  • Aspirin and risk for gastric cancer : a population-based case-control study in Sweden

    • While aspirin and other non-steroid anti-inflammatory drugs (NSAIDs) are associated with gastric mucosal damage, they might reduce the risk for gastric cancer. In a population-based case-control study in 5 Swedish counties, we interviewed 567 incident cases of gastric cancer and 1165 controls about their use of pain relievers. The cases were uniformly classified to subsite (cardia/non-cardia) and histological type and information collected on other known risk factors for gastric cancer. Helicobacter pylori serology was tested in a subset of 542 individuals. Users of aspirin had a moderately reduced risk of gastric cancer compared to never users; odds ratio (OR) adjusted for age, gender and socioeconomic status was 0.7 (95% CI = 0.6-1.0). Gastric cancer risk fell with increasing frequency of aspirin use (P for trend = 0.02). The risk reduction was apparent for both cardia and non-cardia tumours but was uncertain for the diffuse histologic type. No clear association was observed between gastric cancer risk and non-aspirin NSAIDs or other studied pain relievers. Our finding lends support to the hypothesis that use of aspirin reduces the risk for gastric cancer.

    • BACKGROUND:The present study is based on four earlier studies in which the authors classified the relative's involvement in palliative care into different categories and described the involvement as "involvement in the light" or "involvement in the dark".AIM:The aim of the study was to develop a theoretical framework concerning the involvement of relatives based on an in-depth analysis of the results of the four earlier studies.METHOD:Walker & Avant's (1995) strategies for theory construction were used for development of the framework. A number of different concepts, assumptions and statements about relatives' involvement were penetrated in an in-depth analysis.RESULTS:From the concepts two theoretical "blocks" of the relatives' involvement were developed and these constitute the foundation for the framework. One is based on concrete descriptions of the concepts "to know", "to be" and "to do". The other describes how the concepts of "involvement in the light" and "involvement in the dark" differ. Factors that promoted involvement in the light were professional care based on humanistic values, a stronger sense of coherence on the part of relatives, an appropriate illness trajectory, and other available resources. The opposite was the case for those who were involved in the dark. Five assumptions successively developed which together form the theoretical framework.CONCLUSIONS:An important conclusion that can be of importance in palliative care is that the manner in which the staff act toward the patient and relatives influence relatives' possibilities for involvement, patients' possibilities for an appropriate death, and the possibilities the staff have to give good care.

  • Bullying in school and violence on the streets : are the same people involved?

    • Examined the relationship between bullying in school and street violence. 2,915 adolescents (aged 14-15 yrs) completed questionnaires concerning street violence, weapon carrying, violence victimization, loitering, bullying, and nights away from home. Results show that bullying others in school was strongly linked to violent behavior and weapon-carrying on the streets, both among males and females. Bullying others in school was also related to being violently victimized on the streets. Findings suggest that school bullying is in many cases a part of a more general violent and aggressive behavior pattern, and that preventive efforts targeting individuals with bullying behavior in school may decrease violence among adolescents in the community as well.

    • In recent developmental research, sleep-wake patterns and preferences, in other words Morningness-Eveningness, have been shown to be related to various kinds of problem behavior. Previous research on adolescents has demonstrated that individuals with a sleep-wake preference toward staying up late in the evening and arising late in the morning (i.e., Eveningness), are more likely to face problems in development. Accordingly, Eveningness has been proposed as a risk factor in development, but through processes which researchers have not been able to fully explain. The present dissertation focuses on the relationship between Morningness-Eveningness and developmental patterns across contexts in adolescence. Links between Morningness-Eveningness, and negative adjustment, life style, personality characteristics, family relationships, peer networks, and school achievement, are discussed within an interactionistic framework. Also, factors that might explain and/or moderate these associations are elucidated. The empirical material stems from two cohorts of 8th grade adolescents in a middle size Swedish community. Eveningness is proposed to be a concurrent marker of risk behavior. However, there seems to be different kinds of Evening types that encounter different kinds of problems. Also, the present findings do not support the notion of Morningness being protective. How temporal patterns in general, and Eveningness in particular, are connected to other behaviors is emphasized as important knowledge for stemming negative developmental processes and facilitating positive outcomes.

  • The book and its narratives : a critical examination of some synchronic studies of the book of Judges

    • During recent decades, there has been a trend among biblical scholars towards applying methods borrowed from literary studies to the familiar texts of the Old and New Testaments. A major reason for this reorientation is the search for a meaningful and interpretable text; hence, it can be seen as a protest against the historical-critical school and its ambition to reconstruct an authentic text by means of a diachronic analysis. Synchronic scholars argue for a new understanding of the biblical text, claiming that the object of interpretation is the text in its present form, regarded as a literary production. Consequently, they can study texts that are commonly considered to be patchworks or conglomerations as meaningful literary works regardless of their pre-history or authorship. In this thesis I do not focus on studies that concern individual narratives or poems but on those that apply a synchronic approach to large units of texts such as books or collections of books. My example is the book of Judges, and the fundamental issue is whether the synchronists’ description of its structure and of the relationship between the individual narratives and the larger text is sustainable. Through analyses of the book’s introduction and the stories about Ehud, Deborah, Jephthah and Samson, I argue that the scholars under consideration are often compelled to form interpretations that are in conflict with a “natural” or “intuitive” reading. I hence claim that they are not reading these stories in accordance with the conventions that are generally applied to narratives. The arguments in which they refer to implicit devices, allusions and the structure of the larger text are assessed as unconvincing.I argue that that these scholars make two common theoretical mistakes. Firstly, they do not consider the specific restrictions that apply to “the literary point of view”. Secondly, they disregard the fact that narratives are autonomous and hence resistant to reworking. If several independent narratives are put together, they are not thereby transformed into a larger single narrative even though they may contain common patterns and motifs. Hence, the individual story represents the primary level of meaning and discrete elements are understood as motifs within aliterary construction. The stories of the book of Judges are therefore texts within a text. This explains why the book lacks a coherent ideology or morality. The tensions and ambiguities in the book cannot be resolved by classifying it as a literary production and studying it synchronically: on the contrary, doing this confirms and explains the difficulties in the book – that is, the inconsistent character of both the book and its narratives and the bizarre events that are recounted therein – and its polyphonic character.

In short, all the algorithms work well and lead to tests with correct or close to correct size. There is thus little or no reason not to use the bootstrap with error component models.
