    • We observe a deuteron beam polarization lifetime near 1000 s in the horizontal plane of a magnetic storage ring (COSY). This long spin coherence time is maintained through a combination of beam bunching, electron cooling, sextupole field corrections, and the suppression of collective effects through beam current limits. This record lifetime is required for a storage ring search for an intrinsic electric dipole moment on the deuteron at a statistical sensitivity level approaching 10(-29) e cm.

    • The tensor polarisation of omega mesons produced in the pd -> He-3 omega reaction has been studied at two energies near threshold. The 3 He nuclei were detected in coincidence with the pi(0)pi(+)pi(-) or pi(0)gamma decay products of the omega. in contrast to the case of phi-meson production, the omega mesons are found to be unpolarised. This brings into question the applicability of the Okubo-Zweig-lizuka rule when comparing the production of vector mesons in low energy hadronic reactions.

    • Exclusive measurements of the reactions pd -> He-3 pi(+)7 pi(-) and pd -> He-3 pi(0)pi(0) have been carried out at T-p = 0.893 GeV at the CELSIUS storage ring using the WASA detector. The pi(+)pi(-) channel evidences a pronounced enhancement at low invariant pi pi masses-as anticipated from previous inclusive measurements of the ABC effect. This enhancement is seen to be even much larger in the isoscalar pi(0)pi(0) channel. The differential distributions prove this enhancement to be of scalar-isoscalar nature. Delta Delta calculations give a good description of the data, if a boundstate condition is imposed for the intermediate Delta Delta system.

  • A strong H-opacity signal in the near-infrared emission spectrum of the ultra-hot Jupiter KELT-9b

    • We present the analysis of a spectroscopic secondary eclipse of the hottest transiting exoplanet detected to date, KELT-9b, obtained with the Wide Field Camera 3 aboard the Hubble Space Telescope. We complement these data with literature information on stellar pulsations and Spitzer/Infrared Array Camera and Transiting Exoplanet Survey Satellite eclipse depths of this target to obtain a broadband thermal emission spectrum. Our extracted spectrum exhibits a clear turnoff at 1.4 μm. This points to H- bound-free opacities shaping the spectrum. To interpret the spectrum, we perform grid retrievals of self-consistent 1D equilibrium chemistry forward models, varying the composition and energy budget. The model with solar metallicity and C/O ratio provides a poor fit because the H- signal is stronger than expected, requiring an excess of electrons. This pushes our retrievals toward high atmospheric metallicities ([M/H] = 1.98-0.21+0.19) and a C/O ratio that is subsolar by 2.4σ. We question the viability of forming such a high-metallicity planet, and therefore provide other scenarios to increase the electron density in this atmosphere. We also look at an alternative model in which we quench TiO and VO. This fit results in an atmosphere with a slightly subsolar metallicity and subsolar C/O ratio ([M/H] = -0.22-0.13+0.17, log (C/O) = -0.34-0.34+0.19). However, the required TiO abundances are disputed by recent high-resolution measurements of the same planet.

    • First exclusive data for the pp -> nnp(+)pi(+) reaction have been obtained at CELSIUS with the WASA detector setup at a beam energy of T(p) - 1.1 GeV. Total and differential cross-sections disagree with theoretical calculations, which predict the Delta Delta excitation to be the dominant process at this beam energy. Instead, the data require the excitation of one of the nucleons to a higher-lying Delta state, preferably the Delta(1600)P(33), to be the leading process.

    • Rationale: New approaches are needed to guide personalized treatment of asthma. Objectives: To test if urinary eicosanoid metabolites can direct asthma phenotyping. Methods: Urinary metabolites of prostaglandins (PGs), cysteinyl leukotrienes (CysLTs), and isoprostanes were quantified in the U-BIOPRED (Unbiased Biomarkers for the Prediction of Respiratory Diseases Outcomes) study including 86 adults with mild-to-moderate asthma (MMA), 411 with severe asthma (SA), and 100 healthy control participants. Validation was performed internally in 302 participants with SA followed up after 12-18 months and externally in 95 adolescents with asthma. Measurement and Main Results: Metabolite concentrations in healthy control participants were unrelated to age, body mass index, and sex, except for the PGE(2) pathway. Eicosanoid concentrations were generally greater in participants with MMA relative to healthy control participants, with further elevations in participants with SA. However, PGE(2) metabolite concentrations were either the same or lower in male nonsmokers with asthma than in healthy control participants. Metabolite concentrations were unchanged in those with asthma who adhered to oral corticosteroid treatment as documented by urinary prednisolone detection, whereas those with SA treated with omalizumab had lower concentrations of LTE4 and the PGD(2) metabolite 2,3-dinor-11 beta-PGF(2 alpha). High concentrations of LTE4 and PGD(2) metabolites were associated with lower lung function and increased amounts of exhaled nitric oxide and eosinophil markers in blood, sputum, and urine in U-BIOARED participants and in adolescents with asthma. These type 2 (T2) asthma associations were reproduced in the follow-up visit of the U-BIOPRED study and were found to be as sensitive to detect T2 inflammation as the established biomarkers. Conclusions: Monitoring of urinary eicosanoids can identify T2 asthma and introduces a new noninvasive approach for molecular phenotyping of adult and adolescent asthma.

