    • Study Purpose The DRAGON 1 trial aims to assess training, implementation, safety and feasibility of combined portal- and hepatic-vein embolization (PVE/HVE) to accelerate future liver remnant (FLR) hypertrophy in patients with borderline resectable colorectal cancer liver metastases. Methods The DRAGON 1 trial is a worldwide multicenter prospective single arm trial. The primary endpoint is a composite of the safety of PVE/HVE, 90-day mortality, and one year accrual monitoring of each participating center. Secondary endpoints include: feasibility of resection, the used PVE and HVE techniques, FLR-hypertrophy, liver function (subset of centers), overall survival, and disease-free survival. All complications after the PVE/HVE procedure are documented. Liver volumes will be measured at week 1 and if applicable at week 3 and 6 after PVE/HVE and follow-up visits will be held at 1, 3, 6, and 12 months after the resection. Results Not applicable. Conclusion DRAGON 1 is a prospective trial to assess the safety and feasibility of PVE/HVE. Participating study centers will be trained, and procedures standardized using Work Instructions (WI) to prepare for the DRAGON 2 randomized controlled trial. Outcomes should reveal the accrual potential of centers, safety profile of combined PVE/HVE and the effect of FLR-hypertrophy induction by PVE/HVE in patients with CRLM and a small FLR.

  • Detection and localization of early- and late-stage cancers using platelet RNA

    • Cancer patients benefit from early tumor detection since treatment outcomes are more favorable for less advanced cancers. Platelets are involved in cancer progression and are considered a promising biosource for cancer detection, as they alter their RNA content upon local and systemic cues. We show that tumor-educated platelet (TEP) RNA-based blood tests enable the detection of 18 cancer types. With 99% specificity in asymptomatic controls, thromboSeq correctly detected the presence of cancer in two-thirds of 1,096 blood samples from stage I–IV cancer patients and in half of 352 stage I–III tumors. Symptomatic controls, including inflammatory and cardiovascular diseases, and benign tumors had increased false-positive test results with an average specificity of 78%. Moreover, thromboSeq determined the tumor site of origin in five different tumor types correctly in over 80% of the cancer patients. These results highlight the potential properties of TEP-derived RNA panels to supplement current approaches for blood-based cancer screening.

  • an overview of the MHONGOOSE survey: Observing nearby galaxies with MeerKAT

    • MHONGOOSE is a deep survey of the neutral hydrogen distribution in a representative sample of 30 nearby disk and dwarf galaxies with H I masses from ∼ 106 to ∼ 1011 M, and luminosities from MR ∼ 12 to MR ∼ −22. The sample is selected to uniformly cover the available range in log(MHI). Our extremely deep observations, down to H I column density limits of well below 1018 cm−2 — or a few hundred times fainter than the typical H I disks in galaxies — will directly detect the effects of cold accretion from the intergalactic medium and the links with the cosmic web. These observations will be the first ever to probe the very low-column density neutral gas in galaxies at these high resolutions. Combination with data at other wavelengths, most of it already available, will enable accurate modeling of the properties and evolution of the mass components in these galaxies and link these with the effects of environment, dark matter distribution, and other fundamental properties such as halo mass and angular momentum. MHONGOOSE can already start addressing some of the SKA-1 science goals and will provide a comprehensive inventory of the processes driving the transformation and evolution of galaxies in the nearby universe at high resolution and over 5 orders of magnitude in column density. It will be a Nearby Galaxies Legacy Survey that will be unsurpassed until the advent of the SKA, and can serve as a highly visible, lasting statement of MeerKAT’s capabilities.

  • ATOMIUM: halide molecules around the S-type AGB star W Aquilae

    • Context. S-type asymptotic giant branch (AGB) stars are thought to be intermediates in the evolution of oxygen- to carbon-rich AGB stars. The chemical compositions of their circumstellar envelopes are also intermediate but have not been studied in as much detail as their carbon- and oxygen-rich counterparts. W Aql is a nearby S-type star, with well-known circumstellar parameters, making it an ideal object for in-depth study of less common molecules. Aims. We aim to determine the abundances of AlCl and AlF from rotational lines, which have been observed for the first time towards an S-type AGB star. In combination with models based on PACS observations, we aim to update our chemical kinetics network based on these results. Methods. We analyse ALMA observations towards W Aql of AlCl in the ground and first two vibrationally excited states and AlF in the ground vibrational state. Using radiative transfer models, we determine the abundances and spatial abundance distributions of (AlCl)-Cl-35, (AlCl)-Cl-37, and AlF. We also model HCl and HF emission and compare these models to PACS spectra to constrain the abundances of these species. Results. AlCl is found in clumps very close to the star, with emission confined within 0 ''.1 of the star. AlF emission is more extended, with faint emission extending 0 ''.2 to 0 ''.6 from the continuum peak. We find peak abundances, relative to H-2, of 1.7 x 10(-7) for (AlCl)-Cl-35, 7 x 10(-8) for (AlCl)-Cl-37, and 1 x 10(-7) for AlF. From the PACS spectra, we find abundances of 9.7 x 10(-8) and <= 10(-8), relative to H-2, for HCl and HF, respectively. Conclusions. The AlF abundance exceeds the solar F abundance, indicating that fluorine synthesised in the AGB star has already been dredged up to the surface of the star and ejected into the circumstellar envelope. From our analysis of chemical reactions in the wind, we conclude that AlF may participate in the dust formation process, but we cannot fully explain the rapid depletion of AlCl seen in the wind.

  • Chemical tracers of a highly eccentric AGB–main-sequence star binary

    • Binary interactions have been proposed to explain a variety of circumstellar structures seen around evolved stars, including asymptotic giant branch (AGB) stars and planetary nebulae. Studies resolving the circumstellar envelopes of AGB stars have revealed spirals, disks and bipolar outflows, with shaping attributed to interactions with a companion. Here we use a combined chemical and dynamical analysis to reveal a highly eccentric and long-period orbit for W Aquilae, a binary system containing an AGB star and a main-sequence companion. Our results are based on anisotropic SiN emission, the detections of irregular NS and SiC emission towards the S-type star, and density structures observed in the CO emission. These features are all interpreted as having formed during periastron interactions. Our astrochemistry-based method can yield stringent constraints on the orbital parameters of long-period binaries containing AGB stars, and will be applicable to other systems.

