  • Dynamic Fascial Closure With Vacuum-Assisted Wound Closure and Mesh-Mediated Fascial Traction (VAWCM) Treatment of the Open Abdomen—An Updated Systematic Review

    • Introduction: Several different temporary abdominal closure techniques are described in the context of open abdomen treatment. Techniques based on dynamic fascial closure combined with negative pressure therapy have gained popularity and seem to result in the highest fascial closure rates without increased complications and are highlighted in recent guidelines and recommendations. One dynamic closure technique is the vacuum-assisted wound closure and mesh-mediated fascial traction (VAWCM) technique, first described in 2007. The aim of this systematic review was to evaluate the VAWCM technique regarding a number of short- and long-term results. Materials and Methods: A systematic literature search was performed in PubMed, EMBASE, and Cochrane Library databases for articles published between January 1, 2006 and May 8, 2020. The review was independently performed by the two authors according to the PRISMA statements for reporting systematic reviews and meta-analyses. Results were pooled for presentation of weighted means when applicable. Results: A total of 220 articles were screened by title and abstract. Thirty-two articles were assessed for eligibility by full-text review and 15 articles finally remained for review. A total of 600 patients treated with VAWCM were included. The pooled weighted means were as follows: fascial closure, 83.5%; enteroatmospheric fistula, 5.6%; planned ventral hernia, 6.2%; in-hospital survival, 72%; and incisional hernia incidence, 40.5%. Long-term survival ranged between 22 and 72%. Quality of life (SF-36) was reported in two studies showing lower scores than the population mean especially in physical domains. Incisional hernia resulted in lower scores in one but not in the other study. Discussion: The results of 600 VAWCM-treated patients from 15 studies were evaluated in this systematic review. Earlier findings with high fascial closure rates, low enteroatmospheric fistula, and planned ventral hernia rates as well as high incisional hernia incidences were underlined. Permanent mesh for efficient fascial traction and reinforcement at fascial closure seem to be the next step in evolving an optimal temporary closure technique in open abdomen treatment.

  • Pre-commercial thinning in Norway spruce-birch mixed stands can provide abundant forage for ungulates without losing volume production

    • Mixed stands of Norway spruce and birch have the potential to simultaneously produce timber and provide large ungulates with a significant amount of forage during the regeneration phase. While the growth and yield of such mixtures are well studied, little is known about potential trade-offs between timber and forage production and which management techniques are suitable for meeting both goals. In this study, four different pre-commercial thinning (PCT) strategies were used to study the trade-offs between production and available forage for free-ranging ungulates in a Norway spruce-birch mixture. The four PCT strategies were: 1) retaining 2000 birch stems ha(-1) with 2000 Norway spruce ha(-1), 2) removing all birches within a 0.75 m radius around Norway spruce stems, 3) removing all birches and other broadleaves, and 4) no PCT (control). Growth of Norway spruce was higher in the 2000 birch ha(-1) and full removal treatments compared to the untreated control, but these two treatments did not differ from one another in volume production of Norway spruce. We found a negative effect of PCT on forage availability but no effect on ungulate browsing. Therefore, PCT strategies that provide both sufficient birch forage and maximize volume production of Norway spruce can be implemented.

    • I mars 2022 levererade Trafikverket och samverkande myndigheter (Transportstyrelsen, Sjöfartsverket, Luftfartsverket, Trafikanalys och VTI, Statens väg- och transportforskningsinstitut) en rapport som sammanfattade den utforskande utvecklingen att utreda en grunddatadomän för transportsystemet.  Föreliggande rapport är en komplettering till den ursprungliga rapporten och utgör Trafikverkets inklusive samverkande myndigheters sammantagna underlag (utforskande utveckling) för etablering av en grunddatadomän för transportsystemet.     • Utställningen Ett bibliotek i takt med tiden: Linköpings universitetsbibliotek under 50 år skildrade bibliotekets historia och biblioteksteknikens utveckling från 1970-talet till idag.Utställningen fokuserade på olika teman som varit signifikativa för Linköpings universitetsbibliotek, som etablerades 1969 som Linköpings högskolas bibliotek. Dessa teman var den så kallade Linköpingsmodellen och dess kvartersbibliotek, försöksverksamheten med bibliotekssystemet Libris i början av 1970-talet, de första databaserna för att söka vetenskapliga publikationer, högskolereformen 1977 och dess betydelse för universitetsbiblioteket, tillkomsten av Hälsouniversitetets bibliotek 1986, datoriseringen under 1980-talet, användarundervisningens utveckling, tillkomsten av det elektroniska förlaget LiU E-Press 1996, skapande av Campus Norrköpings bibliotek 1998, den tilltagande digitaliseringen under 2000-talet, bibliometrins ökande betydelse samt utvecklingen under 2010-talet med bland annat byte av klassifikationssystem, flyttar och sammanslagningar av bibliotek.

