Within the EUROfusion MST1 work package, a series of experiments has been conducted on AUG and TCV devices to disentangle the role of plasma fueling and plasma shape for the onset of small ELM regimes. On both devices, small ELM regimes with high confinement are achieved if and only if two conditions are fulfilled at the same time. Firstly, the plasma density at the separatrix must be large enough (ne,sep/nG ∼ 0.3), leading to a pressure profile flattening at the separatrix, which stabilizes type-I ELMs. Secondly, the magnetic configuration has to be close to a double null (DN), leading to a reduction of the magnetic shear in the extreme vicinity of the separatrix. As a consequence, its stabilizing effect on ballooning modes is weakened.

  • Outcomes of patients with systemic sclerosis treated with rituximab in contemporary practice: a prospective cohort study

    • To assess the safety and efficacy of rituximab in systemic sclerosis (SSc) in clinical practice.MethodsWe performed a prospective study including patients with SSc from the European Scleroderma Trials and Research (EUSTAR) network treated with rituximab and matched with untreated patients with SSc. The main outcomes measures were adverse events, skin fibrosis improvement, lung fibrosis worsening and steroids use among propensity score-matched patients treated or not with rituximab.Results254 patients were treated with rituximab, in 58% for lung and in 32% for skin involvement. After a median follow-up of 2 years, about 70% of the patients had no side effect. Comparison of treated patients with 9575 propensity-score matched patients showed that patients treated with rituximab were more likely to have skin fibrosis improvement (22.7 vs 14.03 events per 100 person-years; OR: 2.79 [1.47–5.32]; p=0.002). Treated patients did not have significantly different rates of decrease in forced vital capacity (FVC)>10% (OR: 1.03 [0.55–1.94]; p=0.93) nor in carbon monoxide diffusing capacity (DLCO) decrease. Patients having received rituximab were more prone to stop or decrease steroids (OR: 2.34 [1.56–3.53], p<0.0001). Patients treated concomitantly with mycophenolate mofetil had a trend for better outcomes as compared with patients receiving rituximab alone (delta FVC: 5.22 [0.83–9.62]; p=0.019 as compared with controls vs 3 [0.66–5.35]; p=0.012).ConclusionRituximab use was associated with a good safety profile in this large SSc-cohort. Significant change was observed on skin fibrosis, but not on lung. However, the limitation is the observational design. The potential stabilisation of lung fibrosis by rituximab has to be addressed by a randomised trial.

  • Predictors of disease worsening defined by progression of organ damage in diffuse systemic sclerosis: a European Scleroderma Trials and Research (EUSTAR) analysis

    • Mortality and worsening of organ function are desirable endpoints for clinical trials in systemic sclerosis (SSc). The aim of this study was to identify factors that allow enrichment of patients with these endpoints, in a population of patients from the European Scleroderma Trials and Research group database.MethodsInclusion criteria were diagnosis of diffuse SSc and follow-up over 12±3 months. Disease worsening/organ progression was fulfilled if any of the following events occurred: new renal crisis; decrease of lung or heart function; new echocardiography-suspected pulmonary hypertension or death. In total, 42 clinical parameters were chosen as predictors for the analysis by using (1) imputation of missing data on the basis of multivariate imputation and (2) least absolute shrinkage and selection operator regression.ResultsOf 1451 patients meeting the inclusion criteria, 706 had complete data on outcome parameters and were included in the analysis. Of the 42 outcome predictors, eight remained in the final regression model. There was substantial evidence for a strong association between disease progression and age, active digital ulcer (DU), lung fibrosis, muscle weakness and elevated C-reactive protein (CRP) level. Active DU, CRP elevation, lung fibrosis and muscle weakness were also associated with a significantly shorter time to disease progression. A bootstrap validation step with 10 000 repetitions successfully validated the model.ConclusionsThe use of the predictive factors presented here could enable cohort enrichment with patients at risk for overall disease worsening in SSc clinical trials.

    • First systematic measurements of pedestal structure during Ohmic and NBI-assisted Type I ELMy H-modes were performed on the COMPASS tokamak in two dedicated experimental campaigns during 2015 and 2016. By adjusting the NBI heating and a toroidal magnetic field, the electron pedestal temperature was increased from 200 eV up to 300 eV, which allowed reaching pedestal collisionality nu(ped)* < 1 at q(95) similar to 3. COMPASS has approached conditions for the Identity experiment done at JET & DIII-D, complementing the range of scanned rho(ped)*. The pedestal pressure was successfully reproduced by the EPED model. The dependence of pedestal pressure width on nu(ped)* and beta(pol)(ped) is discussed.

