  • Estimation of the Severeness Rate, Death Rate, Household Attack Rate and the Total Number of COVID-19 Cases Based on 16 115 Polish Surveillance Records

    • Background: Estimating the actual number of COVID-19 infections is crucial for steering through the COVID-19 pandemic crisis. It is, however, notoriously difficult, as many cases have no or only mild symptoms. Surveillance data for in-household secondary infections offers unbiased samples for COVID-19 prevalence estimation.Methods: We analyse 16 115 Polish surveillance records to obtain key figures of the COVID-19 pandemic. We propose conservative upper and lower bound estimators for the number of SARS-CoV-2 infections. Further, we estimate age-dependent bounds on the severe case rate, death rate, and the in-household attack rate.Results: By maximum likelihood estimates, the total number of COVID-19 cases in Poland as of July 22nd, 2020, is at most around 13 times larger and at least 1.6 times larger than the recorded number. The lower bound on the severeness rate ranges between 0.2% for the 0–39 year-old to 5.7% for older than 80, while the upper bound is between 2.6% and 34.1%. The lower bound on the death rate is between 0.04% for the age group 40–59 to 1.34% for the oldest. Overall, the severeness and death rates grow exponentially with age. The in-household attack ratio is 8.18% for the youngest group and 16.88% for the oldest.Conclusions: The proposed approach derives highly relevant figures on the COVID-19 pandemic from routine surveillance data, under assumption that household members of detected infected are tested and all severe cases are diagnosed.

  • Occurrence and predictors of obstructive sleep apnea in a revascularized coronary artery disease cohort

    • Background: Knowledge about the prevalence of obstructive sleep apnea (OSA) in coronary artery disease (CAD) is insufficient. The aim of the current report was to evaluate the occurrence and predictors of OSA among revascularized patients with CAD within the framework of a randomized controlled trial (Randomized Intervention with CPAP in Coronary Artery Disease and Sleep Apnea [RICCADSA]), evaluating the impact of continuous positive airway pressure on cardiovascular outcomes in CAD patients with OSA. Material and Methods: All patients undergoing percutaneous coronary intervention or coronary artery bypass grafting between September 2005 and November 2010 (n = 1,291) were invited to participate. Anthropometrics and medical history were obtained, ambulatory sleep recording was performed, and all subjects completed the Epworth Sleepiness Scale (ESS) questionnaire. Results: In total, 662 patients participated in the sleep study. OSA, defined as an apnea–hypopnea index equal to or greater than 15/hour, was found among 422 (63.7%). The prevalence of hypertension was 55.9%; obesity (body mass index ≥ 30 kg/m2), 25.2%; diabetes mellitus, 22.1%; and current smoking, 18.9%. The patients with CAD who did not participate in the study demonstrated an almost similar anthropometric and clinical profile compared with the studied group. The majority (61.8%) of the patients with OSA were nonsleepy (ESS score < 10). Patients with OSA had a higher prevalence of obesity, hypertension, diabetes mellitus, and history of atrial fibrillation, whereas current smoking was more common in the non-OSA group. Age, male sex, body mass index, and ESS score, but not comorbidities, were independent predictors of OSA. Conclusions: The occurrence of unrecognized OSA in this revascularized CAD cohort was higher than previously reported. We suggest that OSA should be considered in the secondary prevention protocols in CAD.     • In most software development companies the road mapping and requirements prioritization process is a complex process in which product management experiences difficulties in getting timely and accurate customer feedback. The feedback loop from customers is slow and often there is a lack of mechanisms that allow for efficient customer data collection and analysis. As a result, there is the risk that requirements prioritization becomes opinion-based rather than data-driven, and that R&D investments are made without an accurate way of continuously validating whether they correspond to customer needs. We call this phenomenon the 'open loop' problem, referring to the challenges for product management to get accurate and timely feedback from customers. To address this problem, we develop the HYPEX model (Hypothesis Experiment Data-Driven Development) that supports companies in running feature experiments to shorten customer feedback loops. We evaluate the model in three software development companies and observe how feature experiments increase the opportunity for data-driven software development.

