    • Many healthcare organizations are currently committed to organizational change and quality improvement projects. Unfortunately, little effort is put into health information system development according to usability requirements and methodology, leading to bottlenecks in the eHealth systems when implemented into daily practice. This paper presents the experiences of applying a participatory approach in a nation-wide project. The aim of this national initiative was to build usability of eHealth systems into the Swedish national eHealth strategy and its action plan to ensure impact on practice. Action researchers within the health informatics domain collaborated with representatives of different care professions to propose high-priority changes necessary to improve the usability of health information systems. Reflections from the first phase of this initiative are further explored in terms of methodological challenges and recommendations, whereof one recommendation is to increase the use of HI-U specialists – health informaticians with usability and human work interaction design expertise within national and local eHealth development.

    • Many new public eHealth Services are now being developed. Often a conventional customer-vendor process is used, where the customer is a public authority, e.g. a county council, and the vendor a commercial actor, e.g. an IT development company. In this case study the engineering process regards a novel eHealth service aiming to provide patients with online access to their electronic health record. A complicating factor in conventional customer-vendor processes for public e-services is that "the future user could be anyone". In the light of best practice in Human-Computer Interaction, this study examines the joint effort of the customer and vendor when developing novel services for citizen use. The results include delimiting factors, recommendations for public authority customers and proposed new actions for the research agenda.

    • As many evaluations show, healthcare organizations do not accomplish the intended effects of their eHealth systems due to inadequate usability. On behalf of the Swedish Ministry of Health and Social Affairs, the usability of current eHealth systems in Swedish healthcare have been analysed from the perspective of healthcare and social service professionals. The objective of the study was to report on current problems, potential solutions as well as to relate these to research in relevant areas. Using a participatory approach, seven workshops were held where researchers within health informatics collaborated with staff from different care providers, representatives of the national associations of health and social care professionals and the national eHealth system vendor organization. This paper presents a foundation for further development of eHealth systems, condensed into 10 issues that the Swedish health and social care professionals find imperative to improve. The study emphasizes that the development of eHealth systems is always a matter of organizational and process development and must be integrated into the care practice improvement process. Further, based on the findings, some identified challenges are discussed.
