- Retinopathy of prematurity is a neurovascular disorder which yearly makes 20 000 infants blind or severely visually impaired worldwide. Hyperoxia is a crucial risk factor in settings with uncontrolled oxygen supplementation, while insufficient nutrient assimilation is a general contributing factor. ROP is a two phase disease with general as well as retinal neurovascular growth arrest after birth and later uncontrolled retinal neovascularization. Laser therapy prevents retinal detachment in most cases. Screening programs need to have inclusion criteria suited for the actual level of neonatal care. With less advanced care, more mature infants are affected by ROP. Risk of severe ROP can be calculated based on neonatal weight gain, especially in high quality care settings. Control of oxygen supplementation can save the sight of many infants in developing countries. Research should focus on prevention of ROP promoting postnatal growth and development. Current off-label use of anti-VEGF treatment is worrying since evidence for e.g. safety is lacking. © 2016, Swedish Medical Association. All rights reserved.
