  • A comprehensive rehabilitation programme tailored to the needs of adults with muscular dystrophy.

    • OBJECTIVE: To assess if activities of daily living (ADL), coping and quality of life could be improved in adults with muscular dystrophy through a comprehensive rehabilitation programme. DESIGN: Quasi-experimental, controlled clinical study comparing patients with similar age and disease aspects. SETTING: Two different counties in Sweden, being either study or control setting. SUBJECTS: The study group comprised 37 adults (21 women, 16 men; mean age 50 years), while the control group comprised 39 people (25 women, 14 men; mean age 46 years). INTERVENTIONS: Four rehabilitation sessions tailored to different medical, physical and psychosocial needs of the patients, comprising a total of 10 days over a period of 18 months. MAIN MEASURES: ADL, the Mental Adjustment to Cancer Scale measuring coping strategies, the Sickness Impact Profile measuring health-related quality of life, the Hospital Anxiety and Depression Scale, and the Psychosocial Well-being Questionnaire. RESULTS: No significant differences were found between groups with regard to the outcome measures. There was increased dependence on others in ADL after 18 months in both groups, but it was more pronounced in the control group. Furthermore, a clear trend was observed in the data with regard to coping patterns, the control group using more coping strategies such as 'Helplessness/hopelessness' (P= 0.057), 'Anxious preoccupation' (P = 0.085) and 'Fatalistic' (P= 0.073) when being compared to the study group. CONCLUSIONS: No apparent effects on ADL were found from the rehabilitation programme, although there was a tendency of reduction of maladaptive coping patterns in the study group. This initial study may provide the rationale and basis for a randomized controlled trial.

  • Slack-Time Aware Dynamic Routing Schemes for on-chip networks

    • Network-on-Chip (NoC) is a new on-chip communication paradigm for future IP-core based System-on-Chip (SoC), designed to remove a number of limitations of today’s on-chip interconnect solutions. A NoC interconnects cores by means of a packet switched micro-network, which improves scalability and reusability, resulting in a shorter time to market. A typical NoC will be running many applications concurrently, which results in shared network capacity between different kinds of traffic flows. Due to the diverse characteristic of applications, some traffic flows will require real-time communication guarantees while others are tolerant to even some loss of data. In order to provide different levels of Quality-of-Service (QoS) for traffic flows, the communication traffic is separated into different service classes. Traffic in NoC is typically classified into two service classes: the guaranteed throughput (GT) and the best-effort (BE) service class. The GT class offers strict QoS guarantees by setting up a virtual path with reserved bandwidth between the source (GT-producer) and destination (GT-consumer), called a GT-path. The BE class offers no strict QoS guarantees, but tries to efficiently use any network capacity which may become available from the GT traffic. The GT traffic may not fully utilize its bandwidth reservation if its communication volume varies, leading to time intervals where there is no GT traffic using the bandwidth reservation. These intervals are referred to as slack-time. If the slack can not be used this leads to unnecessarily reduced performance of BE traffic, since a part of the available network capacity becomes blocked. This thesis deals with methods to efficiently use the slack-time for BE traffic. The contributions include three new dynamic schemes for slack distribution in NoC. First, a scheme to inform the routers of a GT-path about available slack is evaluated. The GT-producer plans its traffic using a special playout buffer, and issues control packets containing the actual amount of slack-time available. The results show that this scheme leads to decreased latency, jitter and packet drops for BE traffic. Secondly, an extension to this scheme is evaluated, where slack is distributed among multiple GT-paths (slack distribution in space). This opens up the possibility to balance the QoS of BE traffic flows which overlap with the GT-paths. Thirdly, a scheme to distribute slack among the links of a GT-path (slack distribution in time) is proposed. In this approach, arriving GT-packets, at a certain router along the GT-path, can wait for a maximum defined amount of time. During this time, any waiting BE traffic in the buffers can be forwarded over the GT-path. The results confirm that this is especially important during high BE-traffic load, where this technique decreases the jitter of BE traffic considerably.

    • PurposeHigher incidence of mental illnesses and less access to care is previously reported concerning migrants but few studies focus on the needs and care of migrant groups in psychiatry. The aim of this study was to compare differences in needs and care between migrant and nonmigrant groups of severely and persistently mentally ill (SMI) after the 1995 Swedish mental health care reform.MethodsIn a Swedish area, inventories were made in 2001 and 2006 of persons considered as SMI. These persons were interviewed and their needs were assessed. In a cross-sectional study in 2006, needs and care were compared between migrants and nonmigrants. In a longitudinal study, migrants and nonmigrants interviewed in both 2001 and 2006 were compared concerning the development of needs and care.ResultsThe needs of the migrant group were less taken care of. In 2006, there were more unmet needs in this group concerning accommodation, physical health, psychological distress, basic education and economy.ConclusionThe improvement of groups considered as SMI concerning functional disability and efforts of care found in the actual area did not seem to include the migrant group, at least not to the same degree.

