    • Due to limited groundwater resources in arid and semi-arid areas, conjunctive use of surface water and groundwater is becoming increasingly important. In view of this, there are needs to improve the methods for conjunctive use of surface and groundwater. Using numerical models, optimization algorithms, and machine learning, we created a new comprehensive methodological structure for optimal allocation of surface and groundwater resources and optimal extraction of groundwater. The surface and groundwater system was simulated by MODFLOW to reflect groundwater transport and aquifer conditions. The important Marvdasht aquifer in the south of Iran was used as an experimental study area to test the methodology. In this context, we developed an optimal conjunctive exploitation model for dry and wet years using two new evolutionary algorithms, i.e., whale optimization algorithm (WOA) and firefly algorithm (FA). These were used in combination with the group method of data handling (GMDH) and least squares support vector machine (LS-SVM) to estimate sustainable groundwater withdrawal. The results show that the FA is more efficient in calculating optimal conjunctive water supply so that about 61% of water needs were met in the worst scenario for surface water resources, while it was 52% using the WOA. By applying the optimal conjunctive model during the simulation period, the groundwater level increased by about 0.4 and 0.55 m using the WOA and FA, respectively. The results of Taylor’s diagram, box plot diagram, and rock diagram with error evaluation criteria, i.e., root mean square error (RMSE), mean absolute error (MAE), and Nash–Sutcliffe efficiency (NSE), showed that the GMDH (RMSE = 6.04 MCM, MAE = 3.89 MCM, and NSE = 0.99) was slightly better than LS-SVM (RMSE = 6.36 MCM, MAE = 4.50 MCM, and NSE = 0.98) to estimate optimal groundwater use. The results show that machine learning models are cost- and time-effective solutions to estimate optimal exploitation of groundwater resources in complex combined surface and groundwater supply problems. The methodology can be used to better estimate sustainable exploitation of groundwater resources by water resources managers.

  • Global, regional, and national burden of stroke and its risk factors, 1990-2019 : a systematic analysis for the Global Burden of Disease Study 2019

    • Background Regularly updated data on stroke and its pathological types, including data on their incidence, prevalence, mortality, disability, risk factors, and epidemiological trends, are important for evidence-based stroke care planning and resource allocation. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) aims to provide a standardised and comprehensive measurement of these metrics at global, regional, and national levels. Methods We applied GBD 2019 analytical tools to calculate stroke incidence, prevalence, mortality, disability-adjusted life-years (DALYs), and the population attributable fraction (PAF) of DALYs (with corresponding 95% uncertainty intervals [UIs]) associated with 19 risk factors, for 204 countries and territories from 1990 to 2019. These estimates were provided for ischaemic stroke, intracerebral haemorrhage, subarachnoid haemorrhage, and all strokes combined, and stratified by sex, age group, and World Bank country income level. Findings In 2019, there were 12.2 million (95% UI 11.0-13.6) incident cases of stroke, 101 million (93.2-111) prevalent cases of stroke, 143 million (133-153) DALYs due to stroke, and 6.55 million (6.00-7.02) deaths from stroke. Globally, stroke remained the second-leading cause of death (11.6% [10.8-12.2] of total deaths) and the third-leading cause of death and disability combined (5.7% [5.1-6.2] of total DALYs) in 2019. From 1990 to 2019, the absolute number of incident strokes increased by 70.0% (67.0-73.0), prevalent strokes increased by 85.0% (83.0-88.0), deaths from stroke increased by 43.0% (31.0-55.0), and DALYs due to stroke increased by 32.0% (22.0-42.0). During the same period, age-standardised rates of stroke incidence decreased by 17.0% (15.0-18.0), mortality decreased by 36.0% (31.0-42.0), prevalence decreased by 6.0% (5.0-7.0), and DALYs decreased by 36.0% (31.0-42.0). However, among people younger than 70 years, prevalence rates increased by 22.0% (21.0-24.0) and incidence rates increased by 15.0% (12.0-18.0). In 2019, the age-standardised stroke-related mortality rate was 3.6 (3.5-3.8) times higher in the World Bank low-income group than in the World Bank high-income group, and the age-standardised stroke-related DALY rate was 3.7 (3.5-3.9) times higher in the low-income group than the high-income group. Ischaemic stroke constituted 62.4% of all incident strokes in 2019 (7.63 million [6.57-8.96]), while intracerebral haemorrhage constituted 27.9% (3.41 million [2.97-3.91]) and subarachnoid haemorrhage constituted 9.7% (1.18 million [1.01-1.39]). In 2019, the five leading risk factors for stroke were high systolic blood pressure (contributing to 79.6 million [67.7-90.8] DALYs or 55.5% [48.2-62.0] of total stroke DALYs), high body-mass index (34.9 million [22.3-48.6] DALYs or 24.3% [15.7-33.2]), high fasting plasma glucose (28.9 million [19.8-41.5] DALYs or 20.2% [13.8-29.1]), ambient particulate matter pollution (28.7 million [23.4-33.4] DALYs or 20.1% [16.6-23.0]), and smoking (25.3 million [22.6-28.2] DALYs or 17.6% [16.4-19.0]). Interpretation The annual number of strokes and deaths due to stroke increased substantially from 1990 to 2019, despite substantial reductions in age-standardised rates, particularly among people older than 70 years. The highest age-standardised stroke-related mortality and DALY rates were in the World Bank low-income group. The fastest-growing risk factor for stroke between 1990 and 2019 was high body-mass index. Without urgent implementation of effective primary prevention strategies, the stroke burden will probably continue to grow across the world, particularly in low-income countries.

  • Prevalence of alcohol consumption in most at-risk groups in Iran : a systematic review and meta-analysis

    • Introduction: Many studies have assessed the prevalence of alcohol consumption in Iran. In this study, we investigated the prevalence of alcohol consumption in different groups. Material and Methods: We searched international and databases including PubMed, Web of Science, Scopus and we searched two main Farsi-language index databases including Scientific Information Database (SID) and the Irandoc. Grey literature search was also performed in Google Scholar, PsycINFO, ProQuest Dissertation and Theses without time limit until June 2020. All studies that reported the prevalence of alcohol consumption among Iranians were included in current study. Results: From 9,038 screened studies, 109 studies with 925,480 participants were included. The pooled prevalence of alcohol consumption was estimated 24% (95% CI: 18.0–31.0), 12% (95%CI: 10.0–14.0), 14% (95%CI: 13.0–15.0), 19% (95%CI: 13.0–26.0), 15% (95%CI: 3.0–28.0) among prisoners, general population, students, drivers, and street children, respectively. Conclusion: This systematic review indicated higher prevalence of alcohol consumption in prisoners, drivers, and street children than the general population and other subgroups. The distribution of the alcohol prevalence studies in different groups and provinces were heterogeneous. The lack of studies among some groups and in regions warrants further, attention.

