    • Constipation is a common and well-studied symptom in palliative care. Most previous studies have focused on the frequency and management of constipation. The current study aimed to investigate the lived experience of constipation among cancer patients in palliative hospital-based home care. Eleven cancer patients admitted to a hospital-based home care unit in Sweden who had experienced constipation participated in this study. Semi-structured interviews were conducted and a qualitative, phenomenological approach was used. Three themes emerged: bodily suffering, mental preoccupation and a reminder of death, and avoidance and social isolation. The experience of constipation was described as an extensive complete person-experience that must be prevented. To prevent unnecessary suffering nurses need to be proactive in the assessment of constipation.

  • Physical and psychosocial impact of xerostomia in palliative cancer care: a qualitative interview study

    • Xerostomia is often considered to be merely a physical problem. The aim of this study was to explore the global effects of xerostomia, with a specific focus on psychological and social consequences. Semi-structured, tape-recorded interviews were conducted with 16 patients with advanced malignancies and symptomatic xerostomia. A phenomenographic (qualitative) analysis was performed. Four main categories were identified in the study: 1) subjective discomfort, e.g. dryness or burning sensation, 2) loss of function, e.g. articulation or swallowing, 3) increased infection (oral thrush and ulcerations), 4) psychosocial effects, including shame, increased feelings of being a patient rather than a person and a tendency to avoid social contact, resulting in loneliness. Xerostomia and its associated symptoms have a considerable, negative global impact, resulting in shame, anxiety, disappointments and verbal communication difficulties. There should therefore be more focus on the management of xerostomia, which is often neglected in palliative care.

  • Pressure ulcers in palliative care: development of a hospice pressure ulcer risk assessment scale.

    • The aim of this study was to construct a pressure ulcer risk assessment scale appropriate for palliative care patients. Risk assessments were performed using the modified Norton scale and nine newly constructed scales, consisting of the modified Norton scale with various changes. Data were collected from 98 patients in a Swedish hospice between April 1999 and September 2000. Pressure ulcer occurrence was registered weekly in the patient record. Increasing age, male gender, physical inactivity, immobility, decreasing food and fluid intake, incontinence, poor general physical condition and lean body constitution were shown to be significant risk factors for development of pressure ulcers in terminally ill cancer patients. After further testing of the data, one scale was found to be superior in identifying patients at risk, with higher validity than the modified Norton scale. The assessment items in this new scale, The Hospice Pressure Ulcer Risk Assessment Scale (in Swedish: Hospice Riskbedöming Trycksår, (HoRT)), are physical activity, mobility and age.