  • ATOMIUM: Molecular inventory of 17 oxygen-rich evolved stars observed with ALMA

    • Context. The dusty winds of cool evolved stars are a major contributor of the newly synthesised material enriching the Galaxy and future generations of stars. However, the details of the physics and chemistry behind dust formation and wind launching have yet to be pinpointed. Recent spatially resolved observations show the importance of gaining a more comprehensive view of the circumstellar chemistry, but a comparative study of the intricate interplay between chemistry and physics is still difficult because observational details such as frequencies and angular resolutions are rarely comparable. Aims. Aiming to overcome these deficiencies, ATOMIUM is an ALMA Large Programme to study the physics and chemistry of the circumstellar envelopes of a diverse set of oxygen-rich evolved stars under homogeneous observing conditions at three angular resolutions between ∼0.02′1.4′. Here we summarize the molecular inventory of these sources, and the correlations between stellar parameters and molecular content. Methods. Seventeen oxygen-rich or S-Type asymptotic giant branch (AGB) and red supergiant (RSG) stars have been observed in several tunings with ALMA Band 6, targeting a range of molecules to probe the circumstellar envelope and especially the chemistry of dust formation close to the star. We systematically assigned the molecular carriers of the spectral lines and measured their spectroscopic parameters and the angular extent of the emission of each line from integrated intensity maps. Results. Across the ATOMIUM sample, we detect 291 transitions of 24 different molecules and their isotopologues. This includes several first detections in oxygen-rich AGB/RSG stars: PO v = 1, SO2 v1 = 1 and v2 = 2, and several high energy H2O transitions. We also find several first detections in S-Type AGB stars: vibrationally excited HCN v2 = 2,3 and SiS v = 4,5,6, as well as first detections of the molecules SiC, AlCl, and AlF in W Aql. Overall, we find strong correlations between the following molecular pairs: CS and SiS, CS and AlF, NaCl and KCl, AlO and SO, SO2 and SO, and SO2 and H2O; meaning both molecules tend to have more detected emission lines in the same sources. The measured isotopic ratios of Si and S are found to be consistent with previous measurements, except for an anomalously high 29Si/30Si ratio of 4 ± 1 in the RSG VX Sgr. Conclusions. This paper presents the overall molecular inventory and an initial analysis of the large ATOMIUM dataset, laying the groundwork for future work deriving molecular abundances and abundance profiles using radiative transfer modeling which will provide more rigorous tests for chemical models.

  • The messy death of a multiple star system and the resulting planetary nebula as observed by JWST

    • Planetary nebulae—the ejected envelopes of red giant stars—provide us with a history of the last, mass-losing phases of 90% of stars initially more massive than the Sun. Here we analyse images of the planetary nebula NGC 3132 from the James Webb Space Telescope (JWST) Early Release Observations. A structured, extended hydrogen halo surrounding an ionized central bubble is imprinted with spiral structures, probably shaped by a low-mass companion orbiting the central star at about 40–60 au. The images also reveal a mid-infrared excess at the central star, interpreted as a dusty disk, which is indicative of an interaction with another closer companion. Including the previously known A-type visual companion, the progenitor of the NGC 3132 planetary nebula must have been at least a stellar quartet. The JWST images allow us to generate a model of the illumination, ionization and hydrodynamics of the molecular halo, demonstrating the power of JWST to investigate complex stellar outflows. Furthermore, new measurements of the A-type visual companion allow us to derive the value for the mass of the progenitor of a central star with excellent precision: 2.86 ± 0.06 M⊙. These results serve as pathfinders for future JWST observations of planetary nebulae, providing unique insight into fundamental astrophysical processes including colliding winds and binary star interactions, with implications for supernovae and gravitational-wave systems.

  • (Sub)stellar companions shape the winds of evolved stars

    • Binary interactions dominate the evolution of massive stars, but their role is less clear for low- and intermediate-mass stars. The evolution of a spherical wind from an asymptotic giant branch (AGB) star into a nonspherical planetary nebula (PN) could be due to binary interactions. We observed a sample of AGB stars with the Atacama Large Millimeter/submillimeter Array (ALMA) and found that their winds exhibit distinct nonspherical geometries with morphological similarities to planetary nebulae (PNe). We infer that the same physics shapes both AGB winds and PNe; additionally, the morphology and AGB mass-loss rate are correlated. These characteristics can be explained by binary interaction. We propose an evolutionary scenario for AGB morphologies that is consistent with observed phenomena in AGB stars and PNe.

    • This overview paper presents atomium, a Large Programme in Cycle 6 with the Atacama Large Millimeter/submillimeter Array (ALMA). The goal of atomium is to understand the dynamics and the gas phase and dust formation chemistry in the winds of evolved asymptotic giant branch (AGB) and red supergiant (RSG) stars. A more general aim is to identify chemical processes applicable to other astrophysical environments. Seventeen oxygen-rich AGB and RSG stars spanning a range in (circum)stellar parameters and evolutionary phases were observed in a homogeneous observing strategy allowing for an unambiguous comparison. Data were obtained between 213.83 and 269.71 GHz at high (0.025-0.050), medium (0.13-0.24), and low (~1) angular resolution. The sensitivity per ~1.3 km s-1 channel was 1.5-5 mJy beam-1, and the line-free channels were used to image the millimetre wave continuum. Our primary molecules for studying the gas dynamics and dust formation are CO, SiO, AlO, AlOH, TiO, TiO2, and HCN; secondary molecules include SO, SO2, SiS, CS, H2O, and NaCl. The scientific motivation, survey design, sample properties, data reduction, and an overview of the data products are described. In addition, we highlight one scientific result - the wind kinematics of the atomium sources. Our analysis suggests that the atomium sources often have a slow wind acceleration, and a fraction of the gas reaches a velocity which can be up to a factor of two times larger than previously reported terminal velocities assuming isotropic expansion. Moreover, the wind kinematic profiles establish that the radial velocity described by the momentum equation for a spherical wind structure cannot capture the complexity of the velocity field. In fifteen sources, some molecular transitions other than 12CO v = 0 J = 2 - 1 reach a higher outflow velocity, with a spatial emission zone that is often greater than 30 stellar radii, but much less than the extent of CO. We propose that a binary interaction with a (sub)stellar companion may (partly) explain the non-monotonic behaviour of the projected velocity field. The atomium data hence provide a crucial benchmark for the wind dynamics of evolved stars in single and binary star models.