    • We study experimentally the ionization of argon atoms by a train of attosecond pulses in the presence of a strong infrared laser field, using a velocity map imaging technique. The recorded momentum distribution strongly depends on the delay between the attosecond pulses and the laser field. We interpret the interference patterns observed for different delays using numerical and analytical calculations within the strong field approximation.

    • We present experiments on field-free molecular alignment of N-2 and CO2 probed with short XUV pulses that are obtained via high-harmonic generation. The XUV pulses induce a dissociative ionization or a Coulomb explosion of the molecule, where the fragment ion recoil (measured using the velocity map imaging technique) provides information on the alignment of the parent molecule at the time of ionization. We discuss how photoelectron detection may in future allow the determination of molecular-frame photoelectron angular distributions and molecular structure.

    • Divertor tiles from the COMPASS tokamak have been examined with ion beam analysis for the determination of deuterium retention and co-deposits. A novel approach was used to draw 2D deuterium retention maps at different depths and correlated to the concentrations of other elements: boron, carbon, oxygen and metals. This approach, which employs all three spatial dimensions, revealed that the deuterium retention pattern is not toroidally symmetric at different depths while the overall deuterium pattern is toroidally symmetric. Analysis of visible-light camera records disclosed that parts of the divertor were not wetted by plasma. The radial profile of deuterium in both wetted and non-wetted ( "shadowed ") divertor regions was compared to strike point positions, angle of incidence, electron temperature T-e, parallel heat flux Q(par) and ion saturation current Isat in order to find the influence of these parameters on deuterium retention. There are certain series of consecutive shots for which correlations between deuterium retention and separate plasma parameters (T-e, I-sat, Q(par)) are high. Combining the three parameters in a linear model or a power law model enabled reconstruction of the radial deuterium retention pattern. After fitting the model to one third of the data points, the deuterium retention for shadowed tiles was reproduced at the other two thirds of the data points within 20% deviation. This study shows the need for detailed tile analysis in three dimensions, the great benefits of broad statistical analysis in solving challenges in fusion, and motivates the investigation of COMPASS tiles after dismantling.

    • We use a train of sub-200 attosecond extreme ultraviolet (XUV) pulses with energies just above the ionization threshold in argon to create a train of temporally localized electron wave packets. We study the energy transfer from a strong infrared (IR) laser field to the ionized electrons as a function of the delay between the XUV and IR fields. When the wave packets are born at the zero crossings of the IR field, a significant amount of energy (similar to 20 eV) is transferred from the field to the electrons. This results in dramatically enhanced above-threshold ionization in conditions where the IR field alone does not induce any significant ionization. Because both the energy and duration of the wave packets can be varied independently of the IR laser, they are valuable tools for studying and controlling strong-field processes.

    • We report the generation, compression, and delivery on target of ultrashort extreme-ultraviolet light pulses using external amplitude and phase control. Broadband harmonic radiation is first generated by focusing an infrared laser with a carefully chosen intensity into a gas cell containing argon atoms. The emitted light then goes through a hard aperture and a thin aluminum filter that selects a 30-eV bandwidth around a 30-eV photon energy and synchronizes all of the components, thereby enabling the formation of a train of almost Fourier-transform-limited single-cycle 170 attosecond pulses. Our experiment demonstrates a practical method for synthesizing and controlling attosecond waveforms.

    • A complete quantum-mechanical description of matter and its interaction with the environment requires detailed knowledge of a number of complex parameters. In particular, information about the phase of wavefunctions is important for predicting the behaviour of atoms, molecules or larger systems. In optics, information about the evolution of the phase of light in time(1) and space(2) is obtained by interferometry. To obtain similar information for atoms and molecules, it is vital to develop analogous techniques. Here we present an interferometric method for determining the phase variation of electronic wave packets in momentum space, and demonstrate its applicability to the fundamental process of single-photon ionization. We use a sequence of extreme-ultraviolet attosecond pulses(3,4) to ionize argon atoms and an infrared laser field, which induces a momentum shear(5) between consecutive electron wave packets. The interferograms that result from the interaction of these wave packets provide useful information about their phase. This technique opens a promising new avenue for reconstructing the wavefunctions(6,7) of atoms and molecules and for following the ultrafast dynamics of electronic wave packets.

