  • Galaxy counterparts of metal-rich damped Ly alpha absorbers - II. A solar-metallicity and dusty DLA at z(abs)=2.58

    • This is the second paper of a series reporting on the results from a survey conducted with the ESO VLT/X-shooter spectrograph. We target high-metallicity damped Lyman alpha absorbers (DLAs) with the aim of investigating the relation between galaxies detected in emission and those detected in absorption. Here, we report on the discovery of the galaxy counterpart of the z(abs) = 2.58 DLA on the line-of-sight to the z = 3.07 quasar SDSS J 091826.16+163609.0 (hereafter Q 0918+1636). The galaxy counterpart of the DLA is detected in the [O iii] lambda 5007 and [O ii] lambda lambda 3726, 3729 emission lines redshifted into the NIR at an impact parameter of 2.0 arcsec (16 kpc at z = 2.58). Ly alpha emission is not detected down to a 3 Sigma detection limit of 5 x 10-18 erg s-1 cm-2, which, compared to the strength of the oxygen lines, implies that Ly alpha emission from this galaxy is suppressed by more than an order of magnitude. The DLA has one of the highest metallicities measured so far at comparable redshifts. We find evidence for substantial depletion of refractory elements on to dust grains. Fitting the main metal line component of the DLA, which is located at z(abs) = 2.5832, we measure the metal abundances from Zn ii, S ii, Si ii, Cr ii, Mn ii, Fe ii and Ni ii to be -0.12 +/- 0.05, -0.26 +/- 0.05, -0.46 +/- 0.05, -0.88 +/- 0.05, -0.92 +/- 0.05, -1.03 +/- 0.05 and -0.78 +/- 0.05, respectively. In addition, we detect absorption in the Lyman and Werner bands of molecular hydrogen (H(2)), which represents the first detection of H(2) molecules with X-shooter. The background quasar Q 0918+1636 is amongst the reddest QSOs at redshifts 3.02 < z < 3.12 from the SDSS catalogue. Its UV to NIR spectrum is well fitted by a composite QSO spectrum reddened by SMC-/LMC-like extinction curves at z(abs) = 2.58 with a significant amount of extinction given by A(V) approximate to 0.2 mag. This supports previous claims that there may be more metal-rich DLAs missing from current samples due to dust reddening of the background QSOs. The fact that there is evidence for dust both in the central emitting regions of the galaxy (as evidenced by the lack of Ly alpha emission) and at an impact parameter of 16 kpc (as probed by the DLA) suggests that dust is widespread in this system.

  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • The prevalence of multiple myeloma (MM) is increasing in Nordic countries and the rest of the western world. Patients aged =75 years at diagnosis constitute an increasing proportion of all MM patients, but are underrepresented in randomized clinical trials. There is an urgent need for studies of the characteristics, treatment and outcome in this cohort. We present data from two nationwide population-based registries of all MM patients diagnosed in Denmark from January 1, 2005 until February 18, 2020, and in Sweden from January 1, 2008 until December 31, 2019, including treatment data for patients diagnosed until 2018 (Denmark) and 2019 (Sweden). In total 4,647 patients were =75 years at diagnosis, compared to 7,378 younger patients. Patients =75 years, accounting for approximately 40% of all MM patients, are a distinct cohort with more advanced disease at diagnosis, reflected by higher International Staging System (ISS) stage, and a higher proportion have renal failure and anemia. We found a more gradual introduction of modern medications in the older cohort than in the younger, despite simultaneous changes in guidelines. Compared to the cohorts in randomized controlled trials that guide the treatment of non-transplant eligible patients, we found a higher proportion of patients =75 years and presenting with ISS III in the real-world populations. Nevertheless, response rates and survival are increasing, indicating that modern treatment regimens are effective and well tolerated also in elderly MM patients in real-world populations.

