    • The weight order – an analytical perspectiveThis article is an outline of a critically oriented and empirically grounded theory of the weight order, as a complement to theories of more widely recognized and studied ordering systems. We 1) expose the weight orders “absent presence” in humanistic and social science-oriented research treating overweight and fatness as a personal or social problem, 2) outline the contours and characteristics of this specific ordering system, and 3) suggest a set of sensitizing concepts for analysis of this ordering system. Two primary forms of activity, maintaining order and putting in order, are analysed. The first is making thin people into order and overweight people into disorder, and thus maintains order in the weight order. The other, putting in order, covers different activities supposed to make sure that people keep their bodies thin or try to become thin. These ordering activities meet resistance when overweight people stop dieting and/or define overweight as a personal choice and themselves as good enough, or even healthy and beautiful. We call these forms of resistance alternative weight-doings.

  • Essentials of Nursing Care in Randomized Controlled Trials of Nurse-Led Interventions in Somatic Care : A Systematic Review

    • Background: Nursing practice has to contribute to evidence pointing out why there is a need for more nurse-designed randomized control trials (RCTs) focusing on evidence-based practice (EBP). How far this EBP has progressed in different health aspects is usually established by systematic reviews of RCTs. Nurse-led RCTs exist but no study has addressed the essentials of nursing care. Aim: The aim was therefore to determine the essentials of nurses’ interventions by means of nurse-led RCTs in somatic care focusing on the stated context, goals, content, strategies as well as the nurse’s role related to effectiveness. Methods: A systematic review was realized according to Cochrane review assumptions to identify, appraise and synthesize all empirical evidence meeting pre-specified eligibility criteria. The PRISMA statement guided the data extraction process (n = 55) from PubMed and CINAHL. Results: Of the RCTs in somatic care, 71% showed a positive effectiveness of nurse-led interventions, of which the nurse had a significant role with regard to being the main responsible in 67% of the studies. Also, 47% of the RCTs presented a theoretical standpoint related to the nurse-led interventions and most prominent were international evidence-based guidelines. Goals were found to have either a patient-centered or a professional-centered ambition. Strategies were based on patient-directed initiatives, nurse-patient-directed initiatives or nurse-directed initiatives, while contents were built upon either a patient-nurse interaction or a nursing management plan. Conclusions: This review underlines the necessity of a holistic view of a person, as nurse-led RCTs comprising a patient-centered ambition, patient-directed initiative and patient-nurse interaction plan showed beneficial nursing care effectiveness, particularly if theory-based. In a nurse-led RCT, a basic theoretical perspective is advantageous as well as to elucidate the role of the nurse in relation to the estimated effects.

  • The Stern Review and its critics : economics at work in an interdisciplinary setting

    • The Stern Review, Stern (2007), received a lot of attention upon its release. It also resulted in an intense and interesting debate within climate change economics. One of the central issues has been the question of the appropriate discount rate to use and more generally the proper role of ethics in an economic analysis of this kind. Some have argued against incorporating ethical considerations at all, while others argue that Stern did not involve ethics enough. There are also those who question whether Stern provided an economic analysis at all. This paper analyzes Stern (2007) and the surrounding debate from a methodological point of view. Basically three conclusions are reached. First, Stern involves a number of different kinds of interdisciplinarity based on the Mäki (2007) characterization of interdisciplinarity. Second, the interaction of economics and ethics is unavoidable within climate change research. Third, Stern uses a modified version of the standard economic approach to climate change and the deviations are well motivated (i.e. it is still a valuable economic analysis).

    • This study starts from Retzinger (1995), Retzinger & Scheff (2000) and Scheff & Retzinger's (2000, 2001), micro-sociological perspective on social bonds. The general aim is to test a new model of operationalized sub-indicators that allows for a simplified ability to analyze the relationship between power relations and the social bond between therapist and clients in cognitive behavior therapy (CBT) for violent men. CBT-therapy often achieves positive effects by giving clients the knowledge and ability to talk about their problems instead of using aggression and drugs as defenses. The therapists make in this case many efforts to balance the relationship between them and the clients, but, there is still a tendency that therapy alienates them from each other and prevents the development of a solidarity social bond, which assuming that the theoretical premise is true, is the prerequisite for a client to build a positive self-image and become reintegrated into society.

    • The study starts from Retzinger (1995), Retzinger and Scheff (2000) and Scheff and Retzinger's (2000, 2001) micro-sociological perspective on social bonds, with the general aim of constructing a model consisting of operationalized indicators that opens for a simplified ability to analyze the relationship between power relations and emotions in private and institutionalized meetings. Scheff & Retzinger have provided a comprehensive guide to how the state of the social bond can be decoded by the direct access to non-verbal data. But since this type of data is both difficult to obtain, difficult to analyze and time consuming, therapists need simplified methods that can provide insight into the state of social bonds. We also need a method that takes into account the issue of power relations — both power relations in society and power relations between the client and the therapist. The treatment of men with violence problems provides an example of how our model can be applied to therapeutic activity. The examination shows that it is possible to construct a model with operationalized indicators that facilitate visualization of the relationship between power and the quality of social bonds.

Inställningen till klienten hos behandlande personal och behandlingsmetodernas tillämpning, kan innebära att klientens självbild som stigmatiserad förstärks, är oförändrad eller konstruktivt rekonstrueras. Med utgångspunkt i behandlade personals erfarenh. av tillämpningen av behandlingsmetoder och klienters erfarenh. av behandl. kan slutsatser dras om hur klientens stigma konstrueras. Syftet är att utveckla och testa hypoteser om skam- och stigmatiseringsprocessers betydelse vid behandling av våldsverkande män. - Har behandlingsmetoder av våldsverkande män skam- eller stolthetsskapande innehåll. Förstärks en stigmatiserad identitet eller rekonstrueras den konstruktivt? - Vilka exempel finns på stolthets- och skamskapande situationer hos klienterna? Data samlas in under två faser, fas 1 omfattar en pilotstudie, där ett urval av behandl. personal samt ett urval klienter intervjuas i avsikt att konkretisera de frågeställningar som studien omfattar. Den första hypotesgenererande fasen kompletteras i fas 2 med delt. observ. och intervjuer samt dokumentanalys. Datainsamlingsmetoderna är intervjuer med personal och klienter, dokumentanalys samt deltagande observ. av klientsamtal. Analysmetoderna är kvalitativa med tonvikt på diskurs- och innehållsanalys. De förväntade resultaten är att behandlingmetoder innefattande en förståelse av att klientens stigma påverkas av stolthets- respektive skamskapande behandling också leder till en konstruktiv rekonstruktion av klientens status som stigmatiserad. Behandlingsmetoder där personalen inte har denna kunskap motverkar en rekonstruktion av en stigmatiserad identitet.
