  • Chronic anterior cruciate ligament tear : knee function and knee extensor muscle size, morphology and function before and after surgical reconstruction

    • Knee function was evaluated by knee score, activity level, clinical findings and performance tests, muscle size by computerized tomography (CT), morphology by light (LM) and electron microscopy (EM), muscle function by electromyography (EMG) and isokinetic performance in 29 patients with chronic anterior cruciate ligament (ACL) tear. Preoperatively CT disclosed a significant mean atrophy of the quadriceps and nonsignificant changes of the other muscle areas of the injured leg. Morphology of m vastus lateralis of the injured leg was normal in more than half of the biopsies preoperatively, the rest showed signs of nonoptimal activation. Significant decreases in all isokinetic parameters were noticed together with significantly decreased EMG of the quadriceps muscle of the injured leg.Âfter surgical reconstruction the knees were immobilized in a cast for 6 weeks at either 30° or 70° of knee flexion. After cast removal CT showed significant decreases of all areas which also remained after training. The 30° group showed larger fibres (intracellular oedema) and more frequent morphological abnormalities than the 70° group. Fourteen weeks postoperatively the patients were allocated to either a combination of isometric and progressive resistance training or isokinetic training for 6 weeks. CT showed slightly larger areas at 20 weeks postoperatively than at 6 weeks. Morphological abnormalities were still prominent at 20 weeks postoperatively. Maximum isokinetic knee extensor mechanical output and endurance were markedly decreased at 14 weeks postoperatively but both improved progressively during the one year rehabilitation, mostly during the intensive 6 week training period but irrespective of training programme used. Fatiguability/endurance level improved over the preoperative level. Muscular work/integrated EMG was stable while EMG/t increased indicating neuromuscular relearning.The clinical result at 28 months foliowup was excellent or good in 93% of the patients and clinical stability improved in 66%. Independent upon primary knee immobilization angle or training programmes no differences could be demonstrated with respect to stability, range of motion, function or isokinetic mechanical output. Isokinetic performance was still significantly lower in the injured compared to the noninjured leg and not significantly different from the preoperative values. Morphology, only 6 cases, showed abnormalities similar to preoperative findings.In conclusion, the reason for the decreased maximum and total knee extensor performance in these patients with ACL tears is suggested to be nonoptimal activation of normal functioning muscle fibres depending on changes in knee joint receptor afferent inflow. No differences concerning the markedly improved postoperative clinical result could be seen between the different treatment modalities used. A nonoptimal muscular activation might explain the still decreased isokinetic performance present at followup.

    • The sex- and age-specific incidence has been calculated for ankle fractures from the first part of the 1950s and compared with the 1980s, a 30-year interval. The fractures were also classified according to Lauge-Hansen. Altogether 1784 fractures were found. During that time interval ankle fractures had become more common, particularly those with extensive skeletal involvement, such as the stage IV supination-eversion fractures, which today have an incidence pattern more typical of a fragility fracture in elderly women.

    • The age and sex-specific incidence and prevalence of vertebral compression fractures were compared between the 1950s and 1980s in an urban population. In women over 80 years of age the incidence of symptomatic vertebral fractures has increased approximately four times; in elderly men even more. Mainly, low-energy trauma has caused these fractures. Thus, one clinical manifestation of osteoporosis--vertebral fracture--has increased in number, incidence, and prevalence over the last 30 years.

    • During 1980-82, 739 ankle fractures occurred in Malmo. These were compared with 383 ankle fractures occurring in 1950-52. In men there was an increase in the age-specific incidence up to the age of 60, especially in fractures of the lateral malleolus. In women there was an increase in the age-specific incidence above the age of 50, both in lateral malleolar fractures and, even more pronounced, in bi- and tri- malleolar fractures.

    • Forty patients who had had a patellar fracture during the years 1950-58 were reevaluated 30 years later. A clinical and radiographic examination was performed. Fourteen patients had subjective complaints. Two thirds of the patients who had more than 2-mm diastasis or 1-mm incongruity had complaints and reduced quadriceps strength. Radiographically, all the patients had a reduction in the lateral patellofemoral distance in the axial view with the greatest reduction in the knees with diastasis or incongruity of the fracture.

  • Lower limb fractures and registration for alcoholism

    • During two consecutive years 607 individuals with lower limb fractures were diagnosed. Half of the individuals, 315, were women and only 14 of them had an earlier registration for alcoholism. In the 292 men, however, 73 men or 25%, had been registered at the Department of Alcohol Diseases at least once during the 13 years of observation, the highest registration frequency (30%) was noted in the malleolar fractures in men. In males, 30-50 years of age, there were 37 per cent who had a registration for alcoholism, the highest registration (44%) was noted for fracture of the tibial diaphysis. Males with fracture of the proximal end of the femur between 16-80 years of age were registered for alcoholism in 23%.

    • 32 patients, suffering from Parkinson's disease, had internal fixation of femoral neck fractures. In 24 displaced fractures, 6 nonunions and 3 segmental collapses were seen; and in 8 undisplaced fractures, 1 case of segmental collapse was diagnosed. Healing complications were thus seen in one third. Total hip replacement for healing complication was performed in 3 of 32 patients. 9 patients died within 2 years. No difference in the rate of healing or mortality was detected compared with hip fracture patients without Parkinson's disease. Our study does not support primary arthroplasty for femoral neck fracture in patients with Parkinson's disease.

    • The gleno-humeral joint space in standard antero-posterior images of 175 normal subjects was measured at three different sites. The average of the three measurements, the integral joint space, was calculated and was found to be between 4 and 5 mm. The value does not change with age, except in women, in whom it increases slightly. A narrow joint space in an elderly patient should not be expected simply because the patient is old. The measuring technique described is useful in population studies. In individual cases a simple measurement with a ruler serves the same purpose.
