  • Dental extractions for patients on oral antiplatelet: a within-person randomised controlled trial comparing haemostatic plugs, advanced-platelet-rich fibrin (A-PRF+) plugs, leukocyte- and platelet-rich fibrin (L-PRF) plugs and suturing alone.

    • To compare the outcome of tooth extractions in patients taking oral antithrombotic without reducing their dose. Four different interventions were compared within the same patient: suturing alone (control group), suturing plus a haemostatic plug, suturing plus advanced-platelet-rich fibrin (A-PRF+) plug, and suturing plus leukocyte- and platelet-rich fibrin (L-PRF) plug into the socket.Forty patients, taking oral antiplatelet agents, requiring the extraction of at least four non-adjacent teeth were selected for the study. After extractions the sockets were randomly allocated to suturing alone (control group), suturing plus haemostatic plug (HAEM), suturing plus advanced-platelet-rich fibrin (A-PRF+) plug, and suturing plus leukocyte- and platelet-rich fibrin (L-PRF) plug into the socket without reducing the dose of oral antiplatelets according to a split-mouth design. Outcome measures were complications, time to complete each procedure, postoperative bleeding, costs of the materials, patient preference and a wound healing index recorded 1 and 2 weeks postextraction by blinded assessors.Two weeks after extraction no patient dropped out and no complication was reported. The average time to complete suturing after tooth extractions was: 1.0 ± 0.00 minutes at control sites, 1.5 ± 0.41 at HAEM sites, 2.8 ± 0.61 at A-PRF+ sites, and 2.8 ± 0.56 at L-PRF sites, the difference being statistically significant between each pairwise comparison except A-PRF+ vs L-PRF. Postoperative bleeding 30 minutes after extractions was present at 8, 5, 1 and 2 sites for control, HAEM, A-PRF+ and L-PRF sites, respectively. A-PRF showed statistically significantly less bleeding compared to the control group (odds ratio = 0.1 (95% CI [0.01;0.86]; P < 0.0361). In all cases bleeding was moderate in nature and not severe. One week after extractions the mean wound healing index was 1.05 ± 0.60 for control, 1.18 ± 0.59 for HAEM, 1.00 ± 0.68 for A-PRF+ and 0.95 ± 0.50 for L-PRF sites. No statistically significant difference was detected across groups (P = 0.633). Two weeks after extractions the mean wound healing index was 0.33 ± 0.53 for control, 0.43 ± 0.50 for HAEM, 0.25 ± 0.49 for A-PRF+ and 0.15 ± 0.36 for L-PRF sites. No statistically significant difference across groups was detected (P = 0.255). One week after extractions, nine patients preferred control sites, eight HAEM, ten A-PRF+, four L-PRF and nine had no preference. No statistically significant differences were detected for control sites (P = 0.6779), HAEM (P = 1.0000), A-PRF+ (P = 0.4055) and L-PRF (P = 0.1472). Two weeks after extractions five patients preferred control sites, three HAEM, eight A-PRF+, eight L-PRF and 16 had no preference. No statistically significant differences were detected for control sites (P = 0.8147), HAEM (P = 0.2363), A-PRF+ (P = 0.3488) and L-PRF (P = 0.3488). Costs without counting sutures and blood centrifuges were 0.00, 14.49, 2.44 and 2.44 Euro for control, HAEM, A-PRF+ and L-PRF sites, respectively.It may not be necessary to discontinue the use of oral antiplatelets in patients undergoing dental extractions and, when present, the minor statistically significant differences between procedures were not clinically relevant; therefore clinicians can use any of the tested interventions according to their preference, keeping in mind that simple suturing is sufficient and is faster and cheaper, and that A-PRF+ was associated with less postoperative bleeding when compared to suturing alone.