    • Background: Efforts to establish the 2015 baseline and monitor early implementation of the UN Sustainable Development Goals (SDGs) highlight both great potential for and threats to improving health by 2030. To fully deliver on the SDG aim of “leaving no one behind”, it is increasingly important to examine the health-related SDGs beyond national-level estimates. As part of the Global Burden of Diseases, Injuries, and Risk Factors Study 2017 (GBD 2017), we measured progress on 41 of 52 health-related SDG indicators and estimated the health-related SDG index for 195 countries and territories for the period 1990–2017, projected indicators to 2030, and analysed global attainment. Methods: We measured progress on 41 health-related SDG indicators from 1990 to 2017, an increase of four indicators since GBD 2016 (new indicators were health worker density, sexual violence by non-intimate partners, population census status, and prevalence of physical and sexual violence [reported separately]). We also improved the measurement of several previously reported indicators. We constructed national-level estimates and, for a subset of health-related SDGs, examined indicator-level differences by sex and Socio-demographic Index (SDI) quintile. We also did subnational assessments of performance for selected countries. To construct the health-related SDG index, we transformed the value for each indicator on a scale of 0–100, with 0 as the 2·5th percentile and 100 as the 97·5th percentile of 1000 draws calculated from 1990 to 2030, and took the geometric mean of the scaled indicators by target. To generate projections through 2030, we used a forecasting framework that drew estimates from the broader GBD study and used weighted averages of indicator-specific and country-specific annualised rates of change from 1990 to 2017 to inform future estimates. We assessed attainment of indicators with defined targets in two ways: first, using mean values projected for 2030, and then using the probability of attainment in 2030 calculated from 1000 draws. We also did a global attainment analysis of the feasibility of attaining SDG targets on the basis of past trends. Using 2015 global averages of indicators with defined SDG targets, we calculated the global annualised rates of change required from 2015 to 2030 to meet these targets, and then identified in what percentiles the required global annualised rates of change fell in the distribution of country-level rates of change from 1990 to 2015. We took the mean of these global percentile values across indicators and applied the past rate of change at this mean global percentile to all health-related SDG indicators, irrespective of target definition, to estimate the equivalent 2030 global average value and percentage change from 2015 to 2030 for each indicator. Findings: The global median health-related SDG index in 2017 was 59·4 (IQR 35·4–67·3), ranging from a low of 11·6 (95% uncertainty interval 9·6–14·0) to a high of 84·9 (83·1–86·7). SDG index values in countries assessed at the subnational level varied substantially, particularly in China and India, although scores in Japan and the UK were more homogeneous. Indicators also varied by SDI quintile and sex, with males having worse outcomes than females for non-communicable disease (NCD) mortality, alcohol use, and smoking, among others. Most countries were projected to have a higher health-related SDG index in 2030 than in 2017, while country-level probabilities of attainment by 2030 varied widely by indicator. Under-5 mortality, neonatal mortality, maternal mortality ratio, and malaria indicators had the most countries with at least 95% probability of target attainment. Other indicators, including NCD mortality and suicide mortality, had no countries projected to meet corresponding SDG targets on the basis of projected mean values for 2030 but showed some probability of attainment by 2030. For some indicators, including child malnutrition, several infectious diseases, and most violence measures, the annualised rates of change required to meet SDG targets far exceeded the pace of progress achieved by any country in the recent past. We found that applying the mean global annualised rate of change to indicators without defined targets would equate to about 19% and 22% reductions in global smoking and alcohol consumption, respectively; a 47% decline in adolescent birth rates; and a more than 85% increase in health worker density per 1000 population by 2030. Interpretation: The GBD study offers a unique, robust platform for monitoring the health-related SDGs across demographic and geographic dimensions. Our findings underscore the importance of increased collection and analysis of disaggregated data and highlight where more deliberate design or targeting of interventions could accelerate progress in attaining the SDGs. Current projections show that many health-related SDG indicators, NCDs, NCD-related risks, and violence-related indicators will require a concerted shift away from what might have driven past gains—curative interventions in the case of NCDs—towards multisectoral, prevention-oriented policy action and investments to achieve SDG aims. Notably, several targets, if they are to be met by 2030, demand a pace of progress that no country has achieved in the recent past. The future is fundamentally uncertain, and no model can fully predict what breakthroughs or events might alter the course of the SDGs. What is clear is that our actions—or inaction—today will ultimately dictate how close the world, collectively, can get to leaving no one behind by 2030.

    • Background: The World Health Organization program on Ending Childhood Obesity (WHO-ECHO) has developed a comprehensive and integrated package of recommendations to address childhood obesity. The present study, entitled IRAN-ECHO, was designed and implemented in the framework of the WHO-ECHO program.Methods: The IRAN-ECHO program is implementing multicomponent interventions by considering life course dimensions. The program has two parts: a population approach and an individual approach. The population approach considers different periods in life, including prenatal, infancy, childhood, and adolescence, as well as family and society. The individual approach targets those children or adolescents with overweight or obesity; this part is conducted as a referral system that is now integrated in the current national health system. As part of the population approach, a quasi-experimental study was conducted in six provinces to compare the status before and after implementing parts of the interventions. By intersectoral collaboration with different organizations, multicomponent interventions are conducted for different age groups.Results: The IRAN-ECHO program is being conducted in six provinces, and will be considered in all provinces in the near future. Its main effects could be assessed in future years. Part of this program that was conducted as a quasi-experimental survey comprised 7149 students and showed that a high percentage of students had acceptable knowledge about adverse health effects of overweight and obesity. However, the knowledge about the low nutritional value of unhealthy snacks such as potato chips, puffs, industrial juices, and carbonated drinks was not appropriate. Many participants had the undesirable attitude of skipping one of the main meals when attempting to lose weight.Conclusions: The IRAN-ECHO program is presenting the feasibility of conducting the WHO-ECHO recommendations in Iran. The scope of potential policy recommendations to decrease childhood obesity is extensive and includes various elements. This program considers multisectoral interventions through population and individual approaches. The multicomponent interventions of this program address the obesogenic environment by considering the life course dimensions. It is expected that, by its life course interventions, it could help in primordial and primary prevention of noncommunicable diseases.

