    • The Cherenkov Telescope Array (CTA) is the major next-generation observatory for ground-based veryhigh-energy gamma-ray astronomy. It will improve the sensitivity of current ground-based instruments by a factor of five to twenty, depending on the energy, greatly improving both their angular and energy resolutions over four decades in energy (from 20 GeV to 300 TeV). This achievement will be possible by using tens of imaging Cherenkov telescopes of three successive sizes. They will be arranged into two arrays, one per hemisphere, located on the La Palma island (Spain) and in Paranal (Chile). We present here the optimised and final telescope arrays for both CTA sites, as well as their foreseen performance, resulting from the analysis of three different large-scale Monte Carlo productions.

    • We use results of our 3 yr polarimetric monitoring programme to investigate the previously suggested connection between rotations of the polarization plane in the optical emission of blazars and their gamma-ray flares in the GeV band. The homogeneous set of 40 rotation events in 24 sources detected by RoboPol is analysed together with the gamma-ray data provided by Fermi-LAT. We confirm that polarization plane rotations are indeed related to the closest gamma-ray flares in blazars and the time lags between these events are consistent with zero. Amplitudes of the rotations are anticorrelated with amplitudes of the gamma-ray flares. This is presumably caused by higher relativistic boosting (higher Doppler factors) in blazars that exhibit smaller amplitude polarization plane rotations. Moreover, the time-scales of rotations and flares are marginally correlated.

    • Think tanks, or policy institutes, are becoming significant ‘sites of normativity’ on the global political scene. While their primary concern often is to provide knowledge, based on which decision makers can make informed choices, they also play a part in setting organizational agendas and priorities, and in mobilizing for political action. Based on ethnographic fieldwork in think tanks in Washington DC, the paper engages with the modes representation used by policy experts as they strive to get traction and establish credibility for their ideas. The work of policy experts can be understood as a form of ‘bricolage,’ in which information and normative perspectives are tinkered with and are thus afforded truth-value. The use of distanciation and proximation techniques facilitates the continuous scale-making processes in which policy experts are involved.

    • Objectives To study the prevalence of antibodies of IgA class against tissue transglutaminase (tTG), endomysium (EMA) and gliadin (AGA) in patients with chronic idiopathic axonal polyneuropathy (CIAP) and to characterize the patients clinically and neurophysiologically. Methods Of 182 patients, 126 patients agreed to blood sampling. Sera were analysed by ELISAs detecting anti-tTG and AGA, whereas EMA was analysed by indirect immunofluorescence (IF) microscopy. Gastrointestinal symptoms were assessed by data from medical records and patient interviews. Results Nine of 126 patients (7%) were seropositive in at least one test (five with positive anti-tTG and/or EMA and four with positive AGA only). One patient with elevated levels of all specificities had laboratory signs of malabsorption and gastrointestinal complaints with abdominal pain and diarrhoea. Conclusions Elevated levels of IgA-AGA were slightly more frequent in patients with CIAP (4%) compared to 2.5% in 1866 healthy blood donors. Highly specific serological markers indicative of coeliac disease (CD) (anti-tTG and EMA) were somewhat more common in our patients with CIAP (4%) than expected from normal reference values and from studies of the prevalence of CD in the general population. Even though these findings may indicate a relationship, the aetiological importance is unclear.

  • Oral Appliances in the Treatment of Obstructive Sleep Apnea and Snoring

    • This overview summarizes current knowledge of effects, side effects and indications for oral appliances in the treatment of patients with obstructive sleep apnea (OSA) and snoring. Mandibular repositioning appliances (MRAs) in various designs represent the most evaluated type of oral appliance. MRAs relocate the lower jaw forward and downward in order to prevent sleep-induced pharyngeal collapse. Short-term success rates with MRAs of over 80% of selected OSA patients have been reported, but figures vary. MRAs are indicated for patients with mild to moderate OSA, men with supine-dependent OSA and women. Patients with initial treatment success may benefit from MRAs over a long time, provided that they do not gain weight and wear high quality devices. There are few randomized controlled trials of treatment effects from MRAs on OSA, particularly regarding symptoms. MRA design, methodology and the teamwork between dentists and physicians has an influence on the treatment outcome. Continuous positive airway presssure is the primary treatment for patients with more severe OSA because of its superior effects compared with MRAs. Another drawback of MRAs is their dependence on healthy teeth and the risk of dental side effects. It is concluded that MRAs are indicated for selected patients with OSA, but more research is needed.
