    • BACKGROUND: All-polyethylene (AP) tibial components in patients aged greater than 60 years have stable tibial migration patterns and favorable survival rates when compared to identical Metal-backed (MB) designs. Tibial component migration in younger patients has not been reported. The aim of this study was to examine the migration characteristics of patients aged less than 60 years compared to a previous cohort of AP and MB tibial components of identical design in older patients.METHODS: A prospective consecutive study examined tibial component migration in 21 patients aged less than 60 years undergoing a cemented total knee arthroplasty with an AP tibial component by radiostereometric analysis (RSA) to 24 months. Results were compared to the authors' previous series of 21 patients aged greater than 60 years that were randomized to either an AP or MB tibial component.RESULTS: Both age groups of patients implanted with an AP component had stable migration patterns with no patient having greater than 0.2° rotation or 0.2 mm maximum total point motion. Five of 11 MB tibial components displayed continued migration between one and two years. Subsidence was similar in all groups, whilst maximum total point motion was greater for the MB cohort (0.34 mm, 0.33 mm, 0.61 mm; AP <60, AP >60, MB).CONCLUSIONS: Young patients implanted with an AP tibial component had stable tibial migration patterns comparable to older patients with the same AP implant. Regardless of age, AP tibial components were at least as stable as MB tibial components.LEVEL OF EVIDENCE: Level II, Prospective comparative study.

  • Curve analyses reveal altered knee, hip, and trunk kinematics during drop-jumps long after anterior cruciate ligament rupture

    • Background: Anterior cruciate ligament (ACL) ruptures may lead to knee dysfunctions later in life. Single-leg tasks are often evaluated, but bilateral movements may also be compromised. Our aim was to use curve analyses to examine double-leg drop–jump kinematics in ACL-reconstructed, ACL-deficient, and healthy-knee cohorts.Methods: Subjects with unilateral ACL ruptures treated more than two decades ago (17–28 years) conservatively with physiotherapy (ACLPT, n = 26) or in combination with reconstructive surgery (ACLR, n = 28) and healthy-knee controls (n = 25) performed 40-cm drop–jumps. Three-dimensional knee, hip, and trunk kinematics were analyzed during Rebound, Flight, and Landing phases. Curves were time-normalized and compared between groups (injured and non-injured legs of ACLPT and ACLR vs. non-dominant and dominant legs of controls) and within groups (between legs) using functional analysis of variance methods.Results: Compared to controls, ACL groups exhibited less knee and hip flexion on both legs during Rebound and greater knee external rotation on their injured leg at the start of Rebound and Landing. ACLR also showed less trunk flexion during Rebound. Between-leg differences were observed in ACLR only, with the injured leg more internally rotated at the hip. Overall, kinematic curves were similar between ACLR and ACLPT. However, compared to controls, deviations spanned a greater proportion of the drop–jump movement at the hip in ACLR and at the knee in ACLPT.Conclusions: Trunk and bilateral leg kinematics during double-leg drop–jumps are still compromised long after ACL-rupture care, independent of treatment. Curve analyses indicate the presence of distinct compensatory mechanisms in ACLPT and ACLR compared to controls.

  • Income change after cruciate ligament injury - A population-based study

    • Objectives: To investigate the association between choice of treatment and patients' income after cruciate ligament (CL) injury and assess the effect of different covariates such as sex, age, comorbidities and type of work.Methods: This entire-population cohort study in Sweden included working patients with a diagnosed CL injury between 2002 and 2005, identified in The National Swedish Patient Register (n = 13,662). The exposure was the treatment choice (operative or non-operative treatment). The main outcome measure was average yearly income five years after CL diagnosis, adjusted for the following covariates: sex, age, comorbidities, type of work, region, calendar year, education and income.Results: Relative to non-operative treatment, operative treatment was associated with greater average yearly incomes (nine to 15%) after injury among patients between 20 and 50 years, patients with partial university education, patients living in large cities and patients with one comorbidity, despite no overall significant association in the national cohort. Delayed operative treatment (>1 year) had no significant association with income change, whereas early operative treatment (<1 year) was associated with higher average yearly incomes (11 to 16%) among females, patients between 20 and 50 years, patients living in large cities and patients with one comorbidity.Conclusions: In a broad sense, treatment choice was not associated with changes in income five years after CL injuries among patients in the workforce, however earlier operative treatment was associated with higher average incomes among patients with ages between 20 and 50, females, living in large cities, with one comorbidity and with a high level of education.