  • Effectiveness of collaborative care in reducing suicidal ideation: An individual participant data meta-analysis

    • To assess whether CC is more effective at reducing suicidal ideation in people with depression compared with usual care, and whether study and patient factors moderate treatment effects. Method: We searched Medline, Embase, PubMed, PsycINFO, CINAHL, CENTRAL from inception to March 2020 for Randomised Controlled Trials (RCTs) that compared the effectiveness of CC with usual care in depressed adults, and reported changes in suicidal ideation at 4 to 6 months post-randomisation. Mixed-effects models accounted for clustering of participants within trials and heterogeneity across trials. This study is registered with PROSPERO, CRD42020201747. Results: We extracted data from 28 RCTs (11,165 patients) of 83 eligible studies. We observed a small significant clinical improvement of CC on suicidal ideation, compared with usual care (SMD, 0.11 [95%CI, 0.15 to 0.08]; I-2, 0.47% [95%CI 0.04% to 4.90%]). CC interventions with a recognised psychological treatment were associated with small reductions in suicidal ideation (SMD, 0.15 [95%CI -0.19 to 0.11]). CC was more effective for reducing suicidal ideation among patients aged over 65 years (SMD, 0.18 [95%CI -0.25 to 0.11]). Conclusion: Primary care based CC with an embedded psychological intervention is the most effective CC framework for reducing suicidal ideation and older patients may benefit the most.

    • Atacama Large Millimetre Array observations show a non-detection of carbon monoxide around the four most luminous asymptotic giant branch (AGB) stars in the globular cluster 47 Tucanae. Stellar evolution models and star counts show that the mass-loss rates from these stars should be similar to 1.2-3.5x10(-7) M-circle dot yr(-1). We would naively expect such stars to be detectable at this distance (4.5 kpc). By modelling the ultraviolet radiation field from post-AGB stars and white dwarfs in 47 Tuc, we conclude that CO should be dissociated abnormally close to the stars. We estimate that the CO envelopes will be truncated at a few hundred stellar radii from their host stars and that the line intensities are about two orders of magnitude below our current detection limits. The truncation of CO envelopes should be important for AGB stars in dense clusters. Observing the CO (3-2) and higher transitions and targeting stars far from the centres of clusters should result in the detections needed to measure the outflow velocities from these stars.

    • Context. Low- and intermediate-mass asymptotic giant stars and massive red supergiant stars are important contributors to the chemical enrichment of the Universe. They are among the most efficient dust factories of the Galaxy, harboring chemically rich circumstellar environments. Yet, the processes that lead to dust formation or the large-scale shaping of the mass loss still escape attempts at modeling. Aims. Through the ATOMIUM project, we aim to present a consistent view of a sample of 17 nearby cool evolved stars. Our goals are to unveil the dust-nucleation sites and morphologies of the circumstellar envelope of such stars and to probe ambient environments with various conditions. This will further enhance our understanding of the roles of stellar convection and pulsations, and that of companions in shaping the dusty circumstellar medium. Methods. Here we present and analyze VLT/SPHERE-ZIMPOL polarimetric maps obtained in the visible (645- 820 nm) of 14 out of the 17 ATOMIUM sources. They were obtained contemporaneously with the ALMA high spatial resolution data. To help interpret the polarized signal, we produced synthetic maps of light scattering by dust, through 3D radiative transfer simulations with the RADMC3D code. Results. The degree of linear polarization (DoLP) observed by ZIMPOL spreads across several optical filters. We infer that it primarily probes dust located just outside of the point spread function of the central source, and in or near the plane of the sky. The polarized signal is mainly produced by structures with a total optical depth close to unity in the line of sight, and it represents only a fraction of the total circumstellar dust. The maximum DoLP ranges from 0.03- 0.38 depending on the source, fractions that can be reproduced by our 3D pilot models for grains composed of olivine, melilite, corundum, enstatite, or forsterite. The spatial structure of the DoLP shows a diverse set of shapes, including clumps, arcs, and full envelopes. Only for three sources do we note a correlation between the ALMA CO ν = 0, J = 2-1 and SiO ν = 0, J = 5-4 lines, which trace the gas density, and the DoLP, which traces the dust. Conclusions. The clumpiness of the DoLP and the lack of a consistent correlation between the gas and the dust location show that, in the inner environment, dust formation occurs at very specific sites. This has potential consequences for the derived mass-loss rates and dust-to-gas ratio in the inner region of the circumstellar environment. Except for π1 Gru and perhaps GY Aql, we do not detect interactions between the circumstellar wind and the hypothesized companions that shape the wind at larger scales. This suggests that the orbits of any other companions are tilted out of the plane of the sky.

    • Aims. Cool, evolved stars have copious, enriched winds. Observations have so far not fully constrained models for the shaping and acceleration of these winds. We need to understand the dynamics better, from the pulsating stellar surface to similar to 10 stellar radii, where radiation pressure on dust is fully effective. Asymmetric nebulae around some red supergiants imply the action of additional forces. Methods. We retrieved ALMA Science Verification data providing images of sub-mm line and continuum emission from VY CMa. This enables us to locate water masers with milli-arcsec accuracy and to resolve the dusty continuum. Results. The 658, 321, and 325 GHz masers lie in irregular, thick shells at increasing distances from the centre of expansion. For the first time this is confirmed as the stellar position, coinciding with a compact peak offset to the NW of the brightest continuum emission. The maser shells overlap but avoid each other on scales of up to 10 au. Their distribution is broadly consistent with excitation models but the conditions and kinematics are complicated by wind collisions, clumping, and asymmetries.

  • MeerGAL: the MeerKAT galactic plane survey

    • Radio surveys of the Milky Way galaxy have transformed our understanding of star formation and stellar evolution. However, due to strong dependence of “survey cost” on frequency most large area surveys have so far been carried out at low frequencies (∼ a few GHz). These surveys select against dense plasma as the free-free turnover frequency scales directly with electron density which means that there are significant biases against the detection of the youngest and densest HII regions, Young Stellar Objects, jets, winds and Planetary Nebulae. Here we describe the MeerKAT Large Project MeerGAL, which aims to address this issue by making the first sensitive high frequency, high resolution multi-epoch survey of the Galactic Plane. Together with its Northern Hemisphere sister project KuGARS (the Ku-band Galactic Reconnaissance Survey), MeerGAL will revolutionise the study of massive star formation and stellar evolution, Galactic structure, and variability.

