  • Caring for people with intellectual and developmental disabilities : how can it be experienced and percieved by registered nurses?

    • Registered nurses [RNs] are within the frontline of professional nursing and are expected to provide a diverse range of health care services to a varied and heterogenic group of patients. They are bound by a code of ethics that mandates that nurses respect all human rights regardless of the patient’s abilities or functional status. However, research implies that RNs do not feel adequately prepared to support patients with intellectual and developmental disabilities [IDD], and that patients with IDD are often misinterpreted and misunderstood in care. Gaining in-depth knowledge about how RNs can experience nursing for this group of patients is therefore of great importance. The overall aim of this thesis was to describe, appraise, integrate and synthesise knowledge concerning nursing for patients with IDD. A further aim was to explore and describe Swedish RNs’ perceptions of providing care for patients with IDD within a home health care setting. This thesis consisted of two studies designed to investigate various aspects of nursing and caring for patients with IDD. Paper I was a systematic review using a meta-ethnographic approach, and Paper II was an interview study using a qualitative descriptive, interpretive design. Data was collected by systematic data base searches (Paper I), and by individual interviews (Paper II). The systematic review comprised 202 RNs (Paper I) and the qualitative descriptive study comprised 20 RNs. In the systematic review, data was analysed by a Line of Argument Synthesis [LOAs] as described by Noblit and Hare (1988), while the data in Paper II was analysed by content analysis.   Nurses’ experiences and perceptions of nursing patients with an IDD could be understood from 14 LOAs. Six of these were interpreted to reflect a tentatively more distinctive and unique conceptualisation of RNs’ experience of nursing for this group of patients. The remaining eight LOAs were interpreted to reflect a conceptualisation of nursing per se that is a universal experience regardless of context or patient group (Paper I). In Paper II, the nurse’s perceptions were interpreted to be reflected by three overarching categories: Nursing held hostage in the context of care, Care dependant on intuition and proven experience and Contending for the patient’s right to adequate care.   Absence of understanding and knowledge about IDD might be an explanation for the “otherness” that still appears to surround this group of patients. Concentrating on the person behind the disabilities label as well as on abilities instead of disabilities could be a reasonable approach in nursing care for patients with IDD. Thus, implementing nursing models focusing on person-centred care could support RNs to moderate the health and care inequalities that are still present among patients with IDD (Paper I).  As a result of the home health care context and its organisation, the RNs perceived themselves as unable to provide care in accordance with their professional values. Not mastering the available augmentative and alternative communication tool additionally meant having to provide care based on second-hand information from support staff. The RNs also perceived that caring for this group of patients involved a daily battle for the patient’s rights to receive the right care at the right place and time and by the right person (Paper II). Hence, a broad base of evidence on what actually works best in clinical practice for this group of patients, particularly in the home care context, is still needed.

    • Den här forsknings- och utvecklingsrapporten cirklar kring en fråga: Vad innebar covid-19-pandemin för det sociala arbetet i Malmö? I rapporten återges några svar på frågan. De är formulerade i möte med en rad personer verksamma inom socialt arbete som delade med sig av sina erfarenheter under pandemins första år. Svaren på frågan är viktiga eftersom de belyser vad en kris bär med sig för det sociala arbetet. Begreppet kris ger uttryck för att situationen i fråga präglas av osäkerhet och brådska och att den hotar centrala värden. Rapporten tar både upp hur det sociala arbetet var del av den omedelbara hanteringen av pandemin i staden och placerar in det i ett bredare perspektiv som låter det sociala arbetets roll i katastrofriskreducering framträda.

  • Drug policing of youth : examining pre- and post-stop conditions and outcomes

    • Given the strong focus on minor drug offenses in Swedish drug control policy and the risk of disparate drug enforcement that may follow from such policies, this thesis explores drug enforcement and the use of coercive measures (enforced drug tests and body searches) towards youths aged 15-20 years. The first study focuses on the circumstances of the detection of minor drug offenses, the grounds for suspicion and use of coercive measures. The second study explores ethnic disparities in exposure to drug tests, as well as ethnic disparities in relation to hitrates for drug tests and body searches. The results show that about one-third of the minor drug situations were detected in a reactive policing manner, and approximately 30-40 percent were detected in association with other offenses. The findings suggest that the grounds for enforcing drug tests and body searches often were based on subjective cues. Results from the second study show that youths subjected to a drug test were male, born outside Europe, and had unemployed fathers to a significantly greater extent than the drug-using youths in the school survey. Additionally, youths born outside Europe were more likely than youths born in Sweden to be submitted to coercive measures that produced a negative result. This finding was dependent on the definition of ethnic background that was employed. The findings suggest that future research should investigate a number of pre- and post-stop conditions and outcomes. Research on pre-stop conditions should further explore: 1) the distribution of "criminal" signs between different sociodemographic groups and their probability for a hit, 2) the importance of concurrent offending for the detection of drug offenses, and 3) the nature of drug use, transactions, and dealing in association with the risk for suspicion. Research on post-stop conditions and outcomes should explore: 1) ethnic disparities in hitrates of body searches and drug tests with more detailed data on ethnic background (specific region-based vulnerabilities), and 2) neighborhood effects on searches and drug tests, including hit-rates, both on an individual level and a neighborhood level. Research on various forms of police bias in drug enforcement should integrate both neighborhood- and individual-level processes in relation to pre-stop and post-stop conditions and outcomes to understand the mechanisms behind ethnic disparities. Finally, future research should focus on the "gender gap" found in this study and previous studies on drug enforcement.

