  • Improving evidence-based social work practice with youths exhibiting conduct problems through structured assessment

    • A key task in evidence-based case management of youth is the assessment of research-based risk and protective factors. In the present study we compare assessments of social workers using a structured assessment instrument with assessments of social workers not using such an instrument. Assessments of the exact same case—a vignette about a 14-year-old boy—conducted by 30 social workers using a structured assessment instrument and 30 social workers not using such an instrument were compared. The 60 assessments were also rated by independent researchers and senior social services managers, blind to whether an instrument had been used in the assessments or not. As hypothesized, using a structured assessment instrument resulted in the identification of a greater number of research-based risk and protective factors, and the assessments were rated as better in terms of general adequacy, quality, accuracy and potential treatment effectiveness, than when an instrument was not used. The present study demonstrates that social workers’ assessments of youth become more evidence-based, adequate and potentially more treatment effective when a structured assessment instrument is used as compared to when it is not.

  • Strategies to handle the challenges of intimacy in nighttime home care services

    • The provision of intimate and personal care constitutes a challenge for both careworkers and care recipients and is still a neglected area of research. An observational study of the interaction between the careworkers and care recipients in night-time home care services was conducted in a large municipality in Sweden. The results were analysed in light of previous research and theorising on strategies for handling intimacy in intimate care. The study highlights what appears to be a tension between the ways in which the recipient of care is conceptualised as an active consumer of care in present-day guidelines and the strategies chosen on the part of both caregivers and care recipients, when intimacy and integrity is most at stake, and framed as it is by the care recipients' situation of dependency and vulnerability. Home care services night-time was shown to be a case that markedly differs from many other settings of intimate care, but in the interactional routines intimate care came forth as a smooth and minimally obtrusive activity. The careworkers and care recipients engaged in strategies such as disattention, eye-discipline, middle-distance orientation, and objectification, thereby serving the purpose of balancing the transgressions of thresholds of intimacy.

    • Avvägningar mellan den biologiska moderns och barnets intressen: En kvalitativ studie av svenska familjerättsekreterares erfarenheter av transnationellt surrogatmoderskapDet finns få studier om hur socialarbetare handlägger ärenden som berör transnationellt surrogatmoderskap. Vår studie har för avsikt att bidra till att fylla detta tomrum. I Sverige är det inte tillåtet för sjukvården att utföra assisterad befruktning vid surrogatmoderskap. Detta har gjort att människor vänt sig utomlands för denna reproduktionsmetod, främst till Indien. Det finns inga lagar som reglerar surrogatmoderskap i Sverige, vilket har lett till svårigheter vid fastställande av rättsligt föräldraskap när föräldrarna återvänt med barnet. Denna kvalitativa intervjustudie med familjerättssekreterare har funnit att det finns rättslig osäkerhet och etiska frågeställningar som påverkar deras hantering av ärendet. Hur föräldraskap konstrueras i nuläget beror till stor del på enskilda familjerättssekreterares uppfattning om hur man bäst väger surrogatmammans intressen mot vad som är bäst för barnet. Avsaknad av riktlinjer och de etiska frågeställningarna har lett till osäker och olika hantering av ärendena, och en reglering behövs för att bättre skydda de inblandade och öka rättssäkerheten i handläggningen.

  • Coping with stressful situations in social work before and after reduced working hours, a mixed-methods study

