    • DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stability limits and regulate β N in the ITER baseline scenario, based on plasma response to probing 3D fields, was demonstrated. Finally, innovative tokamak operation using a negative triangularity shape showed many attractive features for future pilot plant operation.

    • The tokamak a configuration variable (TCV) continues to leverage its unique shaping capabilities, flexible heating systems and modern control system to address critical issues in preparation for ITER and a fusion power plant. For the 2019-20 campaign its configurational flexibility has been enhanced with the installation of removable divertor gas baffles, its diagnostic capabilities with an extensive set of upgrades and its heating systems with new dual frequency gyrotrons. The gas baffles reduce coupling between the divertor and the main chamber and allow for detailed investigations on the role of fuelling in general and, together with upgraded boundary diagnostics, test divertor and edge models in particular. The increased heating capabilities broaden the operational regime to include T (e)/T (i) similar to 1 and have stimulated refocussing studies from L-mode to H-mode across a range of research topics. ITER baseline parameters were reached in type-I ELMy H-modes and alternative regimes with 'small' (or no) ELMs explored. Most prominently, negative triangularity was investigated in detail and confirmed as an attractive scenario with H-mode level core confinement but an L-mode edge. Emphasis was also placed on control, where an increased number of observers, actuators and control solutions became available and are now integrated into a generic control framework as will be needed in future devices. The quantity and quality of results of the 2019-20 TCV campaign are a testament to its successful integration within the European research effort alongside a vibrant domestic programme and international collaborations.

    • A major goal of the Atacama Large Millimeter/submillimeter Array (ALMA) is to make accurate images with resolutions of tens of milliarcseconds, which at submillimeter (submm) wavelengths requires baselines up to similar to 15 km. To develop and test this capability, a Long Baseline Campaign (LBC) was carried out from 2014 September to late November, culminating in end-to-end observations, calibrations, and imaging of selected Science Verification (SV) targets. This paper presents an overview of the campaign and its main results, including an investigation of the short-term coherence properties and systematic phase errors over the long baselines at the ALMA site, a summary of the SV targets and observations, and recommendations for science observing strategies at long baselines. Deep ALMA images of the quasar 3C 138 at 97 and 241 GHz are also compared to VLA 43 GHz results, demonstrating an agreement at a level of a few percent. As a result of the extensive program of LBC testing, the highly successful SV imaging at long baselines achieved angular resolutions as fine as 19 mas at similar to 350 GHz. Observing with ALMA on baselines of up to 15 km is now possible, and opens up new parameter space for submm astronomy.

  • COVID-19 severity and mortality in patients with CLL: an update of the international ERIC and Campus CLL study

    • Patients with chronic lymphocytic leukemia (CLL) may be more susceptible to Coronavirus disease 2019 (COVID-19) due to age, disease, and treatment-related immunosuppression. We aimed to assess risk factors of outcome and elucidate the impact of CLL-directed treatments on the course of COVID-19. We conducted a retrospective, international study, collectively including 941 patients with CLL and confirmed COVID-19. Data from the beginning of the pandemic until March 16, 2021, were collected from 91 centers. The risk factors of case fatality rate (CFR), disease severity, and overall survival (OS) were investigated. OS analysis was restricted to patients with severe COVID-19 (definition: hospitalization with need of oxygen or admission into an intensive care unit). CFR in patients with severe COVID-19 was 38.4%. OS was inferior for patients in all treatment categories compared to untreated (p < 0.001). Untreated patients had a lower risk of death (HR = 0.54, 95% CI:0.41–0.72). The risk of death was higher for older patients and those suffering from cardiac failure (HR = 1.03, 95% CI:1.02–1.04; HR = 1.79, 95% CI:1.04–3.07, respectively). Age, CLL-directed treatment, and cardiac failure were significant risk factors of OS. Untreated patients had a better chance of survival than those on treatment or recently treated.

The tool TMDplotter allows for web-based plotting of distributions implemented in TMDlib2, together with collinear pdfs as available in LHAPDF.

    • The azimuthal correlation, Δ ϕ12, of high transverse momentum jets in pp collisions at s=13 TeV is studied by applying PB-TMD distributions to NLO calculations via MCatNLO together with the PB-TMD parton shower. A very good description of the cross section as a function of Δ ϕ12 is observed. In the back-to-back region of Δ ϕ12→ π, a very good agreement is observed with the PB-TMD Set 2 distributions while significant deviations are obtained with the PB-TMD Set 1 distributions. Set 1 uses the evolution scale while Set 2 uses transverse momentum as an argument in αs, and the above observation therefore confirms the importance of an appropriate soft-gluon coupling in angular ordered parton evolution. The total uncertainties of the predictions are dominated by the scale uncertainties of the matrix element, while the uncertainties coming from the PB-TMDs and the corresponding PB-TMD shower are very small. The Δ ϕ12 measurements are also compared with predictions using MCatNLO together Pythia8, illustrating the importance of details of the parton shower evolution.

