  • The INSIG2 rs7566605 polymorphism is not associated with body mass index and breast cancer risk

    • BACKGROUND: The single nucleotide polymorphism rs7566605, located in the promoter of the INSIG2 gene, has been the subject of a strong scientific effort aimed to elucidate its possible association with body mass index (BMI). The first report showing that rs7566605 could be associated with body fatness was a genome-wide association study (GWAS) which used BMI as the primary phenotype. Many follow-up studies sought to validate the association of rs7566605 with various markers of obesity, with several publications reporting inconsistent findings. BMI is considered to be one of the measures of choice to evaluate body fatness and there is evidence that body fatness is related with an increased risk of breast cancer (BC).METHODS: we tested in a large-scale association study (3,973 women, including 1,269 invasive BC cases and 2,194 controls), nested within the EPIC cohort, the involvement of rs7566605 as predictor of BMI and BC risk.RESULTS AND CONCLUSIONS: In this study we were not able to find any statistically significant association between this SNP and BMI, nor did we find any significant association between the SNP and an increased risk of breast cancer overall and by subgroups of age, or menopausal status.

    • Major societal challenges such as environmental sustainability, availability of energy and raw materials, and globalisation are creating new requirements for many actors in society. These new requirements relate to efficiency, flexibility, sustainability, and competitiveness. While these aspects have all been around for some time, and many systems have been locally optimised with regards to one or a few of these requirements, recent advances in communication and processing capabilities promise increased possibilities for connecting different parts of society, allowing optimal operation on a much larger scale.While industrial production systems have been controlled electronically for decades, the digitisation of market channels and consumer systems, together with the possibility to interconnect different production facilities, now allow for automated interaction along the whole supply chain from raw materials to end users.Simultaneously, increased demand for efficiency forces increased specialisation among actors, which with increased possibilities of interconnectivity, creates large enterprises of cooperating, specialised stakeholders.One of the major remaining obstacles for a widespread adaptation of more intelligent, more connected systems, able to deliver these envisioned results, is a coherent approach to the engineering and management of Systems-of-Systems involving very large numbers of devices and operating across several automation domains.For traditional automation systems there are established engineering procedures and numerous standards for engineering data, although most are focused on the static processes that have traditionally been the norm. For full integration with a digital society many of the existing automation systems will need significant modifications and as many automation systems are ageing and in need of replacement, a suitable solution to this may be a large scale migration to new automation solutions.The work presented in this thesis includes some new approaches and methodologies to utilise the existing engineering procedures and standards, while introducing some of the flexibility proposed by the emerging technologies. The major technical solutions presented consist of a structure way to organise connected systems and how they are related, regardless of engineering standards used to design their interactions, and an approach to allow configuration of heterogeneous systems through service interactions.Further contributions include an approach for migrating certain categories of existing industrial control systems to a service oriented architecture, as a basic outline for adaption of the next generation of automation systems in industry.Certain remaining challenges have been identified, which have to be addressed for a successful launch of widespread interconnected automation systems based on Internet of Things and associated technologies.

  • Clinical Pathway for Coronary Atherosclerosis in Patients Without Conventional Modifiable Risk Factors JACC State-of-the-Art Review

    • Reducing the incidence and prevalence of standard modifiable cardiovascular risk factors (SMuRFs) is critical to tackling the global burden of coronary artery disease (CAD). However, a substantial number of individuals develop coronary atherosclerosis despite no SMuRFs. SMuRFless patients presenting with myocardial infarction have been observed to have an unexpected higher early mortality compared to their counterparts with at least 1 SMuRF. Evidence for optimal management of these patients is lacking. We assembled an international, multidisciplinary team to develop an evidence-based clinical pathway for SMuRFless CAD patients. A modified Delphi method was applied. The resulting pathway confirms underlying atherosclerosis and true SMuRFless status, ensures evidence-based secondary prevention, and considers additional tests and interventions for less typical contributors. This dedicated pathway for a previously overlooked CAD population, with an accompanying registry, aims to improve outcomes through enhanced adherence to evidence-based secondary prevention and additional diagnosis of modifiable risk factors observed.   • Polymorphisms in fatty acid metabolism-related genes are associated with colorectal cancer risk

