  • Treatment with dental implants in patients with severe osteoporosis: a case report.

    • The case report of a woman with severe osteoporosis who was treated with dental implants is presented. Polyarthritis was diagnosed in 1955, and a corticosteroid medication treatment was started in 1960. During the years, the patient has undergone multiple joint surgeries. Dental implants were inserted in the maxilla in 1987 and in the mandible in 1988. Due to a compression of the spine, the patient lost 12 cm in body height between 1991 and 1993; a spontaneous femur fracture was diagnosed in December 1992. However, the arch bone has been stable; the 6- and 5-year follow-up results of the maxillary and mandibular implants, respectively, are presented.

  • Assessment of risk for periodontal disease. I. Risk indicators for attachment loss

    • Specific risk indicators associated with either susceptibility or resistance to severe forms of periodontal disease were evaluated in a cross-section of 1,426 subjects, 25 to 74 years of age, mostly metropolitan dwellers, residing in Erie County, New York, and surrounding areas. The study sample exhibited a wide range of periodontal disease experience defined by different levels of attachment loss. Therefore, it was possible to accurately assess associations between the extent of periodontal disease and patient characteristics including age, smoking, systemic diseases, exposure to occupational hazards, and subgingival microbial flora. Age was the factor most strongly associated with attachment loss, with odds ratios for subjects 35 to 44 years old ranging from 1.72 (95% CI: 1.18 to 2.49) to 9.01 (5.86 to 13.89) for subjects 65 to 74 years old. Diabetes mellitus was the only systemic disease positively associated with attachment loss with an odds ratio of 2.32 (95% CI: 1.17-4.60). Smoking had relative risks ranging from 2.05 (95% CI: 1.47-2.87) for light smokers increasing to 4.75 (95% CI: 3.28-6.91) for heavy smokers. The presence of two bacteria, Porphyromonas gingivalis and Bacteroides forsythus, in the subgingival flora represented risks of 1.59 (95% CI: 1.11-2.25) and 2.45 (95% CI: 1.87-3.24), respectively. Our results show that age, smoking, diabetes mellitus, and the presence of subgingival P. gingivalis and B. forsythus are risk indicators for attachment loss. These associations remain valid after controlling for gender, socioeconomic status, income, education, and oral hygiene status expressed in terms of supragingival plaque accumulation and subgingival calculus. Longitudinal, intervention, and etiology-focused studies will establish whether these indicators are true risk factors.

  • Implant treatment in partially edentulous patients: a report on prostheses after 3 years.

    • There has been an increasing interest in the use of implants for partially edentulous patients. This introduces other biomechanical situations than those experienced in completely edentulous patients. In a prospective multicenter study, 521 implants in 154 patients were loaded with 197 free-standing prostheses. The patients have been followed for 3 years. The cumulative success rate for the prostheses was 94.8% and for the implants it was 93.9%. Most of the lost prostheses were only supported by two implants. A frequent technical complication was fracture and loosening of gold screws, which was more frequent in prostheses supported by only two implants.

    • A survey of approximately 460 patients with Parkinson's disease (PD) or multiple sclerosis (MS) shows that speech and swallowing difficulties are very frequent within these groups. Seventy percent of the PD patients and 44% of the MS patients had experienced impairment of speech and voice after the onset of their disease. Forty-one percent of the PD patients and 33% of the MS patients indicated impairment of chewing and swallowing abilities. The speech disorder was regarded as one of their greatest problems by 29% of the PD patients and by 16% of the MS patients. Only a small number of patients, 3% of the PD and 2% of the MS group, had received any speech therapy.

  • Studies on the relationship between changes in radiographic bone height and probing attachment

    • The aim of this investigation was to determine the relationship between loss of radiographic crestal bone height and probing attachment loss. As part of this aim, we are introducing a new method for determination of a threshold for significant probing attachment loss which takes into account the error contributed by alteration in tissue tone. 57 adults with established periodontitis were selected. Radiographs were taken using the Rinn alignment system. Crestal change was determined from bone height measurements on digitized images of pairs of radiographs using the "side-by-side" technique of analysis developed recently by us. Probing attachment loss was measured using the Florida electronic probe system. Radiographic and probing measurements were made at baseline and after 1 year. No treatment was given during this period. A direct and significant relationship was observed between radiographic bone loss and probing attachment loss on a site basis (p = 0.0001, r2 = 0.018) and between subject means (p = 0.0014, r2 = 0.16). Radiographic and probing attachment change at all categories of sites, dichotomously classified as to not changing or loosing indicated 13% of sites were loosing by measurement of radiographic change and 9.6% were loosing by measurement of attachment change. Concordance in radiographic and attachment level change was found in 82% of sites examined. The relative diagnostic import of change in probing attachment or change in radiographic bone height requires treatment outcome studies based on use of diagnostic information of the 2 measuring techniques used singly and in combination.

    • Seventy-six patients were consecutively treated with fixed prostheses supported by osseointegrated implants in the edentulous maxilla and followed up for 5 years. The mean bone quality and resorption indices were 3.1 and 2.7 at the time of implant placement, respectively. Altogether, 449 standard Brånemark implants were placed. Two patients resumed to complete dentures, and the cumulative implant and prosthesis survival rates were 92.1% and 95.9% for 5 years, respectively. The mean marginal bone level was 0.6 mm below the reference point at the time of placement and 1.2 mm below the same point 5 years later. Speech problems was the most frequent complaint during the first year of function, while resin fractures caused most adjustments during the follow-up period. No implant, abutment or gold alloy screws were found to be fractured, and only 4 patients had their prostheses re-tightened due to loose gold alloy screws.

    • A photogrammetric technique was tested to measure the topography of the mucosa around implants, placed in edentulous upper jaws. Photographs were taken of casts from 6 patients, who all had used a removable overdenture for one year. Another series of photographs was taken on new casts after the use of a fixed prosthesis for a second year. The 6 pairs of photographs were measured and compared in an analytical stereo plotter for surface contour and implant positions. The results from the measurements indicated a trend of general recession of the mucosa after one year with fixed prosthesis, both on the buccal as well as on the palatal side. The mean volume of recession was 222.4 mm3, corresponding to an average of 0.4 mm3/mm2 of mucosa. More recession was generally observed on the palatal side, but obvious variations between the patients were present. In conclusion, the photogrammetric technique was considered to be well suited for analysing tissue contours in various dental situations.

    • The aim of this study was to develop and apply a reliable method of measuring the effects of condylar lesions quantitatively on panoramic radiographs. Three different types of machines were tested. Two dry skulls were exposed in six positions in each machine, and the relative size of the condyle in relation to ramus height was calculated. The results showed good validity for the reference points used. The head position did not contribute to the variation in the measurements, but the type of panoramic machine had some influence. It was concluded that the method may be applied when calculating condylar ratios, provided that the same panoramic machine is used. The relative height of the condyle in relation to ramus height was measured bilaterally in three groups of children, with either normal or postnormal occlusion or with juvenile chronic arthritis (JCA), to detect possible asymmetries and define differences in the relative condylar height. The JCA group had a significantly shorter relative condylar height, and asymmetries were commoner than in the other two groups.
