  • The role of TREM2 R47H as a risk factor for Alzheimer's disease, frontotemporal lobar degeneration, amyotrophic lateral sclerosis, and Parkinson's disease

    • A rare variant in TREM2 (p.R47H, rs75932628) was recently reported to increase the risk of Alzheimer's disease (AD) and, subsequently, other neurodegenerative diseases, i.e. frontotemporal lobar degeneration (FTLD), amyotrophic lateral sclerosis (ALS), and Parkinson's disease (PD). Here we comprehensively assessed TREM2 rs75932628 for association with these diseases in a total of 19,940 previously untyped subjects of European descent. These data were combined with those from 28 published data sets by meta-analysis. Furthermore, we tested whether rs75932628 shows association with amyloid beta (Ab42) and total-tau protein levels in the cerebrospinal fluid (CSF) of 828 individuals with AD or mild cognitive impairment. Our data show that rs75932628 is highly significantly associated with the risk of AD across 24,086 AD cases and 148,993 controls of European descent (odds ratio or OR = 2.71, P = 4.67 x 10(-25)). No consistent evidence for association was found between this marker and the risk of FTLD (OR = 2.24, P = .0113 across 2673 cases/9283 controls), PD (OR 5 1.36, P = .0767 across 8311 cases/79,938 controls) and ALS (OR 5 1.41, P = .198 across 5544 cases/7072 controls). Furthermore, carriers of the rs75932628 risk allele showed significantly increased levels of CSF-total-tau (P = .0110) but not Ab42 suggesting that TREM2's role in AD may involve tau dysfunction.   • Quality of nursing care as perceived by cancer patients : a cross-sectional survey in four European countries

    • Purpose: To explore and compare cancer patients' perceptions on the quality of nursing care in four European countries. Methods: Data were collected in Cyprus, Finland, Greece and Sweden. The sample comprised 596 hospitalized cancer patients. The quality of nursing care was measured using the "Oncology Patients' Perceptions of the Quality of Nursing Care Scale" (OPPQNCS). Patient characteristics were also collected. Analysis of variance was used to examine the effects of country on the perceptions of the quality of nursing care. Results: Patients' age ranged from 18 to 86 years, and 58% were male. The comparison of cancer patients' perceptions regarding the quality of nursing care between the four countries showed a statistically significant difference in the total OPPQNCS scores (p <0.001) as well as in the subscales responsiveness (p <0.001), individualization (p<0.001), co ordination (p<0.001) and proficiency (p<0.001). The Cronbach's alpha coefficient for the OPPQNCS ranged between 0.89 and 0.95. A multivariate analysis of variance for the OPPQNCS controlled by respondents' demographic characteristics revealed that only the patient's country was significantly related with the patients' perceptions of quality care. Conclusion: Quality of nursing care as perceived by cancer patients was high, but differed between the four countries. The impact of the clinical status of cancer patients on the quality of nursing care and managerial factors such as staffing/nursing care delivery models that influence the ability of nurses to offer high quality care should also be explored by more focused studies.

  • Bra liv i Gårdsten: När invånarna själva för råda

    • Denna rapport beskriver ett projekt som drivs av några invånare från Gårdsten och en forskargrupp från Göteborgs Universitet och Västra Götalandsregionen. Målet för projektet är att identifiera vad som gör att man kan leva ett gott och hälsosamt liv i Gårdsten samt att föreslå och initiera förändringar. Vi har vid ett antal träffar pratat om hur vardagen fungerar i Gårdsten, vad som är bra för hälsan och vad som skulle kunna förbättras. I den processen har vi använt en forskningsmetod som innebär att alla ska kunna komma till tals och påverka (GLA – Group Level Assessment). I slutet av varje träff har vi tillsammans bestämt hur vi går vidare och forskargruppen har dokumenterat och sammanställt det som hänt. Bland annat har vi haft en utställning, där vi bjöd in fler invånare och företrädare för myndigheter och organisationer i Gårdsten. Dessa aktiviteter har resulterat i förslag på konkreta insatser. Fler mötesplatser behövs och kommunikationen mellan människorna kan bli mycket bättre. Det har också framkommit att tryggheten och känslan av tillhörighet behöver stärkas. Önskemålen om bättre kommunikation, trygghet och tillhörighet har i sin tur har lett till att några jobbar vidare på att öppna ett språkkafé. En grupp har också tagit initiativ till att bjuda in representanter för hälso- och sjukvården för att prata om munhälsa respektive hälsosam mat. Detta var lätt att ordna eftersom både Folktandvården och Angereds Närsjukhus har sådant utåtriktat arbete som en del av sina uppdrag. Genom projektet tas alltså dessa resurser nu i bruk av invånarna. Slutligen kan man säga om detta projekt att det startat idéer, diskussioner och handlingar som förhoppningsvis kommer att fortsätta bidra till Gårdstens utveckling.