    • Little is known about how reduced working hours relate to social work stressors, coping and work-life balance. An exploratory mixed methods study was therefore performed at a Swedish social service agency participating in a quasi-experimental trial of work-time reduction by 25%, with proportionally reduced workload, and retained full-time pay. Social workers that reduced their working hours reported less emotional exhaustion (n = 28, p < 0.05) on the Maslach Burnout Inventory-Human Services Survey, and less reactivity in stressful situations connected to time urgency and irritation (n = 28, p < 0.05) on the Everyday Life Stress Scale. In interviews, the social workers described that despite using effective, problem-focused coping behaviour at work, both before and after work-time reduction, high caseload remained a central stressor, creating time conflicts that exacerbated stressful situations involving emergencies, practical setbacks, client aggression, report deadlines, and managerial stress. In contrast, the work-time reduction was described as fully resolving time conflicts and stress during free-time in situations that involved finding time for friends, household chores, rest, exercise and childcare. Results suggest that reduced working hours lowered emotional exhaustion and situational reactivity by increasing free-time recovery opportunities and decreasing total daily exposure to work stress, but future trials should also compare reduced work-time with reduced caseload.

    • The purpose of this study is to analyse observed situations of successful cooperation, even if it unfolds during shorter interaction sequences. The aim is to analyse how and when the actors within juvenile care in Sweden present successful cooperation, and which interactive patterns are involved in the construction of this phenomenon. Forming the empirical basis for this study are 119 field observations of organised meetings and informal meetings before and after organised meetings, during visits to youth care institutions in Sweden, social services offices, and the Swedish National Board of Institutional Care. In this study, markers are used to define successful cooperation in the empirical material, so that actors who belong to at least three different categories will be identified (coherent triad). The professional actors can also shape a coherent triad with young people or parents in cases where past conflicts arise. When some professionals create a distance from other professional partners, conflicts can be erased so as to generate new conditions for coherence of the triad. Construction and reconstruction of collaboration success is an ongoing, interactive process. Presentation of the proper interaction moral is created and re-created during interactions and appears in the myriad of everyday interactions.

    • Active labour market policies and programs (ALMPs) have over the lastdecades been established as main instruments to promote the transitionfrom welfare to work. In this article we study strategies employed bySwedish municipalities to help recipients ending spells of socialassistance take-up. By using a combination of quantitative andqualitative data we try to identify which approaches stand out as moresuccessful than others with respect to social assistance duration. Wehave formed pairs of municipalities similar in important respects, butwith different social assistance exit rates as predicted by a statisticalmodel. Then we conducted case studies in the selected municipalities’social assistance units, gathering information on how work is carried outand organized. Information was collected by semi-structured interviewswith department managers and front-line social workers. In theinterviews we identified various elements of practice that emerged asrelevant, suggesting that shorter spells follow from activation with afocus on human resource development, programmes targeted at youngadults, well-functioning collaboration, and use of sanctions or an overallapproach characterized by systemized efforts.

    • School-based social work can reach children at risk through the promotion of children’s participation in seeking support. Drawing on Gibson’s theory of affordances, the aim of this interview-based study was to identify affordances for self-referral associated with school-based social work support. Results reveal three affordances facilitating children’s self-initiated contact: (i) the day-to-day presence of social workers in the school environment supports investment in relationships, (ii) use of communication technologies facilitates contact and (iii) the visibility of the social workers’ practice encourages contact-initiation. Common to all three affordances are the accessibility of the social workers, and the generation of trust.

    • Sense of security is important throughout the lifespan not at least in advanced age with increased risks of functional declines and decreased social capital. Despite this, knowledge concerning older person’s perceptions and experiences of sense of security when moving into nursing homes is scarce. This study is a longitudinal, descriptive, exploratory case study with in-depth interviews and observations of three older persons in the age of 87, 88, and 91 years in a mid-sized municipality in the south of Sweden, in order to highlight how sense of security is experienced when moving into and living in a nursing home. Data were analysed using qualitative content analysis, which resulted in one main theme and four categories. The main theme, ‘Adaptation and sense of security’, indicates older persons’ need to adapt to the new context of the nursing home, and how this relates to their sense of security. The categories – ‘Control’, ‘Struggling for understanding’, ‘Lack of influence’, and ‘Grasping’ – suggest that older persons’ sense of security is reduced when they must adjust to routines without sufficient management and understanding. When able to maintain control over daily routines, and felt as a part of the new context, they perceived a sense of security.