    • Thermal discomfort is one of the main triggers for occupants' interactions with components of the built environment such as adjustments of thermostats and/or opening windows and strongly related to the energy use in buildings. Understanding causes for thermal (dis-)comfort is crucial for design and operation of any type of building. The assessment of human thermal perception through rating scales, for example in post-occupancy studies, has been applied for several decades; however, long-existing assumptions related to these rating scales had been questioned by several researchers. The aim of this study was to gain deeper knowledge on contextual influences on the interpretation of thermal perception scales and their verbal anchors by survey participants. A questionnaire was designed and consequently applied in 21 language versions. These surveys were conducted in 57 cities in 30 countries resulting in a dataset containing responses from 8225 participants. The database offers potential for further analysis in the areas of building design and operation, psycho-physical relationships between human perception and the built environment, and linguistic analyses.

A standardized method to measure and quantify women's birth experiences is required to study satisfaction of childbirth care. Therefore, this study aimed to translate and culturally adapt the Childbirth Experience Questionnaire (CEQ) for use in Iran. Materials and Methods: This was a cross-sectional study including 203 women who attended 2 hospitals and 2 health centers and met the inclusion criteria in Ahvaz city, between February 2013 and June 2014. After forward and backward translation of the Swedish CEQ into Persian language, content validity was assessed by an expert panel. Scale reliability (internal consistency and test-retest reliability) was assessed with respect to the psychometric properties of the scale. Results: Minor cultural differences were identified and resolved during the translation process. One item was excluded. The intraclass correlation coefficient ranging from 0.63 to 0.90 was satisfactory. Conclusions: The Persian version of the CEQ appears to be valid and reliable; hence, it can be an effective tool in designing childbirth experience interventions and also childbirth care and education interventions for the promotion of positive childbirth experience in Iranian women.

  • Maximizing and stabilizing luminescence from halide perovskites with potassium passivation

    • Metal halide perovskites are of great interest for various high-performance optoelectronic applications. The ability to tune the perovskite bandgap continuously by modifying the chemical composition opens up applications for perovskites as coloured emitters, in building-integrated photovoltaics, and as components of tandem photovoltaics to increase the power conversion efficiency. Nevertheless, performance is limited by non-radiative losses, with luminescence yields in state-of-the-art perovskite solar cells still far from 100 per cent under standard solar illumination conditions. Furthermore, in mixed halide perovskite systems designed for continuous bandgap tunability2 (bandgaps of approximately 1.7 to 1.9 electronvolts), photoinduced ion segregation leads to bandgap instabilities. Here we demonstrate substantial mitigation of both non-radiative losses and photoinduced ion migration in perovskite films and interfaces by decorating the surfaces and grain boundaries with passivating potassium halide layers. We demonstrate external photoluminescence quantum yields of 66 per cent, which translate to internal yields that exceed 95 per cent. The high luminescence yields are achieved while maintaining high mobilities of more than 40 square centimetres per volt per second, providing the elusive combination of both high luminescence and excellent charge transport. When interfaced with electrodes in a solar cell device stack, the external luminescence yield—a quantity that must be maximized to obtain high efficiency—remains as high as 15 per cent, indicating very clean interfaces. We also demonstrate the inhibition of transient photoinduced ion-migration processes across a wide range of mixed halide perovskite bandgaps in materials that exhibit bandgap instabilities when unpassivated. We validate these results in fully operating solar cells. Our work represents an important advance in the construction of tunable metal halide perovskite films and interfaces that can approach the efficiency limits in tandem solar cells, coloured-light-emitting diodes and other optoelectronic applications.

    • All studies on oxygen-evolution reaction by Mn oxides in the presence of cerium(IV) ammonium nitrate (CAN) have been so far carried out by synthesizing Mn oxides in the first step. And then, followed by the investigation of the Mn oxides in the presence of oxidants for oxygen-evolution reaction (OER). This paper presents a case study of a new and promising strategy for in situ catalyst synthesis by the adding Mn-II to either CAN or KMnO4/CAN solution, resulting in the formation of Mn-based catalysts for OER. The catalysts were characterized by scanning electron microscopy, energy-dispersive spectroscopy, transmission electron microscopy, X-ray diffraction, Fourier transform infrared spectroscopy, X-ray absorption spectroscopy, and X-ray photoelectron spectroscopy. Both compounds contained nano-sized particles that catalyzed OER in the presence of CAN. The turnover frequencies for both catalysts were 0.02 (mmolO2 /mol(Mn).

  • Effects of chemotherapy and hormonal therapy for early breast cancer on recurrence and 15-year survival: an overview of the randomised trials