    • Exposure to advertisements cannot fully explain the associations between young children's dietary intake and the time they spend in front of the television. It is therefore of importance to study television content other than advertisements in this aspect. The present study aimed to examine the nature and extent of verbal and visual appearances of foods and beverages in children's television programmes on Icelandic public service television.A total of 27 h of children's programmes (domestic and internationally produced) were watched. All verbal and visual appearances of foods and beverages were coded, as well as the context in which the foods/beverages were discussed or appeared.Children's programmes on Icelandic public service television.Two food groups were of special interest for their importance from a public health perspective: high-calorie and low-nutrient (HCLN) foods and fruits and vegetables (F&V). The χ 2 test and logistic regression were performed to analyse if the occurrence of the two groups was associated with the context where foods/beverages appeared.Of the 125 different programmes, a food or beverage appeared in 86 %. Of the total food appearances (n 599), HCLN foods accounted for 26 % and F&V for 23 %. HCLN foods were presented as desirable by appearing more frequently with child characters (P<0·01) than F&V.Public service television has the potential to improve the way food and eating is presented in children's programmes, as young childhood is a critical period for founding healthy habits for later life.

  • Normal weight adiposity in a Swedish population: how well is cardiovascular risk associated with excess body fat captured by BMI?

    • Objective The aim of this study was to examine how well body mass index (BMI) reflects cardiovascular risk associated with excess adiposity in a Swedish population by examining the association between body fat, BMI and cardiovascular risk factors. Methods A total of 3,010 adults participated. Normal weight adiposity was defined as the combination of BMI<25 kg/m2 and percentage body fat ≥35% for women and ≥25% for men. Associations with blood pressure, blood lipids, apolipoproteins and C-reactive protein were analysed in age-adjusted regression models. Results The majority of the individuals with overweight and obesity were correctly classified to adiposity, while a wide range of body fat was observed among the normal weight subjects. In total, 9% of the participants were categorised as normal weight with adiposity. Compared with the normal weight leanness group, participants with normal weight adiposity had higher levels of serum triglycerides, low-density lipoprotein cholesterol, C-reactive protein, apolipoptotein B and the apolipoprotein B/A-I ratio. In normal weight men, adiposity was also associated with higher blood pressure and lower high-density lipoprotein cholesterol. Conclusions Higher percentage of body fat was associated with less favourable risk factor profile even in subjects who were normal weight. Thus, it might be relevant to screen for metabolic risk factors in the upper end of the normal weight category.

    • I ett samarbete mellan de nordiska länderna har ett protokoll för att kartlägga marknadsföring av ohälsosamma livsmedel till barn och ungdomar utvecklats. Protokollet, som publicerats av Nordiska ministerrådet, beskriver metoder för hur man studerar barns exponering för marknadsföring av livsmedel. Den fokuserar på den typ av mat och dryck som inte bör marknadsföras till barn enligt WHO. Det innebär livsmedel som sannolikt bidrar till en överkonsumtion av energi, socker och salt.

    • Sammantaget visar forskning att stora portioner stimulerar överkonsumtion. Att undvika stora portioner av energitäta livsmedel och sockerrika drycker kan därför göra det enklare att hålla kroppsvikten. Att istället äta stora portioner av mat som inte innehåller så mycket energi per gram kan däremot vara en bra strategi, exempelvis att äta en sallad före maten.

    • This review focuses on the influence of portion size and temporal distribution of food intake on weight balance and obesity in adults. The inconsistency of definitions in the area of meal patterns is also discussed. The conclusion is that regular eating habits might facilitate weight balance, while unplanned snacking as well as consuming the major part of the energy intake at the end of the day seem to be unfavourable. Altogether, the research suggests that large portions promote over-consumption and, therefore, limiting portion size of energy dense foods and drinks with added sugar could be recommended. Even if more research is needed, these factors should be taken into consideration in recommendations for obesity prevention.
