkringsprocessen. Sammanfattningsvis visar kartläggningen att det behövs ett större fokus på de olika aktiviteterna som ingår i kvalitetsarbetet inom projekt. Del 2 i rapporten lyfter upp flera goda exempel och best-practices när det gäller automatiska kvalitetskontroller inom husbyggnad och menar att samma koncept för automatiska kvalitetskontroller går att använda inom anläggningsbranschen. Det finns extremt stor potential med automatiserade kvalitetskontroller (visuella, regelbaserade, automatiska och maskinläsbara) inom anläggningsprojekt och detta projekt har visat att det faktiskt fungerar och att det går att börja implementera vissa kontroller för digitala anläggningsmodeller redan nu. Dessa kontroller sparar tid och möjliggör att kontrollerna verkligen blir utförda. Dessutom kan dessa automatiska kvalitetskontroller frigöra tid som istället kan användas för att ta fram bättre tekniska lösningar och utformningar för projektet. Del 3 i rapporten tar upp en diskussion om kvalitetssäkring och digitala och/eller automatiserade kontroller. Utifrån innehåll i rapporten samlas här också ett antal rekommendationer relaterade till organisation, process och teknik/produkt. Rapporten tar upp att man för automatiserad kontroll av kravuppfyllnad vid leverander behövs ett modellorienterat integrerat arbetssätt för kontroll av lösning, en implementerad process för kvalitetssäkring av produktleveranser, ett modellbaserat gränssnitt för hantering av synpunkter och ändringar och en succeessiv uppföljning av projekt med en tillgänglig realtidsuppdaterad digital modell.