    • Background Quinquennial overviews (1985-2000) of the randomised trials in early breast cancer have assessed the 5-year and 10-year effects of various systemic adjuvant therapies on breast cancer recurrence and survival. Here, we report the 10-year and 15-year effects. Methods Collaborative meta-analyses were undertaken of 194 unconfounded randomised trials of adjuvant chemotherapy or hormonal therapy that began by 1995. Many trials involved CMF (cyclophosphamide, methotrexate, fluorouracil), anthracycline-based combinations such as FAC (fluorouracil, doxombicin, cyclophosphamide) or FEC (fluorouracil, epirubicin, cyclophosphamide), tamoxifen, or ovarian suppression: none involved taxanes, trastuzumab, raloxifene, or modem aromatase inhibitors. Findings Allocation to about 6 months of anthracycline-based polychemotherapy (eg, with FAC or FEC) reduces the annual breast cancer death rate by about 38% (SE 5) for women younger than 50 years of age when diagnosed and by about 20% (SE 4) for those of age 50-69 years when diagnosed, largely irrespective of the use of tamoxifen and of oestrogen receptor (ER) status, nodal status, or other tumour characteristics. Such regimens are significantly (2p=0 . 0001 for recurrence, 2p<0 . 00001 for breast cancer mortality) more effective than CMF chemotherapy. Few women of age 70 years or older entered these chemotherapy trials. For ER-positive disease only, allocation to about 5 years of adjuvant tamoxifen reduces the annual breast cancer death rate by 31% (SE 3), largely irrespective of the use of chemotherapy and of age (<50, 50-69, &GE; 70 years), progesterone receptor status, or other tumour characteristics. 5 years is significantly (2p<0 . 00001 for recurrence, 2p=0 . 01 for breast cancer mortality) more effective than just 1-2 years of tamoxifen. For ER-positive tumours, the annual breast cancer mortality rates are similar during years 0-4 and 5-14, as are the proportional reductions in them by 5 years of tamoxifen, so the cumulative reduction in mortality is more than twice as big at 15 years as at 5 years after diagnosis. These results combine six meta-analyses: anthracycline-based versus no chemotherapy (8000 women); CMF-based versus no chemotherapy (14 000); anthracycline-based versus CMF-based chemotherapy (14 000); about 5 years of tamoxifen versus none (15 000); about 1-2 years of tamoxifen versus none (33 000); and about 5 years versus 1-2 years of tamoxifen (18 000). Finally, allocation to ovarian ablation or suppression (8000 women) also significantly reduces breast cancer mortality, but appears to do so only in the absence of other systemic treatments. For middle-aged women with ER-positive disease (the commonest type of breast cancer), the breast cancer mortality rate throughout the next 15 years would be approximately halved by 6 months of anthracycline-based chemotherapy (with a combination such as FAC or FEC) followed by 5 years of adjuvant tamoxifen. For, if mortality reductions of 38% (age <50 years) and 20% (age 50-69 years) from such chemotherapy were followed by a further reduction of 31% from tamoxifen in the risks that remain, the final mortality reductions would be 57% and 45%, respectively (and, the trial results could well have been somewhat stronger if there had been full compliance with the allocated treatments). Overall survival would be comparably improved, since these treatments have relatively small effects on mortality from the aggregate of all other causes. Interpretation Some of the widely practicable adjuvant drug treatments that were being tested in the 1980s, which substantially reduced 5-year recurrence rates (but had somewhat less effect on 5-year mortality rates), also substantially reduce 15-year mortality rates. Further improvements in long-term survival could well be available from newer drugs, or better use of older drugs.

    • Objective: There is a need to develop easy-to-use tools to screen periodontal condition in daily practice. This study aimed to evaluate the FDI World Dental Federation “Chairside Guide” (FDI-CG) developed by the Task Team of the FDI Global Periodontal Health Project (GPHP) as a potential tool for screening. Methods: Databases from 3 centres in Germany, Hong Kong, and Spain (n = 519) were used to evaluate the association of the FDI-CG and its individual items with the periodontitis case definitions proposed by the Centers for Disease Control and Prevention (CDC) and the American Academy of Periodontology (AAP) for population-based surveillance of periodontitis. Results: Statistically significant differences were observed among the databases for the prevalence of periodontitis and the items included in the FDI-CG. The FDI-CG score and its individual components were significantly associated with the periodontal status in the individual databases and the total sample, with bleeding on probing showing the strongest association with severe periodontitis (odds ratio [OR] = 12.9, 95% CI [5.9; 28.0], P < .001, for those presenting bleeding on probing >50%), followed by age (OR = 4.8, 95% CI [1.7; 4.2], P = .004, for those older than 65 years of age). Those subjects with a FDI-CG score >10 had an OR of 54.0 (95% CI [23.5; 124.2], P < .001) and presented with severe periodontitis. A significant correlation was found between the different FDI-CG scoring categories (mild, moderate, and severe) and the categories for mild, moderate, and severe periodontitis using the Centers for Disease Control and Prevention and the American Academy of Periodontology criteria (r = 0.57, Spearman rank correlation test, P < .001). Conclusion: The FDI Chairside Guide may represent a suitable tool for screening the periodontal condition by general practitioners in daily dental practice. © 2020 The Authors

  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • Evaporation is one of the main components of the hydrological cycle, and its estimation is crucial and important for water resources management issues. Access to a reliable estimator tool for evaporation simulation is important in arid and semi-arid areas such as Iran, which lose more than 70% of their received precipitation by evaporation. Current research employs the Bayesian Regularization (BR) and Scaled Conjugate Gradient (SCG) algorithms for training the Multilayer Perceptron (MLP) model (as MLP-BR and MLP-SCG) and comparing their performance with the Levenberg–Marquardt (LM) algorithm (as MLP-LM). For this purpose, 16 meteorological variables were used on a daily scale; including temperature (5 variables), air pressure (4 variables), and relative humidity (6 variables) as input data sets, and pan evaporation as the target variable of the MLP model. The surveys were conducted during the period of 2006–2021 in Fars Province in Iran, which is a semi-arid region and has many natural lakes. Various combinations of input-target pairs were tested by several learning algorithms, resulting in seven input scenarios: (1) temperature-based (T), (2) pressure-based (F), (3) humidity-based (RH), (4) temperature–pressure-based (T-F), (5) temperature–humidity-based (T-RH), (6) pressure–humidity-based (F-RH) and (7) temperature–pressure–humidity-based (T-F-RH). The results indicated the relative superiority of the three-component scenario of T-F-RH, and a considerable weakness in the single-component scenario of RH compared with others. The best performance with a root mean square error (RMSE) equal to 1.629 and 1.742 mm per day and a Wilmott Index (WI) equal to 0.957 and 0.949 (respectively for validation and test periods) belonged to the MLP-BR model. Additionally, the amount of R2 (greater than 84%), Nash-Sutcliff efficiency (greater than 0.8) and normalized RMSE (less than 0.1) all indicate the reliability of the estimates provided for the daily pan evaporation. In the comparison between the studied training algorithms, two algorithms, BR and SCG, in most cases, showed better performance than the powerful and common LM algorithm. The obtained results suggest that future researchers in this field consider BR and SCG training algorithms for the supervised training of MLP for the numerical estimation of pan evaporation by the MLP model.