  • Pain with sexual activity at 1 and 3 years : Comparing total extraperitoneal with Lichtenstein inguinal hernia repair in a randomized setting (TEPLICH trial)

    • Background: Pain at sexual activity induced by an inguinal hernia or as a cause of its repair is not thoroughly studied. Reported frequencies are between 25% and 30% preoperatively and 10% and 15% postoperatively. The primary aim was to analyze pain at sexual activity at 1 year comparing total extraperitoneal with Lichtenstein repair in a randomized setting. Methods: Men, 30 to 60 years old, with a primary inguinal hernia were randomized to total extraperitoneal without mesh fixation or Lichtenstein repair. A questionnaire on pain prevalence, frequency, intensity, and impairment of sexual functions caused by pain at sexual activity was introduced. Clinical examination, a questionnaire on sexual function, and the 36-Item Short Form Survey were performed preoperatively and at 1 and 3 years postoperatively. A risk factor analysis for pain at sexual activity as performed. Results: A total of 243 patients (111 total extraperitoneal and 132 Lichtenstein) were included between 2008 and 2014; 97% remained for 1-year and 90% for 3-year analysis. Preoperative pain at sexual activity was reported in 35%, with a reduction to 5.9% in total extraperitoneal and 12.5% in Lichtenstein (P = .098) at 1 year and 7.0% in total extraperitoneal and 9.3% in Lichtenstein (P = .566) at 3 years. Quality of life preoperatively was markedly reduced in patients with pain at sexual activity but restored almost to norm levels at 1 and 3 years. New pain at sexual activity (harm) was seen in 8 patients (3.6%) at 1 year. Risk factors for having postoperative pain at sexual activity were preoperative pain and Lichtenstein technique. Conclusion: Pain at sexual activity in inguinal hernia patients is more common than suspected and reduces quality of life. Repair will markedly reduce pain at sexual activity and restore quality of life in most patients without difference between techniques. Patients should be informed on a potential relation between having an inguinal hernia and sexual impairment.

    • Utställningen ”Medborgarskap & digitalisering” gestaltar i olika media de digitala utmaningar vi som medborgare möter i vardagen och har hämtat inspiration från forskning som bedrivs vid LiU av forskargruppen DINO. DINO - Digitalisering i nya offentligheter vid Institutionen för ekonomisk och industriell utveckling (IEI) - studerar hur digitalisering av offentliga tjänster påverkar oss som samhällsmedborgare. Forskargruppen undersöker digitaliseringens effekter på det offentliga samtalet och hur offentliga tjänster görs tillgängliga på nya sätt. En viktig del av deras forskning är också att studera vilka som riskerar att hamna utanför och varför. I många fall blir folkbibliotek ett slags stödfunktion i vardagliga situationer när teknik inte fungerar eller inte är tillgänglig så att medborgare kan agera som aktiva aktörer i samhället. DINO:s studier av de frågor om digitala problem som besökare på biblioteken kommer med har visat att dessa problem påverkar människors möjligheter att agera som medborgare. Det kan till exempel handla om att hantera olika e-tjänster, såsom att boka en tid på vårdcentralen, att betala räkningar, att söka jobb, eller att hantera sitt busskort för att åka kollektivtrafik. Med utgångspunkt i denna forskning presenterar utställningen olika situationer och händelser kopplade till vardagens digitala utmaningar.

  • Galectin-9 Signaling Drives Breast Cancer Invasion through Extracellular Matrix

    • Aberrations in glycan and lectin expression and function represent one of the earliest hallmarks of cancer. Among galectins, a conserved family of β-galactoside-binding lectins, the role of Galectin-9 in immune-tumor interactions is well-established, although its effect on cancer cell behavior remains unclear. In this study, we assayed for, and observed, an association between Galectin-9 expression and invasiveness of breast cancer cells in vitro and in vivo. Genetic perturbation and pharmacological inhibition using novel cognate inhibitors confirmed a positive correlation between Galectin-9 levels and the adhesion of invasive cancer cells to─and their invasion through─constituted organomimetic extracellular matrix microenvironments. Signaling experiments and unbiased quantitative proteomics revealed Galectin-9 induction of Focal Adhesion Kinase activity and S100A4 expression, respectively. FAK inhibition decreased S100A4 mRNA levels. Our results provide crucial insights into how elevated Galectin-9 expression potentiates the invasiveness of breast cancer cells during early steps of invasion.

