    • Computer-based quality assurance of specialist eating disorder (ED) care is a possible way of meeting demands for evaluating the real-life effectiveness of treatment, in a large-scale, cost-effective and highly structured way. The Internet-based Stepwise system combines clinical utility for patients and practitioners, and provides research-quality naturalistic data. Stepwise was designed to capture relevant variables concerning EDs and general psychiatric status, and the database can be used for both clinical and research purposes. The system comprises semi-structured diagnostic interviews, clinical ratings and self-ratings, automated follow-up schedules, as well as administrative functions to facilitate registration compliance. As of June 2009, the system is in use at 20 treatment units and comprises 2776 patients. Diagnostic distribution (including subcategories of eating disorder not otherwise specified) and clinical characteristics are presented, as well as data on registration compliance. Obstacles and keys to successful implementation of the Stepwise system are discussed, including possible gains and on-going challenges inherent in large-scale, Internet-based quality assurance.

    • The aim of this 5-year longitudinal study of 593 girls (9-20-year-old) was to examine whether the internalization of the thinness ideal in terms of “a wish to be thinner” might be related to lifestyle factors and longitudinally increase the risk of disturbed eating over time. Results showed that a wish to be thinner was related to lifestyle factors, eating attitudes and BMI longitudinally. Girls who wished to be thinner dieted more often, thought that they would be more popular if they were thinner, skipped meals, were eating breakfast more often alone and had a higher BMI compared to the girls without such a wish. Girls who wished to be thinner were 4 times more likely to develop disturbed eating attitudes over a 5-year period. These findings point to the importance of helping adolescents to establish regular eating habits, to avoid unhealthy dieting practices and to prevent sedentary behaviours that might lead to overweight and or obesity in early childhood.

    • Solutions to climate change have been academically criticized for their continued economic growth, managerialism and lack of real politics. In comparison, the IPCC's socio-economic assessments of climate change have accentuated the ethical implications of their own policy recommendations. Our analysis of ten IPCC reports (1990–2012) shows a turn from a claimed non-political position in human-induced climate change to an outspoken ethical position in climate-induced disasters. We argue that a professionalization of climate ethics is sought through ecological reason, specifically by calls for resilience to foster adaptable subjects. This neoliberal position leans on a problematization of vulnerable subjects' resistance to social adaptation, underpinned by an aim to redirect resistance towards physical disasters to stimulate climate adaptation. Conclusively, climate ethical mastery is formed by detailed elaborations of how the vulnerable subject should not only subsume to ecological reason, but also ethically embrace physical threats and dangers as if productive of life supportive qualities.

  • Poor Catch-up Growth in Late Adolescent Boys with Eating Disorders, Weight Loss and Stunting of Growth

    • Objective The study aims to investigate the catch-up growth of boys presenting with an eating disorder (ED) and a stunting of growth. Method Weight gain and growth of 46 boys with ED were followed up for 1-3years. Results A total of 13 boys who had not started their pubertal growth spurt at presentation started catch-up growth immediately following nutritional rehabilitation and weight gain. After 3years, they had returned to their premorbid growth curve. Thirty-three boys who had started their pubertal growth spurt prior to presentation never caught up in height but continued to catch down despite weight gain. After 3years, they had lost 0.64 +/- 0.55 height standard deviation scores corresponding to approximately 4.5cm of potential height. Conclusions In prepubertal boys with EDs, catch-up growth is possible. Pubertal boys are at a disadvantage in that catch-up growth may not occur despite weight gain.

    • The main aim of this seven-year longitudinal study, of 411 Swedish pre-adolescent girls, (aged 7–11 years, Year 1) was to examine a wish to be thinner, dieting attempts, described motives for wishing to be thinner, and weight-control practices. A further aim was to examine to what extent BMI accounted for motives for wishing to be thinner and weight control practices. A wish to be thinner and dieting attempts increased significantly with increasing age between the ages of 9 and18. The most frequently reported motive for wishing to be thinner was to “feel better about yourself”. Categories that emerged from the qualitative analysis of self-described motives for the wish to be thinner were for example to “correspond to the societal ideal” and to “wear particular clothes”. A majority of the girls adopted weight control practices that would be considered as healthy, but extreme weight control practices increased with age. Girls with BMIs over the 75th percentile reported a greater number of motives for wishing to be thinner and used extreme weight control practices significantly more often than the other girls. However, of the girls who changed BMI from above the 75th percentile to under the 75th percentile, 34.1% reported that they had not used any weight control practices at all. Our results show that girls at a very early age are aware of the ‘thin’ ideal in our society, wish to be thinner, and try to lose weight. The results point to the importance of detecting girls who wish to be thinner as early as possible. If we can employ preventive action in time it is possible that dieting behavior will never develop. By considering the motives for wishing to be thinner, preventive efforts might be perceived as more meaningful and relevant to the girls at risk of developing eating disorders.
