    • A main problem in nearly all contexts is that unguided decision making is tremendously difficult and can lead to inefficient decision processes and undesired consequences. Therefore, decision support systems (DSSs) are of prime concern to any organization and there have been numerous approaches to such from, e.g., computational, mathematical, financial, philosophical, psychological, and sociological angles. However, a key observation is that efficient decision making is not easily performed by using methods from one discipline only. The case is rather that if real world decision making is taken seriously, several aspects must be included. This article describes some efforts of the DECIDE research group for approaching decision making and developing DSSs in a cross-disciplinary environment.

This paper describes some efforts made by the DECIDE Research Group to approach DSS development and decision making  tools in a cross-disciplinary way.

    • This was a cross-sectional study investigating factors related to satisfaction with care among long-term mentally ill patients diagnosed with schizophrenia, selected from an outpatient register. Demographic factors, personality variables, and health-related factors were related to their satisfaction with care. Satisfaction with care showed no relationships to demographic factors such as age, living conditions, or civil status. However, significant associations indicated that patients who had never been hospitalized for mental illness, who were native Swedes, or who had an independent living rated their satisfaction with care higher. Personality, measured with the Temperament and Character Inventory, showed a relationship to satisfaction with care on only one dimension, self-directedness, of seven. Some of the results indicated a relationship between subjective measures and satisfaction with care, and some did not, but, taken together, the findings suggested a partial influence from a subjective factor on both subjective measures of well-being and on satisfaction with care. However, associations between interviewer-rated measures of health-related variables and satisfaction with care proposed that the better-functioning patients were more satisfied with the care, in turn indicating that the services better suited these patients. Thus, assuming that the influence of treatment was controlled for through the selection of long-term mentally ill subjects, this study pointed to two determinants of satisfaction with care: a selectively working subjective factor and the services being better designed for the better-functioning patients.

  • Perceptions of the real and the ideal ward atmosphere among trainees and staff before and after the introduction of a new work rehabilitation model

    • Changes in the ward atmosphere of a psychiatric work rehabilitation unit were investigated. Both trainees and staff filled in the Community-Oriented Programs Environment Scale (COPES) before and after a new rehabilitation model was implemented. It was hypothesised that the levels of autonomy and practical orientation would increase from both the trainees' and staff's perspective concerning the real ward atmosphere and that the staff's perceptions of an ideal ward atmosphere would change in the same way. The staff perceived an increased level of autonomy with respect to both the real ward atmosphere and to what constitutes an ideal ward atmosphere, which partly confirmed the hypotheses, but the level of practical orientation was stable. Concerning the staff's estimate of an ideal ward atmosphere, further changes were an increased level of involvement and a decrease in spontaneity, which was not hypothesised, but was not in conflict with the philosophy behind the new model. From the trainee's perspective there was no change of either autonomy or practical orientation. Instead, there was a decrease in personal problem orientation. Compared to an optimal profile, the ward atmosphere was beneficial, before as well as after implementation of the new programme. Differences were found between the staff and the trainees, but they were not large enough to separate the groups according to what is considered an optimal profile. The few changes found support earlier conclusions that the ward atmosphere is a stable phenomenon over time.

  • Psychometric evaluation of the Swedish version of Rosenberg’s self-esteem scale

    • Background: The widely used Rosenberg’s self-esteem scale (RSES) has not been evaluated for psychometric properties in Sweden. Aims: This study aimed at analyzing its factor structure, internal consistency, criterion, convergent and discriminant validity, sensitivity to change, and whether a four-graded Likert-type response scale increased its reliability and validity compared to a yes/no response scale. Methods: People with mental illness participating in intervention studies to (1) promote everyday life balance (N = 223) or (2) remedy self-stigma (N = 103) were included. Both samples completed the RSES and questionnaires addressing quality of life and sociodemographic data. Sample 1 also completed instruments chosen to assess convergent and discriminant validity: self-mastery (convergent validity), level of functioning and occupational engagement (discriminant validity). Confirmatory factor analysis (CFA), structural equation modeling, and conventional inferential statistics were used. Results: Based on both samples, the Swedish RSES formed one factor and exhibited high internal consistency (>0.90). The two response scales were equivalent. Criterion validity in relation to quality of life was demonstrated. RSES could distinguish between women and men (women scoring lower) and between diagnostic groups (people with depression scoring lower). Correlations >0.5 with variables chosen to reflect convergent validity and around 0.2 with variables used to address discriminant validity further highlighted the construct validity of RSES. The instrument also showed sensitivity to change. Conclusions: The Swedish RSES exhibited a one-component factor structure and showed good psychometric properties in terms of good internal consistency, criterion, convergent and discriminant validity, and sensitivity to change. The yes/no and the four-graded Likert-type response scales worked equivalently.

    • The relationships between characteristics of the ward atmosphere and treatment outcome in a psychiatric day-care unit were investigated. The treatment programme was based on occupational group therapy and included long-term mentally ill patients, equally distributed with regard to psychosis and non-psychosis diagnoses. The ward atmosphere was rated on the Community-Oriented Programs Environment Scale (COPES), and outcome variables concerned symptoms, global mental health, quality of life, and functioning in daily life. The COPES ratings were re-coded in order to control for the fact that different levels of ward atmosphere factors are recommended for psychotic and non-psychotic patients. The results revealed that an optimal initial level of anger and aggression in treatment was significantly associated with improvement in global mental health and functioning in everyday life. Initial optimal levels of staff control and practical orientation were significantly associated with improvement in psychiatric symptoms. The characteristics of the ward atmosphere at the end of the treatment period were significantly correlated with various aspects of functioning in daily life. The study demonstrated associations with outcome variables not previously linked with ward atmosphere characteristics, e.g. functioning in daily life and quality of life.
