    • In three Scandinavian dental facilities, a series of 334 permanent incisors with fractures of the crown or crown and root was treated by reattachment of the fragment with a resin composite. Two centers (Oslo and Stockholm) employed acid etching of enamel alone for fragment bonding (n = 146), while the third center (Copenhagen) used a combination of enamel etching and dentinal bonding (n = 188). Although the final retention rate of fragment bonding was similar in the two groups, it took the dentinal bonding group almost three times as long to drop to 50% fragment retention. This difference could be attributed to greater bonding strength in the dentinal bonding group, greater risk of second injury in the younger acid-etching group, or difficulty in maintaining a dry operative field in the younger age group. The good fragment retention, acceptable esthetics, and pulpal vitality observed in the present series indicate that reattachment of the coronal fragment is a realistic alternative to placement of conventional resin-composite restorations.

    • Thirty-nine temporomandibular joint autopsy specimens were examined by microscopy and tomography for erosive changes. We found two types of erosive changes, an extensive type with complete loss of cartilage and a local type with retained articular cartilage. On microscopic examination nearly twice as many temporal components as condyles were eroded. The erosions were generally more extensive in the condyle. Erosions in the condyle were evenly distributed. In the temporal component there was a slight predominance of erosions located to the lateral part of the tubercle. The radiologic investigation underestimated both the presence and the extent of the erosions. Positive predictive values and negative predictive values were 0.70 and 0.83, respectively, for erosions in the condyle and 0.91 and 0.68 for erosions in the temporal component. It is suggested that the initial event in osteoarthrosis of the TMJ can occur as a subarticular hard-tissue change. The need for more accurate diagnostic tools than radiography should be stressed.

    • The surgical procedure of using a titanium implant in combination with a mandibular bone graft for a single-tooth reconstruction is presented. The donor bone was taken from the vestibular base, close to the symphysis of the mandible. Patients who were missing a maxillary incisor or canine tooth, and exhibiting a loss of supporting bone in the area, were found suitable for the technique.

    • A method using cutting resistance measurements during low-speed threading for identification of various bone densities has been evaluated with regard to its precision and potential. Pig ribs were used as test samples. Differing hand pressure, minor deviation (5 degrees) from a vertical tapping direction and individual threading did not reveal any significant differences in cutting resistance values. After implants were inserted into the threaded canals, the total bone as well as trabecular and compact bone areas surrounding the implants were calculated via a computer program and using microradiographs of the bone test samples. The outcome of the cutting resistance measurements was compared with that of the microradiographic technique, and good agreement was observed between the two procedures in the ability to identify bone density. Therefore, cutting resistance measurements may in the future also be used to clinically identify bone qualities in jaws.

    • Ten autopsy jaw specimens (6 mandibles, 4 maxillae) were used for cutting resistance measurements during low-speed threading. Overall, 31 sites were analyzed where implants were inserted into threaded canals. Bone area measurements were performed around the implants as described previously. The cutting resistance values together with the total bone area values were found to be higher in mandibles than in maxillae, and a tendency towards higher values was seen in incisor regions compared with premolar regions. Furthermore, an intraindividual comparison between the true cutting resistance and the bone density values of prepared sites showed a statistically significant correlation. The method with cutting resistance measurements for evaluation of bone quality seems therefore to be reliable, at least when used in human autopsy jaw bone specimens.

    • Thirty patients with severely resorbed maxillae were treated in a one-stage procedure using bone graft and implant placement. A horseshoe-shaped bone graft was taken from the iliac wing and fixed to the residual maxillary ridge using titanium implants, which supported the prostheses placed after a 6-month healing period. The material constituted two groups: a development group, the first 10 patients; and a routine group, the following 20 patients. At the 3-year follow-up, the implant survival was 87.5% and the prosthesis stability was 100% in the routine group. The probing depth did not change during the follow-up period, while the grafted region decreased in volume during the first postoperative year. The patients reported improved chewing ability and improved life quality. Very few technical and prosthodontic complications occurred.

    • Distortions of 15 routine implant-supported prostheses were measured in relation to the master casts after completion by means of a 3-dimensional (3-D) photogrammetric technique. All prostheses were designed as one-piece gold-alloy castings with resin teeth. Five of the prostheses were placed in the edentulous maxilla, and the remaining were placed in the lower jaw. Distortion of the cylinders was mostly observed in the horizontal plane (x- and y-axis) while the vertical aspect seemed to be more stable. The mean 3-D center point distortion was 42 (SD 15) and 74 (SD 38) microns for the upper and lower jaws, respectively. The measurements revealed a range of 3-D center point distortion from 16 to 80 and 15 to 165 microns for the different jaws, respectively. The corresponding 3-D mean angular distortion of the cylinders was 51 (SD 35) microns in lower and 70 (SD 75) microns in the upper jaws. A correlation was found between 3-D center point distortion and the width as well as the curvature of the implant arch, indicating more displacement the wider and the more curved the arch was. The 3-D center point distortion was also significantly higher in the upper jaws which could possibly be explained by the curvature of the implant arch and higher numbers of implants in the upper jaws. Further problems with the fit of upper jaw castings could be related to more alloy in the castings and poor alignment of implants.