  • The impact of symptomatic knee osteoarthritis on gait pattern and its association with performance-based measures and patient-reported outcomes

    • Background: Limited knowledge exists regarding the impact of symptomatic knee osteoarthritis (OA) on the overall gait pattern; and whether gait deviations are associated with performance based measures (PBMs) and patient-reported outcomes (PROs). This cross-sectional study evaluated overall gait patterns in patients with knee OA using the Gait Deviation Index for kinematics (GDI) and kinetics (GDI-kinetic), and explored associations between gait deviations, PBMs, and PROs. Methods: Forty patients with knee OA and 25 age and gender-matched controls underwent three-dimensional gait analysis. Participants performed the Timed Up and Go (TUG), Five Times Sit-to-Stand (5STS), and Single Limb Mini Squat (SLMS) tests and completed a disease specific PRO. Associations between gait deviations, PBMs, and PROs were assessed by Pearson's correlation and multiple linear regression. Results: Patients with OA demonstrated significantly lower GDI and GDI-kinetic scores of the OA and contralateral limbs compared to controls; with GDI-kinetic scores on the contralateral limb more impacted than the OA limb. On the contralateral limb, GDI-kinetic score significantly correlated with TUG (r = -0.42) and 5STS (r = -033), while on the OA limb with TUG (r = -0.68), 5STS (r = -0.38), SLMS (r = -0.38), activities of daily living (r = -0.35) and Knee-related Quality of Life (r = -035). No significant associations existed between kinematic GDI scores, PBMs and PROs. Conclusion: The overall gait pattern, as represented by GDI and GDI-kinetic scores, in patients with symptomatic knee OA is affected both on the painful OA limb and the contralateral limb. The GDI and GDI-kinetic scores provide different information regarding function that is not revealed by PBMs or PROs.

  • Measuring greenhouse gas emissions from international air travel of a country's residents methodological development and application for Sweden

    • Global civil aviation accounts for 4–5% of total greenhouse gas emissions and these emissions are increasing. In the absence of sufficiently effective global climate instruments, national instruments might be considered as a complement, in which case some way of allocating emissions from international air travel between countries is needed. The purpose of this paper is to develop an accounting method that reflects one country's greenhouse gas emissions from international air travel, and to apply this methodology to Sweden. The new methodology consists of three parts: the number of international air trips made by the country's residents; the average distance of these trips; and the greenhouse gas emissions per passenger km. For Sweden, data for 1990 to 2014 show an increase in the number of trips by Sweden's population of 3.6% per year, resulting in, on average, one international journey (round trip 5800 km) per capita in 2014. The average distance to the final destination has increased only marginally due to simultaneous growth in both long and short trips. However, global average greenhouse gas emissions per passenger km have decreased by 1.9% per year between 1990 and 2014. Because the increase in the number of their trips has outweighed the decrease in emissions per km, the total emissions from Swedish residents' international air travel have increased by 61% between 1990 and 2014. The total emissions from Swedish residents' air travel, including both CO2and non-CO2-effects, were 11 Mt CO2equivalents in 2014, which is the same level as the emissions from Swedish car traffic. This type of reliable data is important when designing policies and for getting public support for new policies.

    • The coastal areas of Eastern Mediterranean have long been the subject of research, due to their rapid geomorphological changes, but also because of their archaeological interest. Our study is focused on a shallow coastal lagoon of Peloponnese, Gialova Lagoon, which for several years has attracted the scientific interest of archaeologists, geomorphologists as well as sedimentologists. Gialova lagoon is located near the ancient city of Pylos, the kingdom of king Nestor during the Mycenaean period (1600-1100 BC). The objectives of this study are: (a) to reconstruct the middle to late Holocene depositional environments of the lagoon and (b) to correlate our data to already existing publications, in order to shed new light on the Holocene evolution of the lagoon and the associated coastal palaeoenvironmental changes. An 8m deep vibracore was drilled and a multi proxy analysis was carried out on the sediment sequence, including sedimentological (grain size analysis and moment measures, total organic carbon - TOC, total nitrogen e TN and total phosphorus - TP), high resolution geochemical (XRF-scanning) and palaeontological (micro-and macro faunal) analysis. The chronological framework is based on five C-14 datings forming the basis for an age depth model, calculated using the OxCal software. The radiocarbon dates from previous studies (6 cores, similar to 20 dates) were also taken into account. The data synthesis and interpretation provided robust and coherent indications regarding the palaeoenvironment, shoreline changes and the rate of geomorphological changes of the coastal area of Gialova Lagoon, as well as useful information about the palaeonvironmental and palaeoclimatic conditions that prevailed during the Mycenaean period. The interpretation, reveal a transition from a shallow marine environment (65005800 yr B.P.) to a brackish/lagoonal (5800-3300 yr B.P.), followed by a shift towards a freshwater/marsh environment (3300 yr B.P. to present).
