  • An investigation of antihypertensive class, dementia, and cognitive decline: A meta-analysis.

    • High blood pressure is one of the main modifiable risk factors for dementia. However, there is conflicting evidence regarding the best antihypertensive class for optimizing cognition. Our objective was to determine whether any particular antihypertensive class was associated with a reduced risk of cognitive decline or dementia using comprehensive meta-analysis including reanalysis of original participant data.To identify suitable studies, MEDLINE, Embase, and PsycINFO and preexisting study consortia were searched from inception to December 2017. Authors of prospective longitudinal human studies or trials of antihypertensives were contacted for data sharing and collaboration. Outcome measures were incident dementia or incident cognitive decline (classified using the reliable change index method). Data were separated into mid and late-life (>65 years) and each antihypertensive class was compared to no treatment and to treatment with other antihypertensives. Meta-analysis was used to synthesize data.Over 50,000 participants from 27 studies were included. Among those aged >65 years, with the exception of diuretics, we found no relationship by class with incident cognitive decline or dementia. Diuretic use was suggestive of benefit in some analyses but results were not consistent across follow-up time, comparator group, and outcome. Limited data precluded meaningful analyses in those ≤65 years of age.Our findings, drawn from the current evidence base, support clinical freedom in the selection of antihypertensive regimens to achieve blood pressure goals.The review was registered with the international prospective register of systematic reviews (PROSPERO), registration number CRD42016045454.

  • Respiratory viral infections in otherwise healthy humans with inherited IRF7 deficiency

    • Autosomal recessive IRF7 deficiency was previously reported in three patients with single critical influenza or COVID-19 pneumonia episodes. The patients’ fibroblasts and plasmacytoid dendritic cells produced no detectable type I and III IFNs, except IFN-β. Having discovered four new patients, we describe the genetic, immunological, and clinical features of seven IRF7-deficient patients from six families and five ancestries. Five were homozygous and two were compound heterozygous for IRF7 variants. Patients typically had one episode of pulmonary viral disease. Age at onset was surprisingly broad, from 6 mo to 50 yr (mean age 29 yr). The respiratory viruses implicated included SARS-CoV-2, influenza virus, respiratory syncytial virus, and adenovirus. Serological analyses indicated previous infections with many common viruses. Cellular analyses revealed strong antiviral immunity and expanded populations of influenza- and SARS-CoV-2–specific memory CD4+ and CD8+ T cells. IRF7-deficient individuals are prone to viral infections of the respiratory tract but are otherwise healthy, potentially due to residual IFN-β and compensatory adaptive immunity.

  • Is there long-term value of pathology scoring in immunoglobulin A nephropathy? : A validation study of the Oxford Classification for IgA Nephropathy (VALIGA) update

    • Background: It is unknown whether renal pathology lesions in immunoglobulin A nephropathy (IgAN) correlate with renal outcomes over decades of follow-up.Methods: In 1130 patients of the original Validation Study of the Oxford Classification for IgA Nephropathy (VALIGA) cohort, we studied the relationship between the MEST score (mesangial hypercellularity, M; endocapillary hypercellularity, E; segmental glomerulosclerosis, S; tubular atrophy/interstitial fibrosis, T), crescents (C) and other histological lesions with both a combined renal endpoint [50% estimated glomerular filtration rate (eGFR) loss or kidney failure] and the rate of eGFR decline over a follow-up period extending to 35 years [median 7 years (interquartile range 4.1-10.8)].Results: In this extended analysis, M1, S1 and T1-T2 lesions as well as the whole MEST score were independently related with the combined endpoint (P < 0.01), and there was no effect modification by age for these associations, suggesting that they may be valid in children and in adults as well. Only T lesions were associated with the rate of eGFR loss in the whole cohort, whereas C showed this association only in patients not treated with immunosuppression. In separate prognostic analyses, the whole set of pathology lesions provided a gain in discrimination power over the clinical variables alone, which was similar at 5 years (+2.0%) and for the whole follow-up (+1.8%). A similar benefit was observed for risk reclassification analyses (+2.7% and +2.4%).Conclusion: Long-term follow-up analyses of the VALIGA cohort showed that the independent relationship between kidney biopsy findings and the risk of progression towards kidney failure in IgAN remains unchanged across all age groups and decades after the renal biopsy.

  • Risk factors for progression in children and young adults with IgA nephropathy : an analysis of 261 cases from the VALIGA European cohort

    • There is a need for early identification of children with immunoglobulin A nephropathy (IgAN) at risk of progression of kidney disease. Data on 261 young patients [age < 23 years; mean follow-up of 4.9 (range 2.5-8.1) years] enrolled in VALIGA, a study designed to validate the Oxford Classification of IgAN, were assessed. Renal biopsies were scored for the presence of mesangial hypercellularity (M1), endocapillary hypercellularity (E1), segmental glomerulosclerosis (S1), tubular atrophy/interstitial fibrosis (T1-2) (MEST score) and crescents (C1). Progression was assessed as end stage renal disease and/or a 50 % loss of estimated glomerular filtration rate (eGFR) (combined endpoint) as well as the rate of renal function decline (slope of eGFR). Cox regression and tree classification binary models were used and compared. In this cohort of 261 subjects aged < 23 years, Cox analysis validated the MEST M, S and T scores for predicting survival to the combined endpoint but failed to prove that these scores had predictive value in the sub-group of 174 children aged < 18 years. The regression tree classification indicated that patients with M1 were at risk of developing higher time-averaged proteinuria (p < 0.0001) and the combined endpoint (p < 0.001). An initial proteinuria of ae0.4 g/day/1.73 m(2) and an eGFR of < 90 ml/min/1.73 m(2) were determined to be risk factors in subjects with M0. Children aged < 16 years with M0 and well-preserved eGFR (> 90 ml/min/1.73 m(2)) at presentation had a significantly high probability of proteinuria remission during follow-up and a higher remission rate following treatment with corticosteroid and/or immunosuppressive therapy. This new statistical approach has identified clinical and histological risk factors associated with outcome in children and young adults with IgAN.