    • The purpose of this study was to compare the status of psychosocial factors and glycemic control in insulin-naïve and insulin-experienced people with type 2 diabetes (T2D). In this observational study on people with T2D, demographic, self-care behavior, resources, and affective variables as well as health-related quality of life were assessed and compared in insulin-naïve and insulin-experienced considering the number of oral glucose-lowering drugs (OGLDs). Measured variable path analysis was used to test the association among variables and their effect on HbA1c in both groups. In total, 215 insulin-naïve and 165 insulin-experienced patients were recruited in this study. The mean duration of diabetes was 11.7 ± 7.0 years in insulin-experienced and 6.8 ± 5.4 years in insulin-naïve (p < 0.001). The mean hemoglobin A1c (HbA1c) was significantly higher in insulin-experienced subjects irrespective of the number of OGLDs [68 ± 20 mmol/mol (8.4 ± 1.8%) vs. 56 ± 16 mmol/mol (7.3 ± 1.4%); p < 0.001]. Moreover, insulin-experienced subjects had significantly higher level of diabetes-related distress (2.2 ± 0.9 vs. 1.9 ± 0.8), depression (9.5 ± 5.5 vs. 8.1 ± 5.1), anxiety (18.3 ± 12.0 vs. 15.1 ± 10.5), and lower knowledge of insulin use considering the results of 9-item insulin-use subscale of Michigan diabetes knowledge test (mean 3.9 ± 1.8) compared to insulin-naïve subjects (p < 0.05). Higher levels of distress, depression, and anxiety are found in insulin-experienced people with T2D. Therefore, one should be aware that, at the time of insulin need/initiation, people with T2D have reached a more vulnerable state and this should be taken into consideration when implementing a complex insulin initiation plan.

    • The expected increase in precipitation and temperature in Scandinavia, and especially short-time heavy precipitation, will increase the frequency of flooding. Urban areas are the most vulnerable, and specifically, the road infrastructure. The accumulation of large volumes of water and sediments on road-stream intersections gets severe consequences for the road drainage structures. This study integrates the spatial and temporal soil moisture properties into the research about flood prediction methods by a case study of two areas in Sweden, Vastra Gotaland and Varmland, which was affected by severe flooding in August 2014. Soil moisture data are derived from remote-sensing techniques, with a focus on the soil moisture-specific satellites ASCAT and SMOS. Furthermore, several physical catchments descriptors (PCDs) are analyzed and the result shows that larger slopes and drainage density, in general, mean a higher risk of flooding. The precipitation is the same; however, it can be concluded that more precipitation in most cases gives higher soil moisture values. The lack, or the dimensioning, of road drainage structures seems to have a large impact on the flood risk as more sediment and water can be accumulated at the road-stream intersection. The results show that the method implementing soil moisture satellite data is promising for improving the reliability of flooding.

    • The licentiate thesis consists of three papers with the particular topic in public housing. They discuss how the public housing companies manage the transition to higher economic demands meeting increased customer and market requirements. These studies focus specifically on how the public housing company deal with market challenges associated with the decision to demolish, maintain and/or new construction. Market-oriented perspective can be a tool for the public housing companies to achieve better customer value and enhance economic development. Although the market orientation concept has contributed to valuable improvements in research, the thesis assumes that it is necessary to distinguish between that the public housing companies operate market-oriented to meet customer requirements and their focus on innovation.Paper I develops market/innovation types and then investigates how public housing companies adapt to these types. It was found that economic conditions in the municipality have a major impact on the housing companies, causing them to act innovatively and create superior customer value by innovations. The study confirms that the implementation of market and innovation orientation contributes to competitive advantages in growing markets, while weak economic conditions impair implementation in declining markets.Paper II addresses how public housing companies in declining markets act based on the concept of market intelligence. This study suggested and tested whether there is a positive link between collecting customer information, disseminating it in the organization, and responding to customer needs, and whether this link has an impact on strategic performance. The result shows that weak links exist in the process; the efficiency of intelligence distribution in public housing companies is affected mainly by their responsiveness to customer needs.Paper III also addresses the public housing companies’ market strategies in declining markets. This study, based on a market-strategic perspective, compares how public housing companies act in relation to customer wants compared to the private housing market. The result shows that public housing companies are more engaged in carrying out new construction, renovation, and reconstruction, as well as taking more social responsibility compared to the private sector. In particular, their concern for the customers’ social needs is evident.

    • Purpose - The purpose of this study is to examine how external factors moderate public housing companies’ market orientation and strategic performance relationship.Design/methodology/approach – The quantitative method was applied to data from a survey sent to 289 public housing companies in Sweden. A hierarchical, multicollinearity diagnostics–moderated regression analysis is used to test the research hypotheses.Findings – The results reveal moderating factors. The companies take several initiatives to inform themselves about customers’ needs and distribute the information within the company, but economic conditions, as well as market and technological turbulence in the municipalities, moderate the relationship between market orientation and strategic performance. Economic conditions make it difficult for public housing companies to strategically act based on market needs when making decisions and planning construction strategies (strategic performance).Research limitations/implications – This study is limited by focusing on public housing companies, a sector that differs radically from the open market. The study highlights the effects of moderating factors that are important for companies’ strategic performance and long-term construction strategies. From this limited focus, researchers might use the results to compare both similar and different market situations.Practical implications – The results of the study are useful for companies facing a similar market situation of external moderating constraints. The result might be used in future research related to the area in focus.Originality –This research adds new knowledge to market research by including the impact of economic conditions, which provide insight into how to develop and use market knowledge in real estate and public housing markets.

    • The purpose of this study is first to examine and compare sustainable strategies within public and private housing companies in declining markets in central Sweden. Then, the study evaluates the impact of new legislation that requires public housing companies to act in a ‘businesslike’ way, in the same way as a long-term private company. A quantitative study was conducted based on a survey sent to 72 housing companies. The results show that public housing companies are more strategy oriented than private housing companies. The results can be viewed as an on-going interaction process, where a company’s strategies affect its profit. The study aims toincrease the understanding of activities within housing companies in adeclining market that engage the companies in sustainable strategies to improve their market knowledge and profit.

    • This study examines how companies in declining markets operate in the context of market intelligence, responding to customer needs and applying them to strategic performance. A quantitative survey was sent to 214 public housing companies. The results indicate that market intelligence creates commitment and is significant. A positive relationship was found between data gathering, dissemination, and responsiveness, which indicates that the companies comprehend market needs but companies have difficult to manage construction strategies that improve strategic performance. There was a low value of strategic performance; a link between market intelligence and the chosen strategy was not confirmed. Companies know what the market wants but base their decision on previous strategic performance on economic conditions in the municipality instead.

