    • At stake in this study is the patient's credibility. The Cartesian philosophical standpoint, which holds sway in western thinking, questions with scepticism whether the reported symptoms are “real.” Do they reside in the body, or are they mentally concocted. However, from the caring perspective any symptom must be both listened and attended to in its own right, not just scrutinized as evidence for an accurate diagnosis.In cognitively and emotionally complex high-tech units caregivers are juggling a precarious handful of cards. Technical tasks take precedence or have more urgency than caring behaviour. Assuming an irremediable tension between object–subject and care–cure in nursing is futile dualism. By addressing the essence of technology—the non-neutral and highly visual technology—this paper aims to find, from a philosophical point of view, a more comprehensive understanding for the dominance visualism and technification within intensive care.Screens give us access to vital signs. Screens record numbers and lines that relate to a graph and afford superfine spiked “readings.” However, the most relevant vital signs may be missing.

    • Procedures for the reductive coupling of carbonyl compounds to alkenes in the literature rely either on a radical coupling strategy, as in the McMurry coupling, or ionic pathways, sometimes catalysed by transition metals, as in more contemporary contributions. Herein, we present the first example of a third strategy that is based on the [2 + 2] cycloaddition of ketone-derived phosphaalkenes. Removal of P-trimethylsilyl groups at the intermediary 1,2-diphosphetane dimer results in its collapse and concomitant release of the tetraaryl-substituted alkene. In fact, the presented strategy is the only alternative to the McMurry coupling in the literature that allows tetraaryl alkene formation from diaryl ketones, with yields as high as 85%. The power of the methodology is illustrated in the reaction of tethered bis-benzophenones which engage in intramolecular reductive carbonyl couplings to form unusual macrocycles without the need for high dilution conditions or templating.

  • Left atrial size as predictor of recurrences after catheter ablation in paroxysmal atrial fibrillation : a systematic review and meta-analysis

    • Background and Aim: Left atrial (LA) enlargement is associated with paroxysmal atrial fibrillation (PAF) incidence and outcome. The predictive role of the LA size in AF treatment with catheter ablation is still controversial. The aim of this meta-analysis was to analyze the potential association between LA diameter in patients with PAF undergoing ablation and AF recurrence after ablation.Methods: We systematically searched PubMed-Medline, EMBASE, Scopus, Google Scholar and the Cochrane Central Registry, up to December 2016 in order to select clinical trial and observational studies, which assessed the predictive role of LA diameter in AF recurrence after catheter-ablation. 2962 patients from 16 studies with paroxysmal AF (PAF) were included.Results: The pooled analysis showed that after a follow-up period of 19. 66± 8.31 months, patients with AF recurrence had larger LA size compared with those without AF recurrence, with a weighted mean difference (WMD) 2.31 ([95% CI 1.27 to 3.34], P < 0.0001). LA diameter ≥40 mm predicted AF recurrence HR:1.04 [95% CI 1.00 to 1.08], P=0.04), but the best cut-off value, in all included patients, was ≥50mm HR:3.08 [95% CI 1.47 to 6.49], P=0.003).Conclusions: Enlarged left atrium in patients with PAF undergoing catheter ablation predicts recurrences. The diameter more than 50 mm is the best cut-off of the recurrences of AF, but diameter of 40 mm also can predict recurrences in these patients.

  • Nanocellulose : its applications, consequences and challenges in papermaking

    • This paper has aimed to bring out the state-of-the-art information about nanocellulose, its application in papermaking process, effect on paper properties and challenges. In papermaking process, nanocellulose is used as bionanofiller and bionanocoating material. The main objective of considering nanocellulose as a bionanofiller is to retain the strength properties after using inorganic filler, such as, GCC (ground calcium carbonate) and PCC (precipitate calcium carbonate); nanocellulose also helps to increase the filler content in paper sheet without loss of paper strength. That’s why, the application of nanocellulose as filler and coating material can solve the issues of using inorganic filler in papermaking industry. Though the production of nanocellulose needs higher energy, which increases the production cost in papermaking process, it can be minimized using different advanced technologies, such as, chemical and enzymatic pretreatment. Additionally, the incorporation of nanocellulose in recycled and virgin fiber increases some physical and strength properties of the paper. Furthermore, the increment of filler content in paper sheet due to incorporating nanocellulose can also reduce the amount of fiber in papermaking process, and it reduces the cost of fiber, which can mitigate the drawback of paper production cost because of adding nanocellulose partially. Therefore, nanocellulose has the potential to use for making value added paper and paper products to compensate the production cost.

  • Transitions in Men’s Caring Identities : Placing a partner in a nursing home

    • Objectives.  The aim of this study is to describe, from a gender identity perspective, the experiences of older men involved in the process of caring for a partner at home and the placement into a nursing home. Background.  Few studies have paid attention to the importance of gender when considering the social experiences of older men providing care for an ill spouse and finally placing a partner in a nursing home. Further understanding is much needed of how older men experience the process of caring for a spouse from a gender identity perspective. Design.  A qualitative constructivist approach was adopted for this study. Participants.  Data consists of interviews with seven men that have been informal carers and experienced the placement of their wife in a nursing home. Methods.  Interviews were analysed with a constructivist approach. Results.  The results indicate that men go through two transitions in their gender identity during the caregiving process and placement. From the mutual loving relationship of being a loving husband, the social responsibility of daily care of their wives changes the situation into that of being a caring husband, and finally with the move to a nursing home there is a transition from intimate care to a relationship based on friendship. Conclusions.  The results show that older caregiving men undergo a process involving a reconstruction of gender identity. To formally recognize men’s caring activities and to make them sustainable, we believe that men in an informal caring relationship need support. Relevance to clinical practice.  Nurses need to recognize the identity struggles resulting in sadness and suffering that are related to changes in men’s lives during the caregiving process. Understanding the dynamics and changes that occur when men take on a caring task is important for the development of their role as carers.

    • Crystals of commercial iodic( V) acid, HIO3, were investigated using single-crystal and powder X-ray diffraction. The crystals turned out to be HI3O8 or (HIO3I2O5)-I-. [iodic acid-diiodine pentaoxide (1/1)] instead, whose structure has been determined previously [Feikema & Vos (1966). Acta Cryst. 20, 769-777]. Redetermination of the structure gave higher precision and allowed the location of the H atom.