    • I föreliggande rapport presenteras resultatet av effektutvärderingen av pilotprojektet Sluta skjut – tillsammans för ett tryggt Malmö. Utvärderingen har genomförts på uppdrag av Brottsförebyggande rådet (Brå) med stöd från Brå och Fonden för inre säkerhet (ISF). Tidigare har enprocessutvärdering publicerats.Den grova våldsbrottsligheten beskrivs som ett av de största samhällsproblemen i Sverige idag och efterfrågan på effektiva åtgärder som kan minska våldet är stor. Sluta skjut bygger på en amerikansk strategi, Group Violence Intervention, som i flera studier visat på positiva resultat när det gäller att minska det allra grövsta våldet. I processutvärderingen av Sluta skjut studerades om det går att tillämpa en amerikansk modell i en svensk kontext. I effektutvärderingen har vi studerat om insatsen lett till minskat antal skjutningar i Malmö. Svensk forskning om vad som fungerar för att minska det grova våldet är begränsad och det behövs mer kunskap för att brottsförebyggande aktörer ska få de verktyg de behöver för att vända utvecklingen.

    • Den grova våldsbrottsligheten beskrivs som ett av de största samhällsproblemen i Sverige idag och efterfrågan på effektiva åtgärder som kan minska våldet är stor. Svensk forskning om vad som fungerar för att minska det grova våldet är begränsad och det behövs mer kunskap för att Polis och andra brottsförebyggande aktörer ska få de verktyg de behöver för att vända utvecklingen. I den här processutvärderingen har vi studerat pilotprojektet Sluta skjut som är ett försök att implementera den amerikanska strategin Group Violence Intervention (GVI) i en svensk kontext. GVI har i flera internationella studier visat på positiva effekter. Resultaten från utvärderingen visar att det går att implementera den amerikanska strategin i en svensk kontext, effekten av insatsen har inte studerats inom ramen för den här utvärderingen. Utifrån det internationella kunskapsläget och erfarenheterna från implementeringen i Malmö är bedömningen att GVI är möjlig att implementera och kan vara värt att pröva i städer med stora problem med grov våldsbrottslighet.Utvärderingen har genomförts på uppdrag av Brottsförebyggande rådet (Brå) med stöd från Brå och Fonden för inre säkerhet (ISF).

  • Betydelsen av kön och ålder i äldreomsorgen : om standardiserade biståndsutredningar och dess diskurser

    • The aim of the study is to explore how social categories, with focus on genderand age, is constructed in the social documentation which is a central part ofthe officially arranged elderly care in Sweden. The analysis consists of a discourseanalysis and has a standpoint in feminist theories about performativityand respectability. The analysis consists of a text analysis of case files and howdiscourses about social categories emerge in the texts. The case files are writtenin a standardized form which is central to the structure of the documentation.The study examines the following questions 1) how is the users of elderlycare and their relatives positioned through the discourse that emerges in thecase files? 2) how is the discourse in the case files a part of doing gender andage as social categories?The study shows that the case files are reproducing stereotyped gender rolesand positions “the elderly” by differencing them from other adults throughconceptions about characteristics and (the lack of) abilities. Women’s andmen’s needs are described differently whereas abilities and strategies areprominent in the women’s case files. The respectable narrative emerges in thedescription of women wanting to perform some of the workload which is absentin case files about men. The moral responsibility of relatives is reinforcedby the description about their care work or the absence of it. Male relatives arementioned in the case files prominently in the lack of female relatives. In thisway both masculinity and femininity are constructed through the implied valuesthat is attributed men versus women in the texts. A clear medical discourseemerges in the needs assessments. The assessment texts reformulate the descriptionsof needs and they are instead formulated as facts which reinforcethe medical discourse. In final, this study shows that users of elderly care andABSTRACT10their relatives are positioned from both gender and age and that these categoriesare integrated with each other. I draw the conclusion that age is a usefulcomplement to the theory of respectability and its importance in the narrativeof expression and assessment of a person’s needs.

