  • Silence as Possibility in Postwar Everyday Life

    • Silence is a form of communication as multifaceted as speech and as such conveys a broad range of contextually situated social meanings. Often silence is understood as a form of denial and inherently detrimental to processes of reconciliation, but it may help create a sense of 'normality' and facilitate encounters between former foes. This article enquires into the role and meanings of silence as tacit forms of communication in postwar social processes and everyday life among people of different ethnic groups in Bosnia and Herzegovina, particularly as used by the displaced and returnees. It argues that in these contentious settings, everyday social interaction employs silence in ways that empower by communicating respect and even trust, thus forming and sustaining relations important to viable local life. Silence can be used to affirm family continuity and protect close relationships. Silent claims may also 'speak' from a marginalized position in relation to hegemonic narratives and make moral claims. Silence may thus be understood as a pragmatic and at times successful strategy for coexistence even when reflecting continued division in the larger society.

  • Questionnaire layout and wording influence prevalence and risk estimates of respiratory symptoms in a population cohort

    • Objective: Results of epidemiological studies are greatly influenced by the chosen methodology. The study aims to investigate how two frequently used questionnaires (Qs), with partly different layout, influence the prevalence of respiratory symptoms.Study Design and Setting: A booklet containing two Qs, the Global Allergy and Asthma European Network Q and the Obstructive Lung Disease in Northern Sweden Q, was mailed to 30 000 subjects aged 16-75 years in West Sweden; 62% responded. Sixteen questions were included in the analysis: seven identical between the Qs, four different in set-up and five with the same layout but different wording. Comparisons were made using differences in proportions, observed agreement and Kappa statistics.Results: Identical questions yielded similar prevalences with high observed agreement and kappa values. Questions with different set-up or differences in wording resulted in significantly different prevalences with lower observed agreement and kappa values. In general, the use of follow-up questions, excluding subjects answering no to the initial question, resulted in 2.9-6.7% units lower prevalence.Conclusion: The question set-up has great influences on epidemiological results, and specifically questions that are set up to be excluded based on a previous no answer leads to lower prevalence compared with detached questions. Therefore, Q layout and exact wording of questions has to be carefully considered when comparing studies.

    • Previous research shows generally low agreement concerning self-rating scales and assessments by staff regarding people considered to be ‘severely mentally ill’. The aims of this study were to measure the level of agreement between patients’ self-rating and key workers’ assessments, and to explore some of the factors that influence level of agreement. The results of questionnaires from 49 severely mentally ill persons concerning needs, therapeutic alliance, and self-esteem were compared with key workers’ estimates. Agreement was found regarding the number of unmet needs. Concerning met needs, therapeutic alliance and self-esteem, no significant agreements were found, however, for met needs, therapeutic alliance or self-esteem.

  • Disproportionate early fetal growth predicts postnatal thymic size in humans

    • Prenatal events can affect neonatal thymus size and adult immune function. The causal insults are unknown, although fetal nutrient restriction is suspected. We used ultrasound at three time points during pregnancy (14, 19 and 30 weeks) to measure the growth of six fetal dimensions in rural Bangladeshi women participating in the Maternal and Infant Nutrition Interventions, Matlab study. Postnatal ultrasound was used to calculate thymic index (TI) at birth, 2, 6 and 12 m. Of the 3267 women recruited, 2861 participated by providing data at least at one fetal biometry and one TI time point. Patterns of fetal growth were summarized using principal components calculated from fetal dimension z-scores. Random effects regression, controlling for infant size and season of measurement were used to relate these patterns to TI. We found that smaller leg length relative to head circumference, characteristic of head-sparing growth restriction, was predictive of lower TI. This association was significant at all time points but strongest in earlier pregnancy. Each standard deviation increase in leg-head proportion was associated with an increase in TI of similar to 5%. We conclude that growth patterns typical of poor fetal nutrition are associated with poor thymic development. The greater strength of this association in the first trimester is consistent with a period of vulnerability during the early ontogeny of the thymus and suggests that preventative intervention would need to be given in early pregnancy.

    • An assemblage of mineralized skeletal fossils containing molluscs, hyoliths, chancelloriids, protoconodonts, lobopods, paleoscolecids, bradoriids, echinoderms and hexactinellid sponges is described from the middle Cambrian part of the Kuonamka Formation, exposed along the Malaya Kuonamka and Bol’shaya Kuonamka rivers, northern Siberian Platform. The sampled succession is attributed to the Kuonamkites and lower Tomagnostus fissus–Paradoxides sacheri biozones of the Amgan Stage of Siberia, correlated with Series 3, Stage 5—lower Drumian Stage of the IUGS chronostratigraphical scheme for the Cambrian. This work complements descriptions of molluscs from the same samples published by Gubanov et al. (2004) with additional material. It contains forms in common with coeval faunas from Australia, China, Western Gondwana, Avalonia, Laurentia and Baltica, increasing potential for global biostratigraphic correlation and understanding of palaeogeographic connections.

