  • Hypercoagulability in acute bleeding peptic ulcer disease assessed by thrombin-antithrombin III concentrations.

    • OBJECTIVE: To find out whether patients with acute bleeding peptic ulcers have hypercoagulable blood.DESIGN: Prospective open study.SETTING: District hospital in Sweden.SUBJECTS: 54 consecutive patients with duodenal ulcer (n = 25) and gastric ulcer (n = 29) admitted with haematemesis or melaena, or both.INTERVENTIONS: Diagnosis verified on admission by endoscopy, and healing was examined at follow up. Consumption of non-steroidal anti-inflammatory drugs, and smoking habits, were recorded.MAIN OUTCOME MEASURES: Coagulation of the blood monitored by concentrations of plasma thrombin-antithrombin III (TAT) complex in samples obtained on admission for the acute bleeding episode and at follow up 1-2 months later.RESULTS: The plasma TAT complex concentrations were raised during the acute bleeding episode (p < 0.001) compared with an age matched reference group. The TAT complex concentrations measured at follow up had returned to the reference range in all patients, whether or not their ulcers had healed.CONCLUSION: The results indicate that patients with acute haemorrhage from peptic ulcers have hypercoagulable blood during the acute episode.

  • Time course of clotting and fibrinolytic markers in acute upper gastrointestinal bleeding : relation to diagnosis and blood transfusion treatment.

    • One hundred consecutive patients with acute upper gastrointestinal bleeding were investigated. Blood coagulation and fibrinolytic activity were monitored by levels of plasma thrombin-antithrombin III (TAT) complex and plasmin-alpha 2-antiplasmin (PAP) complex in samples obtained from patients at admission with haematemesis and/or melana and in samples obtained from patients the first day after admission. Blood was transfused according to a restrictive policy. Median plasma TAT complex was significantly elevated both at admission and on the first day after admission compared with a reference group. Plasma PAP complex levels were normal at admission but decreased on the first day after admission. This decrease was independent of blood transfusion. The results indicate hypercoagulability at admission among patients with upper gastrointestinal haemorrhage reinforced by the development of a hypofibrinolytic state during the first day after admission. Restricted blood transfusion was not associated with any detectable change in blood coagulation or fibrinolysis in these patients.

    • Upper gastrointestinal haemorrhage in 978 unselected patients during a 9-year period was reviewed in regard to incidence, diagnosis, treatment and mortality rate. The annual incidence was 1/1,000 of the catchment population. The source of bleeding was established in 89% of cases. Peptic ulcer accounted for 53% of total admissions. Blood transfusion was given only when the haemoglobin was less than 8.0 g/dl or if the patient was in shock. Emergency surgery was performed on only 31 patients (3%). The perioperative mortality was 13% and the overall mortality 6%. It is concluded that current treatment policy resulted in low rates of operation and mortality in these unselected cases of upper gastrointestinal haemorrhage. The results support the theory that a hypercoagulable state in gastrointestinal bleeding is counteracted by citrated stored blood.

  • Glycolipid- and glycoprotein-based blood group A antigen expression in human thrombocytes. A1/A2 difference.

    • Total non-acid glycolipid fractions and total sodium dodecylsulphate (SDS) solubilized protein fractions were isolated from human thrombocytes obtained from single human donors having different blood group A1/A2 phenotypes. The blood group A glycolipid antigens were characterized by immunostaining of thin layer plates with different monoclonal anti-A antibodies. The glycoproteins carrying blood group A epitopes were identified by SDS-PAGE and Western blot analysis using a monoclonal anti-A antibody. Blood group A glycolipid antigens were found in both A1 and A2 thrombocytes but the A2 individuals expressed at least ten times less A glycolipids compared to the A1 individuals. Expression of A type 3/4 chain and small amounts of A type 1 chain glycolipids were seen in thrombocytes of both A1 and A2 individuals, while the type 2 chain A glycolipids appeared to be missing from the A2 thrombocytes. Blood group A reactive glycoproteins were only found in thrombocytes of A1 individuals and could not be detected in A2 individuals or a blood group O individual. The major blood group A glycoprotein were found as a double band migrating in the 130 kDa region.

  • Green banana protection of gastric mucosa against experimentally induced injuries in rats

    • The protective capacities of fresh green (unripe) sweet bananas and of phosphatidylcholine and pectin (banana ingredients) against acute (ethanol- or indomethacin-induced) and chronic (indomethacin-induced) gastric mucosal lesions were evaluated in rats. Banana pulp was mixed with saline and given by gavage, as a pretreatment in a single dose. The identical protocol was used for pectin and phosphatidylcholine solution, and the dosages were adjusted to equal the amount of ingredients in the banana mixture, but higher concentrations were also given. The banana suspension reduced acute lesions, as did pectin and phosphatidylcholine in higher concentrations, but in concentrations as in fresh fruit no protective effects were observed except by pectin against indomethacin injury. In the model of chronic ulcers the banana suspension provided an incomplete and temporary protective effect. We conclude that the protective capacity of fresh green sweet bananas cannot be confined to only one active component. Pectin and phosphatidylcholine may protect gastric mucosa by strengthening the mucous-phospholipid layer, but the mechanism of protection afforded by bananas has to be further elucidated.

    • The composition and content of the gastric phospholipids were followed during development and healing of indometha-cin-induced chronic, antral ulcers in rats. The individual phospholipids were identified by thin-layer chromatography and quantitatively estimated by spectrophotometric analysis of phosphate. No changes were found in phospholipid composition and content after a 24-hour fast or during the first 24 h after indomethacin was given. The total phospholipid content and the content of lysophosphatidylcholine and phosphatidylcholine were increased on the 5th day, when chronic ulcers were established. After 4 weeks, when the ulcers were healed, the total phospholipid content, and content of phosphatidylcholine, phosphatidylethanolamine and phosphatidylinositol, respectively, were decreased, while the content of lysophosphatidylcholine was increased. In conclusion, the composition and content of gastric phospholipids were altered in rats with ulcers as well as in rats with healed ulcers.

  • Identification of the same factor V gene mutation in 47 out of 50 thrombosis-prone families with inherited resistance to activated protein C

    • Resistance to activated protein C (APC) is the most prevalent inherited cause of venous thrombosis. The APC resistance phenotype is associated with a single point mutation in the factor V gene, changing Arg506 in the APC cleavage site to a Gln. We have investigated 50 Swedish families with inherited APC resistance for this mutation and found it to be present in 47 of them. Perfect cosegregation between a low APC ratio and the presence of mutation was seen in 40 families. In seven families, the co-segregation was not perfect as 12 out of 57 APC-resistant family members were found to lack the mutation. Moreover, in three families with APC resistance, the factor V gene mutation was not found, suggesting another still unidentified cause of inherited APC resistance. Of 308 investigated families members, 146 were normal, 144 heterozygotes, and 18 homozygotes for the factor V gene mutation and there were significant differences in thrombosis-free survival curves between these groups. By age 33 yr, 8% of normals, 20% of heterozygotes, and 40% of homozygotes had had manifestation of venous thrombosis.