    • Context. This is the first of a series of investigations into far-IR characteristics of 11 planetary nebulae (PNe) under the Herschel Space Observatory open time 1 program, Herschel Planetary Nebula Survey (HerPlaNS). Aims. Using the HerPlaNS data set, we look into the PN energetics and variations of the physical conditions within the target nebulae. In the present work, we provide an overview of the survey, data acquisition and processing, and resulting data products. Methods. We performed (1) PACS/SPIRE broadband imaging to determine the spatial distribution of the cold dust component in the target PNe and (2) PACS/SPIRE spectral-energy-distribution and line spectroscopy to determine the spatial distribution of the gas component in the target PNe. Results. For the case of NGC 6781, the broadband maps confirm the nearly pole-on barrel structure of the amorphous carbon-rich dust shell and the surrounding halo having temperatures of 26-40 K. The PACS/SPIRE multiposition spectra show spatial variations of far-.IR lines that reflect the physical stratification of the nebula. We demonstrate that spatially resolved far-IR line diagnostics yield the (T-e, n(e)) profiles, from which distributions of ionized, atomic, and molecular gases can be determined. Direct comparison of the dust and gas column mass maps constrained by the HerPlaNS data allows to construct an empirical gas-to-dust mass ratio map, which shows a range of ratios with the median of 195 +/- 110. The present analysis yields estimates of the total mass of the shell to be 0.86 M-circle dot, consisting of 0.54 M-circle dot of ionized gas, 0.12 M-circle dot of atomic gas, 0.2 M-circle dot of molecular gas, and 4 x 10(-3) M-circle dot of dust grains. These estimates' also suggest that the central star of about 1.5 M-circle dot initial mass is terminating its PN evolution onto the white dwarf cooling track. Conclusions. The HerPlaNS data provide various diagnostics for both the dust and gas components in a spatially resolved manner. In the forthcoming papers of the HerPlaNS series we will explore the HerPlaNS data set fully for the entire sample of 11 PNe.

    • Extracellular vesicles (EVs), through their complex cargo, can reflect the state of their cell of origin and change the functions and phenotypes of other cells. These features indicate strong biomarker and therapeutic potential and have generated broad interest, as evidenced by the steady year-on-year increase in the numbers of scientific publications about EVs. Important advances have been made in EV metrology and in understanding and applying EV biology. However, hurdles remain to realising the potential of EVs in domains ranging from basic biology to clinical applications due to challenges in EV nomenclature, separation from non-vesicular extracellular particles, characterisation and functional studies. To address the challenges and opportunities in this rapidly evolving field, the International Society for Extracellular Vesicles (ISEV) updates its ‘Minimal Information for Studies of Extracellular Vesicles’, which was first published in 2014 and then in 2018 as MISEV2014 and MISEV2018, respectively. The goal of the current document, MISEV2023, is to provide researchers with an updated snapshot of available approaches and their advantages and limitations for production, separation and characterisation of EVs from multiple sources, including cell culture, body fluids and solid tissues. In addition to presenting the latest state of the art in basic principles of EV research, this document also covers advanced techniques and approaches that are currently expanding the boundaries of the field. MISEV2023 also includes new sections on EV release and uptake and a brief discussion of in vivo approaches to study EVs. Compiling feedback from ISEV expert task forces and more than 1000 researchers, this document conveys the current state of EV research to facilitate robust scientific discoveries and move the field forward even more rapidly.

    • Context. Low- and intermediate-mass stars lose most of their stellar mass at the end of their lives on the asymptotic giant branch (AGB). Determining gas and dust mass-loss rates (MLRs) is important in quantifying the contribution of evolved stars to the enrichment of the interstellar medium.Aims. This study attempts to spectrally resolve CO thermal line emission in a small sample of AGB stars in the Large Magellanic Cloud (LMC).Methods. The Atacama Large Millimeter Array was used to observe two OH/IR stars and four carbon stars in the LMC in the CO J = 2−1 line.Results. We present the first measurement of expansion velocities in extragalactic carbon stars. All four C stars are detected and wind expansion velocities and stellar velocities are directly measured. Mass-loss rates are derived from modelling the spectral energy distribution and Spitzer/IRS spectrum with the DUSTY code. The derived gas-to-dust ratios allow the predicted velocities to agree with the observed gas-to-dust ratios. The expansion velocities and MLRs are compared to a Galactic sample of well-studied relatively low MLRs stars supplemented with extreme C stars with properties that are more similar to the LMC targets. Gas MLRs derived from a simple formula are significantly smaller than those derived from dust modelling, indicating an order of magnitude underestimate of the estimated CO abundance, time-variable mass loss, or that the CO intensities in LMC stars are lower than predicted by the formula derived for Galactic objects. This could be related to a stronger interstellar radiation field in the LMC.Conclusions. Although the LMC sample is small and the comparison to Galactic stars is non-trivial because of uncertainties in their distances (hence luminosities), it appears that for C stars the wind expansion velocities in the LMC are lower than in the solar neighbourhood, while the MLRs appear to be similar. This is in agreement with dynamical dust-driven wind models.

  • Dissecting the AGB star L-2 Puppis : a torus in the making

The circumstellar environment of L-2 Pup, an oxygen-rich semiregular variable, was observed to understand the evolution of mass loss and the shaping of ejecta in the late stages of stellar evolution. Methods. High-angular resolution observations from a single 8 m telescope were obtained using aperture masking in the near-infrared (1.64, 2.30 and 3.74 mu m) on the NACO/VLT, both in imaging and polarimetric modes. Results. The aperture-masking images of L-2 Pup at 2.30 mu m show a resolved structure that resembles a toroidal structure with a major axis of similar to 140 milliarcseconds (mas) and an east-west orientation. Two clumps can be seen on either side of the star, similar to 65 mas from the star, beyond the edge of the circumstellar envelope (estimated diameter is similar to 27 mas), while a faint, hook-like structure appear toward the northeast. The patterns are visible both in the imaging and polarimetric mode, although the latter was only used to measure the total intensity (Stokes I). The overall shape of the structure is similar at the 3.74 mu m pseudo-continuum (dust emission), where the clumps appear to be embedded within a dark, dusty lane. The faint, hook-like patterns are also seen at this wavelength, extending northeast and southwest with the central, dark lane being an apparent axis of symmetry. We interpret the structure as a circumstellar torus with inner radius of 4.2 au. With a rotation velocity of 10 kms(-1) as suggested by the SiO maser profile, we estimate a stellar mass of 0.7 M-circle dot.