    • The aim of this licentiate thesis is to investigate how self-determination for users with intellectual disabilities is experienced and described by users themselves, as well as by the personnel working within group homes and personal assistance regulated by the LSS. The essential meaning of self-determination is “the right to decide for oneself”. In international research, the term self-determination is used in the sense of being able to make choices regarding one’s life without undesired external influence or interference. The personnel have a central role in enabling and implementing the intentions of the LSS, a role which in this thesis is primarily focused on support for users in exercising self-determination. This thesis therefore comprises two sub-studies, one that focuses on the users´ experiences of self-determination and their reflections on it, and one that examines the personnel´s view of and attitude to the users´ selfdetermination. The study has a qualitative approach that is based on people´s everyday lives and places the individual at the center. Taken together, the sub-studies provide two distinct perspectives derived from the primary sources, which can contribute knowledge about the everyday reality of users´ self-determination. Two theoretical approaches were used to illustrate the users´ path to selfdetermination. One approach is based on the salutogenic view and the theory of sense of Coherence, whereas the other focuses on the perspective of power and selfdetermination, i.e. empowerment. This study shows that both users and staff identify self-determination as an important factor for users´ well-being. In an in-depth analysis, both possibilities and limitations for this self-determination are revealed. The study also shows that if the contact between users and personnel is characterized by a salutogenic approach based on the theory of sense of Coherence, the users are strengthened in their empowerment process. By gaining empowerment, the users ´ path towards self-determination in their daily life situation is facilitated. It became clear that the personnel are sometimes faced with a dilemma when this salutogenic approach is challenged. Examples of such situations are when the personnel´s responsibility for the care conflicts with the users´ self-determination, or when a third party, such as trustees or parents, has views that go against the user´s wishes. In a group home, rules and routines may also obstruct individually designed salutogenic support.

  • Skam, moralarbete och faderskap : en studie om mötet mellan socialsekreterare och pappor vid våld i nära relationer

    • Men's domestic violence and/or abuse (DVA) is a widespread social issue. Often, the men are fathers, and the victims are mothers and children. In Sweden, social workers (SWs) are expected to involve these fathers in child protection investigations (CPIs) and work towards changing their behavior. However, engaging fathers in CPIs has proven to be a challenging task, especially when they involve DVA by fathers. The overall aim of this licentiate thesis is to examine how fathers can be engaged in CPIs initiated as a result of their suspected DVA against the mothers of their children. The licentiate thesis is a compilation thesis that consist of two articles, with the first having been published in a peer-review journal and the second submitted to a journal awaiting review. Using qualitative methods. I conducted semi structured interviews with 15 social workers on a total of 31 occasions during their work with 12 ongoing CPIs. Seven fathers have also been interviewed. The interviewed fathers had been involved in CPIs as a result of their DVA against the mothers of their children and had undergone treatment targeting their violent/abusive behavior.The interviews with the SWs are addressed in the first article, and the interviews with the fathers in the second article. The data were analysed using thematic analysis. The first article examines the strategies used by SWs to achieve cooperation with fathers in CPIs that involve the father being suspected of DVA against the child's mother. This analysis is rooted in the framework of shame, guilt, and social bonds. The results suggest that whether social workers succeed in achieving cooperation with fathers is related to the social workers' strategies of building secure social bonds with the fathers, which may alleviate the fathers' feelings of shame. However, there is a risk that these strategies may lead to DVA being minimized or overlooked. Striking a balance between building a social bond and addressing DVA is crucial, and the timing of addressing DVA plays a central role. The second article highlights fathers' experiences of interactions with SWs in the context of CPIs initiated as a result of their DVA against the mothers of their children. In particular, the focus is directed at how ideals of fatherhood, power relations, and shame influence fathers' experiences of engaging in these meetings. The analysis is guided by shame, guilt and social bonds linked to a gender theoretical framework. The results suggest that DVA contributes to fathers violating moral ideals of good fatherhood, which threatens to deprive them of contact with their children and also triggers feelings of shame. In the CPIs, it is the SWs, often female, who hold power over the processes while fathers are subordinated, which challenges expected power structures and hegemonic masculinity ideals. Fathers' engagement in CPIs is enabled when fathers adopt a subordinate position, which is facilitated by SWs meeting them with care and non-judgmental attitudes. This subordinate position can be a useful means for fathers to maintain an intact masculine position when DVA is exposed, shame is triggered, and their fatherhood and social bonds are threatened. However, SWs need to be attentive to the risk that fathers may both downplay their DVA and adopt positions that may be perceived as caring, egalitarian, and change-oriented even though their dominance – in the form of DVA – may persist.In summary, the study indicates that it is challenging but possible to successfully engage these fathers in CPIs. The study suggests that SWs need to balance building social bonds with the fathers and addressing the violence without imposing further shame. The findings highlight a particularly crucial aspect in the interaction, which is the SWs' ability to meet the fathers in a way that they do not feel judged for who they are - perpetrators of DVA. Instead, focusing on the violence as actions, on what they have done. In doing so, actions are separated from the self and can instead manifest as guilt. This approach can both facilitate fathers' engagement in CPIs and point towards taking responsibility for and changing the violent/abusive behaviors.