  • An Integrated Antenna System for 4G and Millimeter-Wave 5G Future Handheld Devices

    • In this work, an integrated antenna system with Defected Ground Structure (DGS) is presented for Fourth Generation (4G) and millimeter (mm)-wave Fifth Generation (5G) wireless applications and handheld devices. The proposed design with overall dimensions of 110 mm x 75 mm is modeled on 0.508 mm thick Rogers RT/Duroid 5880 substrate. Radiating structure consists of antenna arrays excited by the T-shape 1 x 2 power divider/combiner. Dual bands for 4G centered at 3.8 GHz and 5.5 GHz are attained, whereas the 10-dB impedance bandwidth of 24.4 - 29.3 GHz is achieved for the 5G antenna array. In addition, a peak gain of 5.41 dBi is demonstrated across the operating bandwidth of the 4G antenna array. Similarly, for the 5G mm-wave configuration the attained peak gain is 10.29 dBi. Moreover, significant isolation is obtained between the two antenna modules ensuring efficient dual-frequency band operation using a single integrated solution. To endorse the concept, antenna prototype is fabricated and far-field measurements are procured. Simulated and measured results exhibit coherence. Also the proposed design is investigated for the beam steering capability of the mm-wave 5G antenna array using CST(R)MWS(R). The demonstrated structure offers various advantages including compactness, wide bandwidth, high gain, and planar configuration. Hence, the attained radiation characteristics prove the suitability of the proposed design for the current and future wireless handheld devices.

    • Takayasu arteritis is a rare inflammatory disease of large arteries. We performed a genetic study in Takayasu arteritis comprising 6,670 individuals (1,226 affected individuals) from five different populations. We discovered HLA risk factors and four non-HLA susceptibility loci in VPS8, SVEP1, CFL2, and chr13q21 and reinforced IL12B, PTK2B, and chr21q22 as robust susceptibility loci shared across ancestries. Functional analysis proposed plausible underlying disease mechanisms and pinpointed ETS2 as a potential causal gene for chr21q22 association. We also identified >60 candidate loci with suggestive association (p < 5 x 10(-s)) and devised a genetic risk score for Takayasu arteritis. Takayasu arteritis was compared to hundreds of other traits, revealing the closest genetic relatedness to inflammatory bowel disease. Epigenetic patterns within risk loci suggest roles for monocytes and B cells in Takayasu arteritis. This work enhances understanding of the genetic basis and pathophysiology of Takayasu arteritis and provides clues for potential new therapeutic targets.

    • A novel, frequency selective surface (FSS) based, data encoding structure amenable to be used as a chipless RFID tag is proposed. The data encoding structure is made up of finite repetitions of a unit cell fabricated on commercially available grounded FR4 substrate having physical dimensions of 15 × 15mm2. The unit cell is composed of numerous T-shaped resonant elements arranged as two atypical sets of concentric nested loops. Alteration in geometry of the encoding circuit, attained by inclusion or omission of nested resonators, corresponds to a particular data sequence. Each encoded data sequence is manifested in the frequency domain as a distinct spectral signature. The proposed 10-bit tag is both compact and robust, and remains interrogable in response to illuminating electromagnetic waves at various angles of incidence.

    • A 27-bit circular shaped, highly-dense, fully printable chipless radio frequency identification (RFID) tag is presented in this letter. High data capacity is provided in a compact size. The total dimension of the tag is 22 × 22mm2. For exciting the tag, the linearly polarized incident plane wave is used. The circular shaped tag structure is analyzed for three different substrates, i.e., Rogers RT/duroid®/5870, Taconic TLX-0 and DuPont™ Kapton® HN. The spectral range for Rogers RT/duroid®/5870 is 3.3-13.5 GHz, 3.4-13.6 GHz for Taconic TLX-0 and 3.7-15.1 GHz for DuPont™ Kapton® HN substrate. Flexibility is achieved by using Kapton® HN substrate. The presented tag is low-cost and flexible; hence it can be easily deployed on wide range of objects.

    • High mobility in ITS, especially V2V communication networks, allows increasing coverage and quick assistance to users and neighboring networks, but also degrades the performance of the entire system due to fluctuation in the wireless channel. How to obtain better QoS during multimedia transmission in V2V over future generation networks (i.e., edge computing platforms) is very challenging due to the high mobility of vehicles and heterogeneity of future IoT-based edge computing networks. In this context, this article contributes in three distinct ways: to develop a QoS-aware, green, sustainable, reliable, and available (QGSRA) algorithm to support multimedia transmission in V2V over future IoT-driven edge computing networks; to implement a novel QoS optimization strategy in V2V during multimedia transmission over IoT-based edge computing platforms; to propose QoS metrics such as greenness (i.e., energy efficiency), sustainability (i.e., less battery charge consumption), reliability (i.e., less packet loss ratio), and availability (i.e., more coverage) to analyze the performance of V2V networks. Finally, the proposed QGSRA algorithm has been validated through extensive real-time datasets of vehicles to demonstrate how it outperforms conventional techniques, making it a potential candidate for multimedia transmission in V2V over self-adaptive edge computing platforms.