    • Asthma phenotyping requires novel biomarker discovery.ObjectivesTo identify plasma biomarkers associated with asthma phenotypes by application of a new proteomic panel to samples from two well-characterised cohorts of severe (SA) and mild-to-moderate (MMA) asthmatics, COPD subjects and healthy controls (HCs).MethodsAn antibody-based array targeting 177 proteins predominantly involved in pathways relevant to inflammation, lipid metabolism, signal transduction and extracellular matrix was applied to plasma from 525 asthmatics and HCs in the U-BIOPRED cohort, and 142 subjects with asthma and COPD from the validation cohort BIOAIR. Effects of oral corticosteroids (OCS) were determined by a 2-week, placebo-controlled OCS trial in BIOAIR, and confirmed by relation to objective OCS measures in U-BIOPRED.ResultsIn U-BIOPRED, 110 proteins were significantly different, mostly elevated, in SA compared to MMA and HCs. 10 proteins were elevated in SA versus MMA in both U-BIOPRED and BIOAIR (alpha-1-antichymotrypsin, apolipoprotein-E, complement component 9, complement factor I, macrophage inflammatory protein-3, interleukin-6, sphingomyelin phosphodiesterase 3, TNF receptor superfamily member 11a, transforming growth factor-β and glutathione S-transferase). OCS treatment decreased most proteins, yet differences between SA and MMA remained following correction for OCS use. Consensus clustering of U-BIOPRED protein data yielded six clusters associated with asthma control, quality of life, blood neutrophils, high-sensitivity C-reactive protein and body mass index, but not Type-2 inflammatory biomarkers. The mast cell specific enzyme carboxypeptidase A3 was one major contributor to cluster differentiation.ConclusionsThe plasma proteomic panel revealed previously unexplored yet potentially useful Type-2-independent biomarkers and validated several proteins with established involvement in the pathophysiology of SA.

    • Fractures of the femoral neck, that is, "cervical hip fractures" constitute 53% of all fractures of the proximal femur (hip fractures) according to the Swedish National Hip Fracture Register linked to SAHFE (Standardised Audit of Hip Fractures in Europe). The most reproducible classification system divides cervical hip fractures into undisplaced (33%) and displaced (67%). Hip fractures are common and costly. Due to the expected increase in the number of elderly in the world during the coming decades the number of hip fractures will increase dramatically, particularly in developing countries. In Sweden three quarters of the patients are women, the mean age is now 81 years and half of the patients are living alone. Hip fractures are rare below 50 years of age. In recent years there has been an incidence increase in the oldest patients, i.e. those over 80 years of age. For these elderly, the incidence in Lund, Sweden, increased from 13.2/1000 in 1966 to 25.5/1000 in 1986. The high number of patients with hip fractures and the cost of treatment increases the need for prevention as well as optimization of operative treatment and rehabilitation. National guidelines are being developed in Europe. Linked with national audits like SAHFE they can improve the quality of care by audit and feedback.

    • Purpose: To evaluate the influence of background factors on the rehabilitation pattern after a hip fracture in the elderly.Method: Prospective registration based on the Swedish national register for hip fracture patients called RIKSHÖFT/SAHFE (Standardised Audit of Hip Fractures in Europe). The place of living was registered both before fracture and during the following four months period (120 days). Graphs were calculated and drawn based on day-to‐day changes. Also influences of age, sex, fracture type and type of operation were analyzed.Results: The patient's pre-fracture functional capacity as evidence by the place they were able to manage to live before the fracture was the most discriminating factor for the rehabilitation; more than sex, fracture type or type of operation. Age was also a highly discriminating factor with a pronounced influence on the rehabilitation pattern.Conclusions: These background parameters are very important factors when planning the rehabilitation of hip fracture patients. A strategy with individualized planning of the rehabilitation procedure will be highly necessary in the future, in view of the increasing amount of elderly with hip fractures prognosticated during the coming decades. The knowledge about influencing factors here presented will be useful when planning and performing the rehabilitation for this resource-consuming group of patients.

