    • Biliary complications are common after orthotopic liver transplantation. Bile leakage in the immediate postoperative period and on removal of the T-tube could possibly be caused by a raised bile duct pressure. In order to test this hypothesis, bile duct pressure was studied in seven consecutive liver transplant patients. During the operation, the common bile duct was anastomosed end-to-end over a T-tube. The initial bile duct pressure measurement was performed a median of 12 days (range 10-17 days) after the transplantation and on one or two more occasions during the following 3 months. Seven cholecystectomized gallstone patients with indwelling T-tubes were used as controls. The bile duct pressure at the level of the xiphoid process in the transplanted group was 7.7 +/- 1.4 cm H2O and in the control group 0.5 +/- 0.8 cm H2O (P < 0.001). The initially increased bile duct pressure after liver transplantation decreased with time (P < 0.05) towards normal during the following 3 months. The raised pressure may increase the risk of bile leakage in the postoperative period.

    • An investigation of patients with pancreatic carcinoma aims to identify those who will benefit from surgery. Physical examination, radiology and laboratory findings are helpful. Most prognostic markers, such as lymph node status, micro metastasis and tumour differentiation, are not preoperatively accessible. Metastatic disease in lymph node 8a (Ln8a) in patients operated for pancreatic carcinoma has been reported to be a predictor of shorter overall survival (OS). This lymph node can be assessed preoperatively through resection (possibly even with laparoscopy) and subsequent histopathology. The value of the procedure is disputed. The aim of this study is to investigate whether metastatic disease in Ln8a is a predictor of decreased OS.In patients with suspected pancreatic or periampullary carcinoma, who were operated with pancreatoduodenectomy (PD), Ln8a was separately resected and analysed with standard and immuno-histochemical methods. Patients with or without metastasis in Ln8a were compared regarding OS.Between 2008 and 2011, 122 consecutive patients were eligible and 87 were resected and had LN8a analysed separately. Sixteen patients were Ln8a+and 71 were Ln8a-. Patients with Ln8a+had a significantly reduced median OS as compared to patients with Ln8a- (0.74 (95% CI 0.26-1.26) versus 5.91 years (95% CI 2.91-), p<.001).Ln8a+was associated with a marked reduction of OS, indicating a possible role in the future preoperative workup in patients with a suspicion of pancreatic cancer.

    • Invertible programming languages specify transformations to be run in two directions, such as compression/decompression or encryption/decryption. Two key concepts in invertible programming languages are partial invertibility and local invertibility. Partial invertibility lets invertible code be parameterized by the results of non-invertible code, whereas local invertibility requires all code to be invertible. The former allows for more flexible programming, while the latter has connections to domains such as low-energy computing and quantum computing. We find that existing approaches lack a satisfying treatment of partial invertibility, leaving the connection to local invertibility unclear. In this paper, we identify four core constructs for partially invertible programming, and show how to give them a locally invertible interpretation. We show the expressiveness of the constructs by designing the functional invertible language Kalpis, and show how to give them a locally invertible semantics using the novel arrow combinator language RRARR—the key idea is viewing partial invertibility as an invertible effect. By formalizing the two systems and giving Kalpis semantics by translation to RRARR, we reconcile partial and local invertibility, solving an open problem in the field. All formal developments are mechanized in Agda.