    • The development of public housing companies in Sweden is strongly influenced by legal, economic, and demographic changes. The companies long tradition of taking responsibility for customer welfare in living have recently been criticized for excessive dominance over price setting, which is claimed to give them competitive advantages in the housing market. New legislation in 2011 requires financial return on investment, implying increased competition with radical changes for the companies. Because of the law, they find themselves transitioning from the role of being responsible for living conditions in Sweden to having to work in a businesslike manner with increased competition.The purpose of this dissertation is to analyze how the public housing companies’ deal with market orientation constructs and what their impact is on construction strategy choices. Theoretically, the market orientation concept is seen as a tool for companies to create superior value for the customers and influence strategic performance, which is a relationship tested and analyzed in this dissertation.The research was conducted through data collection stages using qualitative and quantitative methods. In the first stage, a qualitative pilot study was conducted with interviews of 15 managers in 11 public housing companies in central Sweden. In the second stage, two quantitative comparative studies of public and private housing companies in declining markets in central Sweden were conducted, collecting data from 23 (22 survey respondents) public and 37 private housing companies (16 respondents). In the third and final stage, two quantitative studies were conducted, collecting data from all 289 public housing companies (165 respondents) in Sweden.Results of the five studies show, firstly, that economic condition in the municipality have a major impact on the housing companies’ construction strategies, causing them to act innovatively in order to create superior customer value. Secondly, market orientation efforts contribute to competitive advantages in growing markets, while weak economic conditions limit the companies’ construction strategy choices in declining markets. After analyzing the link between market orientation constructs and construction strategy in declining markets, results showed that the public housing companies demonstrate higher responsiveness to customer demands compared to the private sector, but these have no effects on construction strategy. Thirdly, the analysis of moderating (external) and mediating (corporate social responsibility) effects on the market orientation and construction strategy relationship confirms that economic conditions in society influence construction strategy planning and decisions. The results of the mediating effects of CSR indicate that environmental and social issues have a positive influence on the market orientation and construction strategy relationship.

    • In this paper we propose two new attacks on UMTS network. Both attacks exploit the UMTS-GSM interworking and are possible in the GSM access area of UMTS network. The first attack allows the attacker to eavesdrop on the entire traffic of the victim UMTS subscriber in the GERAN coverage of the UMTS network. The second attack is an impersonation attack i.e. the attacker impersonates a genuine UMTS subscriber to a UMTS network and fools the network to provide services at the expense of the victim subscriber in its GERAN coverage.

    • In this paper we first present three new attacks on Universal Mobile Telecommunication System (UMTS) in access domain. We exploit the interoperation of UMTS network with its predecessor, Global System for Mobile communications (GSMs). Two attacks result in the interception of the entire traffic of the victim UMTS subscriber in the GSM access area of UMTS network. These attacks are applicable, regardless of the strength of the selected GSM encryption algorithm. The third attack is an impersonation attack and allows the attacker to impersonate a genuine UMTS subscriber to a UMTS network and fool the network to provide services at the expense of the victim subscriber. Then, we propose some countermeasures to strengthen the UMTS network against the mentioned attacks with emphasis on the practicality in present networks. The proposed solutions require limited change of the network elements or protocols, insignificant additional computational load on the network elements and negligible additional bandwidth consumption on the network links.

    • Skeletal dysplasias are a heterogeneous group of disorders ranging from mild to lethal skeletal defects. We investigated two unrelated families with individuals presenting with a severe skeletal disorder. In family NMD02, affected individuals had a dysostosis multiplex-like skeletal dysplasia and severe short stature (<-8.5 SD). They manifested increasingly coarse facial features, protruding abdomens, and progressive skeletal changes, reminiscent of mucopolysaccharidosis. The patients gradually lost mobility and the two oldest affected individuals died in their twenties. The affected child in family ID01 had coarse facial features and severe skeletal dysplasia with clinical features similar to mucopolysaccharidosis. She had short stature, craniosynostosis, kyphoscoliosis, and hip-joint subluxation. She died at the age of 5 years. Whole-exome sequencing identified two homozygous variants c.133C>T; p.(Arg45Trp) and c.215dupA; p.(Tyr72Ter), respectively, in the two families, affecting an evolutionary conserved gene TMEM251 (NM_001098621.1). Immunofluorescence and confocal studies using human osteosarcoma cells indicated that TMEM251 is localized to the Golgi complex. However, p.Arg45Trp mutant TMEM251 protein was targeted less efficiently and the localization was punctate. Tmem251 knockdown by small interfering RNA induced dedifferentiation of rat primary chondrocytes. Our work implicates TMEM251 in the pathogenesis of a novel disorder and suggests its potential function in chondrocyte differentiation.

    • PurposeThe purpose of the study is to explore the impact of company leadership and organizational culture in the development of organizational innovation.Design/methodology/approachThe qualitative approach is applied via 12 interviews from six different organizations, encompassing both service providers and product manufacturers, in the countries of Sweden and Sri Lanka.FindingsLeadership and organizational culture together influence the development of innovation. The findings suggest that process- and job-oriented culture, as well as transactional leadership, have both positive and negative impacts in developing innovation. Result-oriented, employee-oriented, pragmatic culture and an open system, together with transformational leadership, are found to be more effective.Research limitations/implicationsThis study was limited to six organizations, specific industries and national cultures. Researchers could explore in depth to identify which factors particularly lead to the failure or success of innovativeness in a particular industry, through comparing national cultures.Practical implicationsThis study helps managers decide how to serve best within the organizational and national culture, while learning about the influential result of the culture and leadership on innovation. With the aid of this study, organizations will be able to identify factors that may help them to be innovative and competitive in the market.OriginalityThis study highlights the importance of leadership and organizational cultures effect on an organization's innovation, as well as showing how it improves the companies ability to both meet consumer expectations and contribute to their national economy.

    • This chapter explores through an understanding of the history and development of the situation of statelessness across the region – a consequence of nation-state formation, state succession and/or discriminatory ethnic, religious and gender law and policy – the striking commonalities in both the causes and consequences of statelessness. It focuses on how the Middle East and North Africa (MENA) region lacks a common language on statelessness which may have contributed to the fact that these groups are often discussed in isolation of one another, with a limited sense of having a shared cause. The chapter also focuses on how the current framework for understating statelessness in the region can obstruct opportunities to address the issue regionally and can even be used to perpetuate statelessness. Discrimination against women in nationality laws can be found in legislation around the world. Statelessness has also resulted from the 'Arabisation' movement that took hold of the region in the 1960s and 1970s.