    • Objectives. To study whether there is an increased fracture incidence following thyrotoxicosis. Design. A case-control study. Setting. Malmo University Hospital, Malmo, Sweden. Subjects: All patients (n = 333) from the population of Malmo who were treated for thyrotoxicosis for the first time during the 5-year period 1970-74. A total of 618 controls were selected from the local municipality registry in Malmo. For each case the aim was to randomly select two age- and gender-specific controls, alive in 1993 and born the same year and month as the case. Main outcome measures. Fracture incidence. Results. Comparing survivors, there were no differences in the percentage of individuals with fractures (all, fragility, non-fragility) between the patients and the controls. Comparing all individuals and including all fractures, the percentage of individuals with fractures in the entire female patient group (24.6%) was lower (P < 0.05) than in female controls (33.1%). There was a similar but non-significant pattern between male patients and controls. The mean number of all fractures was lower in male patients than in controls (P < 0.05), but no significant difference was noted between female patients and controls. For fragility fractures, there were no significant differences in the percentage of individuals with fractures or in the mean number of fractures between female or male patients and controls. Conclusion. In conclusion we found no increased incidence of fragility fractures in patients with previous thyrotoxicosis as compared with controls. Our results do not support the suggestion that screening for osteoporosis should be performed in patients with previous thyrotoxicosis.

    • Our aim was to compare hemiarthroplasty (HA) and osteosynthesis (OS) in the treatment of cervical hip fractures using matched-pair analysis, especially with regard to different age groups. Data concerning all hip fractures (excluding pathological fractures) at the University Hospitals of Lund in Sweden, where osteosynthesis with LIH hook-pins was used exclusively, and of Oulu in Finland, using mainly cementless Austin-Moore hemiarthroplasty, were registered during 1989-1996 using the same standardized hip fracture forms filled in preoperatively and at 4 months follow-up. Altogether 446 pairs matched for age, sex, place of residence and walking ability at the time of fracture were found. Patients aged 55-80 years seemed to benefit more, with regard to function, from OS than older patients. At 4 months follow-up, 38% of HA and 48% of OS patients lived in their own homes, 16% and 27% were able to walk alone outdoors, and 11% versus 16% were able to walk without any aids, respectively. At I year follow-up, mortality was significantly lower among the OS patients, but the reoperation rate was significantly higher. In conclusion, OS is associated with a better function and lower mortality than HA, especially in younger patients, and it is recommended as the primary treatment for cervical hip fractures in patients younger than 80 years and with good ambulatory capacity, whereas the oldest patients can also be safely treated by HA.

    • OBJECTIVE: To investigate the relation between low Apgar score in breech infants and handicap in childhood. STUDY DESIGN: A case-control study. A questionnaire to mothers of 323 non-malformed, singleton infants delivered in breech presentation at term, 105 cases with Apgar score below 7 at 5 min and 218 controls. RESULTS: Four cases (4.6%) and one control (0.5%) had cerebral palsy. In infants without cerebral palsy, speech/language problems were more frequent than controls (10.6 versus 3.2%) (P=0.02). There were no differences in rates of deficits in attention, motor control and perception (DAMP), epilepsy, cognitive developmental delay or learning disabilities. Absence of any handicap or disability was reported in 65 cases (75%) compared to 172 controls (92%) (OR: 3.9; 95% CI: 1.9-7.9). CONCLUSION: Even though low Apgar score indicates an increased risk of neurological sequelae, most (75%) breech infants with low Apgar score are without a handicap/disability at follow-up.

    • Osteosynthesis (OS) and hemiarthroplasty (RA) are the commonest treatments for displaced cervical hip fractures in the elderly, but there is no consensus as to which is better. In this prospective matched-pair study we compared these methods as regards functional outcome. In 1989-1996, using the same standardized forms, all displaced cervical hip fractures were prospectively registered in the university hospitals of Oulu in Finland and Lund in Sweden. Osteosynthesis was performed in Lund and hemiarthroplasty in Oulu. Cross-matching, done for age, sex, preoperative residence, and ambulatory ability, resulted in 357 pairs of displaced fractures. More OS than HA patients could manage in their own homes or live semi-independently at 4 months after the fracture. OS patients had better ambulatory ability (p = 0.001) and used walking aids less than HA ones (p = 0.001). The reoperation rates at 4 months were the same among HA and OS patients, but at 1 year, OS patients had a higher reoperation rate (17%) than HA ones (9.5%).
