  • The Brief Obsessive-Compulsive Scale (BOCS) : a self-report scale for OCD and obsessive-compulsive related disorders

    • Background: The Brief Obsessive Compulsive Scale (BOCS), derived from the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) and the children's version (CY-BOCS), is a short self-report tool used to aid in the assessment of obsessive-compulsive symptoms and diagnosis of obsessive-compulsive disorder (OCD). It is widely used throughout child, adolescent and adult psychiatry settings in Sweden but has not been validated up to date.Aim: The aim of the current study was to examine the psychometric properties of the BOCS amongst a psychiatric outpatient population.Method: The BOCS consists of a 15-item Symptom Checklist including three items (hoarding, dysmorphophobia and self-harm) related to the DSM-5 category "Obsessive-compulsive related disorders", accompanied by a single six-item Severity Scale for obsessions and compulsions combined. It encompasses the revisions made in the Y-BOCS-II severity scale by including obsessive-compulsive free intervals, extent of avoidance and excluding the resistance item. 402 adult psychiatric outpatients with OCD, attention-deficit/hyperactivity disorder, autism spectrum disorder and other psychiatric disorders completed the BOCS.Results: Principal component factor analysis produced five subscales titled "Symmetry", "Forbidden thoughts", "Contamination", "Magical thoughts" and "Dysmorphic thoughts". The OCD group scored higher than the other diagnostic groups in all subscales (P < 0.001). Sensitivities, specificities and internal consistency for both the Symptom Checklist and the Severity Scale emerged high (Symptom Checklist: sensitivity = 85%, specificities = 62-70% Cronbach's alpha = 0.81; Severity Scale: sensitivity = 72%, specificities = 75-84%, Cronbach's alpha = 0.94).Conclusions: The BOCS has the ability to discriminate OCD from other non-OCD related psychiatric disorders. The current study provides strong support for the utility of the BOCS in the assessment of obsessive-compulsive symptoms in clinical psychiatry.

  • Effect of heat on interspecific competition in saprotrophic wood fungi

    • Some boreal wood fungi that are associated with forest fire or open dry habitats have an increased resistance to heat in comparison to species associated with a less specific distribution or species found in mesic forests. We hypothesize that extreme temperature-stress experienced during fires will favor species adapted to heat and, ultimately, the composition of species inhabiting logs in such habitats will change. Competitiveness after temperature stress was examined in three fire-associated species – Dichomitus squalens, Gloeophyllum sepiarium and Phlebiopsis gigantea – and three non fire-associated species – Ischnoderma benzoinum, Phellinus pini and Fomitopsis pinicola. There was a difference between the fire-associated species and the non fire-associated species with respect to competitive strength after heat stress. All fire-associated species had an advantage after heat treatment, colonizing a larger volume of wood than any non-fire-associated competitor. Our findings suggest that increased heat tolerance of mycelia can exert a competitive balance shift after forest fire. It shows that a system governed by forest fire will be dominance controlled under certain conditions. Furthermore, from a management perspective, during a prescribed burning, certain species already present in the ecosystem will be favored if the fire is not allowed to totally consume the substrates.