    • Flat plate solar collectors (FPSCs) are the most often used as solar collectors due to their easiness of installation and usage. The current research investigates the energy efficiency of FPSC using different mass concentration with varied base fluids containing Graphene nanofluids (T-Gr). Mass concentration of 0.1%-wt., 0.075%-wt., 0.050%-wt. and 0.025%-wt. were mixed with ethylene glycol (EG) and distilled water (DW) in different rations. The operating conditions were volumetric flowrate (1.5, 1 and 0.5) LPM 50 °C-input fluid temperature and 800 W/m2-global solar irradiation. Scanning electron microscope (SEM) and energy dispersive X-ray (EDX) were used to synthesize the thermally treated nanomaterial. The theoretical investigation indicated that using T-Gr nanosuspensions increased the FPSC efficiency in comparison with the host fluid for all examined mass concentrations and volumetric flowrates. In quantitative terms, the maximum thermal effectiveness improvement for the EG, (DW:70 + EG:30) and DW:EG (DW:50 + EG:50) and using flowrates of (1.5, 1 and 0.5) LPM were 12.54%, 12.46% and 12.48%. In addition, the research results pointed that the essential parameters (i.e., loss energy (FRUL)) and gain energy (FR (τα)) of the T-Gr nanofluids were increased significantly.

  • Person-centered-care climate in a tertiary hospital: Staff perspective

    • Introduction: Most health care systems strive to improve the quality, safety and value of healthcare, with an emphasis on moving towards patient-centered care/person-centered care (PCC) approach. The aim of the current study was to assess health care providers’ perspectives on PCC climate in hospital setting and to identify the role of providers in determining the perception of the PCC climate.Methods: A survey, using person-centered climate questionnaire-staff version, was employed in health care providers of a tertiary care hospital. Data included variables age, gender, education level, occupation, and years of experience and three PCC dimensions. PCC overall and subscale scores were reported as mean and standard deviation. Factors associated with PCC climate perception were analyzed using a Poisson model.Results: Out of 1216 respondents; the majority 47% aged between 18 and 34 years; 79% women, 68% were nurses. The overall mean score was 45.96±15.36 (range 0–70). Subscale scores were Safety 20.15±5.0 (range 0–30), Everydayness 12.02±3.52 (range 0–20) and Community 13.79±3.34 (range 0–20). Increasing age was a significant factor associated with PCC scores for the overall, safety, everydayness, and community scales, with a positive association. Lower scores were reported more by women compared with men, for overall (p=0.0005), and everydayness (p=0.006) scales. Higher safety scores were reported by health care providers with a diploma compared to master’s degree (p=0.009), Ph.D. (p=0.007), for technicians compared with nurses (p=0.007), and for day shift compared with day/night shift workers (p=0.025). PCC scores were not significantly different across health care providers’ years of experience.Conclusion: There is a room for PCC climate improvement based on the low scores compared to the literature. The study findings indicated that the main factors associated with HCPs’ perception of PCC were higher age and female gender, and these factors would benefit from further research.

    • Assessment is a key activity within the higher education system. From the student´s perspective, assessment shapes what is to be learned; from the perspective of the educator it gives clear inputs about the quality of learning. Thus, as defined by the term, ‘Constructive alignment’, by careful planning of the assessment strategy and wise selection of the assessment methods, the teaching-learning experience can be designed in an aligned manner towards the achievement of the learning outcomes. Although, defined in different ways, the basic purpose of assessment is to work as a feedback tool to both, students and teachers, to improve the student learning. In this work, we review and comment on theoretical aspects of assessment including its significance and characteristics. Moreover, some important considerations regarding the design of the assessment activities are presented, together with the description of some methods for assessing learning. With this work we would like to invite the reader to reevaluate the significance of assessment within the learning experience, and by this, hopefully the educator is “inspired” to improve the assessment activities in the classroom or another less conventional learning space.

  • Investigating the Ability to Identify New Constructions in Urban Areas Using Images from Unmanned Aerial Vehicles, Google Earth, and Sentinel-2

    • One of the main problems in developing countries is unplanned urban growth and land use change. Timely identification of new constructions can be a good solution to mitigate some environmental and social problems. This study examined the possibility of identifying new constructions in urban areas using images from unmanned aerial vehicles (UAV), Google Earth and Sentinel-2. The accuracy of the land cover map obtained using these images was investigated using pixel-based processing methods (maximum likelihood, minimum distance, Mahalanobis, spectral angle mapping (SAM)) and object-based methods (Bayes, support vector machine (SVM), K-nearest-neighbor (KNN), decision tree, random forest). The use of DSM to increase the accuracy of classification of UAV images and the use of NDVI to identify vegetation in Sentinel-2 images were also investigated. The object-based KNN method was found to have the greatest accuracy in classifying UAV images (kappa coefficient = 0.93), and the use of DSM increased the classification accuracy by 4%. Evaluations of the accuracy of Google Earth images showed that KNN was also the best method for preparing a land cover map using these images (kappa coefficient = 0.83). The KNN and SVM methods showed the highest accuracy in preparing land cover maps using Sentinel-2 images (kappa coefficient = 0.87 and 0.85, respectively). The accuracy of classification was not increased when using NDVI due to the small percentage of vegetation cover in the study area. On examining the advantages and disadvantages of the different methods, a novel method for identifying new rural constructions was devised. This method uses only one UAV imaging per year to determine the exact position of urban areas with no constructions and then examines spectral changes in related Sentinel-2 pixels that might indicate new constructions in these areas. On-site observations confirmed the accuracy of this method.

