- I fokus för detta arbete står friskvård inom arbetslivet som pedagogisk praktik med hälsoprofilsbedömning, en metod för undersökning och påverkan av människors hälsostatus och hälsovanor, som konkret exempel. En bakgrund till detta är framväxten av insatser för folkhälsa och den ökade betoning av friskvård inom arbetslivet som skett inom såväl privat som offentlig sektor. Friskvård handlar om människors levnadsvanor när det gäller kost, motion, stress, alkohol, tobak m.m.Hälsoprofilsbedömning och friskvårdspedagogik inom arbetslivet ses här ur två olika perspektiv. Ur det ena perspektivet behandlas verksamheten på dess egna villkor - som en metod att inom ramen för arbetslivet få till stånd ner hälsosamma livsstilar genom kontrakt mellan självständiga parter. Det andra perspektivet kan ses som kritiskt granskande. Här ifrågasätts premissema för hälsoprofilsbedömningen utifrån begrepp som makt och disciplinering. En inspirationskälla har därvid bland annat Michel Foucaults arbeten varit. Som empiriskt material för analysen används två avhandlingar som presenterar hälsoprofilbedömningen som metod.Ur dessa båda perspektiv erhålles två olika betraktelser om hälsoprofilsbedömning och friskvårdspedagogik inom arbetslivet - en rättvis och en orättvis. På basis av en kritisk pragmatisk ansats diskuteras möjligheten till ett samtal baserad på relationen mellan de båda betrakelserna.

Pure Selection. The Ethics of Preimplantation Genetic Diagnosis and Choosing Children without Abortion

- Preimplantation ge¬netic diagnosis (PGD) is taken to mark the starting-point of a new phase in human reproduction, where the possibility of choosing children on genetic grounds without having to resort to ethi¬cally controversial procedures (such as abortion) will grad¬ually increase. Ethical and political issues actu¬alised by this develop¬ment are addressed. The discussion touches upon issues regarding the moral status of em¬bryos and gametes, the moral import of respecting individual auton¬omy and its implications for the requirement of informed consent in health-care, the connec¬tion between sickness, dis¬ability and the value of life, the moral status of possible future people, and the connection between choosing children and eugenic policies of the past. Practical policy issues are adressed on the basis of this, as well as an empirical case-study of the intro¬duction of PGD in Sweden. The book ends up in a set of recommendations regarding the management of re¬search on, introduction and routine use of procedures for pure se¬lection, both within health care and from the point of view of society as a whole. It is argued that research on such procedures should be allowed and supported by society. However, tight restrictions regarding the clinical introduction of new procedures in this area is highly desirable. A rough model for implementing such re¬strictions is also pre¬sented. It is further asserted that, although reasons of economy and safety should limit the access to pure se¬lection, society should not apply any explicit restrictions based on ideasregarding how different traits affect a person’s quality of life. It is stressed that, in order to to avoid a re¬sur¬rection of eugenic policies of the past, the development in this field un¬der¬lines the need for continued and strengthen public support to the sick, dis¬abled and mentally retarded.

- Numerous chemical substances are handled by persons working in the health care sector. At exposure levels that may occur in the occupational setting, some of these substances are potentially harmful to the reproductive processes. Among the potentially harmful substances are anesthetic gases, antineoplastic agents, and sterilants. The epidemiological evidence of increased risks for adverse reproductive effects (eg, subfertility, spontaneous abortions, congenital defects) from such exposure is not unequivocal. However, due to the toxic potential, exposures should be kept at a minimum, and this may be especially important for workers who are pregnant or are planning to achieve pregnancy.

Sense of Coherence and Ill Health Among the Unemployed and Re-employed After Closure of an Assembly Plant

- The aim of this study was to investigate the relationship between employment status, ill health and sense of coherence (SOC). A cross-sectional sample of the former workers at the closed down Volvo Kalmar plant in Sweden was used. A questionnaire was mailed to all identified subjects 2 years after the closure of the plant. Responses were obtained from 344 persons, of whom 173were re-employed and 92were unemployed. They answered the questionnaire anonymously on background variables, SOC and ill health (physical and psychological symptoms). A notable difference was found between the re-employed and unemployed, where the unemployed reported symptoms significantly more often as well as a significantly weaker SOC. The main finding in a four-cell model indicated that unemployment had a rather low impact on symptoms in the presence of a strong SOC. Structural equation modelling (SEM) was performed with simultaneous analysis of the unemployed and reemployed. The SEM analysis indicated that SOC seemed to have a greater impact on psychological symptoms among unemployed compared with re-employed persons. However, it should be noted that the present study does not allow for the determination of cause-effect relationships. A comment: This publication is authored together with a senior research competency within psychology with whom Engström have had long time cooperation procedures.

Cancer patients' experiences of nurses' behaviour and health promotion activities : a critical incident analysis

- Patients with head and neck cancer report several disease- and health-related problems before, during and a long time after completed treatment. Nurses have an important role in educating/supporting these patients about/through the disease and treatment so that they can attain well-being. This study describes the cancer patients' experiences of nurses' behaviour in terms of critical incidents after nurses had given them care to promote health. The study had a qualitative, descriptive design and the method used was the critical incident technique. Twenty-one informants from the Nordic countries diagnosed with head and neck cancer were strategically selected. It was explained to the informants what a critical incident implies before the interviews took place; this was defined as a major event of great importance, an incident, which the informants still remember, due to its great importance for the outcome of their health and well-being. The nurses' behaviour was examined, and critical incidents were involved in 208 cases-150 positive and 58 negative ones-the number of incidents varying between three and 20 per informant. The nurses' health promotion activities or lack of such activities based on the patients' disease, treatment and symptoms, consisted of informing and instructing the patients as well as enabling their participation. Personal consideration and the nurses' cognisance, knowledge, competence, solicitude, demeanour and statements of understanding were found to be important. Continuous health promotion nursing interventions were of considerable value for the majority of this group of cancer patients. Oncology nurses could reconfirm and update the care of head and neck cancer patients by including health promotion activities in individual care plans. By more frequent use of health promotion models, such as the empowerment model, the nurses could identify and focus on those individuals who needed to alter their life-style as well as tailor their approach towards these patient by setting goals for well-being and a healthy life-style.