    • Out-of-hospital cardiac arrest (OHCA) is a major health problem that affects approximately four hundred and thousand patients annually in the United States alone. It is a major challenge for the emergency medical system as decreased survival rates are directly proportional to the time delay from collapse to defibrillation. Historically, defibrillation has only been performed by physicians and in-hospital. With the development of automated external defibrillators (AEDs), rapid defibrillation by nonmedical professionals and subsequently by trained or untrained lay bystanders has become possible. Much hope has been put to the concept of Public Access Defibrillation with a massive dissemination of public available AEDs throughout most Western countries. Accordingly, current guidelines recommend that AEDs should be deployed in places with a high likelihood of OHCA. Despite these efforts, AED use is in most settings anecdotal with little effect on overall OHCA survival. The major reasons for low use of public AEDs are that most OHCAs take place outside high incidence sites of cardiac arrest and that most OHCAs take place in residential settings, currently defined as not suitable for Public Access Defibrillation. However, the use of new technology for identification and recruitment of lay bystanders and nearby AEDs to the scene of the cardiac arrest as well as new methods for strategic AED placement redefines and challenges the current concept and definitions of Public Access Defibrillation. Existing evidence of Public Access Defibrillation and knowledge gaps and future directions to improve outcomes for OHCA are discussed. In addition, a new definition of the different levels of Public Access Defibrillation is offered as well as new strategies for increasing AED use in the society.

    • Context. Born-again stars provide a unique possibility to study the evolution of the circumstellar envelope of evolved stars in human timescales. Up until now, most of the observations of the circumstellar material in these stars have been limited to studying the relatively hot gas and dust. In other evolved stars, the emission from rotational transitions of molecules, such as CO, is commonly used to study the cool component of their circumstellar envelopes. Thus, the detection and study of molecular gas in born-again stars is of great importance when attempting to understand their composition and chemical evolution. In addition, the molecular emission is an invaluable tool for exploring the physical conditions, kinematics, and formation of asymmetric structures in the circumstellar envelopes of these evolved stars. However, up until now, all attempts to detect molecular emission from the cool material around born-again stars have failed. Aims. We searched for emission from rotational transitions of molecules in the hydrogen-deficient circumstellar envelopes of born-again stars to explore the chemical composition, kinematics, and physical parameters of the relatively cool gas. Methods. We carried out observations using the APEX and IRAM 30 m telescopes to search for molecular emission toward four well-studied born-again stars, V4334 Sgr, V605 Aql, A30, and A78, that are thought to represent an evolutionary sequence. Results. For the first time, we detected emission from HCN and (IICN)-C-13 molecules toward V4334 Sgr, and CO emission in V605 Aql. No molecular emission was detected above the noise level toward A30 and A78. The detected lines exhibit broad linewidths greater than or similar to 150 km s(-1), which indicates that the emission comes from gas ejected during the born-again event, rather than from the old planetary nebula. A first estimate of the (HCN)-C-12/(HCN)-C-13 abundance ratio in the circumstellar environment of V4334 Sgr is approximate to 3, which is similar to the value of the C-12/C-13 ratio measured from other observations. We derived a rotational temperature of T-rot = 13 +/- 1 K, and a total column density of N-HCN = 1.6 +/- 0.1 x 10(16) cm(-2) for V4334 Sgr. This result sets a lower limit on the amount of hydrogen that was ejected into the wind during the born-again event of this source. For V605 Aql, we obtained a lower limit for the integrated line intensities I-12CO/I-13CO > 4.

    • We present JWST images of the well-known planetary nebula NGC 6720 (the Ring Nebula), covering wavelengths from 1.6 to 25 m. The bright shell is strongly fragmented with some 20 000 dense globules, bright in H2, with a characteristic diameter of 0.2 arcsec and density nH ∼105-106 cm-3. The shell contains a narrow ring of polycyclic aromatic hydrocarbon (PAH) emission. H2 is found throughout the shell and also in the halo. H2 in the halo may be located on the swept-up walls of a biconal polar flow. The central cavity is filled with high-ionization gas and shows two linear structures which we suggest are the edges of a biconal flow, seen in projection against the cavity. The central star is located 2 arcsec from the emission centroid of the cavity and shell. Linear features ('spikes') extend outward from the ring, pointing away from the central star. Hydrodynamical simulations reproduce the clumping and possibly the spikes. Around 10 low-contrast, regularly spaced concentric arc-like features are present; they suggest orbital modulation by a low-mass companion with a period of about 280 yr. A previously known much wider companion is located at a projected separation of about 15 000 au; we show that it is an M2-M4 dwarf. NGC 6720 is therefore a triple star system. These features, including the multiplicity, are similar to those seen in the Southern Ring Nebula (NGC 3132) and may be a common aspect of such nebulae.

Extreme oxygen-rich giants, OH/IR stars, develop superwinds with the highest mass-loss rates known so far, up to a few 10 −4 solar masses (M ⊙ ) per year 2–12 , informing our understanding of the maximum mass-loss rate achieved during the asymptotic giant branch (AGB) phase. A conundrum arises whereby the observationally determined duration of the superwind phase is too short for these stars to lose enough mass to become white dwarfs 2–4,6,8–10 . Here we report on the detection of spiral structures around two cornerstone extreme OH/IR stars, OH 26.5 + 0.6 and OH 30.1 − 0.7, thereby identifying them as wide binary systems. Hydrodynamic simulations show that the companion’s gravitational attraction creates an equatorial density enhancement mimicking a short, extreme superwind phase, thereby solving the decades-old conundrum. This discovery restricts the maximum mass-loss rate of AGB stars to around the single-scattering radiation pressure limit of a few 10 −5 M ⊙ yr −1 . This has crucial implications for nucleosynthetic yields, planet survival and the wind-driving mechanism.