  • Factors related to self-fated participation in adolescents and adults with mild intellectual disability - A systematic literature review

    • Background Self-rated participation is a clinically relevant intervention outcome for people with mild intellectual disability. The aim of this systematic review was to analyse empirical studies that explored relationships between either environmental factors or individual characteristics and aspects of participation in young adults with mild intellectual disability. Method Four databases were used, 756 abstracts examined and 24 studies were evaluated in-depth. Results Four aspects of participation were found: involvement, perceptions of self, self-determination and psychological well-being. Reported environmental factors were: social support, choice opportunity, living conditions, school, work and leisure, attitudes, physical availability and society. Reported individual characteristics were adaptive and social skills. Conclusions There is a relative lack of studies of factors influencing self-rated participation and existing studies are difficult to compare because of disparity regarding approaches, conceptual frameworks, etc. For adequate interventions, it seems important to study how profiles of participation are influenced by different patterns of environmental factors and individual characteristics. © 2007 Blackwell Publishing Ltd.

    • Cryopreserved peripheral blood mononuclear cells (PBMC) are commonly used when assessing immune responses in clinical trials, both for practical reasons and to minimize interassay variation, as samples are often collected and studied over time. This study investigated the effect of cryopreservation on cytokine and chemokine secretion, and on expression of regulatory T-cell associated markers, in samples from children with type 1 diabetes. PBMC were cultured before and after cryopreservation either with GAD(65) or PHA. Secretion of cytokines (IL-5, -6, -10, -12, -13 -17, IFN-gamma and TNF-alpha) and chemokines (IP-10, MCP-1, MIP-1 alpha, MIP-1 beta and RANTES) was analysed in cell supernatants using multiplex fluorochrome technique (Luminex). Expression of FOXP3 and TGF-beta mRNA was detected by multiplex real-time RT-PCR. Increased spontaneous secretion of IL-6, -10, -12, -13, IFN-gamma and MCP-1, and mRNA expression of FOXP3 and TGF-beta, was detected after cryopreservation. Stimulation with GAD65 induced higher levels of IL-6, IFN-gamma, TNF-alpha and MIP-1 alpha, whereas lower secretion was found for IL-10 and IL-13 in cryopreserved PBMC. Stimulation with PHA induced lower secretion of IP-10, MCPA and RANTES and FOXP3 mRNA expression after cryopreservation. Thus, cryopreserved PBMC were suitable to assess the immunological markers included in this study, even though their expression could differ from freshly handled cells.

    • Different kinds of cultural studies can be used in order to learn more about disability, social policies, attitudes and citizenship. The purpose of this study is to outline some aspects of disability and Swedish society during the 19th century. The ambition is to analyse the issue of the integration of the deaf-mutes and the blind. How did politicians and educators motivate the establishment of compulsory schooling? How was the issue of correction of the body treated? How did they deal with the situation on the labour market? What kind of compensation was contemporary society willing to support? Social policies in the past are likely to be described in terms of control, repression and barriers. This study looks at disability from a more anthropological view which implies the use of hermeneutics, seeking to identify the agent’s own understanding of a problem in order to learn more about how social categorisation and citizenship are integrated and how they change. The use of original sources, such as records from the Swedish parliament and conferences held by experts as well as periodicals, makes this kind of approach possible.This thesis argues that disability must be understood as something that is constantly in the arena of a more dialectical struggle where a number of visions and interests have melted together. In the course of state interventionism and growing social justice, citizenship and disability to a greater extent became a question of honour. Being granted certain rights meant that the individual had passed the test and was now sanctioned as disabled, one who deserved the right to rights. This transition promoted a growing group consciousness. A more dialectical approach perforates the border between social control and humanity since they were not always mutually exclusive.

  • Off-line Testing of Delay Faults in NoC Interconnects

    • Testing of high density SoCs operating at high clock speeds in an important but difficult problem. Many faults, like delay faults, in such sub-micron chips may only appear when the chip works at normal operating speed. In this paper, we propose a methodology for at-speed testing of delay faults in links connecting two distinct clock domains in a SoC. We give an analytical analysis about the efficiency of this method. We also propose a simple digital hardware structure for the receiver end of the link under test to detect delay faults. It is possible to extend our method to combine it with functional testing of the link and adapt it for on-line testing.