  • Importance of factors determining the low health-related quality of life in people presenting with a diabetic foot ulcer: the Eurodialestudy

    • AimTo identify the factors responsible for the low health-related quality of life associated with foot ulcers and the relative importance of these factors. MethodsA total of 1232 patients with a new foot ulcer, who presented at one of the 14 centres in 10 European countries participating in the Eurodiale study, were included in this cross-sectional study. Patient and ulcer characteristics were obtained as well as results from the Euro-Qol-5D questionnaire, a health-related quality of life instrument with five domains (mobility, self-care, usual activities, pain/discomfort and anxiety/depression). To analyse the relative importance of comorbidities and ulcer- and patient-related factors for health-related quality of life, linear regression models were used to calculate the relative contributions of each factor to the fit (R-2) of the model. ResultsPatients reported poor overall health-related quality of life, with problems primarily in the mobility and pain/discomfort domains. Among the comorbidities, the inability to stand or walk without help was the most important determinant of decreased health-related quality of life in all five domains. Among ulcer-related factors, ulcer size, limb-threatening ischaemia and elevated C-reactive protein concentration also had high importance in all domains. The clinical diagnosis of infection, peripheral arterial disease and polyneuropathy were only important in the pain/discomfort domain. ConclusionsThe factors that determine health-related quality of life are diverse and to an extent not disease-specific. To improve health-related quality of life, treatment should not only be focused on ulcer healing but a multifactorial approach by a specialized multidisciplinary team is also important. What's new? The inability to stand or walk without help is the most important determinant of poor health-related quality of life. Other factors with high importance are ulcer size, elevated C-reactive protein concentration and limb-threatening ischaemia. To improve health-related quality of life, treatment should not only be focused on ulcer healing but a multifactorial approach by a specialized multidisciplinary team is also important.

    • The interests in monitoring inflammation by immunoassay determination of blood inflammatory markers call for information on the stability of these markers in relation to the handling of blood samples. The increasing use of stored biobank samples for such ventures that may have been collected and stored for other purposes, justifies the study hereof. Blood samples were stored for 0, 4, 24, and 48 h at 4 degrees C, room temperature (RT), and at 35 degrees C, respectively, before they were separated into serum or plasma and frozen. Dried blood spot samples (DBSS) were stored for 0, 1, 2, 3, 7, and 30 days at the same temperatures. 27 inflammatory markers in serum and plasma and 25 markers in DBSS were measured by a previously validated multiplex sandwich immunoassay using Luminex xMAP technology. The measurable concentrations of several cytokines in serum and plasma were significantly increased when blood samples were stored for a period of time before the centrifugation, for certain cytokines more than 1000 fold compared to serum and plasma isolated and frozen immediately after venepuncture. The concentrations in serum generally increased more than in plasma. The measurable concentrations of inflammatory markers also changed in DBSS stored under various conditions compared to controls frozen immediately after preparation, but to a much lesser degree than in plasma or serum. The study demonstrates that trustworthy measurement of several inflammatory markers relies on handling of whole blood samples at low temperatures and rapid isolation of plasma and serum. Effects of different handling procedures for all markers studied are given. DBSS proved to be a robust and convenient way to handle samples for immunoassay analysis of inflammatory markers in whole blood.

    • The quasar Q0918+1636 (z = 3.07) has two intervening high-metallicity Damped Lyman a Absorbers (DLAs) along the line of sight, at redshifts of z = 2.412 and 2.583. The z = 2.583 DLA is located at a large impact parameter of 16.2 kpc, and despite this large impact parameter it has a very high metallicity (consistent with solar), a substantial fraction of H-2 molecules and it is dusty as inferred from the reddened spectrum of the background QSO. The z = 2.412 DLA has a metallicity of [M/H] = -0.6 (based on Zn II and Si II). In this paper we present new observations of this interesting sightline consisting of deep multiband imaging and further VLT spectroscopy. By fitting stellar population synthesis models to the photometric Spectral Energy Distribution we constrain the physical properties of the z = 2.583 DLA galaxy, and we infer its morphology by fitting a Sersic model to its surface brightness profile. We find it to be a relatively massive (M-* approximate to 10(10) M-circle dot), strongly star-forming (SFR approximate to 30 M-circle dot yr(-1)), dusty (E(B - V) = 0.4) galaxy with a disc-like morphology. We detect strong emission lines from the z = 2.583 DLA ([O II] lambda 3727, [O III] lambda lambda 4960, 5007, H beta and H alpha, albeit at low signal-to-noise ratio except for the [O III] lambda 5007 line). The metallicity derived from the emission lines is consistent with the absorption metallicity (12 + log (O/H) = 8.8 +/- 0.2). We also detect [O III] lambda 5007 emission from the galaxy counterpart of the z = 2.412 DLA at a small impact parameter (<2 kpc). Overall our findings are consistent with the emerging picture that high-metallicity DLAs are associated with relatively luminous and massive galaxy counterparts, compared to typical DLAs.