    • OBJECTIVES: To compare Angio-Seal (AS) and StarClose (SC) and manual compression (MC) on efficacy of hemostasis, complication rate, safety of early mobilization, and patient comfort. BACKGROUND: Closure of the femoral artery after cardiac catheterization can be obtained through different methods. Today, physicians can choose from a number of different devices to achieve arterial closure. METHODS: In a prospective trial 450 patients were randomized to AS, SC, or MC. Patients were mobilized 1 to 2 hr after device placement, and 6 hr after MC. Data were collected during hospital admission and by telephone at one month after hospital discharge. RESULTS: Devices were used in 138/150 allocated to AS and 124/150 allocated to SC patients (92% vs. 83%, P = 0.015) Patients with MC experienced more pain during sheath removal than patients receiving a device, and rated their period of bed rest as less comfortable. Oozing and need for pressure bandage at the puncture site were observed in 37 AS patients and 57 SC patients (25% vs. 38%, P = 0.002). Hematoma occurred in 15 AS patients, in 17 SC patients, and in 14 MC patients (11 vs. 14 vs. 9%, ns). CONCLUSION: There is no difference in safety between the three methods of arterial closure. SC was more often not used or successfully deployed. SC patients more often had continuing oozing. On patient comfort, closure devices performed better than MC. Early ambulation in patients with a closure device is safe. AS is the preferred method of arterial closure after cardiac catheterization.

The fate of a massive star during the latest stages of its evolution is highly dependent on its mass-loss rate and geometry and therefore knowing the geometry of the circumstellar material close to the star and its surroundings is crucial. Aims. We aim to provide insight into the nature (i.e. geometry, rates) of mass-loss episodes, and in particular, the connection between the observed asymmetries due to the mass lost in a fast wind or during a previous, prodigious mass-losing phase. In this context, yellow hypergiants offer a good opportunity to study mass-loss events. Methods. We analysed a large set of optical and near-infrared data in spectroscopic and photometric, spectropolarimetric, and interferometric (GRAVITY/VLTI) modes, towards the yellow hypergiant IRAS 17163-3907. We used X-shooter optical observations to determine the spectral type of this yellow hypergiant and we present the first model-independent, reconstructed images of IRAS 17163-3907 at these wavelengths tracing milli-Arcsecond scales. Lastly, we applied a 2D radiative transfer model to fit the dereddened photometry and the radial profiles of published diffraction-limited VISIR images at 8.59 μm, 11.85 μm, and 12.81 μm simultaneously, adopting a revised distance determination using Gaia Data Release 2 measurements. Results. We constrain the spectral type of IRAS 17163-3907 to be slightly earlier than A6Ia (Teffâ ∼â 8500 K). The interferometric observables around the 2 μm window towards IRAS 17163-3907 show that the Brγ emission appears to be more extended and asymmetric than the Naâ » I and the continuum emission. Interestingly, the spectrum of IRAS 17163-3907 around 2 μm shows Mgâ » II emission that is not previously seen in other objects of its class. In addition, Brγ shows variability in a time interval of four months that is not seen towards Naâ » I. Lastly, in addition to the two known shells surrounding IRAS 17163-3907, we report on the existence of a third hot inner shell with a maximum dynamical age of only 30 yr. Conclusions. The 2 μm continuum originates directly from the star and not from hot dust surrounding the stellar object. The observed spectroscopic variability of Brγ could be a result of variability in the mass-loss rate. The interpretation of the presence of Naâ » I emission at closer distances to the star compared to Brγ has been a challenge in various studies. To address this, we examine several scenarios. We argue that the presence of a pseudo-photosphere, which was traditionally considered to be the prominent explanation, is not needed and that it is rather an optical depth effect. The three observed distinct mass-loss episodes are characterised by different mass-loss rates and can inform theories of mass-loss mechanisms, which is a topic still under debate both in theory and observations. We discuss these in the context of photospheric pulsations and wind bi-stability mechanisms.

    • We present high-angular-resolution observations of Sakurai's object using the Atacama Large Millimeter Array, shedding new light on its morpho-kinematical structure. The millimetre continuum emission observed at an angular resolution of 20 milliarcsec (corresponding to 70 AU) reveals a bright compact central component whose spectral index indicates that it is composed of amorphous carbon dust. Based on these findings, we conclude that this emission traces the previously suggested dust disc observed in mid-infrared observations, and therefore our observations provide the first direct imaging of this disc. The H12CN(J = 4 → 3) line emission observed at an angular resolution of 300 milliarcsec (corresponding to 1000 AU) displays a bipolar structure with a north- south velocity gradient. From the position- velocity diagram of this emission, we identify the presence of an expanding disc and a bipolar molecular outflow. The inclination of the disc is determined to be i = 72. The derived values for the de-projected expansion velocity and the radius of the disc are vexp = 53 km s-1 and R = 277 AU, respectively. On the other hand, the de-projected expansion velocity of the bipolar outflow detected in the H12CN(J = 4 → 3) emission is of approximately 1000 km s-1. We propose that the molecular outflow has an hourglass morphology with an opening angle of around 60. Our observations unambiguously show that an equatorial disc and bipolar outflows formed in Sakurai's object during the 30 years following the occurrence of the born-again event, providing important constraints for future modelling efforts of this phenomenon.

    • BACKGROUND: Primary coronary intervention (PCI) for acute myocardial infarction should be performed as quickly as possible, with a door-toballoon time of less then 90 minutes. However, in daily practice this cannot always be achieved. Prehospital diagnosis of ST-elevation myocardial infarction (STEMI) is of major importance in reducing time to treatment, in particular when patients can be transported directly to a centre with interventional capacities. OBJECTIVES: The aim of the current study was to evaluate the time from prehospital diagnosis of STEMI to balloon inflation and identify factors related to treatment delay in patients directly referred to the catheterisation laboratory of the University Medical Centre of Groningen. METHODS: A cross-sectional descriptive design was used to collect data on patients treated with primary PCI after prehospital diagnosis of STEMI. RESULTS: Median prehospital diagnosis-to-balloon time was 64 minutes for patients directly admitted to the catheterisation laboratory and 75 minutes for patients initially admitted to the coronary care unit. A delay longer than 90 minutes was observed in 18 patients. Higher age was associated with longer delay times (p=0.041). Long delays were not associated with diabetes (p=0.293), time from symptom onset to prehospital diagnosis (p=0.87) or time of day (p=0.09). Initial unavailability of the catheterisation laboratory due to running procedures contributed to longer delay times in ten cases. CONCLUSION: Prehospital diagnosis of STEMI and direct referral to a catheterisation laboratory for primary PCI allows a prehospital diagnosis-toballoon time of less than 90 minutes in 82% of patients. Older patients are at risk of longer delays. (Neth Heart J 2008;16:5-9.).