    • Global Enterprises today are dependent on performance within complex networks. To become effective and efficient, it is more important than ever to organize operations in order to capitalize on resources and to integrate different activities. This means that the supply chain and logistics have become core activities in international enterprises. Transportation has also become an issue based on the most recent ICC and UN reports where the concerns for CO2 emissions have been highlighted. Thus the efficient and effective handling and transportation of goods become central when a company has a policy of buying products from any country and company that produce the best products available, while at the same time needing to deliver the final product all over the world. Packaging is often considered part of logistics: still there is a gap between packaging know-how and logistics know-how both at the company and research levels. There is also a discrepancy between the educational level among personnel in packaging and logistics in most production companies. To address this issue and investigate if there is industrial evidence that packaging and logistics are interrelated, the objective of the research presented in this thesis has been to explore the potential! To develop the packaging logistics system’s perspective along with market driven demands on changes in production. The studies have been carried out at Volvo Cars, where 1 work. This is why it has been natural to use an actor’s approach together with a case study methodology. Based on data collected over more than 20 years, 1 wanted to confront the work carried out and experiences collected in a company with the research carried out at the university involving systematic analyzes. 1 wanted to do this to see if the academic perceptions developed and conclusions drawn were real and ready for application in a wider perspective when new policies are initiated to optimize the supply chain to meet environmental challenges. Four studies were carried out based on the data collected and reported in four articles submitted to international conferences. These studies show that packaging ought to be considered a critical activity closely linked to logistics and considered as company core business. This must be understood when outsourcing decisions are taken. if a company does not see packaging as part of its core competence, it will lose the possibility to control inbound logistics as well as production logistics. All products need packaging in different stages in the supply network; packaging producers deliver into a core” network. Thus they are seldom an integrated actor of the core supply network. Consequently, the more the package and product development can be functionally combined, and integrated to the supply chain system, the greater the possibility of being competitive. This is accepted theoretically, however, industrial consequences are not yet highlighted since empirical knowledge about packaging effects on the entire production and supply network is limited. in addition, the studies show that proper consideration of packaging and logistics in product design makes it possible to influence product costs, and that logistics people with knowledge of packaging need to be engaged early in any new design project. It is not only the specific packaging and logistics knowledge that is required. The study carried out concerning learning during the process in which 1 was involved identified in retrospect that there had been an evolution towards a learning organization. Thus it is recommended to make competence build-up an integral part of future internal! and external educational programs. The multidisciplinary competence established should also be considered when teams are set up for new projects. The thesis describes also the leading role Volvo took in the development of the supply chain thus building a process of great national importance for industrial development.

    • Iron deficiency without anemia and iron deficiency anemia are common and frequently overlooked complications of inflammatory bowel disease. Despite the frequency and impact of iron deficiency in inflammatory bowel disease, there are gaps in our understanding about its incidence, prevalence and natural history and, consequently, patients may be undertreated. Medical registries have a key role in collecting data on the disease's natural history, the safety and effectiveness of drugs in routine clinical practice, and the quality of care delivered by healthcare services. Even though iron deficiency impacts inflammatory bowel disease patients and healthcare systems substantially, none of the established European inflammatory bowel disease registries systematically collects information on iron parameters and related outcomes. Collection of robust iron parameter data from patient registries is one way to heighten awareness about the importance of iron deficiency in this disease and to generate data to improve the quality of patient care, patient outcomes, and thus quality of life. This objective could be achieved through collection of specific laboratory, clinical, and patient-reported measurements that could be incorporated into existing registries. This review describes the status of current European inflammatory bowel disease registries and the data they generate, in order to highlight their potential role in collecting iron data, to discuss how such information gathering could contribute to our understanding of iron deficiency anemia, and to provide practical information in regard to the incorporation of accumulated iron parameter data into registries.

    • Shifting from a dedicated or flexible manufacturing system to a reconfigurable manufacturing system (RMS) requires a significant amount of time, money, and effort. Therefore, it is vital to verify beforehand that the potential reconfigurable solution will be able to achieve the organizational objectives. Discrete event simulation offers the opportunity of assessing several reconfigurable alternatives against the set objectives. This study signifies the importance of using discrete-event simulation as a tool to verify several reconfiguration options. An industrial case example has been presented in the study to elaborate on the role of discrete event simulation in the implementation methodology of RMSs. The study concluded that discrete event simulation is one of the important tools to consider in the RMS implementation methodology.

  • Building Timing Predictable Embedded Systems

    • A large class of embedded systems is distinguished from general purpose computing systems by the need to satisfy strict requirements on timing, often under constraints on available resources. Predictable system design is concerned with the challenge of building systems for which timing requirements can be guaranteed a priori. Perhaps paradoxically, this problem has become more difficult by the introduction of performance-enhancing architectural elements, such as caches, pipelines, and multithreading, which introduce a large degree of nondeterminism and make guarantees harder to provide. The intention of this paper is to summarize current state-of-the-art in research concerning how to build predictable yet performant systems. We suggest precise definitions for the concept of “predictability”, and present predictability concerns at different abstractions levels in embedded software design. First, we consider timing predictability of processor instruction sets. Thereafter, We consider how programming languages can be equipped with predictable timing semantics, covering both a language-based approach based on the synchronous paradigm, as well as an environment that provides timing semantics for a mainstream programming language (in this case C). We present techniques for achieving timing predictability on multicores. Finally we discuss how to handle predictability at the level of networked embedded systems, where randomly occurring errors must be considered.