    • Objective Microbial invasion of the amniotic cavity is a major cause of preterm delivery and the diagnosis is dependent on invasive amniocentesis. The objective was to determine whether specific proteins in amniotic and cervical fluids alone, or in combination, could identify bacterial invasion. Design A prospective follow-up study. Population Women with singleton pregnancies presenting with preterm labour between 22 and 33weeks of gestation (n=89). Setting Sahlgrenska University Hospital, Gothenburg, Sweden. Methods Amniotic and cervical fluid was analysed with polymerase chain reaction for Mycoplasmas, and was cultured for aerobic and anaerobic bacteria. Twenty-seven proteins were analysed using multiplex technology. Individual levels of each protein were compared in order to find associations between different proteins and microbial invasion of the amniotic cavity. Predictive models based on multiple proteins were created using stepwise binary logistic regression. Main outcome measure The main outcome measure was microbial invasion of the amniotic cavity. Results Microbial invasion of the amniotic cavity was present in 17% (15/89) of the women. Concentration levels of several amniotic and cervical proteins were significantly higher in women with microbial invasion of the amniotic cavity. Three multivariate predictive models were found. The predictive power of the non-invasive model (73% sensitivity, 88% specificity, 55% positive predictive value, 94% negative predictive value) was as good as the invasive models. Area under the receiver operating characteristic (ROC) curve and likelihood ratio were 0.87 and 6.0, respectively. Conclusions Prediction of intra-amniotic infection using selected cervical proteins was equally good as prediction using the same proteins collected from amniotic fluid, or a combination of cervical and amniotic proteins.

  • Prediction of spontaneous preterm delivery in women with preterm labor: analysis of multiple proteins in amniotic and cervical fluids

    • OBJECTIVE: To analyze whether specific proteins in amniotic and cervical fluids, alone or in combination with risk factors, can identify women in preterm labor with intact membranes who will deliver spontaneously within 7 days of sampling. METHODS: In a cohort of 89 women in preterm labor, amniotic and cervical fluids were collected between 22 and 33 weeks of gestation. Twenty-seven proteins were analyzed simultaneously using multiplex technology. Individual levels of each protein were compared and calculations performed to find associations among different proteins, background variables, and spontaneous preterm delivery within 7 days of sampling. The area under the curve (AUC) was calculated using receiver operating characteristic curve analysis, and prediction models were created based on stepwise logistic regression. RESULTS: We found two multivariable models that predicted spontaneous preterm delivery better than any single variable. One combined multivariable prediction model was based on amniotic macrophage inflammatory protein-1beta, cervical interferon-gamma, and monocyte chemotactic protein-1. This model predicted outcome with 91% sensitivity, 84% specificity, 78% positive predictive value, and 94% negative predictive value, with a likelihood ratio of 5.6 and AUC of 0.91. An alternative, noninvasive model based on cervical length, cervical interferon-gamma, interleukin-6, and monocyte chemotactic protein-1 predicted delivery within 7 days with 85% sensitivity, 82% specificity, 74% positive predictive value, and 90% negative predictive value, with a likelihood ratio of 4.7 and AUC of 0.91. CONCLUSION: A combination of proteins from amniotic fluid and cervical fluid or cervical length can help determine which women will deliver preterm. LEVEL OF EVIDENCE: II.