    • Aims. We report the first detections of OH+ emission in planetary nebulae (PNe). Methods. As part of an imaging and spectroscopy survey of 11 PNe in the far-IR using the PACS and SPIRE instruments aboard the Herschel Space Observatory, we performed a line survey in these PNe over the entire spectral range between 51 mu m and 672 mu m to look for new detections. Results. The rotational emission lines of OH+ at 152.99, 290.20, 308.48, and 329.77 mu m were detected in the spectra of three planetary nebulae: NGC 6445, NGC6720, and NGC 6781. Excitation temperatures and column densities derived from these lines are in the range of 27-47 K and 2 x 10(10)-4 x 10(11) cm(-2), respectively. Conclusions. In PNe, the OH+ rotational line emission appears to be produced in the photodissociation region (PDR) in these objects. The emission of OH+ is observed only in PNe with hot central stars (T-eff > 100 000 K), suggesting that high-energy photons may play a role in OH+ formation and its line excitation in these objects, as seems to be the case for ultraluminous galaxies.

    • OBJECTIVE To show that limiting dual antiplatelet therapy (DAPT) to six months in patients with event-free ST-elevation myocardial infarction (STEMI) results in a non-inferior clinical outcome versus DAPT for 12 months. Patients with STEMI treated with primary percutaneous coronary intervention (PCI) and second generation zotarolimus-eluting stent. Patients with STEMI aged 18 to 85 that underwent a primary PCI with the implantation of second generation drug-eluting stents were enrolled in the trial. Patients that were event-free at six months after primary PCI were randomised at this time point. Patients that were taking DAPT and were event-free at six months were randomised 1:1 to single antiplatelet therapy (SAPT) (ie, aspirin only) or to DAPT for an additional six months. All patients that were randomised were then followed for another 18 months (ie, 24 months after the primary PCI). The primary endpoint was a composite of all cause mortality, any myocardial infarction, any revascularisation, stroke, and thrombolysis in myocardial infarction major bleeding at 18 months after randomisation. A total of 1100 patients were enrolled in the trial between 19 December 2011 and 30 June 2015. 870 were randomised: 432 to SAPT versus 438 to DAPT. The primary endpoint occurred in 4.8% of patients receiving SAPT versus 6.6% of patients receiving DAPT (hazard ratio 0.73, 95% confidence interval 0.41 to 1.27, P=0.26). Non-inferiority was met (P=0.004 for non-inferiority), as the upper 95% confidence interval of 1.27 was smaller than the prespecified non-inferiority margin of 1.66. DAPT to six months was non-inferior to DAPT for 12 months in patients with event-free STEMI at six months after primary PCI with second generation drug-eluting stents.

    • It is commonly accepted that asymmetries found in the post-AGB stars and planetary nebulae should originate as early as during the AGB phase. We present results from our high-angular resolution observing programs, with an aperture masking technique on the VLT, of a sample of evolved stars that were known to present asymmetries at larger spatial scales (e.g. jets, torii and/or bipolar nebulae). Disk-like structures have been found in the vicinity of at least two of these stars.

    • OBJECTIVE: To investigate whether inhibition of mitochondrial p53 association using pifithrin-μ (PFT-μ) represents a potential novel neuroprotective strategy to combat perinatal hypoxic-ischemic (HI) brain damage. METHODS: Seven-day-old rats were subjected to unilateral carotid artery occlusion and hypoxia followed by intraperitoneal treatment with PFT-μ, an inhibitor of p53 mitochondrial association or PFT-α an inhibitor of p53 transcriptional activity. Cerebral damage, sensorimotor and cognitive function, apoptotic pathways (cytosolic cytochrome c, Smac/DIABLO, active caspase 3), and oxidative stress (lipid peroxidation and PARP-1 cleavage) were investigated. RESULTS: PFT-μ treatment completely prevented the HI-induced increase in mitochondrial p53 association at 3 hours and reduced neuronal damage at 48 hours post-HI. PFT-μ had long-term (6-10 weeks post-HI) beneficial effects as sensorimotor and cognitive outcome improved and infarct size was reduced by ∼79%. Neuroprotection by PFT-μ treatment was associated with strong inhibition of apoptotic pathways and reduced oxidative stress. Unexpectedly, PFT-μ also inhibited HI-induced upregulation of p53 target genes. However, the neuroprotective effect of inhibiting only p53 transcriptional activity by PFT-α was significantly smaller and did not involve reduced oxidative stress. INTERPRETATION: We are the first to show that prevention of mitochondrial p53 association by PFT-μ strongly improves functional outcome and decreases lesion size after neonatal HI. PFT-μ not only inhibits mitochondrial release of cytochrome c, but also inhibits oxidative stress. We propose that as a consequence nuclear accumulation of p53 and transcription of proapoptotic target genes are prevented. In conclusion, targeting p53 mitochondrial association by PFT-μ may develop into a novel and powerful neuroprotective strategy. ANN NEUROL 2011;00:000-000.