  • Use of Landsat 8 and UAV Images to Assess Changes in Temperature and Evapotranspiration by Economic Trees following Foliar Spraying with Light-Reflecting Compounds

    • Pistachio is an important economic crop in arid and semi-arid regions of Iran. A major problem leading to a reduction in crop quality and reduced marketability is extreme air temperature in summer, which causes sunburn of pistachio leaves and fruit. A solution proposed to deal with the negative effects of high temperatures and increase water consumption efficiency in pistachio orchards is use of light-reflecting compounds. This study investigated the effect of foliar application of gypsum, sulfur, and NAX-95 (calcium-based suspension coating) to trees in a pistachio orchard (150 ha) in central Iran. The effect of these foliar products is assessed at plot scale, using control plots sprayed with calcium sulfate, based on temperature and evapotranspiration changes analyzed through remote sensing. Landsat 8 sensor images and RGB images collected by UAVs (spatial resolution of 30 m and 20 cm, respectively), on the same dates, before and after foliar spray application, were merged using the PCA method and bilinear interpolation re-sampling. Land surface temperature (LST) was then estimated using the split-window algorithm, and daily evapotranspiration using the surface energy balance algorithm for land (SEBAL) algorithm. A land use map was prepared and used to isolate pistachio trees in the field and assess weed cover, whose effect was not accounted. The results showed that temperature remained constant in the control plot between the spraying dates, indicating no environmental changes. In the main plots, gypsum had the greatest effect in reducing the temperature of pistachio trees. The plots with foliar spraying with gypsum displayed a mean tree temperature (47–48 °C) decrease of 3.3 °C in comparison with the control plots (>49 °C), leading to an average decline in evapotranspiration of 0.18 mm/day. NAX-95 and sulfur reduced tree temperature by on average 1.3 °C and 0.6 °C, respectively. Thus, gypsum is the most suitable foliar-spraying compound to lower the temperature of pistachio trees, reduce the water requirement, and increase crop productivity.

    • In this study, three types of hydrophobized alkyl-modified magnetic nanoparticles (MNPs) comprising direct alkylated-MNPs (A-MNPs), silica-mediated alkyl MNPs (A-SiMNPs), and arginine (Arg)-mediated alkyl MNPs (A-RMNPs) were synthesized successfully. For this purpose, the co-precipitation method was used to synthesize, and octadecyl trimethoxy silane (OTMS) was used as a functionalizing agent. Accordingly, the hydrophobic octadecyl moieties were connected to MNPs. The nanoparticles (NPs) were characterized by XRD, SEM, FTIR, CHN, DLS, and zeta potential analyses. The synthesized coated MNPs represented a decrease in surface charge and magnetization alongside increased surface hydrophobicity and size. It was revealed that the alkylation process was successfully performed to all three MNPs, but A-SiMNPs showed the highest hydrophobicity. Additionally, the novel A-RMNPs, as the most biocompatible type, and A-MNPs showed the highest magnetization among the synthesized MNPs. The results indicate that synthesized NPs can play an important role in bio applications. However, it was revealed that alkyl chains are easily connected to all three MNPs, and that A-MNPs contained the highest alkyl chains and could affect the re-folding and denaturation process of recombinant proteins.

  • Internet addiction and sleep problems : A systematic review and meta-analysis

    • The pathological use of the internet – conceptualized as ‘internet addiction’ – might be crucial in initiating and increasing sleep disturbances in the community. While inconsistent evidence is reported regarding the association of internet addiction and sleep disturbances, the severity of this association remains unclear. This systematic review and meta-analysis were conducted to increase our understanding of the relationship between internet addiction and sleep disturbances. A systematic review was conducted through Scopus, PubMed Central, ProQuest, ISI Web of Knowledge, and EMBASE using keywords related to internet addiction and sleep problems. Observational studies (cohort, case-control or cross-sectional studies) focusing on association between internet addiction and sleep disturbances including sleep problems and sleep duration were selected. A meta-analysis using random-effect model was conducted to calculate the odds ratio (OR) for experiencing sleep problems and standardized mean differences (SMDs) for sleep duration. Eligible studies (N = 23) included 35,684 participants. The overall pooled OR of having sleep problems if addicted to the internet was 2.20 (95% CI: 1.77–2.74). Additionally, the overall pooled SMDs for sleep duration for the IA group compared to normal internet users was −0.24 (95% CI: −0.38, −0.10). Results of the meta-analysis revealed a significant OR for sleep problems and a significant reduced sleep duration among individuals addicted to the internet.

    • ObjectivesThe present systematic review aimed to investigate how oral health related quality of life (OHQOL) associates with anxiety and depression. The study protocol was registered prospectively in the PROSPERO database (CRD42023389372).Materials and methodsStudies investigating associations between OHQOL and depression and/or anxiety were included. Fisher's Z scores were used to summarize associations between OHQOL and depression/anxiety. Funnel plots and Begg's Tests were used to assess publication bias. Meta-regression was conducted to examine potential moderator effects in the associations. Academic databases including the ISI Web of Knowledge, Scopus, ProQuest and PubMed were systematically searched. The quality of included studies was checked with the Newcastle Ottawa Scale (NOS).ResultsAll 15 included studies were cross-sectional (14,419 participants from nine countries; mean age=43.74 years). The pooled estimates showed weak associations between OHQOL and depression (Fisher's z-score of 0.26 [95% CI = 0.17, 0.35; I2 = 96.2%; tau 2 = 0.03]) and anxiety (Fisher's z-score of 0.22 [95% CI = 0.001, 0.43; I2 = 97.9%; tau 2 = 0.06]). No severe problems in methodology quality, publication biases, or moderator effects were observed.ConclusionBoth depression and anxiety were weakly associated with individuals' OHQOL. Although the synthesized associations were not strong, they were significant, indicating that depression and anxiety are potential factors influencing individuals' OHQOL.

  • Transitional Cancer Care Program from Hospital to Home in the Health Care System of Iran

    • OBJECTIVES: Transitional care program refers to the health care continuity during transferring from one health care setting to another or to home. This is an essential program for cancer patients and reduces the risk of unnecessary hospital admissions as well as the complications of the disease. The aim of this study was to develop a transitional cancer care program from hospital to home in the health care system of Iran.METHODS: This study is a health policy and system research. It was conducted in four stages from October 2019 to January 2020. The first stage was a qualitative study. The qualitative data were collected through semi-structured interviews with 24 participants and a focus group with eight experts. In the second stage, a literature review of transitional care models was carried out. The initial version of the transitional cancer care program was developed based on the qualitative results and the literature review in the third stage. The validity and feasibility of the program were assessed using the Delphi study in the fourth stage.RESULTS: Six major categories were extracted from the qualitative results, consisting of "integrated services for the continuity of care", "holistic care", "care standardization", "the use of telemedicine", "the transparency of rules" and "the care process provision". Using these results and extracted the three common models of transitional care, the initial program was developed in three phases of pre-discharge, post-discharge, and transitional care with six protocols. The content validity of the program (98.7%) and its feasibility (95.8%) were approved by experts in the Delphi rounds.CONCLUSIONS: It is necessary to revise hospitals' discharge program, and home health care center's plan for admission and delivering health care services for cancer patients. Also, a pilot program is necessary to find the system advantages and disadvantages..