    • The ALMA Regional Centres (ARCs) form the interface between the ALMA observatory and the user community from the proposal preparation stage to the delivery of data and their subsequent analysis. The ARCs provide critical services to both the ALMA operations in Chile and to the user community. These services were split by the ALMA project into core and additional services. The core services are financed by the ALMA operations budget and are critical to the successful operation of ALMA. They are contractual obligations and must be delivered to the ALMA project. The additional services are not funded by the ALMA project and are not contractual obligations, but are critical to achieve ALMA full scientific potential. A distributed network of ARC nodes (with ESO being the central ARC) has been set up throughout Europe at the following seven locations: Bologna, Bonn-Cologne, Grenoble, Leiden, Manchester, Ondrejov, Onsala. These ARC nodes are working together with the central node at ESO and provide both core and additional services to the ALMA user community. This paper presents the European ARC, and how it operates in Europe to support the ALMA community. This model, although complex in nature, is turning into a very successful one, providing a service to the scientific community that has been so far highly appreciated. The ARC could become a reference support model in an age where very large collaborations are required to build large facilities, and support is needed for geographically and culturally diverse communities.

  • Prevalence and characteristics of asthma-chronic obstructive pulmonary disease overlap in routine primary care practices

    • Rationale: Adults may exhibit characteristics of both asthma and chronic obstructive pulmonary disease (COPD), a situation recently described as asthma-COPD overlap (ACO). There is a paucity of information about ACO in primary care. Objectives: To estimate the prevalence and describe characteristics of individuals withACOin primary care practices among patients currently diagnosed with asthma, COPD, or both; and to compare the prevalence and characteristics of ACO among the three source populations. Methods: The Respiratory Effectiveness Group conducted a crosssectional study of individuals ≥40 years old and with ≥2 outpatient primary care visits over a 2-year period in theUKOptimum Patient Care Research Database. Patients were classified into one of three source populations based on diagnostic codes: 1) COPD only, 2) both asthma and COPD, or 3) asthma only.ACOwas defined as the presence of all of the following 1) age ≥40 years, 2) current or former smoking, 3) postbronchodilator airflow limitation (forced expiratory volume in 1 second/ forced vital capacity <0.7), and 4) ≥12% and ≥200 ml reversibility in post-bronchodilator forced expiratory volume in 1 second. Results: Among 2,165 individuals (1,015 COPD only, 395 with both asthma and COPD, and 755 asthma only), the overall prevalence of ACO was 20% (95% confidence interval, 18-23%). Patients with ACO had a mean age of 70 years (standard deviation, 11 yr), 60% were men, 73% were former smokers (the rest were current smokers), and 66% were overweight or obese. Comorbid conditions were common in patients with ACO, including diabetes (53%), cardiovascular disease (36%), hypertension (30%), eczema (23%), and rhinitis (21%). The prevalence of ACO was higher in patients with a diagnosis of both asthma and COPD (32%) compared with a diagnosis of COPD only (20%; P<0.001) or asthma only (14%; P<0.001). Demographic and clinical characteristics of ACO varied across these three source populations. Conclusions: One in five individuals with a diagnosis of COPD, asthma, or both asthma and COPD in primary care settings have ACO based on the Respiratory Effectiveness Group ACO Working group criteria. The prevalence and characteristics of patients with ACO varies across the three source populations.

    • Dihydropyrimidine dehydrogenase (DPD) is the initial and rate-limiting enzyme in the catabolism of 5-fluorouracil (5FU). Genetic variations in DPD have emerged as predictive risk factors for severe fluoropyrimidine toxicity. Here, we report novel and rare genetic variants underlying DPD deficiency in 9 cancer patients presenting with severe fluoropyrimidine-associated toxicity. All patients possessed a strongly reduced DPD activity, ranging from 9 to 53% of controls. Analysis of the DPD gene (DPYD) showed the presence of 21 variable sites including 4 novel and 4 very rare aberrations: 3 missense mutations, 2 splice-site mutations, 1 intronic mutation, a deletion of 21 nucleotides and a genomic amplification of exons 9-12. Two novel/rare variants (c.2843T>C, c.321+1G>A) were present in multiple, unrelated patients. Functional analysis of recombinantly-expressed DPD mutants carrying the p.I948T and p.G284V mutation showed residual DPD activities of 30% and 0.5%, respectively. Analysis of a DPD homology model indicated that the p.I948T and p.G284V mutations may affect electron transfer and the binding of FAD, respectively. cDNA analysis showed that the c.321+1G>A mutation in DPYD leads to skipping of exon 4 immediately upstream of the mutated splice-donor site in the process of DPD pre-mRNA splicing. A lethal toxicity in two DPD patients suggests that fluoropyrimidines combined with other therapies such as radiotherapy might be particularly toxic for DPD deficient patients. Our study advocates a more comprehensive genotyping approach combined with phenotyping strategies for upfront screening for DPD deficiency to ensure the safe administration of fluoropyrimidines.

