    • Thirty Nobelpharma implants were retrieved from 17 patients despite a remaining clinical stability, after between 1 and 16 years of clinical function. The reasons for implant removal were bone resorption in combination with soft tissue disorders, psychological causes, implant fracture and post mortem cases. When measured at the cortical passage, there was an average of 84.9% direct bone-to-implant contact and 81.8% average surface bone area in individual threads as evaluated in a computerized morphometric system at the light microscopic level.

  • The absence of correlations between a clinical classification and ultrastructural findings in amelogenesis imperfecta.

    • This study was performed to examine whether a clinical classification of different phenotypes of amelogenesis imperfecta could be discernible at the ultrastructural level. Seventeen primary teeth from 16 children with hypomineralization, hypomaturation, or hypoplastic variants of the disease were collected for histologic studies of the enamel by means of polarized light microscopy, scanning electron microscopy (SEM), and secondary ion mass spectrometry (SIMS). Polarization microscopy showed that the enamel was hypomineralized; in six teeth a wavy configuration of the enamel prisms also appeared. Three histomorphologic main types could be discerned. In 10 of the teeth extensive hypomineralization of the bulk of the enamel was found. One tooth had an unusually thick enamel with only a thin normally mineralized surface layer. SIMS images showed less pronounced signals from Ca2+ and Na+ but with stronger signals from Cl- and CN-, representing the organic component of enamel. The SEM images showed an irregular prism pattern with marked interprismatic areas. Irrespective of the clinical appearance or the hereditary pattern the main findings were hypomineralized enamel with or without wavy bands. Neither of the analytical methods used in this paper distinguishes between the clinical phenotypes of amelogenesis imperfecta.

  • Topical application of tetracycline‐HCl in human periodontitis

    • Previous in vitro studies have suggested that tetracycline‐HCl (TTC‐HCl) is adsorbed and actively released from root dentin. The aim of the current study was to evaluate the binding to and release of TTC‐HCl from human root dentin surfaces in vivo, and to evaluate the clinical utility of TTC‐HCl irrigation as an adjunct to scaling and root planing. Experiment I utilized two contralateral mandibular single‐rooted teeth which were examined in four adults with severe generalized periodontitis. One tooth in each patient was carefully scaled and root planed, under local anesthesia, and the other used as an unsealed control. Each subgingival root surface was irrigated for 5 min with an aqueous TTC‐HCl solution at a concentration of 100 mg/ml. Gingival crevicular fluid samples were collected on paper strips for the next three weeks. The TTC‐HCl concentrations in each sample were determined by the inhibition zone of B. cereus cultured on agar plates. The TTC‐HCl concentrations in gingival crevicular fluid collected 15 min after irrigation were 3100±670 μg/ml from the scaled lesions and 4700±1300 μg/ml from the unsealed root surfaces. The antibiotic concentrations decreased logarithmically over the next 7 days; 1500±270 μ/g/ml and 1100±330μ/g/ml at 2 h. 880±350μ/g/ml and 1300±360 μ/g/ml at 6 h and 19±5μ/g/ml and 31±26 μ/g/ml at 1 week for scaled and unsealed root surfaces, respectively. Results for week two and three indicated an average of over 8 μg/ml. The TTC‐HCl concentrations in gingival crevicular fluid from scaled and unsealed root surfaces were not statistically different at any time point. The tetracycline irrigation resulted in release of tetracycline at concentrations well above therapeutic concentrations for at least 1 week. Experiment II comprised 11 patients with severe adult periodontitis. All subjects were scaled and root planed prior to baseline measurements. The patients were monitored by the following parameters: probing pocket depth (PPD), probing attachment level (PAL), gingival index (GI) and plaque index (PI). 54 contralateral teeth exhibiting residual pocket depths of 5 mm were selected. Within each pair identified for the study, teeth were randomly assigned as test or control sites. After baseline measurement, each subgingival root surface was irrigated for 5 min; either with an aqueous TTC‐HCl solution of 100 mg/ml (test), or a 0.9% NaCl solution (control). At 3 and 6 months post‐treatment, the PI was unchanged for both groups. The GI index was reduced (0.062 > p > 0.001) in a similar manner for both groups. PPD showed statistically significant (p < 0.001) mean/patient decrease of similar magnitudes, 2.3±1.0 mm (test), and ‐1.6±0.8 mm (control) at 3 months, and ‐2.1±1.1 mm (test), and ‐1.4±0.9 mm at 6 months (control), respectively. Also, PAL measurements indicated a statistically significant average gain/patient of 2.1±1.1 mm in the test group (p<0.00l) and again of 1.2±1.0 mm in the controls (p = 0.002) at 3 months, and 1.8±1.1 mm (test; p<0.001) and 1.0±0.9 mm (controls; p= 0.005) at 6 months. Comparisons of the changes, between the groups, indicated statistically greater gain of PAL in the test group at both the 3 (p= 0.042) and 6 months (p= 0.034) intervals. These results suggest that TTC‐HCl irrigation of root surfaces for long periods of time (5 min) results in a subsequent release of active antibiotic into the gingival fluid at therapeutic levels for at least 1 week. TTC‐HCl irrigation resulted in significantly greater attachment gain as compared to scaling and root planing alone over at least a month period of healing.