  • Extensive central nervous system involvement in Merkel cell carcinoma : a case report and review of the literature

    • INTRODUCTION: Merkel cell carcinoma is a rare malignant cutaneous neoplasm that is locally invasive and frequently metastasizes to lymph nodes, liver, lungs, bone and brain. The incidence of Merkel cell carcinoma has increased in the past three decades.CASE PRESENTATION: A 65-year-old Caucasian man presented with a sudden onset of severe headache and a three-month history of balance disturbance. Magnetic resonance imaging revealed a large meningeal metastasis. The radiologic workup showed retroperitoneal and inguinal lymph node metastases. Biopsy of the inguinal lymph nodes showed metastases of Merkel cell carcinoma. Biopsy from three different suspected skin lesions revealed no Merkel cell carcinoma, and the primary site of Merkel cell carcinoma remained unknown. Leptomeningeal metastases, new axillary lymph node metastases, and intraspinal (epidural and intradural) metastases were detected within six, seven and eight months, respectively, from the start of symptoms despite treating the intracranial metastasis with gamma knife and the abdominal metastases with surgical dissection and external radiotherapy. This indicates the aggressive nature of the disease.CONCLUSION: To the best of our knowledge, this is the first report in the literature of an intracranial meningeal metastasis of Merkel cell carcinoma treated with gamma knife and of intraspinal intradural metastases of Merkel cell carcinoma. Despite good initial response to radiotherapy, recurrence and occurrence of new metastases are common in Merkel cell carcinoma.

    • INTRODUCTION:This is the first report on the simultaneous successful treatment of a large ruptured saccular aneurysm and stenotic parent artery with a single flow-diverting stent.CASE PRESENTATION:We report the case of a 68-year-old Caucasian man with occlusion of the right vertebral artery and a ruptured aneurysm at the junction of the left posterior inferior cerebellar artery-left vertebral artery that was successfully treated by the deployment of a single flow-diverting stent in the stenotic left vertebral artery. Stent deployment was complicated by thrombotic occlusion of the basilar artery, which was successfully reopened. The patient recovered completely, and follow-up angiography at 4 months and 1 year showed patent vertebral artery with gradual shrinkage of the aneurysm.CONCLUSIONS:This report contributes to the literature on treatment of large ruptured aneurysms localized in stenotic arteries and in areas of the endocranium where a mass of embolic material in the aneurysm (coils) might compromise the circulation in the parent blood vessel or compress vital brain structures.

  • Lyme neuroborreliosis in HIV-1 positive men successfully treated with oral doxycycline: a case series and literature review.

    • ABSTRACT: INTRODUCTION: Lyme neuroborreliosis is the most common bacterial central nervous system infection in the temperate parts of the northern hemisphere. Even though human immunodeficiency virus (HIV) -1 infection is common in Lyme borreliosis endemic areas, only five cases of co-infection have previously been published. Four of these cases presented with typical Lyme neuroborreliosis symptoms such as meningoradiculitis and facial palsy, while a fifth case had more severe symptoms of encephalomyelitis. All five were treated with intravenous cephalosporins and clinical outcome was good for all but the fifth case Case presentations: We present four patients with concomitant presence of HIV-1 infection and Lyme neuroborreliosis diagnosed in Western Sweden. Patient 1 was a 60-year-old Caucasian man with radicular pain and cognitive impairment. Patient 2 was a 39-year-old Caucasian man with headaches, leg weakness, and pontine infarction. Patient 3 was a 62-year-old Caucasian man with headaches, tremor, vertigo, and normal-pressure hydrocephalus. Patient 4 was a 50-year-old Caucasian man with radicular pain and peripheral facial palsy. Patients one, two, and three all had subnormal levels of CD4 cells, indicating impaired immunity. All patients were treated with oral doxycycline with good clinical outcome and normalization of CSF pleocytosis. CONCLUSION: Given the low HIV-1 prevalence and medium incidence of Lyme neuroborreliosis in Western Sweden where these four cases were diagnosed, co-infection with HIV-1 and Borrelia is probably more common than previously thought. The three patients that were the most immunocompromised suffered from more severe and rather atypical neurological symptoms than are usually described among patients with Lyme neuroborreliosis. It is therefore important for doctors treating HIV patients to consider Lyme neuroborreliosis in a patient presenting with atypical neurological symptoms. All four patients were treated with oral doxycycline with a good outcome, further proving the efficacy of this regime.