    • Background: The World Health Organization (WHO) introduced the International Classification of Functioning, Disability, and Health (ICF) as a scientific method of disability data collection comprised of &gt;1,200 categories describing the spectrum of impairment types (functional, symptoms-based and anatomical) under the bio-psycho-social model with consideration of environmental and personal factors (pf). ICF Core Sets and ICF Checklists are streamlined disease-specific resources for clinical use, service provision, and for use in health economics and health policy. ICF can disclose strengths and weaknesses across multiple patient-reported outcome measures (PROMs) and help consolidate best-fitting question-items from multiple PROMs. Interstitial lung diseases (ILDs), are generally progressive, with restrictive physiology sometimes occurring in the context of multi-organ autoimmunity/inflammatory conditions such as connective tissue diseases (CTDs). In spite of significant associated morbidity and potential disability, ILD has yet to be linked to the ICF.Methods: Each instrument and their question-items within the consensus-recommended core sets for clinical trials in ILD were deconstructed to single concept units, and then linked per updated ICF linkage rules. Inter-linker agreement was established. Three additional subsequently validated measures were also included.Results: One-hundred-eleven ICF categories were identified for ten PROMs and three traditional objective measures that were amenable to ICF linkage. The proportion of agreement ranged from 0.79 (95% CI: 0.62, 0.91) to 0.93 (0.76, 0.99) with the overall proportion of inter-linker agreement being very high 0.86 (0.82, 0.89) for the initial instruments, with 94–100% for the three additional PROMs. Thirty-four new ‘Personal Factors’ emerged to capture disease-specific qualities not elsewhere described in ICF, e.g. ‘pf_embarrassed by cough’ or ‘pf_panic/afraid when can’t get a breath’.Conclusion: This first known effort in ICF linkage of ILD has provided important revelations on the current utility of the ICF in lung disease. Results have indicated areas for meaningful assessment of ICF descriptors for lung impairment. The mapping across PROMs provides insight into possibilities of developing more streamline and precise instrumentation. Finally, familiarity with the ICF in ILD may enable clinicians to experience a smoother transition with the imminent harmonization of ICD and ICF, ICD-11.

    • High-order harmonic generation (HHG) provides scalable sources of coherent extreme ultraviolet radiation with pulse duration down to the attosecond time scale. Efficient HHG requires the constructive interplay between microscopic and macroscopic effects in the generation volume, which can be achieved over a large range of experimental parameters from the driving field properties to those of the generating medium. Here, we present a systematic study of the harmonic yield as a function of gas pressure and medium length. Two regimes for optimum yield are identified, supporting the predictions of a recently proposed analytical model. Our observations are independent on the focusing geometry and, to a large extent, on the pulse duration and laser intensity, providing a versatile approach to HHG optimization.

    • We determine relative photoemission time delays between valence electrons in different noble gas atoms (Ar, Ne and He) in an energy range between 31 and 37 eV. The atoms are ionized by an attosecond pulse train synchronized with an infrared laser field and the delays are measured using an interferometric technique. We compare our results with calculations using the random phase approximation with exchange and multi-configurational Hartree-Fock. We also investigate the influence of the different ionization angular channels.

    • We study temporally localized electron wave packets, generated using a train of extreme ultraviolet (XUV) attosecond pulses to ionize the target atoms. Both the electron wave packets and the attosecond pulse train ( APT) are characterized using the same technique, based on interference of two-photon transitions in the continuum. We study, in particular, the energy transfer from a moderately strong infrared (IR) field to the electron wave packets as a function of time delay between the XUV and the IR fields. The use of an APT to generate the electron wave packets enables the generation at times not accessible through tunneling ionization. We find that a significant amount of energy is transferred from the IR field to the electron wave packets, when they are generated at a zero-crossing of the IR laser field. This energy transfer results in a dramatically enhanced above-threshold ionization even at IR intensities that alone are not strong enough to induce any significant ionization.

    • We characterize the temporal structure of high-order harmonic radiation on both the femtosecond and attosecond time scales. The harmonic emission is characterized by mixed-color two-photon ionization with an infrared femtosecond laser using a Mach-Zehnder interferometer where both pump and probe arms travel completely separate paths. In a first experiment, we measure the duration and chirp of individual harmonics. In a second experiment, we resolve, for the first time with this type of setup, the attosecond beating of several harmonics generated under conditions similar to the first experiment. We suggest that the results of both measurements can be combined to determine the full attosecond time structure of the harmonic emission.

    • The quantum mechanical motion of electrons and nuclei in systems spatially confined to the molecular dimensions occurs on the sub-femtosecond to the femtosecond timescales respectively. Consequently, the study of ultrafast electronic and, in specific cases, nuclear dynamics requires the availability of light pulses with attosecond (asec) duration and of sufficient intensity to induce two-photon processes, essential for probing the intrinsic system dynamics. The majority of atoms, molecules and solids absorb in the extreme-ultraviolet (XUV) spectral region, in which the synthesis of the required attosecond pulses is feasible. Therefore, the XUV spectral region optimally serves the study of such ultrafast phenomena. Here, we present a detailed review of the first 10-GW class XUV attosecond source based on laser driven high harmonic generation in rare gases. The pulse energy of this source largely exceeds other laser driven attosecond sources and is comparable to the pulse energy of femtosecond Free-Electron-Laser (FEL) XUV sources. The measured pulse duration in the attosecond pulse train is 650 ± 80 asec. The uniqueness of the combined high intensity and short pulse duration of the source is evidenced in non-linear XUV-optics experiments. It further advances the implementation of XUV-pump-XUV-probe experiments and enables the investigation of strong field effects in the XUV spectral region.