  • Occultism and Traditionalism: Arturo Reghini and the Antimodern Reaction in Early Twentieth-Century Italy

    • The thesis will aim to analyse the interaction between occultism and different facets of modernity in early twentieth-century Italy, focusing on the figure of Arturo Reghini (1878-1946) as a case study. In his ´The Modernity of Occultism: Reflections on Some Crucial Aspects’, Marco Pasi convincingly evaluates the Hermetic Order of the Golden Dawn (est. 1888) and the Theosophical Society (est. 1875) as modern expressions of Anglo-American occultism, which provide some evidence against the possibly over-simplistic Theses Against Occultism (1946-7) by Theodor Adorno, which associated occult movements with reactionary politics and irrationalism. James Webb echoed Adorno´s theories in his opera prima, Flight From Reason (1971), where the author judges the Occult Revival as a revolt against Enlightenment ideals and as a total embrace of irrationalism. Academic work has been undertaken to readdress the balance of such theories, and far from dismissing occultist currents as irrationalist and out-dated, scholars have judged these milieus to be intrinsic to the making of the modern, constitutive of modern culture, sometimes even permeating the aesthetics of modernism. The occult is seen as bound up with a spiritualised vision of social change, but what that vision turned out to represent varied greatly from country to country. By following S. N. Eisenstadt’s theory on Multiple Modernities, I will attempt to focus on Italian national forms of occultism, chiefly analysing Arturo Reghini and his writings and what has been labelled as Roman Traditionalism: an Italian, autochthone, occultist current deeply interrelated with the particular climate of Italian modern society of the early twentieth century. While witnessing similar processes as the rest of European countries, such as secularization, the disenchantment of the world, and the rise of positivism, studying occultism, Traditionalism and modernity in an Italian context must take into consideration many factors privy to the Italian peninsula. I will make clear that, when analysing the Italian occult milieu, (and Arturo Reghini in more specific terms,) the results obtained will be in some cases vastly different than the conclusions drawn by scholars focusing on other countries, while at the same time retaining some similarities which we could consider a constant of modernity’s influence throughout Europe. For these reasons, in my opinion, the relationship between occultism, Traditionalism and modernity in Italy, especially taking Reghini’s writings and his brand of Italian neo-paganism as a point of reference, is worthy of study and can provide deeper clarity concerning the reassessment of antimodern forms of occultism and a new positioning of occultism and Traditionalism in the intellectual history of early twentieth-century Italy.   • Pagan Rome Was Rebuilt in a Play: Roggero Musmeci Ferrari Bravo and the Representation of Rumon

    • At 17:30 of 6 May 1923, on the Palatine hill, Roggero Musmeci Ferrari Bravo (1866-1937), better known by his pseudonym ignis, staged one of the most unconventional plays to be composed in Italian language: Rumon. With hundreds of extras hired for the occasion and a substantial financing by Benito Mussolini (1883-1945) himself, the play re-enacted the struggle between Romulus and Remus and the successive foundation of the Eternal City. More than a simple, albeit unlikely, example of pagan pageant, Rumon was the most noticeable expression of a cultural current set to bring about a new pagan Golden Age in Rome and, more generally, Italy. More than one critical review described Rumon as a potent rite of birth which heralded a new dawn for Rome and the traditons of its ancient cives, and the play itself features archaic invocations to Roman deities, the exaltation of Roman lineage and the use of the secret and magical names of Rome. In this paper I will briefly analyse the pagan milieu of Rome which no doubt influenced Musmeci Bravo, successively highlighting the most relevant passages of Rumon in which the birth of a newly pagan Rome was so majestically portrayed.

  • 'The Master's Voice' The Hidden Gurdjieff in the Music and Art of Franco Battiato

    • Occultism has been present in popular music since the 1960s, but genuine occult content has been lacking in chart-topping bands, relegating more serious interactions between esoteric message and music to underground phenomena. An exception to this trend is Franco Battiato, who has sold in excess of three million records, one million of his masterpiece, La Voce del Padrone (The Voice of the Master, 1981), alone. This article analyses Battiato's method of communicating occult lore through coded words, which couch esoteric meaning in exoteric forms of expressions. The article provides a substantial analysis of Battiato's career, including his spiritual research through the works of Sufi masters, Rene Guenon and, especially, the teachings of the Armenian mystic G.I. Gurdjieff. Through a textual analysis of Battiato's lyrics of his most mystical works (1979-1982), I aim to identify the Italian songwriter's hidden references to the works of Gurdjieff and his disciple P.D. Ouspensky, proving that Battiato represents a rare case of a pop musician whose works are informed by occult knowledge, and who wants to communicate this hidden lore to listeners.