  • Small Region, Big Impact : Highly Anisotropic Lyman-continuum Escape from a Compact Starburst Region with Extreme Physical Properties

    • Extreme, young stellar populations are considered to be the primary contributor to cosmic reionization. How the Lyman continuum (LyC) escapes these galaxies remains highly elusive, and it is challenging to observe this process in actual LyC emitters without resolving the relevant physical scales. We investigate the Sunburst Arc, a strongly lensed LyC emitter at z = 2.37 that reveals an exceptionally small-scale (tens of parsecs) region of high LyC escape. The small (<100 pc) LyC-leaking region has extreme properties: a very blue UV slope (β = −2.9 ± 0.1), a high ionization state ([O iii] λ5007/[O ii] λ3727 = 11 ± 3 and [O iii] λ5007/Hβ = 6.8 ± 0.4), strong oxygen emission (EW([O iii]) = 1095 ± 40 Å), and a high Lyα escape fraction (0.3 ± 0.03), none of which are found in nonleaking regions of the galaxy. The leaking region's UV slope is consistent with approximately "pure" stellar light that is minimally contaminated by the surrounding nebular continuum emission or extinguished by dust. These results suggest a highly anisotropic LyC escape process such that LyC is produced and escapes from a small, extreme starburst region where the stellar feedback from an ionizing star cluster creates one or more "pencil-beam" channels in the surrounding gas through which LyC can directly escape. Such anisotropic escape processes imply that random sight-line effects drive the significant scatters between measurements of galaxy properties and LyC escape fraction, and that strong lensing is a critical tool for resolving the processes that regulate the ionizing budget of galaxies for reionization.

  • Does software optimization influence the radiologists' perception in low dose paediatric pelvic examinations?

    • Purpose: To investigate whether software optimisation can improve an observers’ perception of image quality in low dose paediatric pelvic examinations. Methods: Twenty-five consecutive patients (3–7 years old) were referred for a pelvic digital radiography (DR) examination. They were prospectively enrolled in the study over a 3-month period. Images were taken at 80 kV and 2–4 mAs depending on pelvic thickness (9–15 cm). A small focal spot, 130 cm SID: 10 cm air gap and 1 mm Al and 0.2 mm Cu additional filtration were also utilised. Images were acquired on a Canon DR detector and optimised using five different combinations of the multi-frequency processing software (Canon DR system version NE, Version 7.1 with SPECTRA) to comply with the ALARA principle. Image quality was blindly evaluated using the subjective Visual Grading Analysis (VGA) by five experienced musculoskeletal radiologists, including the evaluation of six anatomical image quality criteria (scored from 1 to 5). Results: Consistently, the VGA results indicated that by using software optimised parameters, image quality was suitable for diagnosis in 48–71% of all images. Based on a VGC analysis all software optimised images did have significant better image quality then the one with just the clinical settings. Noise reduction was the software setting which influenced the image quality the most, area under the curve (AUC) of 0.8172 95%CI 0.7953–0.8375. Conclusion: Software optimisation improve the radiologists' perception of image quality and should thus be thoroughly considered within clinical practise. Noise reduction is the software parameter which has the greatest influence.

  • The Cosmic Telescope That Lenses the Sunburst Arc, PSZ1 G311.65-18.48 : Strong Gravitational Lensing Model and Source Plane Analysis

