    • Computer-aided diagnosis (CAD) systems have become very important for the medical diagnosis of brain tumors. The systems improve the diagnostic accuracy and reduce the required time. In this paper, a two-stage CAD system has been developed for automatic detection and classification of brain tumor through magnetic resonance images (MRIs). In the first stage, the system classifies brain tumor MRI into normal and abnormal images. In the second stage, the type of tumor is classified as benign (Noncancerous) or malignant (Cancerous) from the abnormal MRIs. The proposed CAD ensembles the following computational methods: MRI image segmentation by K-means clustering, feature extraction using discrete wavelet transform (DWT), feature reduction by applying principal component analysis (PCA). The two-stage classification has been conducted using a support vector machine (SVM). Performance evaluation of the proposed CAD has achieved promising results using a non-standard MRIs database.

    • The “demos paradox” is the idea that the composition of a demos could never secure democratic legitimacy because the composition of a demos cannot itself be democratically decided. Those who view this problem as unsolvable argue that this insight allows them to adopt a critical perspective towards common ideas about who has legitimate standing to participate in democratic decision-making. We argue that the opposite is true and that endorsing the demos paradox actually undermines our ability to critically engage with common ideas about legitimate standing. We challenge the conception of legitimacy that lurks behind the demos paradox and argue that the real impossibility is to endorse democracy without also being committed to significant procedure-independent standards for the legitimate composition of the demos. We show that trying to solve the problem of the demos by appeal to some normative conception of democratic legitimacy is a worthwhile project that is not undermined by paradox.

  • Applying key learnings from the EMAX trial to clinical practice and future trial design in COPD

    • Early MAXimisation of bronchodilation for improving COPD stability (EMAX) was a large, multicentre, multi-national, randomised, double-blind, 24-week trial. EMAX evaluated the efficacy and safety of dual bronchodilator therapy with umeclidinium bromide (UMEC)/vilanterol (VI) versus monotherapy with either UMEC or salmeterol (SAL) in symptomatic patients with chronic obstructive pulmonary disease (COPD) at low exacerbation risk who were not taking concomitant inhaled corticosteroid (ICS). EMAX generated evidence covering a wide range of patient-centred endpoints in COPD in addition to measures of lung function, clinical deterioration and safety. In addition, prospective and post hoc secondary analyses have generated clinically valuable information regarding the effects of baseline patient characteristics on treatment outcomes. Importantly, as concomitant ICS use was not permitted in this study, EMAX compared dual long-acting muscarinic antagonist (LAMA)/long-acting β2-agonist (LABA) therapy with LAMA or LABA monotherapy without potential confounding due to concurrent ICS use or withdrawal. EMAX demonstrated beneficial treatment effects of UMEC/VI over UMEC or SAL monotherapy as maintenance treatment across a range of different patient characteristics, with no forfeit in safety. Thus, the trial provided novel insights into the role of LAMA/LABA versus LABA and LAMA monotherapies as maintenance therapy for patients with symptomatic COPD at low risk of exacerbations. This article will explore the clinical implications of the main findings to date of the EMAX trial and consider the key learnings this trial offers for future trial design in COPD.

    • The point of departure for this article is several Swedish IT policies that articulate goals for further development of the welfare state, which demand and enable active citizenship as well as enrolment of IT in the performance of this active citizenship. This article also examines the performance of active citizenship in a variety of sociotechnical arenas where people and technology coexist. Does the notion of active citizenship turn out a number of performances when translated into materialized technologies, such as Internet portals and web-based services? The authors juxtapose the policies with a construction of agencies in the story of citizens' design. In the last section, the discussions taking place in the parliament of things are summarized and related to the problematizations of citizenship, gender and IT.
