    • BackgroundOak moss absolute, an extract from the lichen Evernia prunastri, is a valued perfume ingredient but contains extreme allergens. ObjectivesTo compare the elicitation properties of two preparations of oak moss absolute: classic oak moss', the historically used preparation, and new oak moss', with reduced contents of the major allergens atranol and chloroatranol. Patients/materials/methodsThe two preparations were compared in randomized double-blinded repeated open application tests and serial dilution patch tests in 30 oak moss-sensitive volunteers and 30 non-allergic control subjects. ResultsIn both test models, new oak moss elicited significantly less allergic contact dermatitis in oak moss-sensitive subjects than classic oak moss. The control subjects did not react to either of the preparations. ConclusionsNew oak moss is still a fragrance allergen, but elicits less allergic contact dermatitis in previously oak moss-sensitized individuals, suggesting that new oak moss is less allergenic to non-sensitized individuals.

    • Background: Contact allergy to oxidized limonene, with hydroperoxides of limonene (Lim-OOHs) as the main allergens, is common. However, high proportions of weak positive and doubtful patch test reactions have been reported. Objectives: To determine the clinical relevance, elicitation threshold and dose-response relationship of Lim-OOHs in individuals with a positive or doubtful patch test reaction to standard Lim-OOHs 0.3% pet. Methods: A multicentre 3-week double-blind vehicle-controlled repeated open application test (ROAT) study with a simulated fine fragrance containing Lim-OOHs at 1260, 420 and 140 ppm, equal to a dose/area per application of Lim-OOHs of 3.0, 0.99 and 0.33 mu g/cm(2), was performed. Results: Among 11 subjects allergic to Lim-OOHs, 11 (100%), 7 (64%), and 3 (27%), respectively, reacted to the applied doses. No reactions were seen in 17 healthy controls exposed to the highest dose. This difference in reactivity was statistically significant (P < 0.0001). Among 13 subjects with doubtful patch test reactions to Lim-OOHs, two (15%) had positive ROAT reactions to the highest Lim-OOH dose applied (P = 0.36 as compared with controls). Conclusions: Contact allergy to Lim-OOHs is of clinical relevance in patients with positive patch test reactions. A doubtful patch test reaction to Lim-OOHs 0.3% pet. can be of clinical relevance.

  • New contact allergens are formed during vulcanization of rubber

    • In rubber materials, both natural rubber and synthetic rubber, several different contact allergenic chemicals are added to the batch before vulcanization. These compounds are relevant for investigation of rubber allergy of workers in rubber factories. The number of workers in the rubber industry has declined, owing to increased automation of the process; however, the number of users of rubber products, such as rubber gloves, is increasing. Furthermore, users of finished rubber products are exposed to allergens other than those affecting workers in rubber factories because of chemical reactions during vulcanization between different additives, altering their structures. Our experience with thiuram mono- and disulfides is that during vulcanization, monosulfides are formed from disulfides. We have also seen that in rubber vulcanized with both thiurams and mercaptobenzothiazole compounds, chemicals are formed that contain both thiocarbamate and a mercaptobenzothiazole structure. Two examples of this are dimethylthiocarbamylbenzothiazole sulfide (DMTBS) and diethylthiocarbamylbenzothiazole sulfide (DETBS). The latter is a commercial accelerator with the name Ethylac. In an earlier study in which we patch tested 24 volunteers with a known allergy to thiuram mix and/or mercapto mix, we observed positive reactions to DMTBS (1% petrolatum) and/or DETBS (1% petrolatum) in 20 subjects. So far we have seen positive reactions to either DMTBS or DETBS in two patients who were investigated because of their dermatitis related to the rubber in their swimming goggles. Chemical analysis of the swimming goggles showed them to contain DMTBS and DETBS, respectively. Furthermore, several patients in the Netherlands and Belgium with allergy to their shoes were found to be allergic to DMTBS. Chemical analysis of these shoes has been performed and DMTBS was identified in the rubber lining. The allergy to DMTBS is an example of how a powerful allergen can be formed during vulcanization as a result of chemical reactions between well-known haptens. It also illustrates that chemical investigations of patients' rubber items can uncover the presence of allergens that were not added during manufacturing and therefore never show up in declarations from the producers.

  • Patch testing children with aluminium chloride hexahydrate in petrolatum: A review and a recommendation

    • Background: According to studies on adults, patch testing with aluminium chloride hexahydrate 2% pet. is insufficient to detect aluminium allergy, and a 10% preparation is recommended. Other studies suggest that a 2% preparation is sufficient for testing children. Objectives: To review three previously published Swedish studies on patch testing children with aluminium chloride hexahydrate 2% pet. Patients/Methods: Altogether, 601 children with persistent itching subcutaneous nodules (granulomas) induced by aluminium-adsorbed vaccines were patch tested with aluminium chloride hexahydrate 2% pet. and metallic aluminium in (a) a pertussis vaccine trial, (b) clinical practice, and (ca) prospective study. Results: Overall, 459 children had positive reactions to the 2% pet. preparation. Another 10 reacted positively only to metallic aluminium. An extreme positive reaction (+++) was seen in 65% of children aged 1 to 2 years as compared with 22% of children aged 7 years. From 8 years onwards, extreme positive reactions were scarce. Conclusions: Aluminium chloride hexahydrate 2% pet. is sufficient to trace aluminium allergy in children. Small children are at risk of extreme reactions. We thus suggest that aluminium chloride hexahydrate 10% pet. should not be used routinely in children before the age of 7 to 8 years.

  • The reactivity of the back revisited. Are there differences in reactivity in different parts of the back?

    • Background: In the contact dermatitis literature, it is regularly stated that the patch test reactivity on various areas of the back differs; this might have a large impact on the reproducibility of patch testing. Objectives: To investigate the reproducibility of patch testing on the upper back with regard to the left as opposed to the right side and the medial as opposed to the lateral part of the upper back. The reproducibility over time and with regard to the reactivity pattern was also investigated. Methods: Thirty-one subjects with contact allergy to the metals gold (n=19) or nickel (n=12) were patch tested with serial dilutions, in triplicate applications, on different locations on the upper back. The Friedman test was used for statistical calculations. Results: No significant differences in the reactivity of the back were found. In all gold-allergic patients and 11 of 12 nickel-allergic patients, the allergy could be reproduced with regard to previous patch testing, but the degree of reactivity differed. Conclusions: When a high level of standardization of the patch test technique with the same test system was used, there were no differences in patch test reactions and sites of application on the upper back.