  • The robustness of the Lu-Hf and Sm-Nd isotopic systems during metamorphism – And case study of the Åker metabasite in southern Sweden

    • While whole-rock Lu-Hf isotope analysis remains one of the only ways to obtain initial Hf isotope signatures of old mafic rocks, Hf isotope analyses of more robust accessory zircon in intermediate to silicic rocks have largely replaced whole-rock analyses during the last decade. This has led to a discrepancy in the amount of existing data from mafic and felsic lithologies. However, especially in mafic, Si-poor rocks with a metamorphic imprint, Hf isotope data rely on whole-rock analysis since baddeleyite, commonly used for U-Pb age analyses of mafic rocks, is sensitive to alteration and metamorphism. Hence, to accurately evaluate the trace element and isotope signatures of altered mafic rocks, it is important to understand the mechanisms of element mobility during metamorphism. Here, we report whole-rock trace element compositions, Lu-Hf and Sm-Nd isotope data from variably deformed and metamorphosed samples of a mafic intrusion in southern Sweden, the Åker metabasite. These data suggest that trace elements were undisturbed on a whole-rock sample scale during deformation at upper amphibolite facies (at least 1000 MPa and 600 °C) metamorphism under hydrated conditions. Despite redistribution of Zr associated with the breakdown of baddeleyite and other igneous phases, the Åker metabasite has retained its chemical and isotopic integrity since igneous crystallisation at ca. 1565 Ma. This study demonstrates and strengthens the feasibility of whole-rock analyses of (meta-)mafic rocks for determining initial εNd and εHf values, despite deformation and metamorphism under hydrated amphibolite-grade metamorphic conditions. Testing the coherence of the calculated initial Nd and Hf isotope ratios by examining variably deformed and metamorphosed varieties of a rock in a single outcrop, could be used as a model for research on more complex Archean rocks.

  • The open abdomen in trauma, acute care, and vascular and endovascular surgery : comprehensive, expert, narrative review

    • BackgroundThe open abdomen is an innovation that greatly improved surgical understanding of damage control, temporary abdominal closure, staged abdominal reconstruction, viscera and enteric fistula care, and abdominal wall reconstruction. This article provides an evidence-informed, expert, comprehensive narrative review of the open abdomen in trauma, acute care, and vascular and endovascular surgery.MethodsA group of 12 international trauma, acute care, and vascular and endovascular surgery experts were invited to review current literature and important concepts surrounding the open abdomen.ResultsThe open abdomen may be classified using validated systems developed by a working group in 2009 and modified by the World Society of the Abdominal Compartment Syndrome-The Abdominal Compartment Society in 2013. It may be indicated in major trauma, intra-abdominal sepsis, vascular surgical emergencies, and severe acute pancreatitis; to facilitate second look laparotomy or avoid or treat abdominal compartment syndrome; and when the abdominal wall cannot be safely closed. Temporary abdominal closure and staged abdominal reconstruction methods include a mesh/sheet, transabdominal wall dynamic fascial traction, negative pressure wound therapy, and hybrid negative pressure wound therapy and dynamic fascial traction. This last method likely has the highest primary fascial closure rates. Direct peritoneal resuscitation is currently an experimental strategy developed to improve primary fascial closure rates and reduce complications in those with an open abdomen. Primary fascial closure rates may be improved by early return to the operating room; limiting use of crystalloid fluids during the surgical interval; and preventing and/or treating intra-abdominal hypertension, enteric fistulae, and intra-abdominal collections after surgery. The majority of failures of primary fascial closure and enteroatmospheric fistula formation may be prevented using effective temporary abdominal closure techniques, providing appropriate resuscitation fluids and nutritional support, and closing the abdomen as early as possible.ConclusionSubsequent stages of the innovation of the open abdomen will likely involve the design and conduct of prospective studies to evaluate appropriate indications for its use and effectiveness and safety of the above components of open abdomen management.
