  • Occupational  performance of persons who have suffered a stroke : a follow-up study

    • The main purpose of this study was to describe the progress made in terms of occupational performance during a two-year period by persons who had suffered a stroke and had been discharged to their home after their initial hospital stay. The population of the study consisted of 49 persons, 30 men and 19 women, with a mean age of 69 years. The Assessment of Motor and Process Skills (AMPS) was used to assess their occupational performance. Of the whole population at discharge, 66% showed increased effort and 29% showed decreased efficiency and independency. The results also showed that the persons who had decreased the most in their efficiency dropped out from the study. Twenty-nine persons participated in the study for 2 years. The best ADL ability was seen in the population at 3 months after discharge. A decrease was seen at 18 months after discharge in AMPS ADL process ability and at 24 months in AMPS ADL motor ability. The results suggest that persons who have suffered a stroke and live in their homes maintain their AMPS ADL ability performance for 1.5 to 2 years. A low AMPS ADL process ability measure in persons who have suffered a stroke signifies a reduced possibility of living at home.

    • The aim of this study was to identify and characterise conceptions of health in a group of experienced occupational therapists. Data-collection involved the occupational therapists´ personal accounts of the meaning of health and data analysis was accomplished qualitatively. The material was found to comprise 52 distinguishable articulations of the meaning of health, which were categorized under three main themes: 22 articulations of Health as “feeling fine“; 20 articulations of Health as ability to act; and 10 articulations of Health as an objective state of body and/or mind. The results show a very strong tendency towards holistic health conceptions and also toward a phenomenological view on the body. If this tendency should be taken to reflect current basic views within the profession this would indicate a movement away from biomedical frames of reference on the theoretical level. This would seem to imply consequent movements on the practice level away from organ focused interventions to the benefit of interventions in the person/context dimension of occupational performance. Studies of processes of change seem to be needed and we suggest that a paradigm theory approach probably be most pertinent.

  • Satisfaction with Daily Occupations: a tool for client evaluation in mental health care

    • Research findings suggest that in order to clarify the relationship between occupation and health, occupation needs to be framed in several different ways. One facet of occupation is satisfaction with daily occupations, assessed using the Satisfaction with Daily Occupations (SDO) instrument. The purpose was to investigate some of the SDO's psychometric properties when applied on people with persistent mental illness. Discriminant validity against occupational value and quality of life was investigated, as was internal consistency and ability to discriminate between people with different types of daily occupation. A total of 103 patients from a psychiatric outpatient unit were selected, representing people in work or studying, people visiting activity centres, and people with no organized daily occupation. The SDO showed no or weak relationships with occupational value and quality of life, and the internal consistency was alpha=0.80. Factor analysis revealed loadings between 0.44 and 0.

  • A group learning programme for old people with hip fracture : a randomized study

    • The aim of this study was to examine whether a group learning programme would influence the participants' perceived activity performance and ability to participate in social life after a hip fracture. The study comprised 35 people aged 54-90 years with hip fracture who had completed ordinary care and rehabilitation after their hip fractures. They were randomized to an intervention group (n=21) or to a control group (n=14). The intervention group participated in the group learning programme, while the control group received no intervention. Directly after the intervention and at 12 months after the intervention, no significant change was seen in either group, regarding the ability to perform ADL activities measured by the Barthel ADL index, or the performance of activities that were identified as important to the individual. However, in the intervention group, the number of ADL items perceived to be performed with difficulties decreased, and the perceived ability to participate in social life increased. These changes were not found in the control group. When analysed between groups, however, the only significant difference was the ability to participate in social life after the intervention. Further research is needed to investigate whether an intensive or prolonged period of rehabilitation, at the hospital or in the patient's home, would increase the ability to resume meaningful participation in social life

    • The importance of patterns of daily occupations to health has been emphasised ever since occupational therapy was founded, but little is known about this relationship. The aim of this study was to describe in detail one woman’s pattern of daily occupations as an initial step in developing strategies for further studies of patterns of daily occupations. Four different methods were used, one on each of four days. Three types of occupations were discerned; main occupations, hidden occupations, and unexpected occupations, which intertwined and formed an occupational pattern. The occupations were, in turn, composed of actions. Using the diary method gave an overall picture of main occupations during a whole day. Direct observation and video-recorded observation added additional information about the actions that were the building blocks of the occupations, as well as about hidden and unexpected occupations. The fourth method, a variant of the experience sampling method, gave deeper knowledge about the informant’s reflections when performing an occupation. It was assumed that a detailed description of patterns of occupation is needed to understand its relationship to health. Therefore, if the diary method is used, it has to be combined with a subsequent interview. Direct observation was the single method that best captured the complexity of the pattern of daily occupations as captured in this study.

  • The Test of Visual Perceptual Skills (non-motor) Upper Level is Not a Valid Predictor for the Outcome of Driver Education

    • In order to reduce the monetary and personal costs of driver training for persons with cerebral palsy, it is of interest to find a predictor that is able to select potential license holders. Previous research has shown that such a predictor could be one that assesses visual perception. In the present study, the Test of Visual Perceptual Skills (non-motor) Upper Level, was validated for predicting the outcome of driver education. It was found that using the test for this purpose could not be recommended. The findings of the present study were not in accordance with the results of other studies on the predictive value of perceptual tests regarding the ability to drive, owing to the use of different methods. Future studies should focus on cross-validation of perceptual tests for different types of independent variables, such as driving license or not, accident involvement and driving ability.

  • The transport mobility situation for children with autism spectrum disorders

    • In order to investigate the transport mobility situation for children with autism spectrum disorders, questionnaires from 1,631 parents were analysed. The results showed that almost 3 out of 4 parents were worried when their child was transported by school transport or by the Special Transport System. Transports in the family vehicle caused worry among almost half of the parents. The parents' worries were justified by the fact that the children were not transported according to general safety recommendations. Moreover, it was common for the children to be transported with unfamiliar drivers, as well as with unknown passengers, which is known to be quite problematic. Transport mobility adaptation to this particular group of children with disabilities refers merely to implementation of knowledge and a specific, well-structured approach among the drivers towards the children during the ride, rather than to physical/mechanical adaptation of the vehicles.
