  • Sedentary Time and Physical Activity Surveillance Through Accelerometer Pooling in Four European Countries

    • Objective: The objective of this study was to pool, harmonise and re-analyse national accelerometer data from adults in four European countries in order to describe population levels of sedentary time and physical inactivity. Methods: Five cross-sectional studies were included from England, Portugal, Norway and Sweden. ActiGraph accelerometer count data were centrally processed using the same algorithms. Multivariable logistic regression analyses were conducted to study the associations of sedentary time and physical inactivity with sex, age, weight status and educational level, in both the pooled sample and the separate study samples. Results: Data from 9509 participants were used. On average, participants were sedentary for 530 min/day, and accumulated 36 min/day of moderate to vigorous intensity physical activity. Twenty-three percent accumulated more than 10 h of sedentary time/day, and 72% did not meet the physical activity recommendations. Nine percent of all participants were classified as high sedentary and low active. Participants from Norway showed the highest levels of sedentary time, while participants from England were the least physically active. Age and weight status were positively associated with sedentary time and not meeting the physical activity recommendations. Men and higher-educated people were more likely to be highly sedentary, while women and lower-educated people were more likely to be inactive. Conclusions: We found high levels of sedentary time and physical inactivity in four European countries. Older people and obese people were most likely to display these behaviours and thus deserve special attention in interventions and policy planning. In order to monitor these behaviours, accelerometer-based cross-European surveillance is recommended.

  • The effect of periodontal therapy on intra-oral halitosis : a case series

    • AIM: The aim of this study was to evaluate the effects of non-surgical periodontal therapy on intra-oral halitosis 3 months after therapy.MATERIAL AND METHODS: Sixty-eight adults with intra-oral halitosis were included in a case series. Intra-oral halitosis was evaluated at baseline, and at 3 months after treatment using the organoleptic scores (OLS), Halimeter® , and a gas chromatograph.RESULTS: Significant reductions for OLS (p < 0.01), total sum of volatile sulphur compounds (T-VSC) (p < 0.01) and methyl mercaptan (MM) (p < 0.05) values were found after treatment. Hydrogen sulphide (H2 S) levels were not significantly reduced. The numbers of probing pockets 4 mm, 5 mm and 6 mm were significantly reduced as a result of therapy (p < 0.001). Bleeding on probing (BOP) and plaque indices were also significantly reduced (p < 0.001). For the 34 individuals with successful periodontal treatment (BOP<20% and a ≥50% reduction of total pocket depth) reductions in OLS (p < 0.01) and T-VSC scores (p < 0.01) were found. Eleven individuals were considered effectively treated for intra-oral halitosis presenting with a T-VSC value <160 ppb, a H2 S value <112 ppb and a MM value <26 ppb.CONCLUSION: Non-surgical periodontal therapy resulted in reduction of OLS, MM and T-VSC values 3 months after therapy. Few individuals were considered as effectively treated for intra-oral halitosis.

  • Tightly Coupled Positioning and Multipath Radio Channel Tracking

    • Radio based localization is an active research topic with a wide range of applications. In this paper, we focus on localization of a radio receiver equipped with an inertial measurement unit. The localization is performed while simultaneously constructing a map of the small scale fading pattern in the local radio environment. The map in our case is a ray-trace-based multipath channel model. This solution is enabled by sensor fusion of information from the channel estimation data and the inertial sensors, and it does not assume any knowledge of, e.g., transmitter locations. The sensor data is fused in a recursive state space model that combines the kinematic motion model with the ray-based radio channel model, and the state vector is estimated using a particle filter. The choice of the particle filter is justified by the multimodal characteristics of the posterior likelihood distributions that follows from the nonlinearities of the problem. The work is assuming a single receiver antenna but the approach can also be transferred to multiple antenna systems. We study the performance of the approach under realistic assumptions, based on the performance of today’s low-cost inertial sensors and radio systems, including accelerometer and gyroscope noise, and also radio receiver frequency error and noise. Simulations show a significant improvement in long-term positioning performance, evaluated against dead reckoning. The work is concluded with experiments which serve as a proof of concept for the proposed technique, using no extra equipment compared to what can be found in a modern cellular phone.

Many inequalities among these constants are proposed in two theorems, 8 corollaries and several propositions. The proofs of these results can be found in a Research report of Lulea Univ., 2003.

  • Differential Genetic Associations for Systemic Lupus Erythematosus Based on Anti-dsDNA Autoantibody Production