    • Patients lacking multifunctional protein 2 (MFP2), the central enzyme of the peroxisomal β-oxidation pathway, develop retinopathy. This pathway is involved in the metabolism of very long chain (VLCFAs) and polyunsaturated (PUFAs) fatty acids, which are enriched in the photoreceptor outer segments (POS). The molecular mechanisms underlying the retinopathy remain, however, elusive. Here, we report that mice with MFP2 inactivation display decreased retinal function already at the age of 3 weeks, which is accompanied by a profound shortening of the photoreceptor outer and inner segments, but with preserved photoreceptor ultrastructure. Furthermore, MFP2 deficient retinas exhibit severe changes in gene expression with downregulation of genes involved in the phototransduction pathway and upregulation of inflammation related genes. Lipid profiling of the mutant retinas revealed a profound reduction of DHA-containing phospholipids. This was likely due to a hampered systemic supply and retinal traffic of this PUFA, although we cannot exclude that the local defect of peroxisomal β-oxidation contributes to this DHA decrease. Moreover, very long chain PUFAs were also reduced, with the exception of those containing ≥ 34 carbons that accumulated. The latter suggests that there is an uncontrollable elongation of retinal PUFAs. In conclusion, our data reveal that intact peroxisomal β-oxidation is indispensable for retinal integrity, most likely by maintaining PUFA homeostasis.

  • Cosmo-tomography toward PKS 1830-211: Variability of the quasar and of its foreground molecular absorption monitored with ALMA

    • Context. Time variability of astronomical sources provides crude information on their typical size and on the implied physical mechanisms. PKS 1830-211 is a remarkable radio-bright lensed quasar with a foreground molecular absorber in the lens galaxy at z = 0.89. Small-scale morphological changes in the core- jet structure of the quasar - which is magnified by the lensing - result in a varying illumination of the absorber screen, which in turn causes variations in the absorption profile. Aims. We aim to study the time variations of the system (the two main lensed images of the quasar and the two corresponding sightlines in the absorber) in order to obtain constraints on both the quasar activity and small-scale structures in the interstellar medium of the absorber. Methods. We used ALMA to monitor the submillimeter continuum emission of PKS 1830-211, together with the absorption spectra of the H2O and CH molecules, with 17 visits spread over six months in 2016. Complementing this, we used available ALMA data to investigate changes in the system in the period 2012-2022. Results. From the continuum data, we followed the evolution of the flux density, flux-density ratio, spectral index, and differential polarization between the two lensed images of the quasar; all quantities show significant variations related to the intrinsic activity of the quasar. We propose a simple parametric model of a core plus a ballistic plasmon to account for the continuum evolution, from which we constrain a time delay of 25 ± 3 days between main lensed images. The spectral lines reveal significant variations in the foreground absorption profile. A principal component analysis highlights apparent wavy time variations, possibly linked to the helical jet precession period of the quasar. From the deep averaged spectra towards the southwest image, we detect the absorption of the rare isotopolog 13CH and estimate an abundance ratio of 12CH/13CH ∼ 150. We also measure the oxygen isotopic ratios, 16O/18O = 65.3 ± 0.7 and 18O/17O = 11.5 ± 0.5 in the z = 0.89 absorber. Finally, we find a remarkable continuous shallow trough in the water absorption spanning a velocity interval of nearly 500 km s-1. This broad absorption could be the signature of an extra-planar molecular component. Conclusions. All together, the system formed by the quasar PKS 1830-211 and its foreground lens- absorber acts as a powerful gravitational microscope, providing us with the possibility to dissect small-scale structures in both the ISM of the foreground absorber and the jet of the background quasar.

    • We discuss our recent results on azimuthal distributions in vector boson + jets and multi-jet production at the LHC, obtained from the matching of next-to-leading order (NLO) perturbative matrix elements with transverse momentum dependent (TMD) parton branching. We present a comparative analysis of boson-jet and jet-jet correlations in the back-to-back region, and a study of the theoretical systematic uncertainties associated with the matching scale in the cases of TMD and collinear parton showers.

