    • BACKGROUND AND OBJECTIVES: During the past 10 years several DNA-typing methods have been developed to complement routine serological typing for determination of polymorphisms in the ABO, RH, KEL, JK and FY blood group genes. However, the molecular basis of blood groups can differ widely between ethnic groups. The purpose of this study was to evaluate selected DNA-based methods for phenotype prediction in a population not previously investigated. MATERIALS AND METHODS: Blood samples from a random sample of Jordanian blood donors were collected and red cells isolated from these blood samples were phenotyped for common ABO (n = 150) and KEL/FY/JK (n = 90) antigens. RHD-negative and -positive donors were selected for RH typing (n = 120 and 30, respectively). DNA was prepared and blood group genotyping performed according to selected methods in current use. Discordant samples required further investigation by extended serology and DNA sequencing. RESULTS: The degree of concordance between phenotype and genotype was high, but some exceptions were noted. Two of 14 A2/A2B samples lacked all mutations associated with known A2 alleles of the ABO system. RH typing revealed four samples with the c(cyt48) marker, causing false-positive RHC typing. A single D-negative sample was positive for D-specific exon 10 markers. The RHD pseudogene was not found in the 150 donors tested. Nine samples revealed discrepancies that were associated with unknown silent or weakly expressing Fyb-like alleles. CONCLUSIONS: With the exception of the FY system, we conclude that the molecular background of the clinically important blood group antigens studied here is similar to that reported for Caucasoids.

    • Background and Objectives: To ensure a proper quality control it is important to use a reliable method to count low numbers of leucocytes in leucocyte-reduced platelet concentrates (PCs). Materials and Methods: A modified flow cytometric method for counting low numbers of leucocytes, based on a reference population contained in tubes with an exact number of fluorescent beads and staining with propidium iodide was used. To increase the number of events, the original sample volume was increased. Results: There was a good correlation in the number of leucocytes (r = 0.99) between the modified flow cytometric method and microscopy of samples from unfiltered and expected numbers from serially diluted PCs. Samples from leucocyte-reduced PCs obtained by apheresis or filtered buffy coats showed no correlation between results from the modified flow cytometric method and microscopy (Nageotte). Conclusion: Counting by microscopy gave a lower number of leucocytes than the modified flow cytometric method when counting a low number of cells. However, analysis of the serially diluted PCs proved that the modified flow cytometric method was reliable and rapid, making it suitable for clinical routine use.

    • Background and Objectives: Our objective was to study the effect of storage time on the filtration of platelet concentrates (PCs). We compared the total number of white blood cells (WBC), as well as the distribution of WBC subsets, in units filtered before and after storage.Materials and Methods: Buffy coat-derived PCs were filtered either fresh or after 5 days of storage, and total WBC were enumerated by flow cytometry. WBC subsets were analyzed by flow cytometry with three-color fluorescence.Results: The total number of white cells before filtration was significantly higher in fresh units compared with stored units, whereas in postfiltration samples the number of white cells was significantly lower in the fresh compared with the stored units. Although absolute numbers were significantly reduced, filtration also induced significant changes in the proportions of subsets in both fresh and stored units; the percentage of T cells was decreased, whereas the percentage of B cells and monocytes was increased after filtration.Conclusion: Our results suggest that prestorage WBC filtration of platelet concentrates is superior in reducing the absolute numbers of WBC. However, both pre- and poststorage WBC filtration significantly affect the proportions of WBC in the final product, decreasing the number of T cells while apparently increasing the proportion of MHC class II-positive cell populations.

    • BACKGROUND AND OBJECTIVES: This study aimed to investigate the impact and effects of blood donation on blood donors as perceived and reported by donors themselves. MATERIALS AND METHODS: A self-administered questionnaire was distributed to 600 consecutive whole-blood donors (who had a history of at least one previous donation) consisting of an open-ended question asking whether the blood donation had any impact on the donor. The answers to this question were considered as descriptions of effects perceived by the donors to be evoked by whole-blood donation. RESULTS: In all, 528 subjects completed the questionnaire (88%; 319 males and 209 females) and answered the question about the effects of blood donation. Altogether, 54% (287 out of 528) of the blood donors reported one or several effects. Exclusively positive effects were described by 29% (151) of blood donors, while exclusively negative effects and mixed effects (i.e. concomitant positive and negative effects in the same subject) were described by 19% (103) and 6% (33), respectively, while no effect was reported by 46% (241) of the donors. A majority of the effects commenced within 1 h of blood donation. The positive effects lasted significantly longer (often for weeks, P < 0.0001) than negative effects (min/h/days). Among positive effects a feeling of satisfaction, of being more alert, and feeling generally better than before the blood donation predominated for both female and male donors. Among negative effects, vertigo/dizziness was reported more frequently by female donors (P < 0.0001). Logistic regression analysis revealed that the negative effects were less likely to occur with increasing age (P < 0.001) and that they were more likely to occur in female donors (P < 0.001) in comparison to male donors, irrespective of age. CONCLUSIONS: The majority of effects elicited by blood donation on blood donors were positive (i.e. feelings of satisfaction, greater alertness, increased wellbeing, etc.). The positive effects did not differ from the negative regarding time of onset, yet their duration was reported to be significantly longer. There was no association between frequency of occurrence of positive effects and the number of blood donations, indicating that there is no 'addictive' relationship between donors and blood donations. The findings in this study of high frequency of occurrence of positive long-lasting effects elicited in blood donors by blood donation may be of great importance for the recruitment of new blood donors as it may make blood donation less frightening and perhaps even attractive.

    • BACKGROUND AND OBJECTIVES: The transmission routes for GB virus-C (GBV-C)/hepatitis G virus (HGV) in blood donors unexposed to hepatitis C virus (HCV) are unknown. We performed a case-control study of risk factors for GBV-C/HGV exposure in blood donors. MATERIALS AND METHODS: After testing stored sera from 458 HCV-negative blood donors for GBV-C/HGV RNA and GBV-C/HGV E2 antibodies, 66 donors with GBV-C/HGV markers and 125 age- and gender-matched controls were interviewed regarding risk factors for viral transmission. RESULTS: Exposure to GBV-C/HGV was strongly associated with previous treatment for a sexually transmitted disease (odds ratio [OR] 4.6; 95% confidence interval [CI] 2.2-9.8), with multiple sexual partners (OR 2.9; 95% CI 1.4-5.7) and with a past history of endoscopy (OR 7.0; 95% CI 3.0-16.4). CONCLUSIONS: In blood donors with GBV-C/HGV markers, sexual contacts and medical procedures appear to be the main transmission routes.