    • We present a strong-lensing analysis of the cluster PSZ1 G311.65−18.48, based on Hubble Space Telescope imaging, archival VLT/MUSE spectroscopy, and Chandra X-ray data. This cool-core cluster (z = 0.443) lenses the brightest lensed galaxy known, dubbed the "Sunburst Arc" (z = 2.3703), a Lyman continuum (LyC) emitting galaxy multiply imaged 12 times. We identify in this field 14 additional strongly lensed galaxies to constrain a strong-lens model and report secure spectroscopic redshifts of four of them. We measure a projected cluster core mass of M(<250 kpc) = M⊙. The two least magnified but complete images of the Sunburst Arc's source galaxy are magnified by ∼13×, while the LyC clump is magnified by ∼4–80×. We present time delay predictions and conclusive evidence that a discrepant clump in the Sunburst Arc, previously claimed to be a transient, is not variable, thus strengthening the hypothesis that it results from an exceptionally high magnification. A source plane reconstruction and analysis of the Sunburst Arc finds its physical size to be 1 × 2 kpc and that it is resolved in three distinct directions in the source plane, 0°, 40°, and 75° (east of north). We place an upper limit of r ≲ 50 pc on the source plane size of unresolved clumps and r ≲ 32 pc for the LyC clump. Finally, we report that the Sunburst Arc is likely in a system of two or more galaxies separated by ≲6 kpc in projection. Their interaction may drive star formation and could play a role in the mechanism responsible for the leaking LyC radiation.

    • A comprehensive set of optical and near-infrared (NIR) photometry and spectroscopy is presented for the faint and fast 2008ha-like supernova (SN) 2010ae. Contingent on the adopted value of host extinction, SN 2010ae reached a peak brightness of -13.8 > M-V > -15.3 mag, while modeling of the UVOIR light curve suggests it produced 0.003-0.007 M-circle dot of Ni-56, ejected 0.30-0.60 M-circle dot of material, and had an explosion energy of 0.04-0.30 x 10(51) erg. The values of these explosion parameters are similar to the peculiar SN 2008ha -for which we also present previously unpublished early phase optical and NIR light curves - and places these two transients at the faint end of the 2002cx-like SN population. Detailed inspection of the post-maximum NIR spectroscopic sequence indicates the presence of a multitude of spectral features, which are identified through SYNAPPS modeling to be mainly attributed to Co II. Comparison with a collection of published and unpublished NIR spectra of other 2002cx-like SNe, reveals that a Co II footprint is ubiquitous to this subclass of transients, providing a link to Type Ia SNe. A visual-wavelength spectrum of SN 2010ae obtained at +252 days past maximum shows a striking resemblance to a similar epoch spectrum of SN 2002cx. However, subtle differences in the strength and ratio of calcium emission features, as well as diversity among similar epoch spectra of other 2002cx-like SNe indicates a range of physical conditions of the ejecta, highlighting the heterogeneous nature of this peculiar class of transients.

    • Oxidation of NH3/n-heptane mixtures at pressures up to 100 atm and temperatures of 400–900 K was characterized experimentally in a laminar flow reactor and a jet-stirred reactor. A detailed chemical kinetic model was developed, updating the hydrogen and amine subsets and introducing a subset for the chemical coupling with emphasis on the NH2+n-heptane reaction. The kinetic model provided a good prediction of the ignition delay times measured in a rapid compression machine by Yu et al. (Combust. Flame 217 (2020) 2–11) as well as the high pressure experimental data obtained in the present work. The results show that it is important to include updated rate constants for NH2 + HO2 and NH2 + n-C7H16 to obtain reliable predictions for ignition and oxidation of NH3/n-heptane mixtures at high pressure. The effectiveness of implementing analogy rules for determining the rate constant of the key reaction NH2 + n-C7H16 was confirmed by the observed results.