    • Forest fires have been the major stand-replacing/modifying disturbance in boreal forests. To adapt to fire disturbance, different strategies have evolved. This thesis focuses on wood fungi, and the effect of forest fire on this organism group. In many ways it is a study on adaptation to forest fire, in concurrence with adaptation to dry open habitats. In Paper I we study increased heat resistance in  mycelia from species prevalent in fire prone environments. Fungi were cultivated on fresh wood and exposed to different temperatures. Species prevalent in fire affected habitats had a much higher survival rate over all combinations of time and temperature compared to species associated with other environments. Based on this results the competitiveness was tested after temperature stress (paper II), three fire associated species, were tested against three non fire associated species. All fire associated species had a clear advantage after heat treatment, conquering a larger volume of wood than its competitor. In paper III we studied the effect of heat shock on decomposition rate, 18 species was tested. Species were cultivated and monitored for CO2 accumulation for 8 weeks and then heat shocked. All species including non fire associated species seemed to up-regulate decomposition after heat shock, this response was more pronounced in fire associated species. To look at the possible effect of forest fire on population structure (Paper IV), we developed 29 SNP/INDELs for Phlebiopsis. gigantea. We amplified the marker containing fragments in 132 individuals of P. gigantea in 6 populations, 3 which were found in areas affected by forest fire and 3 in unaffected areas. We found no genetic structure in accordance to forest fire. However we detected geographic structure, which stands in contrast to earlier studies. This might be due to the method, using SNP´s and number of individuals in the study. Finally we collected cross-sections of decayed logs to evaluate the number of fungal species domains that are likely to be hit when drilling a saw-dust sample in a log.     • Norway spruce (Picea abies) debarking water is an aqueous extract obtained as waste from the debarking of logs at paper mills. The debarking water contains a mixture of natural compounds that can exhibit diverse biological activities, potentially including fungicidal activity on some species of wood-decaying fungi. Thus, we investigated the growth rates of such fungi on agar plates to which debarking water extracts had been added. The experiment included five wood-decaying fungi, viz. Gloeophyllum sepiarium, Oligoporus lateritius, Ischnoderma benzoinum, Junghuhnia luteoalba, and Phlebia sp. Growth reduction was observed for all species at the highest tested concentrations of freeze-dried and ethanol-extracted debarking water, the ethyl acetate-soluble fraction and the diethyl ether-soluble fraction. However, the magnitude of the effect varied between different species and strains of individual species. The brown-rot fungi G. sepiarium and O. lateritius were generally the most sensitive species, with the growth of all tested strains being completely inhibited by the ethyl acetate-soluble fraction. These results indicate that development of antifungal wood-protecting agents from debarking water could potentially be a way to make use of a low-value industrial waste.

  • Meta-analysis of cognitive performance in drug-naive patients with schizophrenia

    • Cognitive deficits represent a significant characteristic of schizophrenia. However, a majority of the clinical studies have been conducted in antipsychotic drug treated patients. Thus, it remains unclear if significant cognitive impairments exist in the absence of medication. This is the first meta-analysis of cognitive findings in drug-na ve patients with schizophrenia. Cognitive data from 23 studies encompassing 1106 patients and 1385 controls published from 1992 to 2013 were included. Tests were to a large extent ordered in cognitive domains according to the Measurement and Treatment Research to Improve Cognition in Schizophrenia (MATRICS) battery. Analysis was performed with STATA using the random-effects model and heterogeneity as well as Egger's publication bias was assessed. Overall the results show that patients performed worse than healthy controls in all cognitive domains with medium to large effect sizes. Verbal memory, speed of processing and working memory were three of the domains with the greatest impairments. The pattern of results is in line with previous meta-analytic findings in antipsychotic treated patients. The present meta-analysis confirms the existence of significant cognitive impairments at the early stage of the illness in the absence of antipsychotic medication.

  • Health-related quality of life and metabolic risk in patients with psychosis

    • Improved Health-related quality of life (HRQoL) is an alternative treatment goal for individuals with psychosis, who have up to two times greater prevalence of type 2 diabetes, hypertension and obesity than the general population. Aim: to compare HRQoL in patients with psychosis, especially schizophrenia, with a reference sample and explore the relationship between HRQoL and metabolic risk factors in these patients. Methods: a prospective cohort study was carried out in specialized psychiatric outpatient departments in Sweden. The patients were invited consecutively. A prospective population-based study of public health in the south-east of Sweden served as reference group. Patients were assessed with psychiatric questionnaires that included Global Assessment of Functioning (GAF). Health-related quality of life was assessed using the questionnaire EQ5D, both for patients and the population, and several other health status outcomes were used. Results: At 73%, schizophrenia and schizoaffective disorder were the most common diagnoses in the patient group. The results in patients (n = 903) and population (n = 7238) showed significant differences in lower EQ5D among patients. According to the definition by the International Diabetes Federation (IDF), elevated blood pressure was the only metabolic risk associated with lower HRQoL in patients. Raised LDL-cholesterol levels were also significantly related to lower HRQoL. Conclusion: patients suffering from psychosis had significantly lower HRQoL regarding all components in EQ5D, except for the pain/discomfort component. Almost half of the patient group met the criteria for metabolic syndrome. According to the IDF criteria, elevated blood pressure was the only metabolic risk factor that had an impact on HRQoL.
