  • Cardiac function in relation to rhythm outcome after intraoperative epicardial left atrial cryoablation.
  • 2011
  • In: Scandinavian cardiovascular journal. - : Informa UK Limited. - 1651-2006 .- 1401-7431. ; 45:6, s. 327-35
  • Journal article (peer-reviewed)abstract
    • Abstract Objectives. To assess the effects of intraoperative left atrial epicardial cryoablation on rhythm and atrial and ventricular function. Design. Thirty five patients with coronary artery disease and documented atrial fibrillation underwent coronary artery bypass surgery and concomitant cryoablation. An age and gender matched control group of 35 patients with atrial fibrillation underwent bypass surgery alone. Echocardiography was performed 9 ? 32 days before and 22 ? 6 months after surgery. Results. The proportion of patients in sinus rhythm at follow-up was 63% and 34% (p = 0.04) in the cryoablation and control groups, respectively. In patients with sinus rhythm both before surgery and at follow-up, the left atrial area increased (p = 0.002) and the mitral annular excursion during atrial contraction decreased (p = 0.01) after cryoablation. The mitral flow velocity during atrial systole decreased after cryoablation (p = 0.002). The LV diameter increased (p = 0.03) and the left ventricular ejection fraction (LVEF) decreased (p = 0.03) in cryoablated but not in control patients. Continued deterioration was seen in patients with atrial fibrillation both pre- and postoperatively. Conclusions. At long-term follow-up, a significantly higher proportion of patients was in sinus rhythm in the cryoablation than in the control group. The atrial and ventricular function had decreased at follow-up two years after surgery. This decrease was small and occurred within or close to the reference values in patients with sinus rhythm at follow-up, while patients remaining in atrial fibrillation showed a significant continued deterioration. Some subgroups were small, and the findings, although statistically significant, should be interpreted with caution.

  • Perceptions of Malawian Nurses about Nursing Interventions for Malnourished Children and Their Parents
  • 2011
  • In: Journal of Health, Population and Nutrition. - : Icddr:b. - 1606-0997 .- 2072-1315. ; 29:6, s. 612-618
  • Journal article (peer-reviewed)abstract
    • in developing countries, malnutrition among children is a major public-health issue. The aim of the study was to describe perceptions of Malawian nurses about nursing interventions for malnourished children and their parents. A qualitative method was used. Data were collected and analyzed according to the phenomenographic research approach. Twelve interviews were performed with 12 nurses at a rural hospital in northern Malawi, Southeast Africa. Through the analysis, two major concepts, comprising four categories of description, emerged: managing malnutrition today and promotion of a favourable nutritional status. The categories of description involved identification and treatment of malnutrition, education during treatment, education during prevention, and assurance of food security. The participating nurses perceived education to be the most important intervention, incorporated in all areas of prevention and treatment of malnutrition. Identification and treatment of malnutrition, education during treatment, education to prevent malnutrition, and assurance of food security were regarded as the most important areas of intervention.

  • Öppna prioriteringar inom nya områden : logopedi, nutritionsbedömning, habilitering och arbetsterapi
  • 2011
  • Reports (other academic/artistic)abstract