Objective To analyse whether specific proteins in maternal serum and cervical length, alone or in combination, can predict the likelihood that women with intact membranes with threatened preterm labour will deliver spontaneously within 7 days of sampling. Design Cohort study. Setting Sahlgrenska University Hospital, Gothenburg, Sweden. Population Women at between 22 and 33 weeks of gestation with threatened preterm labour (n = 142) admitted to the Sahlgrenska University Hospital, Gothenburg, Sweden, in 19952005. Methods Maternal serum was tested for 27 proteins using multiplex xMAP technology. Individual levels of each protein were compared, and calculations were performed to investigate potential associations between different proteins, cervical length and spontaneous preterm delivery. Receiver operating characteristic curves were used to find the best cut-off values for continuous variables in relation to spontaneous preterm delivery within 7 days of sampling. Prediction models were created based on a stepwise logistic regression using binary variables. Main outcome measure Spontaneous preterm delivery within 7 days. Results In order to determine the best prediction model, we analysed models of serum proteins alone, cervical length alone, and the combination of serum proteins and cervical length. We found one multivariable combined model through the data analysis that more accurately predicted spontaneous preterm delivery within 7 days. This model was based on serum interleukin-10 (IL-10) levels, serum RANTES levels and cervical length (sensitivity 74%, specificity 87%, positive predictive value 76%, negative predictive value 86%, likelihood ratio 5.8 and area under the curve 0.88). Conclusions A combination of maternal serum proteins and cervical length constituted the best prediction model, and would help determine whether women with threatened preterm labour are likely to deliver within 7 days of measurement.

    • Larger thalamic volume has been found in children with obsessive-compulsive disorder (OCD) and children with clinical-level symptoms within the general population. Particular thalamic subregions may drive these differences. The ENIGMA-OCD working group conducted mega- and meta-analyses to study thalamic subregional volume in OCD across the lifespan. Structural T1-weighted brain magnetic resonance imaging (MRI) scans from 2649 OCD patients and 2774 healthy controls across 29 sites (50 datasets) were processed using the FreeSurfer built-in ThalamicNuclei pipeline to extract five thalamic subregions. Volume measures were harmonized for site effects using ComBat before running separate multiple linear regression models for children, adolescents, and adults to estimate volumetric group differences. All analyses were pre-registered (https://osf.io/73dvy) and adjusted for age, sex and intracranial volume. Unmedicated pediatric OCD patients (<12 years) had larger lateral (d = 0.46), pulvinar (d = 0.33), ventral (d = 0.35) and whole thalamus (d = 0.40) volumes at unadjusted p-values <0.05. Adolescent patients showed no volumetric differences. Adult OCD patients compared with controls had smaller volumes across all subregions (anterior, lateral, pulvinar, medial, and ventral) and smaller whole thalamic volume (d = −0.15 to −0.07) after multiple comparisons correction, mostly driven by medicated patients and associated with symptom severity. The anterior thalamus was also significantly smaller in patients after adjusting for thalamus size. Our results suggest that OCD-related thalamic volume differences are global and not driven by particular subregions and that the direction of effects are driven by both age and medication status.

  • Dropout from exercise trials among cancer survivors—An individual patient data meta-analysis from the POLARIS study

    • Introduction: The number of randomized controlled trials (RCTs) investigating the effects of exercise among cancer survivors has increased in recent years; however, participants dropping out of the trials are rarely described. The objective of the present study was to assess which combinations of participant and exercise program characteristics were associated with dropout from the exercise arms of RCTs among cancer survivors. Methods: This study used data collected in the Predicting OptimaL cAncer RehabIlitation and Supportive care (POLARIS) study, an international database of RCTs investigating the effects of exercise among cancer survivors. Thirty-four exercise trials, with a total of 2467 patients without metastatic disease randomized to an exercise arm were included. Harmonized studies included a pre and a posttest, and participants were classified as dropouts when missing all assessments at the post-intervention test. Subgroups were identified with a conditional inference tree. Results: Overall, 9.6% of the participants dropped out. Five subgroups were identified in the conditional inference tree based on four significant associations with dropout. Most dropout was observed for participants with BMI >28.4 kg/m2, performing supervised resistance or unsupervised mixed exercise (19.8% dropout) or had low-medium education and performed aerobic or supervised mixed exercise (13.5%). The lowest dropout was found for participants with BMI >28.4 kg/m2 and high education performing aerobic or supervised mixed exercise (5.1%), and participants with BMI ≤28.4 kg/m2 exercising during (5.2%) or post (9.5%) treatment. Conclusions: There are several systematic differences between cancer survivors completing and dropping out from exercise trials, possibly affecting the external validity of exercise effects. © 2024 The Authors.