    • Spin chains are the most elementary entities in the study of magnetic ordering behavior. The authors describe a route toward the realization of artificial XY spin chains. Using thin-film deposition and e-beam lithog., 300 and 550 nm diam. pillars consisting of alternating disks of permalloy and Al2O3 were made. Shape anisotropy forces the magnetization in plane, creating a chain of two-dimensional magnetic moments →i. The disks couple to each other by their dipolar stray fields, resulting in an interaction Jij→i·→j, with Jij the coupling between disks i and j. This form closely resembles a chain of xy spins, coupled by effective interactions Jij. The authors present a magnetic characterization of the pillars, and compare the results with model calcns. Also, spin dynamics and ordering phenomena are addressed.

  • Back-to-back azimuthal correlations in Z + jet events at high transverse momentum in the TMD parton branching method at next-to-leading order

    • Azimuthal correlations in Z + jet production at large transverse momenta are computed by matching Parton-Branching (PB) TMD parton distributions and showers with NLO calculations via MCatNLO. The predictions are compared with those for dijet production in the same kinematic range. The azimuthal correlations Δ ϕ between the Z boson and the leading jet are steeper compared to those in dijet production at transverse momenta O(100) GeV , while they become similar for very high transverse momenta O(1000) GeV . The different patterns of Z + jet and dijet azimuthal correlations can be used to search for potential factorization-breaking effects in the back-to-back region, which depend on the different color and spin structure of the final states and their interferences with the initial states. In order to investigate these effects experimentally, we propose to measure the ratio of the distributions in Δ ϕ for Z + jet- and multijet production at low and at high transverse momenta, and compare the results to predictions obtained assuming factorization. We examine the role of theoretical uncertainties by performing variations of the factorization scale, renormalization scale and matching scale. In particular, we present a comparative study of matching scale uncertainties in the cases of PB-TMD and collinear parton showers.

    • The magnetic and transport properties of Ni81Fe 19/Al2O3 granular multilayer films were studied in relation to their structural properties as the nominal thickness t of the permalloy (Ni81Fe19) layer was varied near the percolation limit: in the range of 8≤t≤ 16 Å while keeping the nominal thickness of the Al2O3 layers constant at 16 Å. A good structural quality of the multilayers was demonstrated by low angle x-ray reflectivity measurements, and transmission electron microscopy showed the transition from continuous permalloy layers separated by aluminium oxide layers for t= 16 Å to metal grains dispersed in the insulator at t=8 Å. Magnetization measurements showed the gradual transition from ferromagnetic layers to superparamagnetic clusters and grains that successively become blocked as the temperature decreases. A strong correlation between transport and structural properties was observed in the temperature (T) dependence of the electrical resistance measured with the current in the plane in the range of 2 ≤T≤300 K: a gradual change of behavior from continuous permalloy layers with conducting interlayer connections for t=16 Å. to isolated permalloy grains in a dielectric for the film with t= 10 Å. The percolation occurs between 12 and 10 Å, as deduced both from the magnetic and resistive properties. The discontinuous metal films were analyzed within models for thermally assisted tunneling, yielding estimates of the tunneling barrier for intralayer conduction of about 20 meV for t= 10 Å. A significant magnetic field dependence of the resistance increasing with decreasing temperature was observed in all samples.

    • Human breath-hold divers usually perform a series of dives with short intervals. Repeated apneas prolong apneic time, and an accentuated diving response has been suggested to be the cause. The aim of this study was to investigate the effect of repeated apneas on apneic time and diving response in humans. Forty-one subjects performed a series of five apneas with face immersion in water of 10 degrees C, separated by 2-min intervals. Apneas were performed at rest and to individual maximal duration. Heart rate, mean arterial pressure, skin capillary blood flow, and respiratory movements were recorded. Thirty-eight of the subjects were used for analysis of cardiovascular parameters, and in 23 subjects the physiologic breaking point could be detected by the involuntary breathing movements. Heart rate reduction and blood pressure increase were most prominent during the first apneic face immersion, whereas skin capillary blood flow reduction was most intense in the second apneic face immersion. Blood pressure and skin capillary blood flow during recovery from apneic episodes also changed throughout the series. Repetition increased apneic time by 55% and postponed the occurrence of involuntary breathing movements by 27% in subjects passing the physiologic breaking point. We conclude that both physiologic factors, associated with the accumulation of CO2, and psychologic factors, related to the capacity to withstand the respiratory drive, contribute to the prolongation of apneic time whereas an increased diving response does not contribute.