    • Background and objectives: Dermoscopy is a useful tool for the early and non-invasive diagnosis of skin malignancies. Besides many progresses, heavily pigmented and amelanotic skin tumors remain still a challenge. We aimed to investigate by dermoscopy if distinctive morphologic characteristics of vessels may help the diagnosis of equivocal nodular lesions. Materials and Methods: A collage of 16 challenging clinical and dermoscopic images of 8 amelanotic and 8 heavily pigmented nodular melanomas and basal cell carcinomas was sent via e-mail to 8 expert dermoscopists. Results: Dermoscopy improved diagnostic accuracy in 40 cases. Vessels were considered the best clue in 71 cases. Focusing on the diameter of vessels improved diagnosis in 5 cases. Conclusions: vascular diameter in addition to morphology and arrangement may be a useful dermoscopic clue for the differential diagnosis of clinically equivocal nodular malignant tumors.

    • Background The European Society of Plastic, Reconstructive and Aesthetic Surgery (ESPRAS) comprises 40 national societies across Europe. In addition to ESPRAS, there are 8 different European Plastic Surgery societies representing Plastic Surgeons in Europe. The 4 th European Leadership Forum (ELF) of ESPRAS, held under the motto "Stronger together in Europe" in Munich in 2023, aimed to collect and disseminate information regarding the national member societies of ESPRAS and European societies for Plastic Surgeons. The purpose was to identify synergies and redundancies and promote improved cooperation and exchange to enhance coordinated decision-making at the European level. Material and methods An online survey was conducted regarding the organisational structures, objectives and challenges of national and European societies for Plastic Surgeons in Europe. This survey was distributed to official representatives (Presidents, Vice Presidents and General Secretaries) and delegates of national and European societies at the ELF meeting. Missing information was completed using data obtained from the official websites of the respective European societies. Preliminary results were discussed during the 4 th ELF meeting in Munich in March 2023. Results The ESPRAS survey included 22 national and 9 European Plastic Surgery societies representing more than 7000 Plastic Surgeons in Europe. Most national societies consist of less than 500 full members (median 182 members (interquartile range (IQR) 54-400); n=22). European societies, which covered the full spectrum or subspecialities, differed in membership types and congress cycles, with some requiring applications by individuals and others including national societies. The main purposes of the societies include research, representation against other disciplines, specialisation and education as well as more individual goals like patient care and policy regulation. Conclusion This ESPRAS survey offers key insights into the structures, requirements and challenges of national and European societies for Plastic Surgeons, highlighting the relevance of ongoing close exchange between the societies to foster professional advancement and reduce redundancies. Future efforts of the ELF will continue to further explore strategies for enhancing collaboration and harmonisation within the European Plastic Surgery landscape.

    • The European Leadership Forum (ELF) of the European Society of Plastic, Reconstructive and Aesthetic Surgery (ESPRAS) previously identified the need for harmonisation of breast reconstruction standards in Europe, in order to strengthen the role of plastic surgeons. This study aims to survey the status, current trends and potential regional differences in the practice of breast reconstruction in Europe, with emphasis on equity and access.A largescale web-based questionnaire was sent to consultant plastic and reconstructive surgeons, who are experienced in breast reconstruction and with understanding of the national situation in their country. Suitable participants were identified via the Executive Committee (ExCo) of ESPRAS and national delegates of ESPRAS. The results were evaluated and related to evidence-based literature.A total of 33 participants from 29 European countries participated in this study. Overall, the incidence of breast reconstruction was reported to be relatively low across Europe, comparable to other large geographic regions, such as North America. Equity of provision and access to breast reconstruction was distributed evenly within Europe, with geographic regions potentially affecting the type of reconstruction offered. Standard practices with regard to radiotherapy differed between countries and a clear demand for European guidelines on breast reconstruction was reported.This study identified distinct lack of consistency in international practice patterns across European countries and a strong demand for consistent European guidance. Large-scale and multi-centre European clinical trials are required to further elucidate the presented areas of interest and to define European standard operating procedures.