    • Colorectal cancer (CRC) is the third most common malignant tumor and the fourth leading cause of cancer death worldwide. The crucial role of fatty acids for a number of important biological processes suggests a more in-depth analysis of inter-individual differences in fatty acid metabolizing genes as contributing factor to colon carcinogenesis. We examined the association between genetic variability in 43 fatty acid metabolism-related genes and colorectal risk in 1225 CRC cases and 2032 controls participating in the European Prospective Investigation into Cancer and Nutrition study. Three hundred and ninety two single-nucleotide polymorphisms were selected using pairwise tagging with an r(2) cutoff of 0.8 and a minor allele frequency of > 5%. Conditional logistic regression models were used to estimate odds ratios and corresponding 95% confidence intervals. Haplotype analysis was performed using a generalized linear model framework. On the genotype level, hydroxyprostaglandin dehydrogenase 15-(NAD) (HPGD), phospholipase A2 group VI (PLA2G6) and transient receptor potential vanilloid 3 were associated with higher risk for CRC, whereas prostaglandin E receptor 2 (PTGER2) was associated with lower CRC risk. A significant inverse association (P < 0.006) was found for PTGER2 GGG haplotype, whereas HPGD AGGAG and PLA2G3 CT haplotypes were significantly (P < 0.001 and P = 0.003, respectively) associated with higher risk of CRC. Based on these data, we present for the first time the association of HPGD variants with CRC risk. Our results support the key role of prostanoid signaling in colon carcinogenesis and suggest a relevance of genetic variation in fatty acid metabolism-related genes and CRC risk.

    • Rapid technology advances and increasing digitalization bring new opportunities but also put new challenges to modern production systems. Industrial agent concepts and technologies can be used to address the needs of modern Cyber-Physical Production Systems (CPPS). To do so, their interplay, especially in industrial settings, needs to be better understood and assessed. Key directions for investigations that are relevant for industrial agent based CPPS are analyzed in this work, mainly the design patterns, interfaces between agents and CPPS, metrics to evaluate the quality of agent based CPPS, and distributed intelligence implemented in or by agents. Although efforts exist both in agent and CPPS domains, their amalgamation still has several aspects that are under-investigated, especially when considering large-scale systems of CPPS, and the proposed four directions discussed in this work, seem promising to address several of the key underlying challenges.

    • Cyber-physical systems constitutes a framework to develop intelligent, distributed, resilient, collaborative and cooperative systems, promoting the fusion of computational entities and physical devices. Agent technology plays a crucial role to develop this kind of systems by offering a decentralized, distributed, modular, robust and reconfigurable control structure. This paper describes the implementation of a summer school aiming to enhance the participants’ knowledge in the field of multi-agent systems applied to industrial environments, being able to gain the necessary theoretical and practical skills to develop real industrial agent based applications. This is accomplished in an international framework where individual knowledge and experiences are shared in a complementary level.

    • Several research agendas worldwide are targeting the development of Industrial Cyber-physical Systems as the next generation of intelligent embedded devices with improved interaction capabilities. These devices, and their potential uses, are though to deliver a radical increase in system sustainability, reconfigurability and flexibility which is perceived to be the root of the so called 4th Industrial Revolution. However such technical systems, at the envisioned revolutionary scale, do not exist just yet and require a convergent and multidisciplinary research and development efforts. The academia curricula are also, albeit slowly, adjusting to the emerging education requirements. The Summer School on Intelligent Agents in Automation is a joint effort from several researchers in core areas of the 4th Industrial Revolution landscape to close the gap and promote advanced education in this context. This paper describes the implementation of the 2nd edition of the event as well as the experience and reflections resultant from it.

    • Learning and intelligent algorithms are the basis for Cyber-Physical Production Systems (CPPS). They enable flexibility through reconfiguration ease and fault tolerance by ensuring that the CPPS adapts to changing conditions. However, in order to exhibit such characteristics, CPPS require a proper support for reliably handling the real time behavior of the physical systems they are in control of. This paper presents and discusses basic requirements for control software, which enables flexible and adaptable automated Production Systems (aPS) modelled according to the CPPS concept. In doing so, the paper exposes the main architectural guidelines and rationale for where to place and operate intelligent algorithms in the context of industrial automation for continuous processes.

    • Industry 4.0, an initiative from Germany, has become a globally adopted term in the past decade. Many countries have introduced similar strategic initiatives, and a considerable research effort has been spent on developing and implementing some of the Industry 4.0 technologies. At the ten-year mark of the introduction of Industry 4.0, the European Commission announced Industry 5.0. Industry 4.0 is considered to be technology-driven, whereas Industry 5.0 is value-driven. The co-existence of two Industrial Revolutions invites questions and hence demands discussions and clarifications. We have elected to use five of these questions to structure our arguments and tried to be unbiased for the selection of the sources of information and for the discussions around the key issues. It is our intention that this article will spark and encourage continued debate and discussion around these topics.