    • Systemic lupus erythematosus (SLE) is a clinically heterogeneous, systemic autoimmune disease characterized by autoantibody formation. Previously published genome-wide association studies (GWAS) have investigated SLE as a single phenotype. Therefore, we conducted a GWAS to identify genetic factors associated with anti-dsDNA autoantibody production, a SLE-related autoantibody with diagnostic and clinical importance. Using two independent datasets, over 400,000 single nucleotide polymorphisms (SNPs) were studied in a total of 1,717 SLE cases and 4,813 healthy controls. Anti-dsDNA autoantibody positive (anti-dsDNA +, n = 811) and anti-dsDNA autoantibody negative (anti-dsDNA -, n = 906) SLE cases were compared to healthy controls and to each other to identify SNPs associated specifically with these SLE subtypes. SNPs in the previously identified SLE susceptibility loci STAT4, IRF5, ITGAM, and the major histocompatibility complex were strongly associated with anti-dsDNA + SLE. Far fewer and weaker associations were observed for anti-dsDNA - SLE. For example, rs7574865 in STAT4 had an OR for anti-dsDNA + SLE of 1.77 (95% CI 1.57-1.99, p = 2.0E-20) compared to an OR for anti-dsDNA - SLE of 1.26 (95% CI 1.12-1.41, p = 2.4E-04), with (Pheterogeneity)<0.0005. SNPs in the SLE susceptibility loci BANK1, KIAA1542, and UBE2L3 showed evidence of association with anti-dsDNA + SLE and were not associated with anti-dsDNA - SLE. In conclusion, we identified differential genetic associations with SLE based on anti-dsDNA autoantibody production. Many previously identified SLE susceptibility loci may confer disease risk through their role in autoantibody production and be more accurately described as autoantibody propensity loci. Lack of strong SNP associations may suggest that other types of genetic variation or non-genetic factors such as environmental exposures have a greater impact on susceptibility to anti-dsDNA - SLE.

    • Baksidestext: Vad hände när invandrade akademiker av Arbetsförmedlingen erbjöds att delta i en kompletteringsutbildning för att sen kunna fortsätta med en svensk lärarutbildning? Samhällets uttalade målsättning är att invandrare skall integreras. Trots detta är många invandrare arbetslösa och det finns många hinder för deras inträde i arbetsmarknaden. Ett speciellt önskemål är att rekrytera fler invandrare till läraryrket så att lärarna kan spegla befolkningens sammansättning. Denna rapport beskriver projektet Kompletteringsutbildning för Invandrade Akademiker (KIA), som startade 1999 med utgångspunkt i ovannämnda målsättningar. Utifrån gruppdiskussioner med kursdeltagare och intervjuer med lärare, beskrivs och tolkas olika händelser och upplevelser samt de problem som uppstod och fick lösas under det första året.

    • BakgrundSjuksköterskor i psykiatrisk vård bör arbeta med att planera vården i samarbete med patienten och spendera tid på att skapa terapeutiska relationer. Samtidigt menar många sjuksköterskor att de saknar den tid som krävs för att samtala med patienterna.SyfteAktuell tidsstudie genomfördes för att undersöka hur sjuksköterskor i psykiatrisk heldygnsvård fördelar den tid de har tillgänglig under ett arbetspass.Metod12 sjuksköterskor på sex vårdavdelningar vid en psykiatrisk klinik i södra Sverige observerades. Totalt utfördes strukturerade observationer under 129 timmar och 23 minuter. Tidfördelning, aktivitetsfrekvens och avbrott registrerades.ResultatLäkemedelshantering var den största kategorin av den uppmätta tiden (17,5 %), följt av indirekt omvårdnad (16 %). Relativt lite tid lades på direkt omvårdnad, den tredje största kategorin (15,3 %) medan en oväntad stor del av tiden (11,3 %) lades på verksamhetsrelaterade uppgifter. Sjuksköterskor avbröts också vid 75 % av alla tillfällen då de arbetade med läkemedelshantering. Således spenderade sjuksköterskor förhållandevis lite tid på direkt omvårdnad och läkemedelshanteringen avbröts ofta. Det är svårt att bygga terapeutiska relationer med syfte att lägga en grund för hållbar psykiska hälsa om relativt lite tid spenderas tillsammans med patienten. Det är oroande att både patientsäkerhet och sjuksköterskors arbetstillfredsställelse riskeras på detta sätt.

  • Testing the SF-36 in Parkinson's disease : Implications for reporting rating scale data.

    • Rating scales are increasingly the primary outcome measures in clinical trials. However, clinically meaningful interpretation of such outcomes requires that the scales used satisfy basic requirements (scaling assumptions) within the data. These are rarely tested. The SF-36 is the most widely used patient-reported rating scale. Its scaling assumptions have been challenged in neurological disorders but remain untested in Parkinson's disease (PD).We therefore tested these by analyzing SF-36 data from 202 PD patients (54% men; mean age 70) to determine if it was legitimate to report scores for the eight SF-36 scales and its two summary measures of physical and mental health, and if those scores were reliable and valid. Results supported generation of the eight SF-36 scale scores and their reliabilities were generally good (>/= 0.74 in all but one instance). However, we found limitations that question the meaningfulness of four scales and other limitations that restrict the ability of four scales to detect change in clinical trials (floor/ceiling effects, 19.6-46.2 %). The two SF-36 summary measures were not found to be valid indicators of physical and mental health. This study demonstrates important limitations of the SF-36 and provides the first evidence- based guidelines for its use in PD. The limitations of the SF-36 demonstrated here may explain some unexpected findings in previous studies. However, the main implication is a general one for the clinical research community regarding requirements for reporting rating scale endpoints. Specifically, investigators should routinely provide scale evaluations based on data from within major clinical trials.