  • Radiological evaluation of marginal bone loss at tooth surfaces facing single Brånemark implants.

    • A radiologic evaluation of marginal bone loss around single implants ad modum Brånemark and their adjacent teeth was performed: 58 adults with 71 fixtures were followed up to 3 years after crown installation. The following factors were considered: age, cause of tooth loss, vertical relation between fixture and teeth, distance between adjacent teeth, distance between fixture and natural tooth, jaw region. Specified distance were measured as well as marginal bone levels around fixtures and tooth surfaces in magnified standardized intraoral radiographs. The results show a loss of marginal bone support at tooth surfaces adjacent to inserted implants during the interval between preoperative examination and crown installation that exceeds the loss during subsequent years. The largest bone loss was observed when implants were placed next to lateral upper incisors. A strong correlation was found between bone loss at adjacent teeth and the horizontal distance fixture-tooth. With decreasing distance the bone loss increased, especially in the upper incisor region. At present it seems difficult to predict which individual or individual condition may have a higher risk for bone loss, due to large intra- and interindividual variation.

    • The aim was to describe the appearance of the calcified cartilage zone (CCZ) and to determine its dimensional relationship to the articular cartilage thickness in the normal human temporomandibular joint. An autopsy material comprising 21 joints from 12 elderly individuals was examined microscopically. The appearance of the CCZ was examined, and the thickness of the CCZ and of the total articular cartilage was measured in 18 different positions in each joint. The CCZ was outlined by a flat or gently undulating tidemark and an irregular osteochondral junction. The cellularity of the CCZ varied extensively. The cells were numerous in the CCZ when the overlying articular cartilage displayed high cellularity. Statistical analysis of the measurements demonstrated a relationship (p < 0.001) between the thickness of the CCZ and of the articular cartilage. Our findings, both qualitative and quantitative, indicate a close relationship between the physiology of the CCZ and of the overlying articular cartilage.

    • Using the Scanora radiography unit, most radiographic examinations in the dento-maxillofacial region can be rapidly performed to produce exceptionally high quality images. The Scanora unit uses the principles of narrow beam radiography and spiral tomography. Starting with a panoramic overview as a scout image, a large number of dental and maxillofacial imaging procedures and projections can be selected, all of which are computer controlled and automatically executed. The authors provide examples from their experience with over 10,000 patients and more than 30,000 examinations.

  • Reactivity of young and old human enamel to demineralization.

    • The aim of this study was to determine the susceptibility of newly erupted and old permanent teeth to artificial, caries-like attacks. Two groups of caries-free teeth were used. Group 1 consisted of 38 teeth extracted for orthodontic reasons (9-12-yr-old children); group 2, of 40 teeth extracted for periodontal reasons (45-65-yr-old patients). After thorough cleaning, a test window was isolated on the incisal two-thirds of the buccal surface. After demineralization with 6% HEC gel at pH 4.9 for 8 days, longitudinal ground sections were prepared for imbibition studies in polarized light and for secondary ion mass spectrometry (SIMS). In the young teeth, the lesions appeared to be uniform in their extension in the enamel, whereas the old teeth showed less marked and thinner surface zones and greater depth of the positively birefringent body of the lesion. Polarized light microscopy and SIMS data support the hypothesis that there are different enamel pathways in the initiation of the natural carious process.

  • A 3-year follow-up study on single implant treatment.

    • The purpose of this study was to investigate restorative and postinsertion problems in patients provided with single implant supported restorations. Fifty consecutive single implant patients were reviewed over a period of 3 years following placement of artificial crowns. One (1.4%) of the 70 inserted implants was lost during the follow-up period, which gives a cumulative success rate of 98.5%. The most frequent complication was loosening of the single tooth abutment screw. This problem was associated with fistulas during the first year of clinical service. A more severe complication was that three adjacent teeth had to be endodontically treated due to accidental devitalization from surgical trauma during implant insertion. The mean marginal bone level adjacent to the implants was reduced 0.5 mm from crown insertion to the third annual review.

    • A group of 48 patients, all more than 80 years old (mean age 82.7 years) at first implant surgery, who received a total of 254 implants, were followed in one clinical center. Of the exposed implants, 6 of 238 (2.5%) were found to be loose at second-stage surgery, and another 3 implants were lost during the follow-up period. Most patients had minimal postplacement problems, similar to what has been observed in younger patients. However, some patients (10%) experienced obvious problems with general adaptation and muscle control, which has not been observed in younger patients.