  • The Riks-Stroke story : building a sustainable national register for quality assessment of stroke care
  • 2011
  • In: International Journal of Stroke. - : SAGE Publications. - 1747-4930 .- 1747-4949. ; 6:2, s. 99-108
  • Journal article (peer-reviewed)abstract
    • Background Riks-Stroke, the Swedish Stroke Register, is the world's longest-running national stroke quality register (established in 1994) and includes all 76 hospitals in Sweden admitting acute stroke patients. The development and maintenance of this sustainable national register is described. Methods Riks-Stroke includes information on the quality of care during the acute phase, rehabilitation and secondary prevention of stroke, as well as data on community support. Riks-Stroke is unique among stroke quality registers in that patients are followed during the first year after stroke. The data collected describe processes, and medical and patient-reported outcome measurements. The register embraces most of the dimensions of health-care quality (evidence-based, safe, provided in time, distributed fairly and patient oriented). Result Annually, approximately 25 000 patients are included. In 2009, approximately 320 000 patients had been accumulated (mean age 76-years). The register is estimated to cover 82% of all stroke patients treated in Swedish hospitals. Among critical issues when building a national stroke quality register, the delicate balance between simplicity and comprehensiveness is emphasised. Future developments include direct transfer of data from digital medical records to Riks-Stroke and comprehensive strategies to use the information collected to rapidly implement new evidence-based techniques and to eliminate outdated methods in stroke care. Conclusions It is possible to establish a sustainable quality register for stroke at the national level covering all hospitals admitting acute stroke patients. Riks-Stroke is fulfilling its main goals to support continuous quality improvement of Swedish stroke services and serve as an instrument for following up national stroke guidelines.

  • Effects of androgen and leptin on behavioral and cellular responses in female rats.
  • 2011
  • In: Hormones and behavior. - : Elsevier BV. - 1095-6867 .- 0018-506X. ; 60:4, s. 427-38
  • Journal article (peer-reviewed)abstract
    • The causes of anxiety and depression in women with polycystic ovary syndrome (PCOS) remain elusive. To identify steps linking androgen signaling to the regulation of affective symptoms in vivo, we compared behavioral responses in female rats continuously exposed to DHT from puberty (a model of DHT-induced PCOS) and in rats exposed to DHT for 1week. Continuous and 1week of DHT exposure resulted in a general decrease in locomotor activity and time spent on the open arms in the elevated plus maze, indicating anxiety-like behavior. Rats with DHT-induced PCOS have increases in adiposity and circulating leptin levels accompanied by leptin resistance. One week of DHT exposure decreased androgen receptor (AR) expression in the hypothalamus and leptin synthesis and function in adipocytes; it also inhibited signal transducer and activator of transcription 3 (STAT3) and attenuated leptin activity by increasing levels of soluble leptin receptor, a leptin-binding protein, in the hypothalamus. This may affect the androgen-induced anxiety-related behavior in female rats. In conclusion, our results highlight the central role of androgens in behavioral function in female rats and suggest that androgens directly regulate the AR by decreasing its hypothalamic expression. Androgens also increase leptin synthesis in adipocytes, which drives central leptin signaling, and may regulate anxiety-related behaviors. Elucidating mechanisms by which androgens modulate female anxiety-like behavior may uncover useful approaches for treating women with PCOS who have symptoms of anxiety.

  • Insändare i Kamratposten : Uttryck för villkor i barns kulturella sammanhang

  • Doctoral thesis (other academic/artistic)abstract
    • The aim of this thesis is to elevate children's conditions as they are expressed in the specific genre of letters to the editor column. The empirical material consists of 1054 child writer contributions in the children's magazine 'Kamratposten' 1977-2007. In the thesis the letters to the editor column is considered to constitute a social place wherein a meeting occurs between ideas, norms and values from children's culture and culture for children, creating conditions for how the children writers express themselves. It concerns conditions the children experience, interpret and reproduce from their daily lives, as well as conditions formed by different editors in a magazine for children. Drawing on a theoretical framework of semiotics and culture pedagogy in combination with a methodological approach of hermeneutics and abduction, the concepts 'develomental task' and 'affordance' were initially used to interpret data. During the interpreting process two main phenomena have been conceptualised as prominent conditions in children's everyday life: Belonging and Place. The abductive method used in the study revealed how the dialectics of belonging and place as conditional phenomena create feelings of discomfort, as well as feelings of pleasure. The results show that these feelings are mainly due to a sense of belonging in families, peer groups and romantic relations. The results also illustrate that belonging and place concerns more than being part of different social and cultural settings. It concerns feelings of real participation and involvment, of taking part, in family life and social interaction with peers, girlfriends and boyfriends. The children's written contributions reveal that belonging and place are issues of vital democratic quality in their daily life and world.