- Denna bok är skriven i dialogens form. Den utgör ett försök att förmedla resultatet av ett forskningsarbete i en annan form än den traditionellt akademiska. Det är dock en form som har antika anor och som under klassisk tid användes särskilt för att förmedla filosofiska kunskaper. Min egen forskning är filosofisk, och de idéer jag vill förmedla har många gånger kommit till i ett slags dialogsituation, antingen en äkta dialog med en kollega eller vän, eller en inre dialog med en fiktiv samtalspartner. På ett sätt är dialogen därför den mes naturliga formen för presentation både av filosofiska problem och av filosofiska lösningsförslag.I boken ställs många frågor kring hälsans natur och andra närbesläktade begrepp. Svaren överensstämmer i allt väsentligt med mina egna åsikter idag. Jag har emellertid för läsbarhetens skull inte alltid skrivit så precist och omständligt som en analytisk filosof förväntas göra i en akademisk avhandling. Jag uppfyller sådana krav något bättre i skrifter som On the Natur of Health (1987, 1995), Quality of Life and Happiness (1993) och Action, Ability and Health (under utgivning). Den forskare som vill kritiskt granska mina idéer bör därför i första hand uppsöka dessa skrifter. Avsikten med denna bok har inte varit att presentera mina teorier och resultat för forskarsamhället. Min huvudavsikt har istället varit att på ett någorlunda lättläst och livfullt sätt förmedla tankar om så abstrakta ting som begreppen hälsa och lycka till en större publik. Här och var har detta intressant nog inneburit att jag har gått längre i mina analyser än jag gjort i de vetenskapliga skrifterna.Mina ambitioner på det litterära planet är mycket blygsamma. Jag inbillar mig inte att jag genom ett enkelt knep som att skriva i dialogform skall kunna närma mig dialogens stora mästare Platon och Cicero. Jag har därför inte heller försökt beskriva en bakgrund eller ett sammanhang där dessa samtal utspinner sig. Läsaren kan tänka sig att de äger rum i ett seminarierum, eller kanske ännu hellre under filosofiska promenader i natursköna miljöer som den bohuslänska kusten, det öländska allvaret eller den lappländska fjällvärlden. Jag har funnit att skådandet av öppna vidder har varit befruktande för filosofisk reflektion.I dialogerna uppträder två personer benämnda I och F. ”I” står helt enkelt för intervjuaren, ”F” för filosofen. Saken är inte alltid så enkel att F är mitt alter ego. Några gånger har jag krupit in under intervjuarens skinn. Läsaren kan kanske ana när jag h yser dubier om mina egna åsikter. Oftast är dock I en person utanför mig själv. Ibland har I varit förkroppsligad i någon av de många personer som jag mött som handledare eller föreläsare om hälsans filosifi.Som alla vetenskapliga författare idag har jag fått kämpa med användningen av personliga pronomina när jag avser att tala om en människa i allmänhet och inte om en människa av ett bestämt kön. Jag använder ömsom ”han” ömsom ”hon” i dessa sammanhang,. Det är min förhoppning att jag lyckats med detta utan att resultatet har blivit vare sig förvirrande eller könsdiskriminerande.Linköping våren 1996Lennart Nordenfelt

- Health has its basis in the relationships between human societies and cultures and the environment. The health of indigenous peoples living in traditional territories is threatened today by the destruction of this environment and the introduction of new contagious diseases; those who move to urban areas suffer even worse health because of ‘civilization’ diseases and the loss of the social networks that provide personal support and health care. Contrary to expectations, an epidemiological transition has not occurred in developing countries where many of these groups live. Two apparent underlying causes—persistent poverty and political inequality—are among the factors that will affect the future health of indigenous groups, together with trends in population, urbanization, and the environment. Indigenous groups can play a major role in determining that future and are already doing so through territorial-control projects aimed at protecting the environment on which their health and well-being depend.

Variability in Skin Exposure in Machine Operators Exposed to Cutting Fluids

- Objective. This study describes a new technique for measuring skin exposure to cutting fluids and evaluates the variability of skin exposure among machine operators performing cyclic (repetitive) work. Methods. The technique is based on video recording and subsequent analysis of the videotape by means of computer-synchronized video equipment. The time intervals at which the machine operator's hand was exposed to fluid were registered, and the total wet time of the skin was calculated by assuming different evaporation times for the fluid. The exposure of twelve operators with different work methods was analyzed in six different workshops, which included a range of machine types, from highly automated metal cutting machines (ie, actual cutting and chip removal machines) requiring operator supervision to conventional metal cutting machines, where the operator was required to maneuver the machine and manually exchange products. Results; the relative wet time varied between 0% and 100%. A significant association between short cycle time and high relative wet time was noted. However, there was no relationship between the degree of automatization of the metal cutting machines and wet time. Conclusions. The study shows that skin exposure to cutting fluids can vary considerably between machine operators involved in manufacturing processes using different types of metal cutting machines. The machine type was not associated with dermal wetness. The technique appears to give objective information about dermal wetness. A comment: This publication is a result of a cooperation between the authors at Chalmers University of Technology and Gothenburg University, i.e. between the Department of Transportation and the Department of Occupational Health and the Department of Sociology.