    • Context. This is the first publication from the DEATHSTAR project. The overall goal of the project is to reduce the uncertainties of the observational estimates of mass-loss rates from evolved stars on the Asymptotic Giant Branch (AGB). Aim. The aim in this first publication is to constrain the sizes of the (CO)-C-12 emitting region from the circumstellar envelopes around 42 mostly southern AGB stars, of which 21 are M-type and 21 are C-type, using the Atacama Compact Array (ACA) at the Atacama Large Millimeter/submillimeter Array. The symmetry of the outflows is also investigated. Methods. Line emission from (CO)-C-12 J = 2 -> 1 and 3 -> 2 from all of the sources were mapped using the ACA. In this initial analysis, the emission distribution was fit to a Gaussian distribution in the uv-plane. A detailed radiative transfer analysis will be presented in a future publication. The major and minor axis of the best-fit Gaussian at the line center velocity of the (CO)-C-12 J = 2 -> 1 emission gives a first indication of the size of the emitting region. Furthermore, the fitting results, such as the Gaussian major and minor axis, center position, and the goodness of fit across both lines, constrain the symmetry of the emission distribution. For a subsample of sources, the measured emission distribution is compared to predictions from previous best-fit radiative transfer modeling results. Results. We find that the CO envelope sizes are, in general, larger for C-type than for M-type AGB stars, which is as expected if the CO/H-2 ratio is larger in C-type stars. Furthermore, the measurements show a relation between the measured (Gaussian) (CO)-C-12 J = 2 -> 1 size and circumstellar density that, while in broad agreement with photodissociation calculations, reveals large scatter and some systematic differences between the different stellar types. For lower mass-loss-rate irregular and semi-regular variables of both M- and C-type AGB stars, the (CO)-C-12 J = 2 -> 1 size appears to be independent of the ratio of the mass-loss rate to outflow velocity, which is a measure of circumstellar density. For the higher mass-loss-rate Mira stars, the (CO)-C-12 J = 2 -> 1 size clearly increases with circumstellar density, with larger sizes for the higher CO-abundance C-type stars. The M-type stars appear to be consistently smaller than predicted from photodissociation theory. The majority of the sources have CO envelope sizes that are consistent with a spherically symmetric, smooth outflow, at least on larger scales. For about a third of the sources, indications of strong asymmetries are detected. This is consistent with what was found in previous interferometric investigations of northern sources. Smaller scale asymmetries are found in a larger fraction of sources. Conclusions. These results for CO envelope radii and shapes can be used to constrain detailed radiative transfer modeling of the same stars so as to determine mass-loss rates that are independent of photodissociation models. For a large fraction of the sources, observations at higher spatial resolution will be necessary to deduce the nature and origin of the complex circumstellar dynamics revealed by our ACA observations.

    • Purpose: Positron emission tomography (PET) with Zr-89-britumomab tiuxetan can be used to monitor biodistribution of Y-90-ibritumomab tiuxetan as shown in mice. The aim of this study was to assess biodistribution and radiation dosimetry of 90Y-ibritumomab tiuxetan in humans on the basis of Zr-89-ibritumomab tiuxetan imaging, to evaluate whether co-injection of a therapeutic amount of Y-90-ibritumomab tiuxetan influences biodistribution of Zr-89-ibritumomabtiuxetan and whether pre-therapy scout scans with Zr-89-ibritumomab tiuxetan can be used to predict biodistribution of Y-90-ibritumomab tiuxetan and the dose-limiting organ during therapy. Methods: Seven patients with relapsed B-cell non-Hodgkin's lymphoma scheduled for autologous stem cell transplantation underwent PET scans at 1, 72 and 144 h after injection of similar to 70 MBq Zr-89-ibritumomab tiuxetan and again 2 weeks later after co-injection of 15 MBq/kg or 30 MBq/kg Y-90-britumomab tiuxetan. Volumes of interest were drawn over liver, kidneys, lungs, spleen and tumours. Ibritumomab tiuxetan organ absorbed doses were calculated using OLINDA. Red marrow dosimetry was based on blood samples. Absorbed doses to tumours were calculated using exponential fits to the measured data. Results: The highest Y-90 absorbed dose was observed in liver (3.2 +/- 1.8 mGy/MBq) and spleen (2.9 +/- 0.7 mGy/MBq) followed by kidneys and lungs. The red marrow dose was 0.52 +/- 0.04 mGy/MBq, and the effective dose was 0.87 +/- 0.14 mSv/MBq. Tumour absorbed doses ranged from 8.6 to 28.6 mGy/MBq. Correlation between predicted pre-therapy and therapy organ absorbed doses as based on Zr-89-ibritumomab tiuxetan images was high (Pearson correlation coefficient r=0.97). No significant difference between pre-therapy and therapy tumour absorbed doses was found, but correlation was lower (r=0.75). Conclusion: Biodistribution of Zr-89-ibritumomab tiuxetan is not influenced by simultaneous therapy with Y-90-ibritumomab tiuxetan, and Zr-89-ibritumomab tiuxetan scout scans can thus be used to predict biodistribution and dose-limiting organ during therapy. Absorbed doses to spleen were lower than those previously estimated using In-111-ibritumomab tiuxetan. The dose-limiting organ in patients undergoing stem cell transplantation is the liver.

  • ALMA Compact Array observations of the Fried Egg nebula: Evidence for large-scale asymmetric mass-loss from the yellow hypergiant IRAS 17163-3907

    • Yellow hypergiants are rare and represent a fast evolutionary stage of massive evolved stars. That evolutionary phase is characterised by a very intense mass loss, the understanding of which is still very limited. Here we report ALMA Compact Array observations of a 50??-mosaic toward the Fried Egg nebula, around one of the few Galactic yellow hypergiants IRAS 17163-3907. The emission from the 12CO J = 2-1 line, H30? recombination line, and continuum is imaged at a resolution of ~8??, revealing the morphology of the molecular environment around the star. The continuum emission is unresolved and peaks at the position of the star. The radio recombination line H30? shows unresolved emission at the star, with an approximately Gaussian spectrum centered on a velocity of 21 ± 3km s-1 with a width of 57 ± 6km s-1. In contrast, the CO 2-1 emission is complex and decomposes into several components beyond the contamination from interstellar gas in the line of sight. The CO spectrum toward the star is a broad plateau, centered at the systemic velocity of +18 km s-1 and with an expansion velocity of 100 ± 10km s-1. Assuming isotropic and constant mass-loss, we estimate a mass-loss rate of 8 ± 1.5 × 10-5M? yr-1. At a radius of 25?? from the star, we detect CO emission associated with the dust ring previously imaged by Herschel. The kinematics of this ring, however, is not consistent with an expanding shell, but show a velocity gradient of vsys ± 20km s-1. In addition, we find a puzzling bright feature radially connecting the star to the CO ring, at a velocity of +40 km s-1 relative to the star. This spur feature may trace a unidirectional ejection event from the star. Our ACA observations reveal the complex morphology around IRAS 17163 and illustrate the breakthroughs that ALMA will bring to the field of massive stellar evolution.