    • High-order harmonics generated through the interaction of atoms and strong laser fields are a versatile, laboratory-scale source of extreme ultraviolet (XUV) radiation on a femtosecond or even attosecond time-scale. In order to be a useful experimental tool, however, this radiation has to be well characterized, both temporally and spectrally. In this paper we discuss how multi-photon, multi-colour ionization processes can be used to completely characterize either individual harmonics or attosecond pulse trains. In particular, we discuss the influence of the intensity and duration of the probe laser, and how these parameters effect the accuracy of the XUV characterization.

    • A novel multilayer mirror was designed and fabricated based on a recently developed three-material technology aimed both at reaching reflectivities of about 20% and at controlling dispersion over a bandwidth covering photon energies between 35 and 50 eV. The spectral phase upon reflection was retrieved by measuring interferences in a two-color ionization process using high-order harmonics produced from a titanium: sapphire laser. We demonstrate the feasibility of designing and characterizing phase-controlled broadband optics in the extreme-ultraviolet domain, which should facilitate the manipulation of attosecond pulses for applications.

An inherent difficulty with techniques involving the tight focus of a laser beam is the continuous distribution of intensities contained within the focus, which can vary over several orders of magnitude. The present study adopts time of flight mass spectrometry coupled with a high intensity (8 X 10(15) Wcm(-2)), ultra-short (20 fs) pulse laser in order to investigate the ionization and dissociation of the aromatic molecule benzene-d1 (C6H5D) as a function of intensity within a focused laser beam, by scanning the laser focus in the direction of propagation, while detecting ions produced only in a "thin" slice (400 and 800 mum) of the focus. The resultant TOF mass spectra varies significantly, highlighting the dependence on the range of specific intensities accessed and their volumetric weightings on the ionization/dissociation pathways accessed.

    • The temporal confinement of high harmonic generation (HHG) via modulation of the polarization of the fundamental pulse is studied in both temporal and spectral domains. In the temporal domain, a collinear cross-correlation setup using a 40 fs IR pump for the HHG and a 9 fs IR pulse to probe the generated emission is used to measure the XUV pulse duration. The observed temporal confinement is found to be consistent with theoretical predictions. An increased confinement is observed when a 9 fs pulse is used to generate the harmonics. An important spectral broadening, including a continuum background, is also measured. Theoretical calculations show that with 10 fs driving pulses, either one or two main attosecond pulses are created depending on the value of the carrier envelope phase.

    • We use a train of sub-200 attosecond extreme ultraviolet (XUV) pulses with energies just above the ionization threshold in argon to create a train of temporally localized electron wave packets. We study the energy transfer from a strong infrared (IR) laser field to the ionized electrons as a function of the delay between the XUV and IR fields. When the wave packets are born at the zero crossings of the IR field, a significant amount of energy (∼20 eV) is transferred from the field to the electrons. This results in dramatically enhanced above-threshold ionization in conditions where the IR field alone does not induce any significant ionization. Because both the energy and duration of the wave packets can be varied independently of the IR laser, they are valuable tools for studying and controlling strong-field processes.

    • We present experimental measurements of attosecond pulse trains. The characterization of the pulses uses a spectral interferometry technique that is implemented with a Mach-Zehnder interferometer. This allows us to manipulate independently the pump and probe pulses for a wide range of applications. By letting the attosecond pulses pass through metallic films, we can in particular compensate for the intrinsic chirp of the attosecond pulses corresponding to the plateau spectral region, thus getting pulses as short as 170 attoseconds-only 1.2 optical cycles at the central frequency. The measurement technique is also applicable for determination of the group delay of thin XUV-transparent films and relative delay in the photoionization process. Our experimental method is applied to attosecond pulse trains created by 35- and 9-fs laser pulses, and the shortest train observed consists of three or four pulses.

    • We have generated attosecond pulse trains in an ensemble of randomly aligned nitrogen molecules. Measurements of the high-order harmonic relative phases and amplitudes allow us to reconstruct the temporal profile of the attosecond pulses. We show that in the considered spectral range, the latter is very similar to the pulse train generated in argon under the same conditions. We discuss the possible influence of the molecular structure in the generation process, and how it can induce subtle differences on the relative phases.
